Intraprocedural Cardiac Arrest in Patients Undergoing Percutaneous Coronary Interventions in the United States: An Analysis of the NCDR CathPCI Registry.

BACKGROUND: Cardiac arrest (CA) during percutaneous coronary interventions (PCI) is a catastrophic procedural complication. Contemporary rates of CA during PCI are unknown. OBJECTIVES: This study sought to understand contemporary rates of CA, associated mortality, and identify predictors of CA during PCI. METHODS: Patients with CA during PCI from 2018 to 2023 were identified from the National Cardiovascular Data Registry CathPCI Registry. Trends in intraprocedural CA (IPCA) incidence and in-hospital death were evaluated. Predictors of IPCA and risk-adjusted mortality were identified. RESULT

Temperature-Controlled Radiofrequency for Severe Nasal Airway Obstruction: A Non-Inferiority Comparison With Surgical Intervention.

INTRODUCTION: Temperature-controlled radiofrequency (TCRF), septoplasty (ST) with turbinate reduction, and functional rhinoplasty (FR) are treatment options for nasal airway obstruction (NAO) and nasal valve dysfunction (NVD), but no direct comparison of these procedures has been performed. METHODS: This prospective, open-label, non-inferiority (NI) trial, conducted at 29 US sites, used a prespecified NI margin and inverse probability of treatment weighting to simulate randomization and facilitate an unbiased comparison of TCRF to ST and FR in adults with Nasal Obstruction Symptom Evaluation

Consensus-based perspectives on integrating IPX203 into Parkinson's disease management.

Motor fluctuations are a common and burdensome complication of Parkinson's disease (PD) when the disease progresses, largely driven by the short half-life and variable absorption of immediate-release (IR) levodopa. IPX203 is a next-generation modified-release levodopa/carbidopa formulation that delivers rapid onset and sustained plasma levodopa concentrations through IR granules and extended-release pellets. Clinical studies have shown that IPX203 increases Good ON time (ON time without dyskinesia plus ON time with non-troublesome dyskinesia, equivalent to ON time without troublesome dyskinesi

Neurite orientation dispersion and density imaging for preoperative evaluation of cerebral microstructure and neurodevelopment in children with congenital heart disease.

BACKGROUND: Congenital heart disease (CHD) is a type of heart structure defect caused by the abnormal development of the heart and major blood vessels during the embryonic stage. With the continuous innovation of surgical treatment techniques and the improvement of perioperative management, the survival rate of children with CHD has significantly improved. However, accompanying central nervous system development problems have become increasingly prominent. This study investigated the clinical utility of neurite orientation dispersion and density imaging (NODDI) in evaluating preoperative cereb

Perioperative Hospital-at-Home Program: A Scoping Review.

BACKGROUND: Hospital-at-Home (HaH) is an established model of care for medical patients, particularly following rapid expansion during the COVID-19 pandemic. In contrast, perioperative HaH remains in its early stages, with limited understanding of appropriate care delivery models, staffing, feasibility, cost-effectiveness, and relevant clinical outcomes. Surgical-specific demands limit the operational transferability of HaH from nonsurgical care. This scoping review synthesizes existing evidence on perioperative HaH to clarify current practices and identify key domains for future research and

Low-intensity versus medium-intensity continuous renal replacement therapy for critically ill patients (LIMIT) trial: Protocol summary and statistical analysis plan of a randomised clinical trial.

BACKGROUND: Severe acute kidney injury in critically ill patients may require continuous renal replacement therapy (CRRT), yet mortality remains high. A standard CRRT intensity is 20-25 mL/kg/hr, because higher doses have not improved outcomes. Excessive treatment may cause harm by removing antibiotics and other essential substances and potentially impairing renal recovery. Observational data suggest that lower-intensity CRRT may be effective, warranting a clinical trial to compare low-versus standard-intensity therapy. OBJECTIVE: The objective of this study was to describe the prespecified p

Psychosis and Pericardial Effusion in Neuropsychiatric Systemic Lupus Erythematosus: Diagnostic Challenges in a Language-Discordant Encounter.

Neuropsychiatric systemic lupus erythematosus (NPSLE) is a diagnostically complex manifestation of systemic lupus erythematosus (SLE) that may initially present with significant psychiatric symptoms, contributing to delayed recognition and treatment. We describe a 36-year-old non-English-speaking female patient with a recent history of intermittent medical and psychiatric encounters over 1 year, during which progressive psychosis was attributed to a primary psychiatric illness. Her course was characterized by fragmented care across institutions, limited collateral history, language discordance

Vascular Surgery Training and Practice in West Africa: A Narrative Review of Current Capacity and Challenges.

