Mesenteric Meckel's Diverticulum as a Missed Diagnosis in a Child With Recurrent Rectal Bleeding: A Report of a Challenging Case.

Meckel's diverticulum is a common congenital anomaly of the gastrointestinal tract and is a remnant of the omphalomesenteric duct. One of the classical identification and diagnostic criteria of Meckel's diverticulum is its location along the anti-mesenteric border of the ileum. Mesenteric Meckel's diverticulum (MMD) is an unexpected occurrence, rarely reported, with confounding diagnostic tests due to the atypical location, often forming a gray zone in diagnosis due to its similarity to an enteric duplication cyst. A 1.5-year-old male child presented to the pediatric surgery outpatient departm

Cholangitis, Choledocholithiasis and a Cholecystocolonic Fistula: A Case Report on a Rare Triple Surgical Challenge.

Cholecystocolonic fistula (CCF) is an uncommon complication of gallstone disease and is usually diagnosed intraoperatively. Concomitant choledocholithiasis and acute cholangitis create competing priorities: urgent biliary drainage and definitive treatment of the fistula. A 78-year-old woman presented with 3 days of upper abdominal pain, fever, vomiting and jaundice. She was febrile and hypotensive but stabilised after intravenous fluid resuscitation without vasopressors. Laboratory and imaging findings met Tokyo Guidelines 2018 criteria for severe acute cholangitis. Computed tomography demonst

Extramedullary plasmacytoma of the colon manifesting with intussusception.

The WHO categorises plasma cell tumours into two primary types: multiple myeloma and plasmacytoma. Plasmacytoma is further classified into solitary extramedullary plasmacytoma (EMP) and solitary plasmacytoma of bone. The upper respiratory tract and nasopharynx are the sites most commonly affected by EMP. Gastrointestinal tract involvement occurs in approximately 10% of EMP cases, while colonic involvement-particularly when associated with intussusception-is exceedingly rare. We present the case of a middle-aged man in his 40s who reported haematochezia, fatigue and weight loss over a 6-month p

A Rare Case of Squamous Cell Carcinoma of the Gallbladder Presenting as a Bleeding Colonic Mass.

Gallbladder carcinoma (GBC) is the most common malignancy of the biliary tract, with adenocarcinoma being the predominant histological subtype. Squamous cell carcinoma (SCC) is rare, accounting for approximately 1%-4% of gallbladder carcinomas, and is characterized by aggressive local invasion and poor prognosis. We report a 53-year-old man with a one-year history of recurrent painless lower gastrointestinal bleeding and a six-month history of progressive constipation, abdominal bloating, and unintentional weight loss. His bleeding had previously been attributed to a known grade 4 hemorrhoid.

Mass-forming ischemic colitis mimicking colon cancer: A case report of early repeat colonoscopy.

RATIONALE: Ischemic colitis is the most common form of intestinal ischemia, typically affecting elderly individuals with vascular risk factors such as hypertension, diabetes mellitus, atherosclerosis, and chronic kidney disease. In rare cases, a mass-forming variant may mimic malignancy or inflammatory bowel disease, creating a diagnostic challenge. We report a case of mass-forming ischemic colitis, initially suspected to be colon cancer, in which the correct diagnosis was established through short-term follow-up colonoscopy with biopsy, thereby preventing unnecessary surgical intervention. Poorly cohesive carcinoma diffusely involving the whole gastrointestinal tract: a case report.

BACKGROUND: Gastric cancer is the fourth leading cause of cancer death worldwide. We reported a 64-year-old male patient with paroxysmal dull pain around the periumbilical region after meals. After admission, the patient underwent contrast-enhanced computed tomography (CT) scans of the upper and lower abdomen, which revealed marked thickening of the gastric antrum and multiple enlarged lymph nodes around the gastric antrum and in the retroperitoneum. Esophagogastroduodenoscopy revealed possible tumor lesions at the cardia, gastric body, duodenal bulb, and descending part of the duodenum. Colon

Robot-Assisted Ileocecal Resection Using the Saroa Surgical System.

