BCG Induces PD-1 Upregulation on Circulating CD8+ T Cells and IL-6 and IL-8 Secretion In Vitro.

Backgrounds: Bacillus Calmette-Guérin (BCG) remains the most effective adjuvant therapy for non-muscle-invasive bladder cancer (NMIBC); however, the immunological underpinnings of its efficacy remain incompletely understood. This study aims to elucidate the dual immunomodulatory roles of BCG by integrating systemic and tumor-intrinsic analyses, through determining the systemic effects of BCG instillation on immune checkpoint expression and direct inflammatory response in a previously established in vitro tumor model. Methods: We investigated systemic and tumor-intrinsic immune responses to BCG

Emerging Intravesical Therapies for Non-Muscle-Invasive Bladder Cancer.

Non-Muscle-Invasive Bladder Cancer poses significant therapeutic challenges due to high recurrence and progression rates. While intravesical Bacillus Calmette-Guérin (BCG) has long been the standard, limitations in efficacy, global shortages, and patient-specific contraindications to radical cystectomy have fueled the development of novel bladder-sparing therapies. Recent advances include UGN-102 for low-grade/intermediate-risk disease and multiple FDA-approved agents-nadofaragene firadenovec, TAR-200, and nogapendekin alfa inbakicept-for BCG-unresponsive disease. Ongoing trials of TAR-210, cr

Key Clinical Trials Shaping the Future of Superficial High-Risk Bladder Cancer Management.

High-risk non-muscle-invasive bladder cancer (HR-NMIBC) represents a biologically aggressive disease state with substantial risk of recurrence, progression, and need for radical cystectomy. While intravesical Bacillus Calmette-Guérin (BCG) remains first-line for HR-NMIBC is standard of care, both recurrence and progression have led to the development of novel intravesical agents therapies. This article summarizes key clinical trials that are reshaping the management of HR-NMIBC, with particular emphasis on BCG-unresponsive disease. Approved intravesical and systemic agents demonstrate meaningf

Longitudinal circulating tumour DNA identifies patients at high risk of upstaging and recurrence in non-muscle-invasive bladder cancer.

OBJECTIVES: To evaluate whether longitudinal tumour-informed circulating tumour DNA (ctDNA) testing identifies patients at increased risk of upstaging and recurrence in high-risk non-muscle-invasive bladder cancer (NMIBC). PATIENTS AND METHODS: A cohort of 52 patients with high-risk NMIBC underwent serial plasma ctDNA testing using a personalised tumour-informed assay. The ctDNA was obtained at initial presentation and at approximately 3-month surveillance intervals during appropriate routine clinical NMIBC therapy and follow-up. A subset of tumours underwent tumour-normal whole-exome sequenc

ACC1 inhibition enhances BCG-induced trained immunity by reprogramming acetyl-CoA metabolism.

Trained immunity enhances long-term innate immune responsiveness through metabolic and epigenetic rewiring. Using Bacille Calmette-Guérin (BCG) as a model, we demonstrate that inhibition of acetyl-CoA carboxylase 1 (ACC1) enhances BCG-induced trained immunity by increasing intracellular acetyl-CoA (ACoA) availability. This shift redirects ACoA from lipid biosynthesis toward enhanced tricarboxylic acid cycle flux and histone acetylation, reinforcing metabolic and epigenetic programs that sustain trained immunity. ACC1 inhibitors amplify cytokine production, mitochondrial respiration, and glutam

BCG-trained macrophages promote pan-anti-tumor activity through epigenetic rewiring of NOX2-ROS axis.

BACKGROUND: Bacillus Calmette-Guerin (BCG)-induced trained immunity in the macrophages is characterized by exaggerated inflammatory cytokine production with favourable effects on disease controls. However, how BCG-trained macrophages exert anti-tumor effects and underlying mechanisms remain to be clarified. METHODS: Pan-anti-tumor activity induced by BCG training was evaluated in mouse models with grafted tumors. The proportion and function of tumor infiltrating macrophages (TAMs) and CD8+T cells, as well as the level of reactive oxygen species (ROS), were detected by flow cytometry. The secre

Sarcopenia and recurrence risk in high-risk non-muscle-invasive bladder cancer: a nonlinear modeling approach.

BACKGROUND: Sarcopenia and altered body composition are established prognostic markers in several malignancies, but their relevance in non-muscle-invasive bladder cancer (NMIBC) remains unclear. This study evaluated associations between body composition indices-body mass index (BMI), skeletal muscle index (SMI), and psoas muscle index (PMI)-and recurrence risk in high-risk NMIBC patients treated with intravesical Bacillus Calmette-Guérin (BCG). METHODS: Body composition was assessed on computed tomography at primary diagnosis. Sarcopenia was defined using published sex-specific cut-offs. Recu

PCK2-Mediated PQBP1 Lactylation Promotes Asthmatic Inflammation through PRMT5 Inhibition.

