Factors influencing adherence to intravesical instillation therapies for non-muscle-invasive bladder cancer: a scoping review protocol.

INTRODUCTION: Non-muscle-invasive bladder cancer is often treated with repeated weekly intravesical instillations of chemotherapy or immunotherapy. The number of instillations received influences the risk of recurrence and progression; thus, treatment completion is crucial. However, previous research indicates that nearly half of patients do not adhere to treatment for various reasons. This scoping review aims to define the concept of treatment adherence to intravesical instillation therapies and identify and map the factors that influence treatment adherence in intravesical instillation thera

[Expert consensus on intravesical therapy for non-muscle invasive bladder cancer (2025 edition)].

Bladder cancer is one of the common malignant tumors. In 2022, China is estimated to have approximately 92,900 new cases of bladder cancer, including about 73,200 cases in males and 19,700 cases in females. It is projected that around 41,400 deaths will be attributed to bladder cancer, with about 32,500 deaths among males and 8,800 among females. With 75% of bladder cancers being non-muscle invasive and having a high recurrence rate after surgery. Intravesical therapy is a useful method to either directly kill tumor cells by infusing cytotoxic drugs into the bladder, or directly or indirectly

Predictors of BCG Therapy Complications in Non-Muscle-Invasive Bladder Cancer: A Clavien-Dindo-Based Study.

BACKGROUND: Intravesical Bacillus Calmette-Guérin (BCG) therapy is the standard adjuvant treatment for non-muscle-invasive bladder cancer (NMIBC), particularly in patients with carcinoma in situ, following transurethral tumor resection. Despite its proven oncological benefits, BCG therapy may be associated with a wide range of local and systemic adverse events. This study aimed to evaluate the clinical significance of BCG-related complications across the full severity spectrum and to assess their impact on treatment continuation using the Clavien-Dindo classification system. METHODS: This ret

Cost-Effectiveness of Immune Checkpoint Inhibitor Therapy Plus Bacillus Calmette-Guérin for High-Risk Nonmuscle-Invasive Bladder Cancer: Analyses of CREST, POTOMAC, and ALBAN.

PURPOSE: Three recent trials evaluated immune checkpoint inhibitors (ICIs) + bacillus Calmette-Guérin (BCG) for treatment of high-risk nonmuscle-invasive bladder cancer (NMIBC). Two of these trials (CREST, POTOMAC) demonstrated that ICI + BCG improved event-free survival compared with BCG alone but resulted in higher rates of treatment-related adverse events. In this article, we evaluated the cost-effectiveness of ICI + BCG compared with BCG alone. We then created a publicly available cost-effectiveness calculator to facilitate future NMIBC drug value comparisons. MATERIALS AND METHODS: We us

Urine Tumor DNA Testing Identifies Recurrence and Monitors Therapy Response in Patients With High-Risk Nonmuscle-Invasive Bladder Cancer Receiving Intravesical Bacillus Calmette-Guérin.

PURPOSE: Intravesical bacillus Calmette-Guérin (BCG) is the preferred treatment for high-risk nonmuscle-invasive bladder cancer. However, many patients recur within the first year despite optimal therapy. Urine tumor DNA (utDNA) offers promise in stratifying therapy response to identify patients who may benefit from treatment intensification or de-escalation. Our study aimed to validate utDNA testing in a multicenter cohort of patients receiving BCG. MATERIALS AND METHODS: Pretreatment/posttreatment urine from patients undergoing BCG for nonmuscle-invasive bladder cancer was analyzed by utDNA

Individual Patient Data Validation of the EAU Definitions of BCG Failure in Patients with Non-muscle-invasive Urothelial Carcinoma of the Bladder: An International Multicenter Retrospective Study.

BACKGROUND: In non-muscle-invasive bladder cancer (NMIBC), the occurrence of a high-grade (HG) recurrence following Bacillus Calmette-Guérin (BCG) treatment identifies a disease category that is associated with a particularly poor prognosis and for which radical cystectomy (RC) is the gold standard. The latest definition of European Association of Urology (EAU) BCG-unresponsive disease has been endorsed by the United States Food and Drug Administration for clinical trials and adopted by EAU guidelines, despite limited clinical data supporting it. OBJECTIVE: To provide a clinical validation of

Pain Response and Associated Factors During Bacillus Calmette-Guérin Vaccination in Newborns: A Cross-Sectional Study.