Saroa is a novel robotic surgical system that can estimate the gripping forces of robotic forceps and convey this information to the console surgeon via a remote-control interface. A patient with a polyp located adjacent to the Bauhin valve was diagnosed with adenocarcinoma with submucosal and vascular invasion after endoscopic resection and subsequently underwent additional surgical resection. Port placement was modified twice intraoperatively due to the limited range of motion of the robotic arms. During the first setting, the ileocolic artery and vein were resected, and the ascending colon

Minimally invasive management of colonic lipoma causing intussusception with endoloop in an adult.

Intussusception is a condition in which one segment of the intestine (intussusceptum) telescopes into the lumen of an adjacent segment (intussuscipiens), potentially leading to obstruction. Giant colonic lipomas are rare and often asymptomatic, but when large enough, can serve as a lead point for intussusception. We present the case of a 62-year-old male patient who presented with nausea and vomiting. A computed tomography (CT) abdomen/pelvis without contrast showed intussusception of the ascending colon into the transverse colon without bowel obstruction, along with a lobulated fatty mass at

Relationship between ileocolic arterial stump length and lymph node yield - a retrospective study using machine learning.

PURPOSE: In right-sided colon cancer surgery, ileocolic artery stump length may reflect the extent of mesenteric resection and lymph node harvest. This study evaluated the association between postoperative stump length measured on CT, lymph node yield, and recurrence or progression within 24 months. METHODS: This single-center retrospective study included 64 patients who underwent right or extended right hemicolectomy for right-sided colon adenocarcinoma between 2016 and 2024. Right-sided tumors were defined as those located in the cecum, ascending colon, hepatic flexure, or proximal transver

A Case of Colon Cancer-Induced Hemophagocytic Lymphohistiocytosis with Lymphangitic Carcinomatosis.

INTRODUCTION: Hemophagocytic lymphohistiocytosis (HLH) is a syndrome involving extreme hyperinflammation driven by a cytokine storm, which leads to the pathological autophagocytosis of blood cells. Although secondary HLH is frequently associated with underlying malignancies, the majority of which are hematological, cases triggered by colorectal cancer are quite rare. We report herein a case of HLH induced by ascending colon cancer. CASE PRESENTATION: An 83-year-old female patient presented to her primary care physician with a 1-month history of anorexia, persistent fever (>38°C), and cognitiv

Proximal Peril From a Distal Malignancy: Impending Colonic Perforation in Obstructing Sigmoid Adenocarcinoma.

Sigmoid colon adenocarcinoma is a common cause of large bowel obstruction in elderly patients. However, progression to impending proximal colonic perforation due to distal malignant obstruction represents a rare and catastrophic complication associated with high morbidity and mortality. A 74-year-old female presented with acute constipation of five days' duration, progressive abdominal distension, nausea, vomiting, and one episode of fever. Contrast-enhanced computed tomography demonstrated diffuse dilatation of the large bowel, multiple air-fluid levels, and an irregular circumferential wall

Right kidney contusion following endoscopic mucosal resection of hepatic flexure lymphangioma: a rare case report.

Endoscopic mucosal resection (EMR) is widely utilized for the removal of benign colonic lesions; nevertheless, it carries certain inherent risks. We present a rare case of right kidney contusion following EMR for hepatic flexure lymphangioma, representing an uncommon and underreported complication. A 50-year-old woman underwent colonoscopy, which identified a 3 cm cystic mass at the hepatic flexure, subsequently diagnosed as a lymphangioma. EMR was performed with electrocoagulation and mechanical clamping. Postoperatively, she developed progressively worsening upper abdominal pain but did not

Gangrenous cecal volvulus with 720° torsion and prolonged intraoperative hypotension in a resource-limited setting: a case report.

