An integrated fluid-dynamic and structural pipeline to assess atrial hemodynamics before and after left atrial appendage occluder deployment in a patient-specific anatomy.

Atrial fibrillation is associated with a high risk of thromboembolic events, often mitigated by left atrial appendage occlusion (LAAO) when anticoagulation is contraindicated. This study presents a patient-informed computational workflow integrating pre-operative computational fluid dynamics (CFD), finite element (FE) modeling of device deployment, and post-operative CFD informed by the FE-deformed configuration. A proof-of-concept case was analyzed considering three atrial wall stiffness values to explore biomechanical variability. Pre-implant CFD simulations identified regions of blood stasi

Contemporary Economic, Hospitalization, and Mortality Burden Among Pulmonary Arterial Hypertension Patients: A Real-World US Claims Data Analysis.

This study quantified the recent real-world burden of pulmonary arterial hypertension (PAH) using US real-world data. Adults (18+) with treated PAH were identified from the MarketScan Commercial/Medicare Databases using a claims-based algorithm requiring pulmonary hypertension diagnosis, PAH-specific treatment, and right heart catheterization from January 1, 2017 to August 31, 2024 (earliest treatment claim = index). Patients were followed for ≥ 30 days post-index, overall and stratified by payer and by treatment categories. Subsets of PAH-hospitalized and newly treated patients were also anal

Persistent Supraventricular Tachycardia Following Central Venous Catheter Insertion: When Withdrawal of the Mechanical Stimulus Is Not Enough.

Cardiac arrhythmias are a recognised complication of central venous catheterisation and are usually attributed to mechanical irritation of the endocardium by the guidewire or catheter tip. These arrhythmias typically resolve following withdrawal of the mechanical stimulus. We report a 42-year-old man with heart failure with reduced ejection fraction and diabetic ketoacidosis who developed an abrupt regular narrow-complex tachycardia at approximately 217 beats/min immediately following advancement of a right internal jugular central venous catheter. The arrhythmia was associated with transient

Robust Approach to Measure Time-Resolved Septal Curvature in Pediatric Pulmonary Hypertension Patients Using Standard Short-Axis Ventricular Volume Contours: A Retrospective, Cross-Sectional Study.

BACKGROUND: Right heart catheterization is the gold standard for diagnosis and monitoring of pediatric pulmonary hypertension (PH). Noninvasive cardiovascular magnetic resonance (CMR) and echocardiography can additionally provide volumetric and functional assessment of the right ventricle and possibly serve as a non-invasive method for surveillance of PH severity using ventricular septal curvature (SC), interventricular septal (IVS) angle, or eccentricity index (EI). However, these septal wall measurements require manual interaction and can be observer dependent. This study presents the result

Estimation of Endothelin Levels in Chronic Obstructive Pulmonary Disease Patients with Pulmonary Hypertension: A Clinical Insight.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is a major global health issue, with pulmonary hypertension (PH) as a severe complication linked to increased morbidity and mortality. Endothelin-1 (ET-1) is a vasoconstrictor peptide and a potential biomarker for PH in COPD due to its role in vascular remodeling and hemodynamic stress. OBJECTIVES: This study aimed to assess serum ET-1 levels in COPD patients with and without PH to evaluate its correlation with hemodynamic and metabolic parameters and evaluate its diagnostic potential. METHODOLOGY: A cross-sectional study enrolled 90 s

Left ventricular transmural pressure and ventilatory inefficiency during exercise in advanced heart failure.

BACKGROUND: Baseline intracavitary haemodynamic measurements frequently demonstrate an inconsistent relationship with exertional performance. This may reflect an incomplete interpretation of left ventricular (LV) filling pressures. LV transmural pressure difference (ΔPTM = LV end-diastolic pressure [LVEDP]-right atrial pressure [RAP]) integrates LV filling pressure with right ventricular/pericardial constraint and may provide a composite representation of resting haemodynamic conditions. Ventilatory efficiency, assessed by the minute ventilation-carbon dioxide production (VE/VCO2) slope during

Elevated plasma eosinophil-derived neurotoxin is associated with echocardiographic pulmonary vascular involvement in stable COPD.