Based on their immunodominant nature and ability to induce appropriate immune responses in the host, several antigens of Mycobacterium tuberculosis have shown promise of protection. However, most of the candidate vaccines developed by employing various strategies have afforded protection that is at best comparable with bacillus Calmette-Guérin (BCG) in animal models. Due to the inherent ability of BCG to prime cellular responses in the host, it has become an attractive vehicle for development of a vaccine against intracellular infections. In this study, we have cloned the genes of three immuno

Sarcopenia Weakens the Benefit of Bacillus Calmette-Guérin in Non-Muscle-Invasive Bladder Cancer: A Computed Tomography-Based Cohort Study.

OBJECTIVES: Non-muscle-invasive bladder cancer (NMIBC) commonly recurs despite intravesical Bacillus Calmette-Guérin (BCG) therapy. Host factors that modify the effectiveness of BCG have not been completely defined. We evaluated whether sarcopenia is associated with outcomes after BCG in Japanese patients. METHODS: We conducted a retrospective single-center cohort study (2010-2023) including high-grade (HG) NMIBC patients treated with BCG (n = 139) and those not receiving BCG (n = 100). Sarcopenia was defined a priori using sex- and BMI-specific skeletal muscle index (SMI) and psoas muscle in

Evaluating the non-specific effects of BCG vaccination on the immune system and serological response to influenza vaccination in the elderly: A randomised controlled trial.

OBJECTIVES: The Bacillus Calmette-Guerin (BCG) vaccine has beneficial effects on the immune system, which may lead to non-specific protection against non-tuberculous infections and increase the response to subsequent vaccinations. Seasonal influenza vaccination is used to protect senior citizens against influenza, but the serological response to the inactivated influenza vaccine (IIV) decreases in the elderly due to immunosenescence. The aim of this study was to test the capacity of BCG to boost the specific immune response to IIV and explore the effect of BCG on the innate immune system and h

Hypoxia restricts the generation of BCG-responsive polycytotoxic T cells.

The Bacillus Calmette-Guérin (BCG) vaccine protects infants from severe tuberculosis by eliciting cellular immune responses, particularly through the activation of CD8+ cytotoxic T lymphocytes. Given that hypoxia is a defining feature of the pulmonary microenvironment during Mycobacterium tuberculosis infection, especially within granulomatous lesions, we investigated how limited oxygen availability influences the expansion and effector functions of BCG-induced cytotoxic T cells. Mycobacteria-specific short-term T-cell lines were generated and restimulated with BCG-infected macrophages under n

Peptide nanofibers and PAMP agonist combinations enhance pulmonary CD4+T cell functionality of immune recall to vaccination.

Vaccine adjuvants typically contain pathogen-associated molecular patterns (PAMPs) or danger-associated molecular patterns (DAMPs) that activate innate immune cells. The FDA's approval of multiple PAMP-DAMP combinations for clinical use has generated increased momentum in this field in recent years. Here, we report the use of DAMP-inducing peptide nanofibers (PNFs) and CL429 (PAMP) combinations as subunit boosters for Bacille Calmette-Guérin (BCG). We demonstrate that pulmonary boosting with PNFs and CL429 enhances the lung-resident memory phenotype, effector cytokine profiles, and transcripti

Immunogenicity and Protective Effects of an Ag85B Tuberculosis Subunit Vaccine Formulated with Synthetic TLR4 Agonists in BCG-Boosted Mice.

BACKGROUND/OBJECTIVES: Tuberculosis (TB) remains a major global health challenge, and the Bacillus Calmette-Guérin (BCG) vaccine has limited efficacy against adult pulmonary disease. Protein subunit vaccines are a promising alternative but require strong adjuvants to induce cell-mediated immunity. Synthetic agonists targeting toll-like receptor 4 (TLR4) and stimulators of interferon genes (STINGs) have emerged as effective immunostimulants. Therefore, we aimed to evaluate the immunogenicity and protective efficacy of Ag85B-based subunit vaccines formulated with synthetic TLR4 and STING agonist

Functional analysis of the rv1371/2/3 operon in mycobacteria.

UNLABELLED: Mycobacterium tuberculosis is known for its lipid-rich cell wall. Additionally, lipids are also involved in various mycobacterial pathways, such as signaling and energy storage. In this study, using BCG as a model, we aimed to characterize the rv1371/2/3 operon, which is predicted to be involved in lipid production. Western blotting reveals that single nucleotide polymorphisms in BCG Moreau's rv1371 and rv1373 do not hinder the expression but alter the primary structure of BCG's Rv1371. Mass spectrometry analysis with the extracted lipids shows that the complemented strains' (with
