Frailty as a Predictor of Mortality in Dialysis: A Multicenter Validation of the Clinical Frailty Scale.

RATIONALE & OBJECTIVE: Frailty is highly prevalent in dialysis and is associated with poor outcomes, yet scalable assessment in routine care remains challenging. We evaluated the feasibility and prognostic performance of the clinical frailty scale (CFS) applied by dialysis nurses and compared it with the REIN predictive score (RPS), a comorbidity-based mortality tool routinely available in French dialysis care. STUDY DESIGN: A prospective multicenter observational cohort. SETTING & PARTICIPANTS: Adults receiving maintenance dialysis at 6 centers in northeastern France (n = 792 prevalent pati

DIalysis Symptom COntrol-Pruritus Outcome Trial (DISCO-POT): A Multicenter Randomized Blinded Placebo Controlled Crossover Trial.

RATIONALE & OBJECTIVE: Uremic pruritus affects up to 50% of patients with kidney failure and negatively impacts quality of life. Itch often persists and remains insufficiently controlled despite standard of care treatments. We evaluated the effects of nabilone, a synthetic cannabinoid, in patients with kidney failure receiving dialysis who had symptoms of chronic itch. STUDY DESIGN: Placebo-controlled randomized crossover trial. SETTING & PARTICIPANTS: Adults ≥25 years old receiving maintenance hemodialysis at least twice weekly or peritoneal dialysis daily with at least moderate itch. INTE

New potassium binders: we need them-we do not need them.

New potassium binders may facilitate contemporary chronic kidney disease care by controlling hyperkalemia and thereby facilitating the continued use of kidney- and cardioprotective therapies, particularly renin-angiotensin-aldosterone system inhibitors. The opposing view is that sodium polystyrene sulfonate remains effective, familiar, inexpensive, and often well tolerated, while direct comparative evidence has not yet shown clinically meaningful superiority of newer binders. This debate weighs clinical utility, comparative efficacy, safety, and cost, concluding that new binders are valuable o

Immune mechanisms underlying endothelial and placental dysfunction in leptospirosis during pregnancy.

Leptospirosis is a zoonotic disease caused by pathogenic bacteria of the genus Leptospira and represents a major public health concern worldwide. Although its systemic manifestations are well documented, the consequences of leptospirosis for reproduction and pregnancy, particularly in humans, remain poorly understood. This narrative review summarizes the principal reproductive outcomes associated with leptospirosis and examines the mechanisms that may contribute to pregnancy complications. In livestock, leptospirosis is a well-established cause of reproductive failure, including abortion and i

H1N1-Related Hemophagocytic Lymphohistiocytosis Complicated by Severe Rhabdomyolysis in a Pediatric Patient: A First Case Report.

BACKGROUND: Hemophagocytic lymphohistiocytosis (HLH) is a rare and often life-threatening hyperinflammatory disorder, mainly caused by infections, malignancies, or autoimmune diseases. Influenza A (H1N1) has occasionally been linked to pediatric HLH cases. Rhabdomyolysis, relatively uncommon in children, can be a complication of viral infections, including influenza. To our knowledge, this is the first reported pediatric case where H1N1 infection was complicated by both HLH and severe rhabdomyolysis. CASE PRESENTATION: We describe the case of a previously healthy 4-year-old girl who was hospi

Isolated Acute Tubulointerstitial Nephritis in a Myeloperoxidase-ANCA-Positive Patient Without Crescentic Glomerulonephritis.

Kidney involvement in ANCA-associated vasculitis is usually recognized by its hallmark lesion, necrotizing crescentic glomerulonephritis, and clinicians naturally anchor their diagnostic expectations to this pattern. Rarely, however, the injury appears largely confined to the tubulointerstitium, with glomeruli that show no necrotizing or crescentic lesions, a presentation that is easily mistaken for drug-induced or infection-related interstitial nephritis. We describe an elderly woman who developed dialysis-requiring severe kidney failure soon after a brief febrile illness and was found to hav

Circadian rhythms and sleep disturbances: a conceptual perspective with implications for autosomal dominant polycystic kidney disease.

Autosomal dominant polycystic kidney disease (ADPKD) is a genetic disorder characterized by cyst formation, kidney enlargement, and extrarenal complications. While the role of genetic mutations in PKD1, PKD2, and other minor genes is well established, the influence of circadian rhythms and sleep disturbances on disease progression remains underexplored. The circadian timing system, coordinated by the central clock in the suprachiasmatic nucleus and synchronized with peripheral clocks in the kidneys, regulates key physiological processes, including glomerular filtration rate (GFR), sodium handl

Retinal Disease Risk With Hypoxia-Inducible Factor Prolyl Hydroxylase Inhibitors Versus Long-Acting Erythropoiesis-Stimulating Agents in Patients With Non-Dialysis Chronic Kidney Disease: A Real-World Database Analysis in Japan.

AIM: Hypoxia-inducible factor prolyl hydroxylase (HIF-PH) inhibitors constitute novel oral pharmacotherapy for anaemia in chronic kidney disease (CKD). Although a possible HIF-PH inhibitor-induced increase in vascular endothelial growth factor could exacerbate retinal diseases, real-world evidence is limited, particularly among Japanese non-dialysis patients. METHODS: We conducted a retrospective cohort study using the DeSC administrative claims database (DeSC Healthcare Inc., Tokyo, Japan; December 2019 to June 2023). We identified patients with CKD who newly used HIF-PH inhibitors or long-a

Discordance Between Gated Single-Photon Emission Computed Tomography Myocardial Perfusion Imaging and Coronary Angiography in Dilated Cardiomyopathy.