BACKGROUND: Pulmonary vascular involvement is an important dimension of chronic obstructive pulmonary disease (COPD) heterogeneity, but its biological features may not be fully represented by airflow limitation, gas-transfer impairment, or resting hypoxemia. eosinophil-derived neurotoxin (EDN) is a circulating eosinophil-derived protein whose relationship with pulmonary vascular involvement in stable COPD is unknown. METHODS: This single-center observational cohort study included 53 clinically stable participants with COPD and 10 healthy controls. COPD participants were classified by echocard

Functional Recovery Patterns and Baseline Cardiac Power Output During Impella 5.5 Support in Heart Transplant Candidates: A Descriptive Study.

BACKGROUND: The Impella 5.5 microaxial pump facilitates ambulatory mechanical circulatory support via axillary implantation, providing a model for active prehabilitation in critically ill heart transplant candidates. Whether patients with severely reduced baseline cardiac power output (CPO) can achieve meaningful ambulation during Impella 5.5 support remains uncertain. METHODS: This is a retrospective descriptive study of consecutive adults with cardiogenic shock (SCAI stages C-E) who received an Impella 5.5 as a bridge to heart transplantation between April 2022 and September 2024. Pulmonary Artery Denervation for Persistent Pulmonary Hypertension After Transcatheter Mitral Edge-to-Edge Repair: A Pilot Study and First Application in the Post M-TEER Setting.

Pulmonary hypertension (PH) is common in patients undergoing transcatheter mitral edge-to-edge repair (M-TEER) and is an established determinant of adverse outcomes after intervention. Importantly, persistent PH after technically successful M-TEER remains associated with ongoing symptoms, right ventricular dysfunction and poor prognosis, yet no dedicated treatment strategy has been established. The aim of this study is to evaluate the safety and efficacy of pulmonary artery denervation (PADN) in patients with persistent PH following successful M-TEER. Six patients (median age 69.0 years; 1 fem

Diagnostic performance of artificial intelligence-enabled electrocardiography for pulmonary hypertension: a systematic review and meta-analysis.

Pulmonary hypertension (PH) is a progressive, under-recognized condition with substantial morbidity and mortality. Its non-specific presentation and multimodal diagnostic pathway contribute to delayed diagnosis. Artificial intelligence (AI)-enabled electrocardiography (ECG) may offer a scalable approach to case finding. This PRISMA-DTA systematic review and meta-analysis was registered in PROSPERO (CRD420251241142). MEDLINE, Embase, Web of Science, Scopus, and IEEE Xplore were searched through July 2026, with Google Scholar and citation searching used as supplementary sources. Eligible studies

EACTS Expert Consensus Document on Civil Aviation Following Cardiothoracic Surgery and Transcatheter Procedures.

Civil aviation medicine and cardiothoracic surgery intersect at a point where individual clinical outcomes and public safety are tightly coupled. Professional aircrew and other safety-critical aviation personnel must meet legally defined medical standards that are designed around an engineering approach to risk, including a low annual tolerance for sudden incapacitation, whereas passengers and cabin crew are exposed to the physiological constraints of the flight environment (hypobaric hypoxia, gas expansion, limited access to medical care, immobility, and circadian rhythm disruption). Yet, the

The Left Atrial Appendage: Anatomy, Electrophysiology, and Clinical Implications for Atrial Fibrillation and Stroke.

PURPOSE OF REVIEW: The left atrial appendage (LAA) is nowadays recognized as both an arrhythmogenic substrate and the dominant source of thromboembolism in atrial fibrillation (AF). In this review we tried to summarize recent data on the anatomy, physiology, and imaging of the LAA, and examine how these findings help guide rhythm-control and stroke-prevention strategies. RECENT FINDINGS: High-resolution mapping confirms the LAA as a non-pulmonary-vein trigger source in persistent and post-ablation AF, supporting LAA electrical isolation in selected patients, though isolation may induce mechan

Cause-specific outcomes in systemic sclerosis-associated, systemic lupus erythematosus-associated, and idiopathic pulmonary hypertension with significant pre-capillary components: a competing risk analysis.

BACKGROUND: Connective tissue disease-associated pulmonary hypertension (CTD-PH) comprises subtypes with divergent prognoses, yet its established endpoint, transplant-free survival (TFS), aggregates death and lung transplantation and cannot reveal whether subtypes differ in death, transplantation, or both. We decomposed TFS into its competing components in systemic sclerosis-associated (SSc-PH), systemic lupus erythematosus-associated (SLE-PH), and idiopathic PH (iPH). METHODS: We analyzed 197 patients with right heart catheterization-confirmed precapillary PH (pulmonary vascular resistance >
