Pre-hospital time and risk of perforation in patients with acute appendicitis in 112 countries (AlliGatOr): an international, prospective, observational cohort study.

BACKGROUND: Acute appendicitis is the most common surgical emergency worldwide. Concern that delayed treatment might increase perforation can lead to unnecessary surgery and high rates of negative appendicectomy, where a normal, non-inflamed appendix is removed. However, whether perforation is related to pre-hospital time (symptom onset to first surgical assessment) has not been explored in contemporary global data. METHODS: We conducted an international, multicentre, prospective, observational cohort study (AlliGatOr) of patients undergoing emergency appendicectomy for suspected acute append

Accelerating the clinical development of treatments for dengue: WHO dengue therapeutics landscape analysis and target product profiles.

Dengue, an Aedes mosquito-borne viral infection, is on the rise with climate and demographic change. In 2023, WHO declared dengue its highest grade of emergency, following the largest number of cases and deaths in recorded history. This emergency highlighted the absence of safe and effective dengue-specific treatments as a gap in the product landscape for care for people with dengue. To accelerate development of dengue-specific treatments for wide implementation and access in all dengue-endemic countries, WHO conducted a landscape analysis of the dengue therapeutics pipeline and convened a glo

Extracellular vesicle signature is associated with cardio-metabolic improvement after bariatric surgery.

AIMS: Bariatric surgery (BS) reduces obesity-associated systemic inflammation leading to multiple cardiovascular (CV) and metabolic benefits. Here, we tested whether measuring vaso-inflammatory cytokines, gut hormones, and circulating extracellular vesicles (EV) provide vaso-inflammatory-metabolic signatures that better correlate to CV-metabolic outcomes after BS, compared to a standard clinical assessment including body weight (BW) loss and traditional CV risk factors. METHODS AND RESULTS: In 111 patients with severe obesity, conventional clinical-biochemical parameters and non-conventional

Beyond the protocol: adherence to ECPR selection criteria and outcomes after Out-of-Hospital Cardiac Arrest (OHCA)-insights from the German Cardiac Arrest Registry (G-CAR).

BACKGROUND: Extracorporeal cardiopulmonary resuscitation (ECPR) is a potential treatment for selected patients with refractory out-of-hospital cardiac arrest (OHCA). Randomized controlled trials (RCTs) emphasize the importance of appropriate patient selection. Expert consensus statements recommend six criteria for ECPR suitability: age ≤ 75 years, witnessed OHCA, bystander cardiopulmonary resuscitation (CPR), no-flow time ≤ 5 min (min), low-flow time ≤ 60 min, and an initial shockable rhythm. Although widely accepted, these items are not supported by robust data and adherence varies greatly. W

[Ethical Decision-Making in Late-Termination Requests: Recommendations for Developing In-Hospital Policies].

BACKGROUND: Late terminations of pregnancy (after 14 weeks of gestation) can present physicians and other healthcare professionals with complex medical and ethical challenges. In Germany, such terminations require a specific form of medical indication (a danger of grave impairment to the pregnant woman's physical or mental health). Despite these legal provisions, the decision-making process in individual cases often remains characterized by uncertainty, a lack of transparency and insufficient structural support. AIM: This article offers recommendations for the development of institutional eth

Early Analysis of the Composite Allocation Score for Lung Transplantation on Race-Based Outcomes.

INTRODUCTION: The United Network for Organ Sharing recently transitioned from the lung allocation score to the composite allocation score (CAS) to optimize organ distribution. Although race is not directly incorporated, the CAS was designed to increase equity. This study evaluates race-related outcomes following implementation of the CAS in lung transplantation. METHODS: The United Network for Organ Sharing database was queried for lung transplants between March, 2022 and March, 2024. Patients were classified by transplant date before (lung allocation score) or on/after (CAS) September 3, 202

A global, descriptive analysis of the sources used to regulate abortion care.

An enabling environment is essential for delivering quality, comprehensive abortion care. According to the WHO, abortion should be treated like other healthcare services, regulated by healthcare law, and guided by evidence-based standards and guidelines. This paper examines abortion regulations across 183 countries, drawing on data from the Global Abortion Policies Database, up to May 2025. It divides sources relevant to the regulation of abortion care into seven categories: Criminal/Penal Code, Abortion-specific Law, Health Regulation/Clinical Guidelines, Constitution, Case Law, Essential Med

Regulatory complaint patterns in physical medicine and rehabilitation: A population-based cohort study.

BACKGROUND: Regulatory complaints provide insight into physician performance and professionalism. Although complaint patterns have been described for the collective physician workforce, little is known about complaint risk among physical medicine and rehabilitation (PM&R) specialists (physiatrists). OBJECTIVE: To examine the rate, type, and predictors of complaints among Alberta physiatrists and to compare complaint patterns across specialties. DESIGN: Population-based cohort study using linked administrative data on physician demographics, practice characteristics, prescribing, and complain

Pregnancy after the Manchester-Fothergill procedure: a case report.

INTRODUCTION AND IMPORTANCE: Cervical elongation is commonly associated with uterine prolapse. The Manchester-Fothergill procedure offers a fertility-preserving surgical option for its correction. However, conception following this procedure is exceedingly rare, with an estimated incidence of 1 in 10 000. CASE PRESENTATION: We present the case of a 35-year-old multiparous woman (G3P2L2) who conceived 18 months after undergoing the Manchester-Fothergill procedure for cervical elongation. Her obstetric history included one vaginal delivery and one cesarean section performed for cervical elongat

Evaluating a Bilingual, Community-Based Skin Cancer Education Program in an Underserved Hispanic Population: An Item Response Theory Analysis.

