Pelvic splenosis and endometriosis in adolescent with hereditary spherocytosis.

The patient is a 16-year-old with a history of hereditary spherocytosis. At age 12, she underwent a laparoscopic partial converted to total splenectomy for intraoperative bleeding. The spleen was placed in a retrieval bag and removed via morcellation. She first reported abdominal pain at age 8, with dysmenorrhea after menarche at age 13. Her dysmenorrhea was managed with a progesterone IUD. Due to worsening pain, she underwent MRI which showed multiple lobulated structures throughout the pelvis and ventral abdomen, suggestive of endometriomas versus splenules. Laparoscopy revealed dozens of bl

GASTROINTESTINAL TRANSIT TIME OF HEALTHY DOGS BEFORE AND AFTER ENEMA ASSESSED BY CONTRAST RADIOGRAPHS WITH BARIUM-IMPREGNATED POLYETHYLENE SPHERES.

In this prospective experimental study, we aimed to assess the gastrointestinal (GI) transit of healthy dogs with and without an enema, using barium-impregnated polyethylene spheres (BIPS). Six healthy dogs underwent two experimental steps: evaluation of the GI transit using BIPS (BIPS group) before and after enema (BIPS+ENEMA group). All animals were fasted for 12hours, the transit was assessed in serial radiographs from 0 to 12h, and BIPS were counted in each segment of the GI tract, calculating the gastric emptying time and the time the spheres reached the descending colon. Blood cell count

Robot-assisted surgery for rectal cancer with persistent descending mesocolon: analysis of vascular anatomical features and short-term outcomes.

PURPOSE: Persistent descending mesocolon (PDM) is a rare congenital anomaly characterized by a medial shift of the descending colon and vascular variations that can complicate colorectal surgery. This study aimed to clarify the PDM-associated vascular anatomy and describe the short-term outcomes of robot-assisted surgery. METHODS: We retrospectively analyzed 541 patients with primary rectal cancer who underwent robot-assisted surgery (2017-2024) and compared the short-term outcomes between the PDM and non-PDM groups. The vascular anatomy, including inferior mesenteric artery (IMA) branching p

Assessment of bowel preparation quality in dogs undergoing colonoscopy using the Boston Bowel Preparation Scale.

OBJECTIVE: To evaluate the efficacy of a bowel preparation protocol consisting of at-home administration of clear broth and polyethylene glycol 3350 followed by colonic flushing immediately before colonoscopy in dogs. METHODS: In this single-referral-center retrospective study, dogs with complete video documentation of the colon were included. Colonic cleanliness was assessed using the Boston Bowel Preparation Scale (BBPS), which scores the 3 colonic segments individually (total score range, 0 to 9 with higher scores indicating better preparation quality). Adequate preparation was defined as

A case of intestinal mucosa-associated lymphoid tissue (MALT) lymphoma characterized by diffuse capillary hyperplasia nodules with hemorrhage.

A 52-year-old man presented to our department, complaining of recurrent episodes of hematochezia for half a month. Colonoscopy showed diffuse nodular mucosal eminences of approximately 2-3 mm in size in the colorectum and terminal ileum, presenting a "cobblestone" appearance, with visible capillary hyperplasia that are prone to rupture and bleeding upon light touch. Narrow-band imaging (NBI) showed that the hyperplastic capillaries presented "root-like" branching. We performed deep biopsies in areas with dense capillary networks or prone-to-bleeding regions at the nodules of the rectum, sigmoi

Inter-rater Reliability and Accuracy of NBI Score for Activity Evaluation in Ulcerative Colitis.

INTRODUCTION: Colonoscopy is a critical tool for the management of Ulcerative Colitis (UC). In this study, we aim to explore the accuracy of Virtual Chromoendoscopy (VCE) using Narrow Banding Imaging (NBI), and magnification using Near Focus (NF) for a better definition of endoscopic inflammation grade in UC patients compared to standard white light (WL) endoscopy alone. METHODS: This is a non-randomized prospective study including all the patients who underwent a colonoscopy (for any reason) between April and September 2023 (with protocol number n. 60/2019.20). During the endoscopic evaluati

Anti-TNF Therapies Promote a Proximal-to-Distal Healing Pattern in Moderate-to-Severe Ulcerative Colitis.