BACKGROUND: Globally, the burden of peripheral vascular disease and other diseases in the remit of the vascular surgeon has been on the rise. Health systems globally have responded to the rise in the burden of peripheral vascular disease, particularly in their regions, by investing in training, research, and other initiatives to improve access and the quality of care. Unfortunately, regions in the Global South, especially West Africa, have significantly lagged behind. METHODS: This narrative review synthesised existing evidence on the current state of vascular surgical training and clinical p

Transesophageal Echocardiography for Intraoperative Hemodynamic Assessment: A Physiology-Based, Phase-Oriented Practical Framework.

Intraoperative transesophageal echocardiography (TEE) has evolved from a diagnostic adjunct into a real-time tool supporting perioperative decision-making in cardiac surgery. Although the 2025 American Heart Association Scientific Statement and the 2025 American Society of Echocardiography updates on diastolic and right heart function introduce new parameters and severity grading relevant to intraoperative interpretation, an integrated framework linking these advances to perioperative fluid stewardship has been lacking. This review synthesizes contemporary guidelines and pertinent recent evide

Clinical practice guidelines for neonatal therapeutic hypothermia across European Society for Paediatric and Neonatal Intensive Care (ESPNIC) member countries: a multidisciplinary blind comparison.

BACKGROUND: Therapeutic hypothermia (TH) decreases mortality and neuro-developmental disability in neonates greater or equal than 36 weeks' gestation with moderate-to-severe hypoxic-ischaemic encephalopathy (HIE) due to perinatal asphyxia. TH is often applied in different ways, as national guidelines and target populations may significantly differ. We aimed to compare TH guidelines from various European countries as we hypothesised that guidelines are significantly different and often of insufficient quality. METHODS: Multidisciplinary, blind, critical appraisal of clinical practice guideline

Orthopedic Risks and Recovery Considerations in Long COVID: A Narrative Review.

The COVID-19 pandemic left a lasting imprint on orthopedic surgery, initially through widespread reductions in care delivery and subsequently through the complex challenges posed by Long COVID syndrome. Emerging literature describes Long COVID-related alterations in bone mineral density, delayed wound healing, prolonged inflammation, persistent chronic pain, and increased perioperative risks. Long COVID may adversely affect skeletal integrity through inflammatory, hypoxic, and angiotensin-converting enzyme 2-mediated pathways, potentially leading to bone loss and increased fracture susceptibil

Traumatic High Cervical Spine Injury Presenting as Cardiac Arrest: A Case Report.

Traumatic high cervical spinal cord injury is a potentially devastating condition that may cause profound cardiovascular and respiratory instability through disruption of descending sympathetic pathways. In severe cases, autonomic dysfunction may contribute to marked hypotension, bradycardia, and cardiac arrest. We report the case of a 47-year-old man who sustained blunt trauma of incompletely characterized mechanism in a workplace accident and was found in cardiac arrest by a prehospital emergency medical team. Advanced life support was initiated, and return of spontaneous circulation was ach

Understanding respiratory rate as a predictor of clinical deterioration and mortality.

Respiratory rate (RR) is a fundamental physiological observation and a key component of early warning systems used to identify deterioration in hospitalised adults. Accurate RR measurement and interpretation, alongside other clinical observations and timely escalation of abnormal findings, are essential to ensure safe patient care. However, despite its clinical importance, RR remains one of the least reliably measured vital signs in practice and it is frequently estimated rather than counted accurately. This article explores the evidence on RR as a predictor of clinical deterioration and morta

Ocular Manifestations in Rheumatic Diseases.

Ocular involvement is common in systemic rheumatic diseases and may precede or reflect systemic activity, serving as an important diagnostic clue. Inflammation can affect any ocular structure, with presentations ranging from mild to vision-threatening. Early recognition is critical, as ocular findings can guide diagnosis and management. A systematic approach combining clinical evaluation, imaging, and laboratory testing is essential to identify disease-specific patterns. Advances in immunomodulatory and biologic therapies have improved outcomes. Optimal care requires a multidisciplinary approa

Australian Practical Guide on the Management of Muscle-Invasive Bladder Cancer.

Muscle-invasive bladder cancer presents a complex clinical challenge in Australia, where survival outcomes lag behind those in other high-income nations. Radical cystectomy remains the most frequently used curative approach. This review highlights a significant shift in the Australian landscape, with neoadjuvant chemotherapy uptake increasing from historic lows to over 60% according to recent registry data. Clinical unsuitability for cisplatin use and other factors, such as geographic disparities in access to multidisciplinary care, continue to be barriers to the more consistent use of periope

Effect of virtual reality during cycle ergometer session in the intensive care unit: A pilot randomised cross-over trial.

BACKGROUND: Early mobilisation, including a cycle ergometer, is a cornerstone of best practices in the intensive care unit (ICU). Virtual reality (VR) has increasingly been integrated into healthcare, showing benefits in reducing depressive symptoms, improving mobility, and managing pain. However, the combined use of VR and cycle ergometry to enhance physical performance in the ICU has not been investigated. OBJECTIVES: The primary objective of this study was to assess whether a single session of VR-based cycle ergometry increases active cycling distance (ACD) compared with a conventional ses
