Perioperative Hydrogen Inhalation and Postoperative Pain After Endoscopic Discectomy: A Pilot Non-Randomized Clinical Study with Exploratory Murine Data.

Background and Objectives: Postoperative pain after endoscopic discectomy may reflect acute surgical nociception superimposed on pre-existing radicular symptoms. Molecular hydrogen (H2) has antioxidant and anti-inflammatory properties, but clinical evidence for perioperative analgesia is limited. We evaluated the association between perioperative H2 inhalation and postoperative pain and analgesic requirements. Materials and Methods: This open-label, non-randomized pilot study enrolled 37 patients in two consecutive periods (H2, n = 25; control, n = 12). Because original outcome records for 12

Preoperative Medial Proximal Tibial Angle of Less Than 84° Does Not Compromise Two-Year Clinical Outcomes After Unrestricted Kinematic Alignment Total Knee Arthroplasty: A Propensity Score-Matched Study.

INTRODUCTION: Unrestricted kinematic alignment (KA) total knee arthroplasty (TKA) may accept tibial component varus beyond conventional thresholds, yet outcomes in marked proximal tibial varus remain understudied in Southeast Asian populations. Our study evaluated two-year outcomes after unrestricted KA-TKA in patients who had severe proximal tibial varus, using mechanically aligned TKA (MA-TKA) as control. METHODS: This retrospective, propensity score-matched cohort study included 238 unrestricted KA-TKAs and 476 MA-TKAs with minimum two-year follow-up. The KA cohort was stratified by preope

Rotational Deformity of Long Finger Caused by Isolated Metacarpophalangeal Joint Capsular Injury: A Case Report.

CASE: A 49-year-old woman presented after a leash-pulling injury with ring and small finger proximal phalanx fractures. Pain and supination of the long finger metacarpophalangeal (MCP) joint were noted without collateral ligament injury on examination or imaging. Surgical exploration noted isolated capsular avulsion, which was repaired with restoration of alignment and full range of motion with relief of pain and instability at 1-year follow-up. CONCLUSION: This is a unique case of isolated finger MCP capsular injury causing rotatory instability. Surgeons should consider exploration and repai

Clinical value of fifth metatarsal osteotomy in hallux valgus with concomitant bunionette deformity: a symptom-based comparative study.

BACKGROUND: Hallux valgus is frequently accompanied by bunionette deformity, which may contribute to lateral forefoot pain and increased forefoot width. Although fifth metatarsal osteotomy is an established treatment for symptomatic bunionette deformity, the necessity of this procedure in patients with asymptomatic deformity remains unclear. PURPOSE: This retrospective cohort study aimed to evaluate the clinical necessity of symptom-based fifth metatarsal osteotomy in patients with hallux valgus and concomitant bunionette deformity. STUDY DESIGN: Retrospective comparative cohort study. METH

Sustainable high performance computing in the UK: a position paper on public investment, accountability, and the path to net zero.

BACKGROUND: Factors associated with early re-collapse after posterior fixation of thoracolumbar burst fractures have been sparsely examined in previous studies. Early re-collapse is challenging to predict due to multifactorial risk. Therefore, we aimed to investigate factors associated with vertebral re-collapse within the first 3 months following posterior fixation. METHODS: This retrospective study used data from the Swedish Fracture Register (2015-2019) and included patients aged 18 to 66 years with a single-level thoracolumbar burst fracture between T11 and L3 treated with posterior fixat

Delayed Metachronous Multicentric Giant Cell Tumor of Bone after a 16-Year Disease-Free Interval: A Case Report.

INTRODUCTION: Multicentric giant cell tumor (GCT) of bone is rare and usually occurs synchronously or within a few years of the primary lesion. Delayed metachronous presentation is exceedingly uncommon. CASE REPORT: We present the case of a 48-year-old man with progressive left hip pain 16 years after wide resection and endoprosthetic reconstruction for GCT of the proximal right humerus. Imaging demonstrated well-defined lytic lesions involving the bilateral proximal femur, left iliac bones, with cortical breach in the periacetabular region and right iliac bone. Predictors and Prevention of Hallux Valgus Recurrence After Primary Surgical Correction: A Structured Narrative Review.

Hallux valgus recurrence remains one of the most important causes of unsatisfactory outcomes following corrective surgery, with reported rates varying widely because of differences in patient selection, surgical techniques, follow-up duration, and recurrence definitions. Although numerous operative procedures have been described, growing evidence indicates that durable correction depends less on the specific procedure performed than on appropriate patient selection and complete correction of the underlying deformity. A structured literature search was conducted in PubMed from database inceptio

Clinically constrained Bayesian network analysis of immune-metabolic factors associated with bone mineral density in a real-world orthopedic cohort.

