Ketogenic diet fails to mitigate seizures and neuroinflammatory responses in a mouse model of virus-induced epilepsy.

OBJECTIVE: The ketogenic diet (KD) is a high-fat, low-carbohydrate intervention widely used to treat drug-resistant epilepsy, thought to reduce seizures through a combination of metabolic, neuronal, and microbiota-dependent mechanisms. Additionally, recent studies suggest that the anticonvulsant effects of KD require the gut microbiota, with taxa such as Akkermansia and Parabacteroides contributing to seizure protection by modulating host neurotransmitter balance and neural excitability. Whereas KD has been shown to be effective in reducing seizure burden across different epilepsies, its antis

[Qihuang Jianpi Zishen Granule ameliorates renal injury in patients with systemic lupus erythematosus by regulating immunometabolic reprogramming of macrophages].

OBJECTIVES: To investigate the mechanism of Qihuang Jianpi Zishen Granule (QJZG) for regulating macrophage polarization in renal damage in patients with systemic lupus erythematosus (SLE). METHODS: Public databases were used to screen the targets of QJZG, SLE and renal damage. An active ingredient-target network and a protein-protein interaction network were constructed followed by molecular docking. Sixty-two SLE patients were randomized into QJZG treatment group and control group (n=31), with 30 healthy individuals as the normal control group. Macrophage polarization markers (iNOS and Arg-1

Dynamics of virological suppression and systemic immune-inflammatory response following antiretroviral therapy in pediatric HIV infection.

BACKGROUND: Persistent systemic inflammation remains a hallmark of treated HIV infection and is not reliably resolved by virological suppression alone. The Systemic Immune-Inflammation Index (SII), a composite hematologic index [(neutrophil × platelet)/lymphocyte count], has emerged as a candidate inflammatory biomarker. Longitudinal SII data following antiretroviral therapy (ART) initiation in pediatric HIV are lacking. METHODS: This single-center retrospective paired longitudinal study included 30 pediatric patients with HIV with laboratory data at baseline and 12 months post-ART. The prima

Treponema pallidum suppressive microenvironments and central nervous system damage: from mechanisms to tools and prevention.

The global resurgence of syphilis, marked by a dramatic 150% increase in cases over the last decade and the rise of the dominant SS14-Ω strain cluster, represents a critical threat to public health. This review explores the complex molecular mechanisms by which Treponema pallidum functions as a "stealth pathogen," establishing suppressive chemotactic microenvironments that facilitate neuroinvasion and persistent central nervous system (CNS) damage. We delineate a biomechanical and molecular cascade wherein the spirochete utilizes its unique "naked" outer membrane and hypervariable Treponema pa

Efficacy and safety of first-line immunotherapy-related induction and maintenance therapy for extensive-stage small cell lung cancer: a systematic review and Bayesian network meta-analysis.

BACKGROUND: Extensive-stage small cell lung cancer (ES-SCLC) is highly aggressive with dismal prognosis, and platinum monochemotherapy offers modest long-term survival. Multiple first-line regimens integrating immunotherapy, anti-angiogenic agents and chemotherapy have been approved, yet direct head-to-head comparisons are scarce, and the optimal induction plus maintenance strategy remains undefined. In this study, a Bayesian network meta-analysis (NMA) was performed to hierarchically assess the efficacy and safety of available first-line regimens and identify balanced treatment strategies for

Transcriptomics, identification, and testing of manganese metabolism-related genes in cervical squamous cell carcinoma.

BACKGROUND: Cervical squamous cell carcinoma (CSCC) remains a leading cause of cancer-related mortality in women, and few effective prognostic biomarkers have been identified. Although manganese metabolism (MAM) has been implicated in tumorigenesis and immune regulation, its prognostic relevance in CSCC has not been systematically explored, and existing prognostic models for CSCC have largely ignored MAM-related genes. METHODS: CSCC transcriptomic and clinical datasets were obtained from public databases. MAM-related differentially expressed genes (DEGs) were identified by intersecting DEGs,

Case Report: Reconciling genomic ambiguity with clinical certainty in refractory panniculitis.

