Analysis of clinical factors for hyperoxaluria in patients with calcium oxalate stones: a single-center retrospective cross-sectional study.

BACKGROUND: Hyperoxaluria is associated with calcium oxalate (CaOx) stone disease and is assessed by quantitative urinary oxalate excretion. We aimed to identify routinely available clinical factors associated with concurrent study-defined hyperoxaluria among adults with CaOx stones. METHODS: This single-center retrospective cross-sectional study included 385 adults with infrared-spectroscopy-confirmed CaOx stones who underwent stone surgery and completed at least one valid preoperative 24-hour urine collection 3-10 days before surgery between January 1 and December 1, 2024. Study-defined hyp

Clinically adaptable size cartilage grafts generated from human embryonic stem cells enable tracheal repair in a porcine model.

INTRODUCTION: Autologous costal cartilage grafting is commonly used for pediatric tracheal reconstruction; however, it has several limitations, including restricted availability and surgical invasiveness. In this study, we investigated the feasibility of a novel tracheal reconstruction approach using human embryonic stem cell-derived cartilage tissue (ESCaT). METHODS: ESCaT was differentiated from human embryonic stem cells (SEES2), and large-scale cartilage constructs were generated by integrating tissues derived from multiple culture dishes within a nylon mesh scaffold. Decellularized ESCaT

How-I-do-it: bottom dock eTEP with bilateral TAR for incisional hernia after liver transplantation: early outcomes and feasibility.

BACKGROUND: Incisional hernias represent a significant source of morbidity for patients following liver transplantation, and repair of these defects remains challenging. Transversus abdominis release (TAR) is a posterior component separation technique enabling wide sublay prosthetic reinforcement which is suitable for the treatment of complex incisional hernias. TAR can be used with the enhanced view totally extraperitoneal (eTEP) approach, with favorable outcomes in non-transplant patients being reported. METHODS: This is a single center retrospective review of adult liver transplant recipie

Severe Plasmodium falciparum infection with multi-organ involvement in an asplenic patient: a case report.

Plasmodium falciparum is the leading cause of severe malaria. The spleen plays a central role in the clearance of the infection in conjunction with antimalarial treatment. Splenectomized patients lack sufficient clearance functions. We report a case of severe P. falciparum infection with multi-organ involvement acquired in Uganda in a splenectomized patient with a persistently high parasitemia level despite appropriate antimalarial treatment. The case emphasizes the challenges associated with severe malaria in asplenic patients.

Extracorporeal photopheresis for the treatment of steroid-refractory immune related adverse events from immune checkpoint inhibitor therapy: a multi-center phase Ib/II clinical trial.

Severe immune-related adverse events (irAE) in cancer patients receiving immune checkpoint inhibitors (ICI) require treatment interruption and immunosuppression. Based on prior preclinical data, in this prospective, single arm, non-randomized phase-Ib/II clinical trial we test the activity of Extracorporeal Photopheresis (ECP) against steroid-refractory irAEs in 30 patients (EudraCT-No.:2021-002073-26). The primary endpoint is the evaluation of the safety of ECP treatment for patients with ICI-induced irAEs. The study meets its pre-specified primary endpoint of less than 50% of patients develo

Uniportal versus multiportal robotic-assisted thoracic surgery for anatomical pulmonary resection: a systematic review and meta-analysis of perioperative recovery and surgical quality.

Uniportal robotic-assisted thoracic surgery (uRATS) seeks to combine the dexterity of robotic platforms with reduced access trauma, yet its comparative value over multiportal RATS (mRATS) remains uncertain. We systematically evaluated perioperative recovery and surgical quality after anatomical pulmonary resection. PubMed/MEDLINE, Web of Science, and the Cochrane Library were searched through 2026. Comparative studies of uniportal/single-port versus multiportal robotic anatomical lung resection were included. Random-effects meta-analyses pooled risk ratios (RRs) and mean differences (MDs). Ris

Navigating anticoagulation in heart transplantation with thrombin-antithrombin complex: when conventional monitoring fails.

