Robot assisted end-to-end hand-sewn anastomosis after segmental colectomy for transverse and splenic flexure tumors: A preliminary cohort study.

Tumors of the transverse colon and splenic flexure represent a technically demanding subset of colorectal cancer, characterized by complex anatomy. Robotic approach, with complete mesocolic excision (CME) and the use of indocyanine green (ICG) fluorescence may confer advantages for these complex resections. Furthermore, end to end hand-sewn anastomosis can potentially overcome troubles of mechanical side to side anastomosis. This retrospective single-center observational study reports 19 consecutive patients undergoing robotic segmental resection: 10 for transverse colon cancer and 9 for splen

Contained transverse colonic perforation caused by an ingested fish bone presenting as chronic abdominal pain: a case report.

INTRODUCTION AND IMPORTANCE: Foreign body ingestion is common; however, gastrointestinal perforation is rare and often poses a diagnostic challenge, particularly when symptoms are chronic and nonspecific. Colonic perforation due to ingested foreign bodies is uncommon, with the transverse colon being an especially rare site due to its mobility and wide lumen. CASE PRESENTATION: We report the case of a 19-year-old male who presented with central abdominal pain and intermittent diarrhea for 1 month, followed by acute worsening of abdominal pain, fever, nausea, and vomiting over the proceeding 3

Incidental giant subserosal lipoma of the transverse colon with fat necrosis and abscess formation discovered during emergency laparotomy for blunt abdominal trauma: a case report.

Giant subserosal colonic lipomas are rare, benign tumors that are typically asymptomatic. However, they can mimic malignancy or become complicated by inflammation and necrosis. We present a 59-year-old male who underwent emergency laparotomy for a traumatic splenic rupture following blunt abdominal trauma. During surgery, a 12 × 10 cm incidental mass in the transverse colon was discovered, mimicking a malignant tumor. Due to gross purulent intra-abdominal contamination from an intratumoral abscess and patient instability, a segmental colonic resection with an end colostomy was performed alongs

Artificial intelligence-based phase recognition for workflow analysis in robotic choledochal cyst excision: a proof-of-concept study.

BACKGROUND: Automated surgical phase recognition may convert routinely recorded robotic surgery videos into objective workflow metrics, but its clinical interpretability in complex robotic pediatric and hepatobiliary procedures remains unclear. METHODS: In this retrospective proof-of-concept study, we developed and internally evaluated a video-based phase-recognition model for robotic choledochal cyst surgery using nine predefined operative phases. The model was trained on 20 robotic cases and evaluated on an independent internal dataset of 10 cases. AI-derived phase durations were applied to

Association Between Polyp Size and Malignant Invasion in Colorectal Polyps <20 mm: A Single-Trust Observational Study.

Introduction Colorectal polyps are precursors to colorectal cancer. Accurate identification and follow-up of polyps during colonoscopy depend on reliable localization techniques, including tattooing. However, the optimal minimum polyp size threshold for routine tattooing remains poorly defined in current clinical guidelines. This study evaluated the association between polyp size and malignant invasion among colorectal polyps measuring smaller than 20 mm, with consideration of its implications for lesion localization and tattooing. Methods A retrospective observational study was conducted at M

Iatrogenic Large Bowel Perforation Following Image-Guided Pericardiocentesis: A Case Report.

BACKGROUND Image-guided pericardiocentesis is a diagnostic and potentially life-saving intervention, yet even under guidance, rare complications can occur. Among them, bowel perforation with subsequent fecal pericarditis is an exceptionally uncommon and often under-recognized occurrence. CASE REPORT We describe the case of a 72-year-old man with multiple comorbidities, including acute colonic pseudo-obstruction (Ogilvie syndrome), Parkinson disease, and chronic kidney disease, who underwent urgent fluoroscopy-guided pericardiocentesis for a large pericardial effusion. After the procedure, his

Probable everolimus-associated severe colitis with hematochezia in a patient with metastatic hormone receptor-positive, HER2-negative breast cancer: a case report.

BACKGROUND: Everolimus, an inhibitor of the mechanistic target of rapamycin (mTOR), is used with endocrine therapy for endocrine-resistant hormone receptor-positive, human epidermal growth factor receptor 2 (HER2)-negative advanced breast cancer. Although diarrhea is a recognized adverse effect, severe inflammatory colitis accompanied by gastrointestinal bleeding is rare and remains insufficiently characterized. CASE PRESENTATION: A 60-year-old woman with metastatic estrogen receptor-positive, progesterone receptor-positive, HER2-negative breast cancer involving the bone and liver presented w

Pneumatosis coli, a rare cause of chronic abdominal pain.