Panniculitis in adolescents often poses a diagnostic challenge for physicians. We report an adolescent girl presenting with recurrent steroid-responsive lymphocytic septal panniculitis, genital ulcer, and constitutional symptoms, later complicated by macrophage activation syndrome and steatohepatitis. Extensive evaluation excluded infectious, autoimmune, and malignant causes. Histopathology revealed septal panniculitis without vasculitis or atypical lymphoid cells. Whole-exome sequencing identified a heterozygous PSMB9 variant (c.595A>G; p.Thr199Ala) and a homozygous HAVCR2 variant (c.290T>C;

An anoikis-related four-gene prognostic signature and functional characterization of ANXA5 in laryngeal squamous cell carcinoma.

BACKGROUND: Laryngeal squamous cell carcinoma (LSCC) remains one of the most lethal malignancies of the head and neck region. Tumor metastasis, the primary cause of treatment failure, is intimately linked to the evasion of anoikis. Yet the molecular drivers of this process in LSCC are incompletely understood. This study systematically investigates the prognostic and functional landscape of anoikis-related genes (ARGs) in LSCC, with an emphasis on their interplay with the tumor immune microenvironment and therapeutic vulnerability. METHODS: RNA-sequencing profiles and clinical data from the TC

Immune checkpoint inhibitor-related pancreatitis with fulminant type 1 diabetes mellitus: a case report.

BACKGROUND: Immune checkpoint inhibitors (ICIs) are widely used in tumor therapy, and the immune-related adverse events (irAEs) they induce can involve multiple organs. As an organ with both endocrine and exocrine functions, pancreatic involvement may manifest as acute pancreatitis, new-onset diabetes mellitus, or even a combination of both. Among these, ICI-induced acute pancreatitis concurrent with fulminant type 1 diabetes mellitus (FT1DM) is extremely rare but highly critical and easily missed in clinical practice. CASE PRESENTATION: A 59-year-old male patient with squamous cell carcinoma

Systematic evaluation of MAIT and iNKT abundance as checkpoint blockade biomarkers.

INTRODUCTION: MAIT and iNKT cells are unconventional T cell lineages that bridge innate and adaptive immunity and have been implicated in antitumor immune responses and immunotherapy outcomes. However, evidence to date has largely derived from individual studies of selected cancer types or specialized immune profiling cohorts. Whether MAIT and iNKT abundance provides reproducible clinical information across large, multi-cancer immune checkpoint blockade (ICB) cohorts remains insufficiently evaluated. METHODS: We applied an AI-based TCRβ inference framework to estimate MAIT and iNKT relative a

Enzyme-mediated remodeling of the extracellular matrix and glycocalyx to enhance immunotherapy in solid tumors.

The tumor-associated extracellular matrix (ECM) and glycocalyx form a dynamic barrier that provides physical shielding and immunosuppressive signaling, leading to therapeutic resistance. A growing body of evidence supports enzyme-based strategies to remodel these barriers and improve therapeutic access to tumors. This review examines enzymes that act as "path cleansers" by cleaving components of the ECM or the glycocalyx to restore the efficacy of immunotherapies. Moreover, some enzymes may exert effects beyond barrier remodeling, including modulation of signaling pathways, enhancement of immu

Clinical improvement of pemphigus-like mucocutaneous disease in a patient with CLL following initiation of zanubrutinib: a case report.

Pemphigus is an autoimmune bullous disease. PNP/PAMS represents a rare subtype of autoimmune bullous disease, and typically associated with an underlying neoplasm, particularly lymphoproliferative disorders. Current management of PNP primarily involves glucocorticoids, immunosuppressants, and rituximab. This article reports a 67-year-old male who presented with generalized erythema, blisters, and oral erosions with pain for one month. Initially diagnosed with pemphigus, he received methylprednisolone treatment, which led to slight improvement in skin lesions but minimal resolution of oral ulce

Pan-Cancer Landscape of the Novel Oxygen Sensor ADO and Its Potential Role in Hepatocellular Carcinoma.

