Severe cytomegalovirus gastritis and hypogammaglobulinemia in a patient treated with pembrolizumab.

A 66-year-old man with stage IVB oesophageal carcinoma treated with cisplatin, 5-fluorouracil, and pembrolizumab developed gastric erosions and ulcers with lymphoplasmacytic and neutrophilic infiltrates suggestive of immune checkpoint inhibitor-induced gastritis. High-dose steroids yielded transient relief. Cytomegalovirus (CMV) DNA was later found in gastric biopsies; steroids were withdrawn, and 3 weeks of intravenous ganciclovir led to partial improvement. Subsequent haemorrhagic gastritis raised concern for underlying immunodeficiency; serum immunoglobulin G (IgG), IgA, and IgM were undete

Gastroparesis: A Review.

IMPORTANCE: Gastroparesis is a condition of delayed gastric emptying in the absence of gastric outlet obstruction. Based on a 2018 US administrative health insurance claims database study, prevalence of definite gastroparesis (with documented delayed gastric emptying) was 21.5 per 100 000 persons. OBSERVATIONS: Gastroparesis, which is caused by reduced contractions of the distal stomach or abnormal pyloric relaxation, is more common among females than males (ratio, 2:1-4:1) and typically causes nausea, vomiting, early satiety, bloating, and abdominal pain or discomfort. In a large US epidemio

Sustained abnormal bilirubin and leukocyte levels after ERCP with acute suppurative obstructive cholangitis: a case report with 2 months follow-up observation.

Acute Suppurative Obstructive Cholangitis (AOSC) typically resolves post-ERCP. However, a 53-year-old male had persistent hyperbilirubinemia and leukocytosis (WBC 28.12×109/L) along with marked eosinophilia (20.26×109/L) despite standard antibiotic therapy. Systematic diagnosis ruled out residual calculi, Endoscopic Nasobiliary Drainage (ENBD) dysfunction, biliary neoplasms, and sclerosing cholangitis. A history of undercooked freshwater fish consumption and eosinophilia prompted suspicion of occult liver fluke infection, despite negative initial stool tests. Diagnostic treatment with albendaz

The uninvited guest: live fasciola hepatica as a cause of obstructive jaundice-a case report.

BACKGROUND: Hepatic fascioliasis is a zoonotic infection caused by Fasciola hepatica. It is underdiagnosed due to its non-specific clinical presentation and limited diagnostic tools in many healthcare settings. Human infections, though uncommon, are rising in endemic areas. CASE PRESENTATION: We report a 40-year-old South Asian female presenting with a 3-month history of vague right hypochondrial pain, nausea, and poor appetite. Laboratory tests showed cholestatic jaundice. Abdominal ultrasound revealed gallbladder sludge and a dilated common bile duct. ERCP was performed and revealed live Fa

Incidental Detection of Asymptomatic Colonic Anisakiasis During Surveillance Colonoscopy: A Case Report and Literature Review.

Anisakiasis is a parasitic infection caused by ingestion of raw or undercooked seafood containing Anisakis larvae. Although gastric involvement is common, colonic anisakiasis is rare and may be asymptomatic, making diagnosis difficult without endoscopic evaluation. We report a case of asymptomatic colonic anisakiasis incidentally detected during surveillance colonoscopy in a 53-year-old man 2 years after endoscopic submucosal dissection (ESD) for a sessile serrated lesion. The patient had consumed raw squid the evening before colonoscopy but had no abdominal symptoms. Bowel preparation was per

Unmasking the Parasite: When Giardiasis Looks Like Gastroenteritis.

Giardiasis is a protozoal infection that typically presents with diarrhea, abdominal pain, and malabsorption. Peripheral eosinophilia is unusual and may confound diagnosis by suggesting eosinophilic gastrointestinal disease (EGID). We present a 41-year-old woman with abdominal pain, diarrhea, malabsorption, and peripheral eosinophilia. Endoscopic biopsy performed shortly after anti-parasitic therapy demonstrated eosinophilic duodenitis, raising concern for eosinophilic gastroenteritis. However, repeat biopsy after recovery revealed resolution of eosinophilia, excluding EGID. This case highligh

Predictors of cystobiliary communication in patients undergoing surgery for hepatic hydatid cysts.

Cystobiliary communication (CBC) is a common complication of hepatic hydatid cysts, increasing surgical complexity and postoperative morbidity. Reliable preoperative prediction and intraoperative detection of occult CBC remain challenging. We retrospectively analyzed 106 patients undergoing surgery for hepatic hydatid disease. Patients were classified as CBC or non-CBC based on intraoperative findings. Clinical, laboratory, radiologic, and operative variables were evaluated. Multivariate logistic regression identified independent predictors, and Receiver Operating Characteristic (ROC) analysis

A Case of Nasal Myiasis Without Any Predisposing Factors: An Uncommon Presentation.

Nasal myiasis is an uncommon human parasitic disease affecting the nasal cavity. In otolaryngology, myiasis is mostly encountered as aural myiasis and oral myiasis, but occurs rarely as nasal myiasis. It typically affects individuals with poor hygiene, chronic illness, or immunosuppression. Cases in immunocompetent individuals are exceptional and can present diagnostic challenges. We report the case of a 32-year-old immunocompetent female who presented with the spontaneous expulsion of small larvae through the nostrils. Live larvae were observed in both nasal cavities during endoscopic examina

Hookworm infestation in the biliary tract: a case report.

BACKGROUND: Hookworms are parasitic nematodes that primarily infect humans in impoverished communities. They attach to the intestines to feed on blood, which results in anemia and other health complications. While parasitic infections of the biliary tract can occur and lead to significant issues such as cholangitis, pancreatitis, and biliary obstruction, hookworms are not documented to be associated with these biliary infestations. CASE PRESENTATION: A 53-year-old female Tibeto-Burman ethnicity following Buddhism, with a significant smoking history and no alcohol consumption presented to Trib

A new era in the early treatment of dioctophymosis - The first laparoscopic nephrotomy without ischemia in a dog.