INTRODUCTION AND IMPORTANCE: Cecal volvulus is an uncommon cause of large-bowel obstruction, but when it occurs, it can rapidly become life-threatening, with ischemia and gangrene developing quickly. Explicitly quantified 720° torsion is rarely described. CASE PRESENTATION: A 45-year-old man presented after 5 days of abdominal pain, distension, obstipation, fever, and vomiting. His pulse rate was 124 beats per minute, his blood pressure was 100/65 mmHg, and he demonstrated clear signs of peritonism. Based on his history, examination findings, elevated white blood cell count, and abdominal X-r

Chilaiditi Syndrome: A Case Report and Review of the Literature.

The Chilaiditi sign refers to the interposition of a bowel segment, most commonly the colon, between the liver and the right hemidiaphragm. The term Chilaiditi syndrome is reserved for cases in which this radiographic finding is accompanied by clinical manifestations that can reasonably be attributed to it. Respiratory or thoracic presentations are uncommon. We report the case of an 80-year-old woman with hypertension and knee osteoarthritis who presented with a persistent dry cough, right-sided lower chest pain, and progressive fatigue, without active gastrointestinal symptoms at presentation

Synchronous right colon adenocarcinoma and sigmoid endometriosis mimicking a second primary carcinoma: a case report.

Intestinal endometriosis is a benign gynecological condition that frequently involves the rectosigmoid colon. Its synchronous occurrence with colorectal cancer is exceptionally rare and diagnostically challenging, particularly when mimicking a second primary malignancy. We report a case of a 48-year-old woman presenting with altered bowel habits. Colonoscopy revealed synchronous lesions: an adenocarcinoma at the hepatic flexure and a suspicious mass in the sigmoid colon. Repeated biopsies of the sigmoid lesion were inconclusive, ranging from dysplasia to chronic inflammation, raising concern f

Pancreas-preserving segmental duodenectomy for duodenal malignancy: two case reports.

The extent of resection required for duodenal malignancy has long been debatable. Compared with pancreaticoduodenectomy (PD), pancreas-preserving segmental duodenectomy (PPSD) is a less demolitive procedure that provides definitive treatment for a range of duodenal pathologies, including adenocarcinoma. However, the rarity of these malignancies limits large-scale clinical studies on the feasibility and safety of PPSD. Thus, case reports and small case series may provide some clinical guidance for this rare situation. We herein report two cases of patients with duodenal malignancy who were trea

The Stent That Went Too Far: Ascending Colon Perforation Following Gastric Stent Migration.

A 32-year-old male patient presented with symptoms of gastric outlet obstruction secondary to distal gastric adenocarcinoma. A fully covered self-expanding metal stent (SEMS) measuring 20 mm × 130 mm was placed to relieve obstruction and restore oral intake, with the aim of optimizing the patient for planned perioperative chemotherapy. The patient subsequently received neoadjuvant FLOT (fluorouracil, leucovorin, oxaliplatin, and docetaxel) chemotherapy as per recommendations of the multidisciplinary team and completed three cycles uneventfully. Before scheduled restaging imaging and completion

Successful Colonic Self-Expandable Metal Stent Placement Using a Rotatable Sphincterotome After Conventional Guidewire Insertion Failure: A Case Report.

Self-expandable metal stent (SEMS) placement is the standard endoscopic treatment for malignant colorectal obstruction, but its technical success depends on successful guidewire passage, which may fail when a sharply angulated or eccentrically oriented stricture prevents the operator from aligning the catheter tip coaxially with the luminal course. We report a 90-year-old woman with complete transverse colon obstruction caused by an adenocarcinoma near the hepatic flexure, in whom guidewire passage across the tumor was achieved with a rotatable sphincterotome after conventional catheter-based

Extended Lymphadenectomy for Right-Sided Colon Cancer: A Systematic Review and Narrative Synthesis of Evidence, Technique, and Patient Selection.

Right-sided colon cancer represents a substantial portion of all colorectal malignancies worldwide. Extended lymphadenectomy techniques, such as complete mesocolic excision (CME) with central vascular ligation (CVL) and D3 lymph node dissection, aim to improve oncological outcomes through enhanced nodal harvest and dissection along embryological planes. Despite two decades of investigation and six prospective studies, the survival benefit remains unproven in unselected populations, and patient selection criteria remain poorly defined. This International Prospective Register of Systematic Revie
