Splenic Flexure Colon Cancer Presenting as a Perisplenic Abscess With Subsequent Intrasplenic Abscess Formation and Portal Venous Gas: A Case Report.

Splenic abscess associated with colon cancer is rare, and progression from a perisplenic abscess to an intrasplenic abscess is particularly unusual. We report the case of a 70-year-old man who presented in February 2023 with abdominal pain and fever. Contrast-enhanced computed tomography showed a perisplenic abscess and wall thickening at the splenic flexure. After percutaneous drainage and antibiotic therapy, an intrasplenic abscess and portal venous gas developed. Following infection control, colonoscopy and biopsy confirmed splenic flexure colon adenocarcinoma, and elective open left hemico

Quantitative analysis of colon-lengthening techniques in laparoscopic low rectal cancer surgery with left colic artery preservation: a fresh cadaveric model study.

PURPOSE: To quantify colonic lengthening achieved by complete splenic flexure mobilization (SFM) and high inferior mesenteric vein (IMV) ligation while preserving the left colic artery (LCA) in laparoscopic low rectal cancer surgery. METHODS: Standardized LCA-preserving anterior resection was performed on 18 fresh cadavers. The additional colonic length gained after each maneuver was measured under tension-free conditions. High inferior mesenteric artery (IMA) ligation, which sacrifices the LCA, was also measured as a reference comparator. RESULTS: The mean baseline resection length was 10.4

Association of proximal adenomas with high-grade dysplasia with increased cumulative incidence of postpolypectomy colon cancer: A Surveillance, Epidemiology, and End Results-Medicare analysis.

BACKGROUND: After polypectomy, patients with adenomas containing high-grade dysplasia (HGD) are recommended to undergo surveillance at 3 years because of an increased risk of metachronous neoplasia. Although proximal colon cancers are associated with a worse prognosis compared with distal tumors, limited data exist regarding how the laterality of an initial HGD adenoma influences the subsequent risk of colon cancer. This study aimed to analyze the role of the laterality of HGD adenoma on the incidence of metachronous colon cancer. METHODS: The Surveillance, Epidemiology, and End Results-Medic

Surgical and clinical predictors of successful Hartmann reversal procedure after perforated diverticulitis.

BACKGROUND: The feasibility of Hartmann reversal after an Hartmann procedure for perforated diverticulitis has long been debated. This retrospective multicentered study assessed the outcomes of Hartmann reversal and the risk factors of its failure. METHODS: Patients who underwent Hartmann reversal for perforated diverticulitis in the last 5 years with at least 12 months of postoperative follow-up within the Diverticular Disease Registry (ClinicalTrials.gov NCT04907383) were recruited. A successful Hartmann reversal was defined as a minimally invasive surgery without conversion, without protec

Colonic Continuity After Splenic Flexure Resection: Does the Orientation of the Anastomosis Matter? A Retrospective Cohort Study.

AIM: Splenic flexure resection (SFR) is a technically demanding procedure due to the region's complex vascular anatomy and limited colonic mobility. The orientation of the colonic anastomosis-either isoperistaltic or anisoperistaltic-represents a critical yet understudied technical factor that may influence operative complexity and postoperative recovery. This study aimed to compare short-term outcomes between these two anastomotic configurations. METHODS: We conducted a retrospective cohort study at a tertiary colorectal center, including consecutive patients who underwent elective SFR for c

Endoscopic Visualization of a Jejuno-Colonic Fistula in a Case of Monomorphic Epitheliotropic Intestinal T-Cell Lymphoma.

Monomorphic epitheliotropic intestinal T-cell lymphoma (MEITL) is a rare primary intestinal T-cell lymphoma newly defined in the 2016 WHO classification, characterized by rapid progression and a poor prognosis. Fistula formation caused by intestinal lymphoma is extremely uncommon, and no previous reports have described MEITL presenting with a jejunocolic fistula. An 83-year-old woman presented with a 20-kg weight loss and generalized fatigue over the preceding year. Laboratory tests revealed hypoalbuminemia and elevated inflammatory markers. Computed tomography demonstrated wall thickening of

Management of two extremely aggressive signet ring cell colon carcinoma: a case series.

Signet ring cell carcinoma (SRCC) of the colon is a rare histologic subtype of colorectal cancer, accounting for <1% of cases, and is associated with aggressive clinical behavior and poor prognosis. We report two cases with advanced colonic SRCC. The first case involved a 58-year-old female with obstructing rectal SRCC and extensive pelvic invasion with adnexal metastasis. The second is a 45-year-old male with an obstructing splenic flexure SRCC and peritoneal dissemination, found to have microsatellite instability and multiple oncogenic alterations. Both cases required aggressive surgical int

Splenic flexure colon cancer may represent a distinct prognostic subtype associated with elevated systemic inflammation and impaired nutritional status.

BACKGROUND: Splenic flexure colon cancer (SFCC) is a rare gastrointestinal malignancy distinguished by its unique anatomical location and dual vascular supply. Nevertheless, its clinical behavior, prognostic outcomes, and underlying risk factors remain inadequately defined. METHODS: A two-cohort study design enrolled 478 patients with stage II-III colon cancer. A colon cancer cohort (2012-2017) compared survival outcomes across tumor locations, while a separate SFCC-specific cohort (2012-2022) evaluated prognostic biomarkers in SFCC patients. A composite nutrition-inflammation integrating ind

Short- and Long-Term Outcomes of Laparoscopic Segmental Left Colectomy for Splenic Flexure Colon Cancer: A Multicenter Propensity Score-Matched Analysis from the Catholic Colorectal Group.