BACKGROUND: Ulcerative colitis (UC) is a chronic inflammatory disease of the colonic mucosa, extending proximally from the rectum. However, the segmental pattern of healing in UC remains unclear. Endoscopic improvement (EI), a key therapeutic endpoint, is typically assessed using the Mayo endoscopic score (MES), which scores the worst affected area and may miss partial/segmental healing. This study evaluates healing patterns in UC and compares conventional MES with a 3-segment MES approach for detecting treatment effects in clinical trials. METHODS: A post hoc analysis of HIBISCUS I/II and GA

Effect of κ-carrageenan with different molecular weights and its acetylated derivatives on the structure and metabolites of the intestinal flora.

The digestive fermentation characteristics of κ-carrageenan with different molecular weights and its acetylated derivatives were investigated by analyzing the variation patterns of the intestinal flora and metabolites. The results showed that κ-carrageenan with different molecular weights and its acetylated derivatives were not digested in the oral-gastric-small intestinal digestion process but could be fermented by the colonic flora and were mainly degraded in the ascending colon stage. The lower molecular weight κ-carrageenan was more likely to increase the abundance of Proteobacteria and En

CV-SCAN (Crystal Violet Staining for Colitis-Associated Neoplasia): A Novel Endoscopic Staining Method to Detect Paneth Cell Metaplasia and Ulcerative Colitis (UC)-Associated Neoplasia in UC.

OBJECTIVES: Paneth cell metaplasia (PCM), a metaplastic change associated with chronic inflammation in ulcerative colitis (UC), may be linked to UC-associated neoplasia (UCAN). However, no endoscopic method currently exists for detecting PCM. This study aimed to develop and validate a novel endoscopic staining technique-CV-SCAN-for identifying PCM and UCAN, and to explore the molecular characteristics of the stained areas. METHODS: This retrospective observational study included 131 patients with UC undergoing surveillance colonoscopy. CV-SCAN involved spraying an ultra-diluted solution (0.00

Epstein-Barr virus colitis on a background of Crohn's disease.

A middle-aged woman with a 6-year history of stable inflammatory bowel disease (IBD) presented with progressively worsening abdominal pain, increased stool frequency and rectal bleeding. Colonoscopy revealed patchy inflammation in the descending colon, and PCR testing detected 6961 copies/mL of Epstein-Barr virus (EBV) DNA in inflamed tissue. Initial treatment with steroids and azathioprine led to partial improvement, but azathioprine had to be discontinued due to significant adverse effects. A follow-up colonoscopy demonstrated persistent inflammation, despite declining EBV DNA levels in infl

Obstructive colonic FGESF associated with feline infectious peritonitis in a cat: a case report.

BACKGROUND: Feline infectious peritonitis (FIP) is a severe immune-mediated disease that develops in a small proportion of cats infected with feline coronavirus (FCoV). Clinical manifestations are variable and most commonly involve systemic inflammatory disease with or without effusion. Gastrointestinal signs, including vomiting or diarrhea, may be observed; however, focal intestinal lesions leading to mechanical obstruction are rare. Histopathologic changes resembling feline gastrointestinal eosinophilic sclerosing fibroplasia (FGESF) have only rarely been reported in association with FIP. CA

Colonic polyp as a rare aetiology of paediatric colocolic intussusception.

Intussusception is a common cause of abdominal pain and bowel obstruction in infants and children. Colocolic intussusception is a rare form, for which diagnostic imaging should be critically reviewed for identification of a lead point. This is the case of a young child who presented with abdominal pain and was found to have colocolic intussusception. Complete reduction was achieved with a pneumatic enema followed by a contrast enema. A pathological lead point was suspected based on radiological findings during reduction. A colonoscopy revealed a solitary, large juvenile polyp in the descending

A two-stage deep learning prediction system for colon cancer microsatellite instability status using CT images.