Pneumatosis coli is a rare entity, characterised by the formation of intramural air cysts in the colonic wall. Diagnosis can be made by CT and colonoscopy, which may suggest this aetiology. We present the case of a 51-year-old patient who consulted for abdominal pain and diarrhoea and underwent colonoscopy with findings of multiple lesions between 5-10 mm with polypoid morphology and submucosal appearance in the transverse colon and splenic angle. Given the suspicion of pneumatosis, these lesions were punctured with a 3-mm injection needle and then aspirated with a syringe, extracting gas and

Very rare isolated colonic inflammatory cap polyps in a patient with known long-standing ulcerative colitis.

Inflammatory cap polyps (ICP) are an extremely rare finding during digestive endoscopy, typically presenting as multiple polyps in the form of cap polyposis. Among the few reported cases of ICP, some have been associated with multiple polyposis forms, showing a clinical presentation similar to inflammatory bowel disease (IBD), which was subsequently ruled out following further diagnostic evaluation. In this report, we present an exceptionally rare case of two isolated ICPs (not in the form of cap polyposis) in a patient with a long-standing, well-established histological diagnosis of IBD locat

Gastro-Entero-Colic Fistula After Successful EUS Guided Gastroenterostomy: Initial Lessons Learned From an International Cohort.

BACKGROUND: EUS-guided gastroenterostomy (EUS-GE) is an effective option for the management of gastric outlet obstruction (GOO). Due to the poor prognosis of patients undergoing it, data on its long-term adverse events (AE) is scarce. Herein, we describe a cohort of patients with gastro-entero-colic fistula (GECF) as a late AE of previously successful EUS-GE. METHODS: Patients who developed a GECF after EUS-GE from 10 high-volume centers were analyzed. Information collected included demographic characteristics, indications, clinical and technical success rates, stent type and size, fistula ch

Neoantigen dendritic cell vaccination combined with conventional chemotherapy for a patient with advanced transverse colon cancer.

Over the past decade, neoantigen dendritic cell (DC) vaccination has emerged as a promising personalized cancer immunotherapy based on genomic analysis. To further assess the clinical utility of this approach, additional data on the efficacy of neoantigen DC vaccination are needed. A 65-year-old male patient underwent curative surgical resection for pStage IIIB transverse colon cancer, followed by adjuvant DC vaccination combined with conventional chemotherapy for 6 months postoperatively. The neoantigen DC vaccination effectively reduced elevated carcinoembryonic antigen levels. Moreover, cyt

Gut proteome and microbiome alterations: Analysis of transverse colon samples from pathologically confirmed Alzheimer's disease patients.

INTRODUCTION: Alzheimer's disease (AD) has been regarded as a brain-first disorder. Emerging evidence suggests that the gut may influence central nervous system pathology, but the mechanisms remain unclear. METHODS: We conducted a proteomic and microbial analysis of transverse colon samples from clinically and pathologically confirmed AD and control cases. RESULTS: In the AD gut samples, antimicrobial humoral response and oxidative stress response were downregulated, while catabolic processes and insulin signaling were upregulated. Several complement (e.g., C5) and synaptic (e.g., synaptophy

Diffuse large B-cell lymphoma with intussusception in adults: a case report and literature review.

Intussusception secondary to gastrointestinal diffuse large B-cell lymphoma (DLBCL) is exceedingly rare in adults. We present a case of a 23-year-old male admitted with abdominal pain as the primary symptom. Computed tomography (CT) revealed an ileocolic intussusception involving the ascending and transverse colon, demonstrating a characteristic "target." Conservative treatment (including air enema) was ineffective, and the patient finally received surgical treatment. The resected lesion was pathologically confirmed as DLBCL. The patient received postoperative chemotherapy with the R-CHOP regi

Submucosal colonic fecalith mimicking a submucosal malignant tumor at the anastomotic site after colon cancer: A case report.

Submucosal fecaliths occurring at the anastomotic site after intestinal surgery are rare. In addition, endoscopic histological diagnosis of submucosal lesions is challenging. The present study describes a rare case of a submucosal lesion suspected of being a submucosal tumor at the anastomotic site following colectomy, resulting in a postoperative diagnosis of submucosal fecalith. A 70-year-old man had a history of abdominal surgery, including colectomy for colon cancer and small bowel resection for gastrointestinal stromal tumor (GIST). During follow-up, a calcified submucosal lesion was dete

Transcriptomic integration nominates FOXN2 as a candidate schizophrenia risk gene.

BACKGROUND: Schizophrenia (SCZ) is increasingly recognized as a complex, systemic disorder involving interactions between the central nervous system and peripheral biological processes. Growing evidence implicates dysregulation of the gut-brain axis in SCZ pathophysiology; however, the genetic mechanisms linking schizophrenia susceptibility to gut-related biological pathways remain poorly understood, particularly with respect to causal gene prioritization across tissues. METHODS: To address this gap, we conducted a cross-tissue transcriptome-wide association study (TWAS) using FUSION across f