Background: Splenic flexure colon cancer (SFCC) is a relatively rare subtype of colon cancer, and the optimal extent of resection and safety of laparoscopic surgery remain controversial. We conducted a multicenter comparative analysis to evaluate the short- and long-term outcomes of laparoscopic segmental left colectomy (LC) for SFCC using laparoscopic anterior resection (AR) as a comparator. Methods: We retrospectively reviewed patients with stage I-III colon cancer involving the distal transverse to sigmoid colon who underwent laparoscopic surgery at four hospitals between March 2004 and Dec

Laparoscopic Sigmoid Colectomy in a Patient With Congenital Left Renal Agenesis: A Case Report.

Congenital renal agenesis is a rare anomaly that can alter retroperitoneal anatomy and affect the spatial relationships between adjacent organs, potentially complicating colorectal surgery. Here, we present the case of a 54-year-old woman with congenital left renal agenesis who was diagnosed with an obstructing sigmoid colon tumor. She initially underwent transanal decompression with an ileus tube and was referred to our institution for further management. Contrast-enhanced computed tomography revealed no distant metastasis and incidentally revealed the absence of the left kidney. After adequa

Left Clavicular Pain Following Splenic Rupture After Colonoscopy: A Variant of Kehr's Sign?

Colonoscopy is a widely performed diagnostic and therapeutic procedure in modern gastrointestinal practice and is generally considered safe. However, splenic injury remains a rare but potentially life-threatening complication. We present the case of a 67-year-old woman who developed severe epigastric and left-sided pain six days following an uncomplicated colonoscopy. Notably, the patient reported pain localised to the left clavicular region rather than the classical left shoulder-tip pain associated with Kehr's sign. She also experienced a syncopal episode and presented with hypotension and t

Selective non-diverting strategy after restorative rectal cancer surgery: a single-center experience combining optimized anastomotic technique and intensive surveillance.

The role of routine diverting ileostomy after restorative rectal cancer surgery remains controversial. We evaluated a selective non-diverting strategy implemented together with technical modifications of the anastomosis and an intensified postoperative surveillance protocol. This retrospective single-center study included 300 patients who underwent restorative surgery for rectal cancer between 2017 and 2026. Patients were categorized according to reconstruction technique: end-to-end stapled colorectal anastomosis, side-to-end stapled colorectal anastomosis, or delayed coloanal anastomosis usin

Colonic pleomorphic undifferentiated sarcoma with a leukemoid reaction: case report.

BACKGROUND: Undifferentiated pleomorphic sarcoma (UPS) is an aggressive mesenchymal tumor that typically occurs in the extremities or retroperitoneum. Primary colonic involvement is relatively rare, and those accompanied by a leukemoid reaction are even rarer. This case of primary colonic UPS with a significant leukemoid reaction highlights the unique nature of this rare paraneoplastic syndrome, which may mimic hematologic malignancy and delay appropriate surgical intervention. It also contributes new clinical evidence to the literature by indicating that surgical reduction of tumor burden can

Robotic Single-Port Trans-stomal Hartmann Reversal.

BACKGROUND: The colostomy site provides natural access for a single-port Hartmann reversal, making it an ideal scar-sparing procedure. Although single-incision laparoscopic surgery for this purpose is technically challenging, the da Vinci SP robotic platform may overcome single-incision laparoscopic surgery limitations: its wristed instruments and 3-dimesional visualization could enhance the feasibility of a full single-port approach. IMPACT OF INNOVATION: The robotic single-port approach for Hartmann reversal retains all the advantages of a single-port access through the stoma site while ove

Operative Approach for Robotic Descending Colon Cancer Requiring Bidirectional Mobilization of the Splenic Flexure and Sigmoid-Descending Junction.

INTRODUCTION: Robotic Descending Colectomy Occasionally Requires Splenic Flexure (SF) and Sigmoid-Descending Junction (SDJ) Mobilization. In Multiport Robotic Surgery, This Bidirectional Mobilization Can Be Technically Demanding Because the Operative Field Extends in Two Directions, and Conventional Curved-Line or Stepped-Line Configurations May Limit Operative Envelope or Cause Arm Interference. MATERIALS AND SURGICAL TECHNIQUE: For lesions requiring continuous SF-SDJ access, an oblique-line configuration using the da Vinci Xi (DVXi) or the da Vinci SP (DVSP) system is typically required for

A population-based analysis of the elective surgical management for non-metastatic splenic flexure cancers, the ongoing question.

AIM AND BACKGROUND: Malignancies found at the splenic flexure correspond to only 2-4% of all colorectal cancers and no standardised surgical technique exists for its treatment. Our study analyses the surgical and oncological outcomes between Extended Right Hemicolectomy (ERH) and Left Hemicolectomy (LH), the operations commonly performed to resect tumours in this location. METHODOLOGY: A retrospective cohort study of patients with non metastatic splenic flexure cancer treated between 1997 and 2017 within the NHS in England. The surgical approach was defined through OPCS-4 codes. Patients were