BACKGROUND: Bacillus Calmette-Guérin (BCG) vaccination is routinely administered shortly after birth in many countries, including Vietnam. Although brief, this procedure may induce substantial pain during a critical period of neonatal neurodevelopment. Evidence on pain responses during BCG vaccination and modifiable procedural factors remains limited. This study aimed to assess neonatal pain during BCG vaccination and identify factors associated with severe pain. METHODS: We conducted a cross-sectional observational study involving 172 newborns aged less than 48 hours who received intradermal

Urinary pH and oncologic outcomes after bacillus calmette-guérin in non-muscle invasive bladder cancer: a retrospective cohort study.

INTRODUCTION: Urinary pH may influence intravesical Bacillus Calmette-Guérin (BCG) efficacy by modulating the local immune microenvironment. We evaluated baseline urinary pH as a continuous variable and its association with oncologic outcomes in BCG-treated non-muscle invasive bladder cancer (NMIBC). METHODS: In this retrospective cohort, 248 NMIBC patients receiving adequate BCG therapy were analyzed. Recurrence-free survival (RFS) and progression-free survival (PFS) were assessed using Cox proportional hazards models. Given limited progression events, ridge (L2-penalized) multivariable regr

Nano-armed bcg delivers Cu-Fe MOF to trigger dual cell death in bladder cancer.

Intravesical Bacillus Calmette-Guérin (BCG) perfusion is widely regarded as a standard clinical intervention for non-muscle-invasive bladder cancer. Despite its established efficacy, this treatment is associated with a high tumor recurrence rate of approximately 50%, highlighting a significant clinical challenge. To address this limitation, combination strategies integrating BCG with other antitumor modalities have emerged as a promising direction for enhancing therapeutic outcomes. In this study, we engineered a novel "super-BCG" formulation by functionalizing BCG with a copper-iron metal-org

Decoding Immune Mechanisms in BCG-unresponsive Non-muscle Invasive Bladder Cancer.

Non-muscle-invasive bladder cancer (NMIBC) accounts for roughly 75% of all bladder cancer cases. For patients with intermediate- and high-risk disease, intravesical Bacillus Calmette-Guérin (BCG) remains the standard treatment, yet it fails in up to 40% of cases. While radical cystectomy is the most effective salvage option, it carries significant morbidity and long-term quality-of-life consequences, highlighting the urgent need for bladder-sparing alternatives. Advancing such therapies requires a deep understanding of the immunologic mechanisms within the tumor microenvironment (TME). This re

A machine-learning approach to predict additional treatment after Bacillus Calmette-Guérin induction in non-muscle-invasive bladder cancer.

Non-muscle invasive bladder cancer (NMIBC) comprises ~ 75% of newly diagnosed bladder cancer, with high-risk NMIBC associated with high rates of recurrence and progression. Nearly 40% of patients experience a lack of efficacy with gold standard Bacillus Calmette-Guérin (BCG) therapy and current methods for predicting BCG response are limited. This multicentre real-world study developed and evaluated machine learning (ML) models using data (April 2008-March 2024) from the Japan Medical Data Vision database to predict whether patients with NMIBC who received BCG induction would require additiona

From Patient Selection to Surveillance: Artificial Intelligence Applications in Radioiodine Therapy - A Systematic Review.

UNLABELLED: Dietary phytochemicals exhibit anticancer activity, and increasing evidence suggests that the combinations of co-occurring compounds may provide greater therapeutic efficacy than individual compounds. Eruca sativa Mill. (rocket salad) contains both erucin and kaempferol; however, their synergistic anticancer activity against non-small cell lung cancer (NSCLC) has not been systematically investigated. Network pharmacology, molecular docking, and molecular dynamics (MD) simulations were performed to identify molecular targets of erucin and kaempferol. Their synergistic anticancer act

Predictive value of toll-like receptors, myeloid differentiation factor 88, and nuclear factor kappa-B regarding the response to Bacillus Calmette-Guérin intravesical therapy in patients with high-risk non-muscle-invasive bladder cancer: A controlled cohort clinical study.

BACKGROUND: Intravesical Bacillus Calmette-Guérin (BCG) is the standard adjuvant therapy for high-risk non-muscle-invasive bladder cancer (HR-NMIBC). Toll-like receptors (TLRs) modulate the BCG immune response. Myeloid differentiation factor 88 (MyD88) is a canonical adapter for TLR signaling pathways, which ultimately activates the nuclear factor kappa-B (NF-κB) transcription factor, triggering cytokine expression and immune activation. Validated biomarkers to detect recurrences or progression of T1G3 bladder cancer (BC) are lacking. PURPOSE: Our study aimed to analyze the role of TLRs, MyD8