OBJECTIVES: Osteoporosis substantially influences surgical decision-making, implant fixation, fracture risk, and postoperative outcomes in orthopaedic practice. However, the systemic biological interactions underlying bone loss remain difficult to characterize using conventional statistical models. We aimed to integrate clinically constrained Bayesian network learning with routine biochemical data to identify immune-metabolic factors associated with bone mineral density (BMD) and to provide a clinically interpretable framework for bone health assessment. METHODS: In a retrospective real-world

Three-Dimensional Planning and Surgical Correction of Bilateral Developmental Valgus Deformity of the Manus in a Dog.

A one-year-old, 48 kg, male neutered, Great Dane presented for bilateral thoracic limb lameness ascribed to an apparent developmental valgus deformity of both manus. Corrective radial-shaped osteotomies of the four weight-bearing metacarpal bones (II-V) were virtually planned based on CT scan data, and bone models of each corrected manus were three-dimensionally (3D)-printed, sterilized, and used intra-operatively to facilitate the procedure. Corrective radial-shaped osteotomies of metacarpals II-V were performed on the right and then left manus, nine weeks apart. In the right manus, metacarpa

Early recognition of limb necrotizing fasciitis and surgery within six hours is associated with a lower mortality risk and improved patient-reported outcomes.

AIMS: Necrotizing fasciitis (NF) is a rapidly progressive surgical emergency. The effects of time from hospital presentation to theatre on short- and long-term outcomes, as well as specific modifiable aspects of the care pathway that would enable early surgery, remain poorly defined. METHODS: A retrospective cohort study of all suspected cases of limb NF presenting across a large health board region over ten years (January 2015 to April 2025) was undertaken. Early surgery was defined as ≤ six hours from presentation (triage time) to theatre (operation start time). The independent associations

Why Do We Lose Bone During Weight Loss: Can It Be Prevented?

PURPOSE OF REVIEW: Weight loss is recommended for obesity management, but it also increases bone turnover and reduces bone mineral density (BMD) and bone quality (microarchitecture, geometry, and strength). This review examines mechanisms and the multiple factors that influence skeletal responses to caloric restriction. RECENT FINDINGS: Bone loss during weight loss likely reflects multiple interacting mechanisms, including alterations in endocrine signaling, impaired calcium absorption, reduced mechanical loading, and alterations in the gut microbiota. The extent of bone loss is further influ

Toward Evidence-Informed Staging in Septic Revision Total Knee Arthroplasty: One-Stage, 1.5-Stage, or Two-Stage?

Periprosthetic joint infection (PJI) after total knee arthroplasty (TKA) remains one of the most consequential complications in adult reconstruction because successful treatment requires simultaneous infection control, restoration of a mechanically durable knee, preservation of function, and minimization of surgical morbidity. Two-stage exchange has traditionally been regarded as the reference strategy for chronic knee PJI, but contemporary comparative evidence increasingly challenges its routine use as the default for all patients. This narrative review synthesizes evidence from systematic re

Hip Arthroscopy in Dysplasia: Addressing Anatomy and Setting Yourself up for Periacetabular Osteotomy Success.

Combined hip arthroscopy and periacetabular osteotomy (PAO) is associated with improved outcomes in patient who have symptomatic hip dysplasia with concomitant intra-articular or extra-articular hip pathology. Modifications to the arthroscopic procedure include illiocapsularis dissection from the underlying capsule after capsular closure to aid in and begin the dissection for the PAO. The outcomes of combined PAO and hip arthroscopy have been published in several recent studies, demonstrating favorable patient reported outcomes of the combined procedures. When used in combination, these proced

The use of the Neviaser portal for antegrade intramedullary nailing in humerus fractures: a scoping review.

PURPOSE: Proximal humerus and humeral shaft fractures are common orthopaedic injuries that can be managed using antegrade intramedullary nailing. The Neviaser portal allows a more medial start point at the apex of the humeral head. The published literature on humeral intramedullary nailing through the Neviaser portal is limited and heterogeneous. This scoping review aims to map the existing literature on the use of the Neviaser portal for antegrade intramedullary nailing of humeral fractures, summarise reported indications, surgical techniques, radiographic and functional outcomes, complicatio

[Surgical access routes and their selection for osteosynthesis of pilon fractures].

The operative management of pilon fractures requires individually tailored preoperative planning, taking the fracture morphology, the intended reconstruction strategy and the local soft tissue situation into account. The goal is the anatomical restoration of the articular surface by means of open reduction and internal fixation (ORIF), combined with soft tissue-preserving stabilization of the metadiaphyseal component using minimally invasive plate osteosynthesis (MIPO). The choice of surgical approach is based on the analysis of computed tomography (CT) imaging, particularly the location of th
