Preoperative antibiotics for mild or moderate acute cholecystitis (POMMAC): study protocol for a non-inferiority, open-label, randomized controlled trial.

BACKGROUND: Evidence is lacking on the effect of preoperative antibiotic therapy on postoperative infections in acute cholecystitis. The POMMAC trial aims to investigate whether a single dose of antibiotic prophylaxis is non-inferior to continuous preoperative antibiotic therapy in terms of postoperative infections. METHODS: This is a non-inferior, open-label randomized controlled trial aiming to include 456 participants. Eligible participants with mild or moderate acute cholecystitis will be randomized at a 1:1 ratio to receive either continuous or a single dose of preoperative antibiotic pr

Intravitreal methotrexate as a prophylactic strategy for proliferative vitreoretinopathy in high-risk retinal detachment.

BACKGROUND: Proliferative vitreoretinopathy (PVR) remains the major cause of surgical failure in rhegmatogenous retinal detachment (RRD). This study evaluates the anatomical and functional outcomes, as well as the safety, of a standardized intraoperative intravitreal methotrexate infusion protocol during pars plana vitrectomy (PPV) in eyes with early stage PVR (grade B) or with advanced PVR (grade C). METHODS: A retrospective case series was conducted on 60 consecutive eyes of 59 patients with RRD and PVR grade B (n = 29) or grade C (n = 31) undergoing PPV with adjunct intraoperative methotre

Molecular epidemiology of carbapenemase-producing gram-negative bacilli in diabetic foot infections: association with glycemic control.

BACKGROUND: Carbapenem-resistant Gram-negative bacilli are an increasing threat in diabetic foot ulcer (DFU) infections, limiting therapeutic options and worsening outcomes. However, data linking specific carbapenemase genes to clinical and metabolic factors remain limited. OBJECTIVES: To isolate Gram-negative bacilli from deep tissue biopsies of DFUs, determine carbapenem susceptibility by minimum inhibitory concentration (MIC) testing, detect major carbapenemase genes by multiplex qPCR, and evaluate their association with glycemic control and Wagner grade. METHODS: This cross-sectional stu

Comparative histopathological and immunohistochemical features of central nervous system and pulmonary tuberculosis: an exploratory retrospective study.

Central nervous system tuberculosis (CNS-TB) is a rare and particularly severe form of extrapulmonary tuberculosis (EPTB). Early diagnosis and precise treatment are crucial in managing this condition. Pathological and immunohistochemical features may differ between CNS-TB and pulmonary tuberculosis (PTB). This study included 9 patients of CNS-TB, and analyzed their clinical manifestations, pathological features, and the immunohistochemical(IHC) features. The findings were then compared with PTB to explore the similarities and differences in how tuberculosis manifests in the brain versus the lu

Diagnosing and treating influenza in the emergency department: an evidence-based update.

Influenza is a common emergency department presentation that can cause serious complications, particularly in young children, older adults, pregnant and postpartum patients, and patients with underlying conditions. Because its clinical features overlap with those of other respiratory illnesses, emergency clinicians must determine when testing will inform care, which patients need prompt antiviral treatment, and who can be safely discharged. This review presents evidence-based recommendations for the evaluation and management of influenza in the emergency department, with attention to diagnosti

Duration of single-agent perioperative antibiotic prophylaxis and postoperative meningitis after endoscopic endonasal skull base surgery: a systematic review and meta-analysis of 7,981 patients.

Postoperative meningitis is a rare but serious complication in endoscopic-endonasal skull-base surgery. Although prophylactic antibiotics are routinely administered, the optimal regimen and duration remain unclear. This systematic review and meta-analysis evaluated the incidence of meningitis and intraoperative cerebrospinal fluid (CSF) leakage associated with single-agent perioperative antibiotic prophylaxis. Following PRISMA 2020 guidelines, PubMed, EMBASE, and Web of Science were searched through February 2025. Eligible studies reported postoperative meningitis or intraoperative cerebrospin

Mycobacterium tuberculosis complex Lineage 8 causes drug resistant pulmonary tuberculosis also in South Kivu, Eastern Democratic Republic of Congo.