BACKGROUND: This study seeks to build a two-stage deep learning approach for identifying the microsatellite instability (MSI) status of colon cancer based on computed tomography (CT) scans without the requirement for manual segmentation. METHODS: This study included 108 enhanced CT scans of colon cancer, including 68 cases of ascending colon, 14 cases of transverse colon, 18 cases of descending colon, and 8 cases of sigmoid colon; there were 56 cases of MSI-H and 52 cases of microsatellite stability (MSS). In the first stage, the segmentation model MSI-SAM was trained to accurately segment th

Surgical management of giant Xp11.2 translocation renal cell carcinoma with multivisceral invasion: a case report of en bloc resection and targeted-immunotherapy success.

A 36-year-old man presented with a 1-month history of abdominal distension and a 3-day palpable mass. Imaging demonstrated a 20-cm left renal tumor with adjacent organ infiltration and suspicious lymphadenopathy, radiologically suggestive of sarcoma or advanced malignancy. He underwent radical en bloc resection including left nephrectomy, distal pancreatectomy, splenectomy, and partial colectomy due to tumor invasion of the pancreatic tail, splenic vessels, and descending colon mesentery. Histopathology confirmed Xp11.2 translocation/TFE3 gene fusion renal cell carcinoma. Surveillance MRI at 3

Laparoscopic Abdominoperineal Resection for Ischemic Colitis after Laparoscopic Partial Resection of the Descending Colon: Case Report.

INTRODUCTION: With advances in laparoscopic surgery, more sophisticated vessel-preserving techniques have become standardized. Laparoscopic partial colectomy aimed at maximizing colonic preservation is now widely performed. Along with this trend, cases of ischemic colitis developing after colorectal cancer surgery have occasionally been reported; however, cases requiring surgical resection remain exceedingly rare. Here, we report a case of ischemic colitis that developed 2 years and 2 months after partial laparoscopic resection of the descending colon, necessitating laparoscopic abdominoperine

MRGPRX2-expressing mast cells are increased in the GI tract of individuals with active inflammatory bowel disease and hereditary α-tryptasemia.

INTRODUCTION: Hereditary α-tryptasemia (HαT), defined by increased TPSAB1 copy number and elevated basal serum tryptase, is associated with mast cell (MC)-mediated symptoms. However, its role in gastrointestinal disease remains unclear. Because intestinal MCs express the non-IgE-dependent activation receptor MRGPRX2, we investigated whether MRGPRX2 expression and MC phenotypes are altered in individuals with HαT in the context of inflammatory bowel disease (IBD). METHODS: We genotyped 854 biobanked IBD samples to identify individuals with HαT. Spatial transcriptomic analysis was performed on

Adult Pan-Colonic Intussusception Secondary to Poorly Differentiated Cecal Adenocarcinoma: A Rare Presentation of Stage IIA Colon Cancer.

Adult intussusception is rare, typically associated with an underlying pathology, and often presents with nonspecific symptoms. We report the case of a 68-year-old man presenting with diarrhea, abdominal pain, and weight loss. CT suggested volvulus versus intussusception, and emergent laparotomy revealed massive pan-colonic intussusception extending from the small bowel into the right colon and continuing through the transverse and descending colon to the sigmoid. A subtotal colectomy with end ileostomy was performed. The postoperative course was complicated by high serous drain output and tra

Ischemic Colitis Associated With Tirzepatide Therapy in a Young Female Patient: A Case Report.

Tirzepatide has been approved for the management of obesity, obstructive sleep apnea, and type 2 diabetes mellitus; while gastrointestinal side effects are commonly seen, ischemic colitis has been only rarely described. We present the case of a 32-year-old woman with no prior medical history who developed hematochezia and abdominal pain after six weeks of tirzepatide therapy for weight loss. Laboratory evaluation showed neutrophilic leukocytosis without anemia or electrolyte abnormalities, and stool studies were negative for infection. A CT of the abdomen and pelvis excluded obstruction or wal