BACKGROUND: Hypoxia is a key driver of tumor progression across cancers, yet oxygen-sensing mechanisms beyond HIFs remain underexplored. 2-Aminoethanethiol dioxygenase (ADO) has recently been identified as an oxygen sensor, but its role in malignancy is poorly defined. We conducted a pan-cancer analysis of ADO with a special focus on hepatocellular carcinoma (HCC), to assess its oncogenic significance and clinical potential. METHODS: A multi-omics pan-cancer analysis of ADO expression and survival was performed using TCGA and GTEx, with validation in HCC across ICGC, GEO, and CNHPP proteomic

Temporal changes in auditory adverse event reporting in the U.S. vaccine adverse event reporting system associated with the COVID-19 vaccine rollout: an interrupted time series analysis with negative control evaluation.

OBJECTIVES: To assess whether auditory adverse event (AE) reporting changed after the COVID-19 vaccine rollout, and whether any change reflected a true safety signal or a shift in reporting behavior. METHODS: We analyzed U.S. Vaccine Adverse Event Reporting System (VAERS) data from 2018 through 2025 using interrupted time series (ITS) analysis. The COVID-19 vaccine Emergency Use Authorization (EUA), granted in the fourth quarter of 2020, was treated as a time-level ecological intervention. Because VAERS receives reports for all vaccines, we included every report regardless of vaccine type. We

Incidence and implications of anti-rituximab antibodies in nephropathies and other immune-mediated diseases: a narrative review.

Rituximab is a chimeric anti-CD20 monoclonal antibody widely used in immune-mediated and hematologic diseases, but its efficacy may be limited by the development of anti-rituximab antibodies (ADAs). This narrative review summarizes current evidence on the incidence, detection, and clinical implications of ADAs in nephropathies and other immune-mediated disorders. ADA prevalence varies markedly across diseases and is influenced by assay methodology, timing of testing, and patient immune background. Although not all ADAs are clinically relevant, their presence has been associated with lower ritu

Case Report: Severe mixed autoimmune hemolytic anemia associated with acute cytomegalovirus infection in an immunocompetent adult.

BACKGROUND: Cytomegalovirus (CMV)-associated hemolytic anemia (HA) is rare in immunocompetent hosts and may present with heterogeneous serologic phenotypes, including direct antiglobulin test (DAT)-negative hemolysis, warm autoimmune hemolytic anemia (AIHA), cold antibody-mediated AIHA, and rarely mixed AIHA. Because mixed AIHA requires evidence of both warm autoantibodies and clinically significant cold autoantibody activity, this phenotype may be underrecognized without comprehensive immunohematologic testing. CASE PRESENTATION: We report a case of CMV-associated HA that was diagnosed as mi

TREM2-directed therapy in Alzheimer's disease: from therapeutic window to clinical translation.

Alzheimer's disease (AD) is characterized by amyloid-β (Aβ) deposition, tau pathology, synaptic dysfunction, and a sustained neuroimmune response. Among immune-related targets, triggering receptor expressed on myeloid cells 2 (TREM2) is of particular interest because it is supported by human genetics, microglial biology, and expanding therapeutic development. TREM2 regulates microglial survival, phagocytosis, lipid handling, metabolic fitness, and plaque-associated responses, yet its therapeutic significance is more complex than a simple protective receptor model suggests. Although multiple TR

In vitro diagnostics of drug hypersensitivity reactions.

In vitro diagnostics of drug hypersensitivity reactions comprises different tests that can add important information for identifying the culprit drug. Depending on the patients' clinical history and underlying immune mechanisms, these tests can be classified into immediate-type and late-type in vitro assays. While they are currently mostly not part of routine diagnostics, knowledge of when to apply which test and what distinct benefits and specific limitations each assay carries, can be helpful when considering the referral of patients with adverse drug reactions for appropriate further testin