BACKGROUND: Heart transplant recipients often face the clinical dilemma of coexisting bleeding and thrombotic risks postoperatively. Conventional anticoagulation monitoring parameters (e.g., platelet count, fibrinogen, D‑dimer, and anti‑Xa activity) frequently exhibit limitations in evaluating complex postoperative patients, including delayed response, poor sensitivity, and lack of clear target ranges. This case report describes the use of thrombin-antithrombin complex (TAT) to guide anticoagulation management for complex thrombotic events after heart transplantation and explores its potential

Precise immunomodulation by mesenchymal stem cells mitigates hepatitis B virus-specific CD8⁺ T cell-mediated acute liver injury without compromising viral clearance.

Hepatitis B virus-related acute-on-chronic liver failure (HBV-ACLF) is associated with a high mortality rate. Although mesenchymal stem cell (MSC) therapy markedly improves liver function and survival in patients with HBV-ACLF, it remains unclear whether the immunomodulatory effects of MSCs suppress antiviral CD8⁺ T cell function and trigger HBV reactivation. Here, we developed a novel in vitro HBV-specific CD8⁺ T cell-mediated acute liver injury organoid model, which was co-cultured with either menstrual blood-derived MSCs (MenSCs) or umbilical cord-derived MSCs (UC-MSCs). We found that both

Association of the lactate dehydrogenase-to-albumin ratio with all-cause and non-relapse mortality in patients with aGVHD after allogeneic HSCT: a single-center retrospective cohort study.

BACKGROUND: Acute graft-versus-host disease (aGVHD) remains a leading cause of non-relapse mortality after allogeneic hematopoietic stem cell transplantation (allo-HSCT). The lactate dehydrogenase-to-albumin ratio (LAR), integrating metabolic stress and nutritional reserve, has demonstrated prognostic value across malignancies but has not been investigated in aGVHD. We evaluated the association between LAR measured at aGVHD diagnosis and both all-cause mortality (ACM) and non-relapse mortality (NRM). METHODS: This retrospective cohort study enrolled 109 consecutive allo-HSCT recipients who de

Therapeutic Hybrid Intelligence with Neural and Knowledge-based Expert Reasoning for SRS (THINKERS): an AI Model for Functional Outcome Prediction After Gamma Knife Thalamotomy for Essential Tremor.

Outcome prediction after Gamma Knife thalamotomy for essential tremor remains largely guided by baseline tremor severity, target anatomy, radiosurgical dose parameters, and institutional experience rather than individualized estimates of functional improvement, latency to benefit, and adverse radiation effect risk. We developed THINKERS-ET, a mixture-of-experts (MoE) artificial intelligence framework for personalized functional outcome prediction and dose-policy evaluation after Gamma Knife thalamotomy. We performed a retrospective single-center treatment-level study of patients with medically

Effectiveness of artificial intelligence-assisted colonoscopy for colorectal lesion detection: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Colonoscopy is the gold standard for colorectal cancer detection and prevention, but its performance depends on operator skill and lesion features. Artificial intelligence (AI)-assisted colonoscopy has been introduced to improve adenoma detection. METHODS: A comprehensive search was conducted in PubMed, Embase, the Cochrane Library, Web of Science, ClinicalTrials.gov, and major Chinese databases to identify randomized controlled trials (RCTs) comparing AI-assisted and conventional colonoscopy. Meta-analyses were performed using Review Manager 5.4 and Stata 14.0. The quality of evi

Diagnostic accuracy of CSF tap-test parameters in predicting shunt responsiveness in normal pressure hydrocephalus: a cohort study.

BACKGROUND: The cerebrospinal fluid tap test (CSF-TT) is widely used as an ancillary test to select patients with idiopathic normal pressure hydrocephalus (iNPH) for ventriculoperitoneal (VP) shunt surgery; however, its diagnostic utility remains unclear. OBJECTIVES: To evaluate the diagnostic utility of CSF-TT in predicting outcomes following VP shunts. METHODS: Patients with probable iNPH underwent assessments of gait, urinary, and cognitive function using the Dutch gait scale, timed up and go (TUG) gait score, iNPH grading scale, and modified Rankin Scale (mRS) at baseline and 24 h after
