5-Year effectiveness of an oral cholera vaccine in a cholera outbreak in rural Haiti: a case-control study.

BACKGROUND: The World Health Organization recommends use of killed oral cholera vaccines (kOCV) as part of cholera prevention and control, but data on the duration of effectiveness of the most widely available vaccine, Euvichol® is limited. We evaluated the effectiveness of Euvichol® 5 years after a kOCV campaign conducted in Haiti in 2017. METHODS: We undertook a frequency-matched case-control study in Mirebalais, Haiti between October 2022 and February 2023. Cases were patients with acute watery diarrhea, had a stool sample positive for V. cholerae by polymerase chain reaction, and had been

Lessons learned from the deployment of oral cholera vaccine by maintaining the controlled temperature chain (CTC) in an urban setting, Bangladesh.

Cholera, caused by Vibrio cholerae, remains a significant public health issue, especially in regions prone to outbreaks. While the oral cholera vaccine (OCV) is an effective preventive tool, its use is often limited by the traditional cold chain requirements. The controlled temperature chain (CTC) provides an alternative by allowing vaccines to be stored at ambient temperatures for limited periods. This study explored participants' hands-on experiences with CTC compared with standard cold chain (SCC). This qualitative study during the March 2024 OCV campaign in Dhaka, Bangladesh, used purposiv

Development of DuoChol, a Thermostable Inactivated Whole-Cell/B-Subunit Oral Cholera Vaccine in Enteric Capsule.

Background/Objectives: Cholera remains an important global health problem. Inactivated oral cholera vaccines (OCVs) are essential in the WHO/GTFCC (World Health Organization/Global Task Force on Cholera Control) strategy to end cholera by 2030; however, global supply is insufficient, they require partial cold-chain storage, and their formulation and antigen contents leave room for improvement. We describe here the development and preclinical evaluation of DuoChol OCV, a next-generation thermostable oral vaccine designed to address these gaps. Methods: DuoChol is a lyophilized dry-powder formul

The forgotten foe returns: cholera's warning to a complacent world demanding urgent action.

Cholera, a deadly bacterial infection, affects 1.3-4 million people annually. Reports in early 2025 show a sharp rise in cases, especially in the Eastern Mediterranean and African regions, posing a serious global threat. This perspective argues that this reappearance is not just a periodic outbreak, but rather the result of interconnected global crises: climate change, armed conflicts, and a fragile healthcare system. This article analyzes these factors driving the spread of cholera and critically evaluates the roles of the World Health Organization (WHO) and other international bodies. While

Pre-vaccination biomarkers of enteric dysfunction, inflammation and nutritional status associate with seroconversion to oral cholera vaccine and are perturbed during natural infection.

BACKGROUND: Oral cholera vaccines (OCVs) show variable immunogenicity in endemic settings. Host factors including enteric dysfunction, systemic inflammation, and nutritional status may influence responses, yet their role in shaping OCV immune responses is not fully understood. This study examined associations between pre-vaccination biomarkers spanning these domains and vibriocidal seroconversion to an OCV, while characterizing how the same biomarker profiles are altered during acute natural V. cholerae infection. METHODS: We conducted a retrospective analysis of stored samples from two Zambi

Time to invest in cholera.

The recent surge in cholera cases globally calls for urgent evaluation of current approaches to prevention and control of the disease. Malawi was one of the worst affected countries in 2022-2023 with the highest number of deaths due to cholera in the world. In this personal view, we look at Malawi as a case example to illustrate how current approaches lack sufficient investment. We review the history of cholera in Malawi and compare previous outbreaks to the 2022/23 outbreak. We discuss contributing factors to the outbreak including a lack of investment in water, sanitation and hygiene (both h

Evaluation of oral cholera vaccine (Euvichol-Plus) effectiveness against Vibrio cholerae in Bangladesh: an interim analysis.

INTRODUCTION: Millions of Euvichol-Plus doses have been deployed from the global oral cholera vaccine stockpile in over 20 cholera-affected countries. However, information on Euvichol-Plus's effectiveness is limited. Using this vaccine in a cholera epidemic in Dhaka, Bangladesh, provided the opportunity to evaluate the vaccine effectiveness (VE) using a test-negative design. METHODS: A two-dose regimen of Euvichol-Plus was administered to individuals aged >1 year in a population of ca. 900 000 in two campaign rounds between June and August 2022, with prospective registration of all persons wh

Epidemiologic and Genomic Surveillance of Vibrio cholerae and Effectiveness of Single-Dose Oral Cholera Vaccine, Democratic Republic of the Congo.

We conducted 4 years of epidemiologic and genomic surveillance of single-dose effectiveness of a killed whole-cell oral cholera vaccine (kOCV) and Vibrio cholerae transmission in the Democratic Republic of the Congo. We enrolled 1,154 patients with diarrhea; 342 of those had culture-confirmed cholera. We performed whole-genome sequencing on clinical and water V. cholerae isolates from 200 patient households, which showed annual bimodal peaks of V. cholerae clade AFR10e infections. A large clonal cholera outbreak occurred 14 months after a kOCV campaign of >1 million doses, likely because of lo