BACKGROUND: Lineage 8 (L8) occupies a unique phylogenetic position as a sister clade to all other known Mycobacterium tuberculosis complex (MTBC) lineages. Although the initial identification of L8 strains was limited to Rwanda and Uganda, the complete geographical distribution of this lineage remains undetermined due to limited genomic surveillance across eastern and central Africa. METHODS: We performed whole genome sequencing (WGS) on MTBC isolates from rifampicin-resistant tuberculosis (TB) patients' sputum in Bukavu, South Kivu province, eastern Democratic Republic of Congo (DRC). Clinical Insights Into Linear Enamel Hypoplasia: A Case Report.

Linear enamel hypoplasia (LEH) is a failure or aberration of enamel to form correctly, resulting in bands of decreased enamel on the tooth surface. Many reasons, including genetics, malnutrition, and childhood diseases, can cause LEH, which solely affects the neonatal line. LEH is a kind of environmental hypoplasia with many symmetrical and ring-like abnormalities. Amelogenesis is a one-time procedure, and any abnormality might result in permanent enamel abnormalities. Enamel, unlike other human tissues, cannot be restored. The consequences of enamel hypoplasia vary based on illness severity,

Unmasking Brugada Syndrome With Oral Propafenone in a Resource-Limited Setting: A Case Report.

Brugada syndrome (BrS) is a cardiac channelopathy predisposing patients to malignant ventricular arrhythmias and sudden cardiac death (SCD) in the absence of structural heart disease. The diagnosis requires documentation of a type 1 Brugada electrocardiographic pattern, which can be unmasked using sodium channel-blocking agents in symptomatic patients presenting with non-diagnostic baseline patterns. We report a case of a 24-year-old male patient with a history of unexplained syncope who exhibited a non-diagnostic type 2 Brugada pattern on a baseline electrocardiogram (ECG). Due to the local u

Antimicrobial activity of twelve plant-derived compounds against fish-associated non-tuberculous mycobacteria.

Nontuberculous mycobacteria (NTM) are important pathogens of fish, causing chronic infections that are difficult to control in aquaculture. In this study, we evaluated the in vitro antimicrobial activity of twelve plant-derived compounds against six fish-associated NTM strains using a broth microdilution assay. Minimum inhibitory concentrations (MICs) ranged from 0.25 to >32 µg/mL. Our results demonstrated that cinnamaldehyde exhibited the strongest antimycobacterial effect, followed by sesamol and farnesol, while several other phytochemicals showed moderate activity and salicylic acid was ina

Nanotechnology-Based Theranostic Strategies for Triple-Negative Breast Cancer.

Triple-negative breast cancer (TNBC) is a heterogeneous type of breast cancer characterized by the absence of estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor 2 (HER2) expression. Because of the lack of ER, PR, and HER2 targets, TNBC is unsuitable for endocrine therapy and HER2-targeted therapy and is associated with a higher risk of early recurrence, distant metastasis, and poorer prognosis than many other breast cancer subtypes, particularly in advanced or metastatic disease. Although surgery, chemotherapy, radiotherapy, and immunotherapy have be

Delayed and selective bone health assessment following cancer diagnosis: a real-world gap in survivorship care.

BACKGROUND: Cancer treatment-induced bone loss (CTIBL) is well-recognized in hormone-sensitive malignancies such as breast and prostate cancer but is less consistently implemented in patients with other solid tumors and hematologic malignancies. Furthermore, the long-term skeletal effects of newer systemic therapies, including immune checkpoint inhibitors and targeted agents, remain incompletely characterized. Although bone mineral density (BMD) assessment by dual-energy X-ray absorptiometry (DXA) is recommended for patients at increased skeletal risk, real-world utilization patterns remain po