The parasite Dioctophyme renale is a nematode that infects carnivores and often causes total and irreversible destruction of renal tissue. In cases of early diagnosis, nephrotomy is an alternative for renal preservation. This report describes the first laparoscopic nephrotomy performed without ischemia in the treatment of a female dog with renal dioctophymosis, using an ultrasonic scalpel for hemorrhage control. The technique stood out for its precision in simultaneous renal parenchyma incision and coagulation, eliminating the need for temporary blood flow interruption to the organ. It allowed

Endoscopic Detection and Molecular Confirmation of Ancylostoma ceylanicum with Atypical Gastrointestinal Manifestations: A Case Series.

BACKGROUND: Hookworm is a widely known soil-transmitted helminth (STH) traditionally linked to iron-deficiency anemia caused by chronic blood loss in the small intestine. The classical human pathogens are Ancylostoma duodenale and Necator americanus. However, emerging evidence indicates that Ancylostoma ceylanicum, a hookworm of cats and dogs, is a significant cause of human infection, particularly in Southeast Asia and other tropical regions. Its detection in human populations signals an evolving epidemiological landscape, challenging the previous notion that zoonotic hookworms have a negligi

A Rare Cause of Gastrointestinal Bleeding: Taeniasis in a Roux-en-Y Gastric Bypass Patient.

Taenia infection is a globally distributed parasitic disease that is uncommon in developed countries. Taeniasis typically presents with mild or no symptoms, and anemia is not a usual feature. We report the case of a 50-year-old woman with a history of Roux-en-Y gastric bypass who presented with a history of abdominal pain, asthenia, and melena. Blood tests revealed severe anemia, prompting endoscopic evaluation. EGD showed only postsurgical changes, and lower endoscopy revealed no bleeding source. Video capsule endoscopy demonstrated blood near the Roux-en-Y anastomosis and in the ileum. Enter

Surgical Management of Pulmonary Hydatid Disease.

Pulmonary hydatid cysts (PHCs), caused by Echinococcus granulosus, remain endemic in livestock-raising regions worldwide. Surgical removal of cysts is the mainstay of treatment, with the aim of eliminating the parasite while preserving the lung parenchyma and minimizing postoperative complications and recurrences. This article synthesizes the latest evidence on the optimal surgical management of PHCs, emphasizing practical recommendations for operating surgeons based on current best practices and the highest level of available evidence.

A rare case of abdominal pain caused by Enterobius vermicularis.

Small bowel involvement of Enterobius vermicularis infection is exceptionally rare, with few cases reported. A female presented with persistent abdominal pain for one month. Gastroscopy and colonoscopy were unremarkable, but computed tomography revealed localized small bowel wall thickening. Laboratory tests showed elevated eosinophil counts, though parasite antibody screening was negative. Double-balloon enteroscopy identified multiple shallow ulcers and a live worm in the small intestine, confirmed as Enterobius vermicularis on pathological examination. The patient was treated with anthelmin

Laparoscopic Splenectomy: Outcomes and Conversion Trends from A 20-Year Single-Center Experience.

Background: Laparoscopic splenectomy has become the preferred approach for most elective splenic indications; however, conversion to open surgery remains a relevant concern, particularly in complex cases. Long-term institutional experiences provide valuable insight into outcomes, learning curves, and evolving indications. Methods: A retrospective observational study was conducted at a tertiary referral center, including all consecutive patients who underwent laparoscopic splenectomy over a 20-year period. Demographic data, surgical indications, conversion to open surgery, and postoperative hos

Isolated colonic strongyloidiasis presenting as iron deficiency anemia with negative duodenal biopsies and stool studies.

BACKGROUND: Strongyloides stercoralis is a parasitic nematode capable of lifelong autoinfection, often resulting in delayed diagnosis because of nonspecific symptoms and the low sensitivity of stool testing. Isolated colonic involvement is uncommon and may be overlooked in the absence of hyperinfection. CASE PRESENTATION: A 67-year-old man with longstanding iron deficiency anemia presented with fatigue and intermittent eosinophilia. Upper endoscopy with duodenal biopsies was unrevealing. First report of gasless laparoscopy for the treatment of severe ectopic dioctophymosis in a puppy.

Dioctophymosis is uncommon in juvenile dogs, both in renal and ectopic presentations. Caused by the nematode Dioctophyme renale, which shows low sensitivity to anthelmintic drugs, surgery is considered the treatment of choice. An alternative approach is gasless laparoscopic surgery, in which pneumoperitoneum is not established and the surgical working space is created by mechanical elevation of the abdominal wall. This technique maintains normobaric intra-abdominal conditions and avoids the cardiorespiratory alterations, ischemia-reperfusion injury, and inflammatory response associated with pn

A Case Report of an Asymptomatic Cecum Ulceration Colonoscopically and Radiologically Mimicking Inflammatory Bowel Disease Caused by Amebic Colitis.

INTRODUCTION: Amebiasis is a disease caused by the protozoan parasite Entamoeba histolytica (Eh) and remains the second leading cause of death due to parasitic infections worldwide. Asymptomatic Eh infections are the most common. Infection with Eh can transition from a harmless colonic colonizer to a pathogenic form, leading to amebic colitis. The clinical presentation of amebic colitis is varied and may include abdominal pain, bloody-mucoid diarrhea, tenesmus, and low-grade fever. These symptoms are often nonspecific and can mimic many other gastrointestinal conditions, both infectious and no
