Priority dissecting of the inferior mesenteric artery combined with complete medial approach: a novel laparoscopic approach for left-sided colon cancers.

PURPOSE: To explore the application effect of the technique of "priority dissecting of the inferior mesenteric artery combined with complete medial approach (IMA-CMA)" in laparoscopic left-sided colon cancer radical resection. METHODS: A total of 99 patients who underwent laparoscopic left-sided colon cancer radical resection with splenic flexure mobilization between September 2021 to May 2023 were included. Sixty-eight of these patients were analyzed after propensity score matching (PSM). The perioperative characteristics were compared. RESULTS: Among these enrolled patients, 45 underwent t

Splenic Flexure Volvulus Managed With Left Hemicolectomy and Primary Anastomosis: A Case Report.

Volvulus of the splenic flexure is a rare cause of large bowel obstruction. We present the case of a male in his mid-20s who presented with a three-day history of colicky left-sided abdominal pain. A computed tomography scan revealed severely dilated large bowel loops with a closed-loop obstruction in the left upper quadrant, which was thought to originate from the transverse colon. An urgent laparoscopy was performed, and a splenic flexure volvulus was found with a 720-degree rotation around its mesentery. The bowel was viable with no evidence of ischemia or perforation. There was a congenita

Endoscopic submucosal dissection for proximal colonic lesions: An effective therapeutic option.

BACKGROUND AND STUDY AIMS: Due to the greater risks of adverse events (AEs) and the lower rate of submucosal invasive cancer (SMIC), large proximal colonic polyps are frequently treated by piecemeal endoscopic mucosal resection (EMR) in the West. However, this implies the risk of surgery to radicalize non-curative endoscopic resection in case of early colorectal cancer (CRC). We evaluated procedure outcomes in patients undergoing ESD for proximal colonic lesions at risk of SMIC. PATIENTS AND METHODS: All consecutive patients with lesions at risk of SMIC proximal to splenic flexure referred fo

Mobilization of the splenic flexure in laparoscopic colorectal surgery: Why and how?

Whether or not to mobilize the splenic flexure during laparoscopic colorectal surgery remains a subject of debate. Its usefulness in decreasing the rate of anastomotic leak has not been demonstrated. The difficulty of performing splenic flexure mobilization via laparoscopy and the increase in operative duration are its principal drawbacks. The splenic flexure is an anatomic threshhold zone with complex anatomy, particularly with numerous vascular variations. The laparoscopic approach to splenic flexure mobilization must take into account the embryologic planes while respecting its vascular sup

Faecal microbiota transplantation in Crohn's disease: an Australian randomised placebo-controlled trial protocol.

INTRODUCTION: The enteric microbiota drives inflammation in Crohn's disease. Yet, there are no placebo controlled trials evaluating the efficacy and safety of faecal microbiota transplantation (FMT) in inducing and maintaining remission in patients with active Crohn's disease. The Microbial Restoration (MIRO) study aims to establish this evidence. METHODS AND ANALYSIS: At two specialist inflammatory bowel disease centres, 120 enrolled patients will have a 3-week period of diet optimisation (removal of ultra-processed foods) together with a 7-day course of antibiotics (to facilitate subsequent

Variation in Proximal Sessile Serrated Lesion Detection Rates During Non-screening Colonoscopies.

AIMS: Proximal sessile serrated lesions (PSSL) are increasingly recognized as significant precursors of interval colon cancer. We aimed to assess the PSSL detection rates during non-screening colonoscopies and whether there are associations between PSSL detection rate and the established colonoscopy key performance indicators (KPI). METHODS: We retrospectively collected data of all non-screening colonoscopies performed by independent endoscopists at a large teaching hospital between June and December 2019. Data regarding endoscopists' KPIs, including polyp detection rate (PDR), cecal intubati

Post-Colonoscopy Perforation in a Patient With Ulcerative Colitis.

Ulcerative colitis, a chronic inflammatory bowel disease, renders the colonic mucosa particularly vulnerable to complications during endoscopic procedures. This case illustrates a serious complication following colonoscopy in active disease. A 25-year-old female presented with two weeks of hematochezia. Colonoscopy revealed severe active inflammation with ulcerations, friability, and spontaneous bleeding throughout the examined colon, consistent with chronic ulcerative colitis. Histopathology confirmed chronic architectural distortion with acute inflammatory infiltrates. Four hours post-proced

The Effect of the Second Forward View on the Detection Rate of Sessile Serrated Lesions in the Proximal Colon: A Single-Center Prospective Randomized Controlled Study.

INTRODUCTION: The detection rate of proximal sessile serrated lesion (PSSLDR) is linked to the incidence and mortality of colorectal cancer. However, research on second forward view (SFV) examinations for PSSLDR remains limited. This first randomized controlled trial assessed the impact of the proximal SFV on the PSSLDR. METHODS: Patients were randomized into 2 groups during proximal colonoscopy: standard colonoscopy (SC) and SFV. The SC group underwent a standard examination, whereas the SFV group underwent a second examination of the proximal colon (cecum to splenic flexure). The primary ou

Laparoscopic Resection for Colonic Obstruction Due to Immunoglobulin G4-Related Disease: A Case Report.

An 89-year-old woman was referred to our hospital with complaints of body weight loss and anemia. Lower gastrointestinal imaging revealed a 3.5-cm stenosis in the descending colon near the splenic flexure. Endoscopy did not pass through the stricture. A biopsy showed no evidence of malignancy or lymphoma. Enhanced abdominopelvic computed tomography showed a thickened descending colon wall with swollen lymph nodes around the colon. Because descending colon cancer was suspected, laparoscopic-assisted left hemicolectomy with D2 lymph node dissection was performed. Hematoxylin and eosin staining s

Hemoperitoneum due to aneurysm rupture related to systemic lupus erythematosus: a case report.

Although systemic lupus erythematosus (SLE) is often associated with aneurysm formation, hemoperitoneum due to SLE-related abdominal visceral aneurysm is rare. A 53-year-old man with a 21-year history of SLE presented to our hospital with abdominal pain. Contrast-enhanced computed tomography revealed extravasation around a branch artery of the inferior mesenteric artery (IMA), suggesting hemorrhage from the same region. Emergency laparotomy was performed due to patient's declining condition. A large hematoma in the mesentery of the splenic flexure of the descending colon was observed, and rese

Predictors of difficulty in robotic splenic flexure mobilization during rectal cancer surgery.

PURPOSE: In surgery for rectal cancer, splenic flexure mobilization is sometimes necessary to ensure a tension-free colorectal anastomosis with adequate blood supply. Splenic flexure mobilization is regarded as a challenging and risky maneuver, but there are no clear indicators of its difficulty in rectal cancer surgery. This study evaluated the impact of clinical and anatomical factors, including splenic flexure height measured qualitatively on the basis of vertebral level using computed tomography, on the difficulty of splenic flexure mobilization during rectal cancer surgery. METHODS: The