Background Gated single-photon emission computed tomography myocardial perfusion imaging (SPECT-MPI) provides functional information that may help distinguish ischemic from nonischemic dilated cardiomyopathy (DCM). However, its classification may differ from that of coronary angiography (CAG). Examining these discordant cases may improve SPECT-MPI interpretation in patients with DCM. This study aimed to assess the pattern of discordance between gated SPECT-MPI and CAG in patients with DCM and describe the features of false-positive and false-negative SPECT-MPI findings. Methodology This cross-

Epimeric Vitamin D is Not Associated With Arterial Stiffness or Cardiovascular Outcomes in Patients With Advanced CKD.

RATIONALE & OBJECTIVE: 3-epi-25(OH)D3, an epimeric form of 25(OH)D3 generated by C-3 epimerization, declines with advancing chronic kidney disease (CKD), yet its role in vascular health and related outcomes remains largely unknown. We aimed to determine whether circulating 3-epi-25(OH)D3 levels are associated with arterial stiffness and longitudinal cardiovascular (CV) outcomes in advanced CKD (stages 5 and 5D). STUDY DESIGN: A prospective observational study. SETTING & PARTICIPANTS: A total of 270 patients with advanced CKD using the Cambridge Coventry Vascular cohort. EXPOSURE S OR PREDIC

The pharmacokinetics of a single intravenous dose of difelikefalin in adolescents aged 12 to 17 years on hemodialysis.

BACKGROUND: The aim of this study was to evaluate the pharmacokinetic profile, safety, and tolerability of a single intravenous bolus injection of difelikefalin in adolescents on maintenance hemodialysis. METHODS: In this multicenter, open-label study, adolescents (12 to 17 years old) received a single difelikefalin dose of 0.5 μg/kg dry body weight at the end of the dialysis. Blood samples were collected over 3 days to assess difelikefalin plasma concentrations, and participants were followed up for adverse events (AEs) for 7-10 days. Key pharmacokinetic parameters were maximum plasma concen

Therapy for kidney vasculitis in children during the acute phase and maintenance: updates and insights from new adult therapies.

Kidney vasculitides in children encompass a heterogeneous group of immune-mediated disorders with distinct pathogenic mechanisms but a shared risk of irreversible kidney injury. Although pediatric vasculitides are rare, advances in understanding disease biology, coupled with significant therapeutic developments in adult vasculitides, have reshaped treatment paradigms. This review summarizes current evidence for the acute-phase and maintenance treatment of pediatric kidney vasculitis and examines how emerging adult therapies, including B-cell depletion, complement inhibition, cytokine-directed

When to start continuous kidney replacement therapy in critically ill children: a practical, PICU-focused, evidence-based review.

Acute kidney injury (AKI) and fluid overload (FO) are common in critically ill children and have been shown to be associated with adverse outcomes. Continuous kidney replacement therapy (CKRT) is the preferred renal support modality in paediatric intensive care to treat children with AKI and FO and may be necessary for non-renal indications like life-threatening toxin or electrolyte removal. However, the timing of CKRT initiation remains a contentious and highly variable clinical decision. Classic indications for dialysis are well established, yet modern paediatric intensive care practice incr

Left Ventricular Assist Device Implantation in Dextrocardia: Lessons From a Complex Redo Case.

Dextrocardia with situs solitus poses significant surgical challenges for left ventricular assist device (LVAD) implantation, particularly in patients with prior sternotomy and complex valvular disease. We report on a 60 year old man with dextrocardia, advanced heart failure, prior mechanical mitral valve replacement, and bicuspid aortic valve disease who underwent successful Corheart 6 LVAD implantation during redo sternotomy with concomitant bioprosthetic aortic and mitral valve replacement. Intraoperative findings included extensive pannus/thrombus on the mechanical mitral prosthesis (causi

Twice isolated, not twice ignored: Bacillus cereus catheter-related bloodstream infection in a haemodialysis patient.

A 53-year-old man with chronic kidney disease on maintenance haemodialysis via a right internal jugular tunnelled catheter (permcath) presented with recurrent fever during dialysis sessions. Paired blood cultures from the catheter and peripheral vein yielded Bacillus cereus. As the organism is frequently regarded as a contaminant, repeat paired cultures were obtained, which again grew B. cereus susceptible to vancomycin and gentamicin. Catheter-drawn cultures flagged positive earlier than peripheral cultures in both sets (differential time to positivity approximately 6.2 h and 5.5 h). Echocard

A Scoping Review on Decision-Making Interventions for Kidney Failure Treatment Options by the Kidney Disease Aging Research Collaborative.

BACKGROUND: Patients with kidney disease face complex treatment decisions amid prognostic uncertainty. These decisions may be particularly challenging for older adults, who often experience cognitive and functional changes and rely on caregivers. However, limited evidence describes treatment decision-making interventions for older adults, including the decision points addressed, use of decision aids, and geriatric-relevant SDM features, including caregiver involvement, values clarification, multiple touchpoints, and prognostic information. METHODS: We conducted a scoping review of studies exa

The evolving role of cardiac CT: Emerging technologies and clinical impact.

Cardiac computed tomography (CT) has progressed from a non-invasive test primarily used to exclude obstructive coronary artery disease into a comprehensive cardiovascular imaging platform for coronary artery disease, myocardial disease, valvular heart disease, and structural heart disease. Coronary CT angiography (CCTA) is now endorsed as a first-line test in appropriately selected patients with stable chest pain or suspected chronic coronary syndromes because it can exclude obstructive disease, depict coronary atherosclerotic burden, support preventive treatment, and inform referral for invas