BACKGROUND: Skin cancer prevention is often underemphasized in medically underserved regions such as the Rio Grande Valley (RGV), where residents, predominantly Hispanic and lower-income individuals, face limited access to dermatologic care and may be unaware of their risk for skin cancer. METHODS: The intervention included a bilingual educational session with a board-certified dermatologist and a seven-item knowledge survey administered before and immediately after the session, adapted from a previously validated instrument. A convenience sample of 73 participants completed both surveys. The

GLS2 Promotes Early Ferroptosis Followed by Apoptosis and p53 Signaling in Renal Ischemia-Reperfusion Injury.

Renal ischemia-reperfusion injury (IRI) is a major cause of acute kidney injury (AKI), particularly in the settings of kidney transplantation and perioperative care. In IRI-induced AKI, ferroptosis, an iron-dependent, lipid peroxidation-driven form of regulated cell death, occurs early in proximal tubular epithelial cells; however, the precise molecular mechanisms underlying ferroptosis, its crosstalk with other forms of regulated cell death, and its clinical relevance remain incompletely understood. We previously identified glutaminase 2 (GLS2) as a key regulator of ferroptosis via α-ketoglut

Prevalence and risk factors of persistent and neuropathic pain following resection of pediatric extremity soft tissue sarcomas: A nationwide cohort study.

INTRODUCTION: Resection of extremity soft tissue sarcoma (eSTS) in children carries a substantial risk of chronic postsurgical pain (CPSP) development. However, the exact prevalence and the clinical predictors of CPSP, with and without neuropathic characteristics, remain poorly defined. Therefore, we aimed to quantify outcome after pediatric eSTS surgery and to identify risk factors. METHODS: We extracted data from the Dutch Childhood Cancer Group (DCOG) registry for patients <18 years who underwent eSTS resection between 2003 and 2025. Of 109 patients identified, 78 were evaluable for the an

Neurosurgical Outcomes of Epilepsy Surgery Between 2017 and 2025: A Retrospective ESSQ-19 Study From a Tertiary Care Center.

Introduction Epilepsy is a common neurological disorder. Despite being on anti-epileptic drug therapy, a subset of patients continue to experience seizures. In such patients with drug-resistant epilepsy, surgery remains a viable treatment option. Patient-reported satisfaction provides an important perspective on post-epilepsy surgery outcomes. This study aimed to descriptively assess postoperative patient satisfaction using the validated Epilepsy Surgery Satisfaction Questionnaire-19 (ESSQ-19). Materials and methods This was a retrospective observational study conducted at a tertiary care cent

Pediatric Brain Tumors: A Clinicopathological Study and Assessment of Caregiver Quality of Life at a Tertiary Neurosurgical Referral Center in Northern India.

BACKGROUND: Pediatric brain tumors are the leading cause of cancer-related death in children, yet in low- and middle-income countries (LMICs) their outcomes are shaped as much by socio-environmental factors as by tumor biology. The child and caregiver form a single therapeutic unit whose combined trajectory is rarely studied together. We analyzed the clinicopathological profile and short-term surgical outcomes of pediatric brain tumor patients alongside the quality of life (QoL) of their caregivers. MATERIALS AND METHODS: In this prospective observational study, 56 consecutively admitted chil

Preoperative disability is independently associated with postoperative care intensity following anterior cervical discectomy and fusion.

PURPOSE: Postoperative outcomes following anterior cervical discectomy and fusion (ACDF) are traditionally assessed using complications or long-term patient-reported outcomes (PROMs). However, determinants of immediate postoperative recovery and in-hospital resource utilization remain less well defined. This is particularly relevant with the increasing adoption of outpatient ACDF pathways, where accurate preoperative risk stratification is critical. The objective of this study was to evaluate whether preoperative PROMs are associated with postoperative care intensity following ACDF, in additio

Chinese Patients' Attitudes Toward the WHO Surgical Safety Checklist.

BACKGROUND AND OBJECTIVES: Strategies to reduce adverse events have mainly focused on systems of care and professional behavior despite a growing interest in involving patients in safety initiatives. Only some studies were published in developing countries. This study sought to clarify the patients' views on the surgical safety checklist in China. METHODS: We developed a research-based patient questionnaire and delivered it through a ubiquitous social media platform. The survey was sent to patients over 12 weeks from May to July 2020. Responses were accepted up to the end of July 2020. This s

Transformer-Based Preoperative Invasiveness Identification and Postoperative Recurrence Risk Stratification in Stage T1N0M0 Lung Adenocarcinoma: A Multi-Center Cohort Study.

Surgery is the primary curative treatment for early-stage lung adenocarcinoma (LUAD), with accurate identification of tumor invasiveness critical for determining pulmonary resection extent. Due to significant variability in postoperative recurrence among early-stage LUAD patients with aggressive tumors, a more effective risk stratification system is urgently needed to improve prognosis. This study developed the T1LUT system, a transformer-based tool for T1N0M0 LUAD management, covering preoperative tumor invasiveness identification and postoperative recurrence risk stratification. Preoperative
