Unexplained intraoperative cardiac tamponade during thoracoscopic lobectomy: a case report.

BACKGROUND: Cardiac tamponade is a life-threatening emergency characterized by reduced cardiac output and circulatory failure. It most commonly occurs during cardiac surgery, interventional cardiology procedures, pericardiocentesis, or central venous catheterization, as well as in the setting of acute myocardial infarction or aortic dissection. To the best of our knowledge, cardiac tamponade during video-assisted thoracoscopic surgery (VATS) lobectomy has not been reported and may has an insidious presentation, making diagnosis challenging in the absence of obvious pericardial injury. CASE PR

Beyond detection: developing probabilistic hemodynamic phenotyping after a positive point-of-care pulmonary hypertension test.

BACKGROUND: Pulmonary hypertension (PH) is a heterogeneous condition in which hemodynamic subtype directs management and specialist referral. PH can be identified non-invasively at the point of care by a previously developed, regulatory-cleared machine-learned diagnostic (area under the receiver operating characteristic curve 0.95, sensitivity 82%, specificity 92%), but a positive test result raises a second clinical question: which patients have pre-capillary disease and therefore warrant referral to a specialist PH center. A probabilistic post-test phenotyping model was developed to estimate

Right Ventricular Stroke Work Index and Its Prognostic Value in a Lung Transplant Population With Group 3 Pulmonary Hypertension.

BACKGROUND: Lung transplantation requires meticulous preoperative assessment. Recipients with Group 3 pulmonary hypertension can be at higher risk of mortality and primary graft dysfunction (PGD). We conducted a study to determine whether the right ventricular stroke work index (RVSWI), a right-heart catheterisation haemodynamic indicator assessing right ventricle function, could predict 1-year survival in this setting. METHODS: Monocentric, retrospective, observational cohort study approved by our local ethics review board, between January 2013 and December 2023. We included lung transplant

Short- and long-term outcomes of balloon pulmonary valvuloplasty in children ≤24 months with critical and haemodynamically significant pulmonary valve stenosis: a 20-year single-centre experience in Medellín, Colombia.

BACKGROUND: Balloon pulmonary valvuloplasty is the treatment of choice for critical and haemodynamically significant pulmonary stenosis, but long-term outcome data in infants, particularly in Latin America, remain limited. We evaluated long-term outcomes and factors associated with adverse outcomes in children ≤24 months. METHODS: We retrospectively included consecutive patients ≤24 months who underwent balloon pulmonary valvuloplasty for critical or haemodynamically significant pulmonary stenosis between January 2004 and July 2024. Outcomes included significant restenosis, moderate-or-greate

Nox4 Mediates Diastolic Function in a Genetic Model of Pitx2 Haploinsufficiency.

Heart failure with preserved ejection fraction (HFpEF) commonly coexists with atrial fibrillation (AF), but shared mechanisms remain unclear. In this study, we hypothesized that Pitx2, a transcription factor located near the strongest genetic locus associated with AF in humans, increases susceptibility to HFpEF-like remodeling. We also sought to understand pathways that might be central to this increased risk. Male and female Pitx2+/- mice and wild-type littermates received 3-week subcutaneous osmotic pump infusion of saline or angiotensin II (Ang II; 500 ng/kg/min). Cardiac structure and func

Percutaneous Transaxillary Access for Large-Bore Cardiovascular Interventions: Evidence, Technique, and Complication Management.

Percutaneous transaxillary access is an established alternative to transfemoral large-bore arterial access for transcatheter structural heart interventions and mechanical circulatory support (MCS). Although useful for overcoming hostile iliofemoral anatomy and providing prolonged support and patient mobilization in case of MCS, the axillary/subclavian territory poses distinct technical and anatomical challenges that demand meticulous pre-procedural imaging, patient selection, accurate puncture under ultrasound and angiographic guidance, preservation of bailout access, and familiarity with sutu

Human vs Artificial Intelligence in Detecting Coronary Occlusions in ST-Elevation Myocardial Infarction: Retrospective Study.

BACKGROUND: Occlusion myocardial infarction (OMI) on the electrocardiogram (ECG) can present as ST-elevation myocardial infarction (STEMI), non-STEMI, ST depression, or other patterns (eg, de Winter T waves and hyperacute T waves), posing a diagnostic challenge. Even experienced physicians may overlook OMI on ECG, leading to delays in catheterization laboratory activation. Recently, smartphone apps using AI have been shown to exhibit promising accuracy in ECG interpretation in large datasets. OBJECTIVE: The objective of our study was to retrospectively compare the diagnostic performance of an

Late-onset bilateral pulmonary vein stenosis after living-donor lobar lung transplantation managed with balloon angioplasty: a case report.

BACKGROUND: Pulmonary vein stenosis (PVS) is a rare but potentially life-threatening complication after lung transplantation. Most reported cases occur during the early postoperative period and are associated with technical issues at the venous anastomosis. Late-onset PVS is extremely uncommon and may be overlooked because its clinical presentation mimics infection, rejection, or chronic lung allograft dysfunction (CLAD). CASE DESCRIPTION: A woman in her 20s with familial interstitial pneumonia underwent living-donor lobar lung transplantation (LDLLT). Her postoperative course was uneventful,

Life-Threatening Ventricular Tachycardia and Transient Brugada-Like Electrocardiographic Abnormalities Caused by Severe Hyperkalemia Due to Urinary Retention in a 55-Year-Old Man.

BACKGROUND Hyperkalemia is a potentially fatal electrolyte disturbance that typically manifests as progressive conduction abnormalities, bradyarrhythmias, and sine-wave electrocardiographic changes. In rare cases, however, severe hyperkalemia also presents with malignant ventricular tachyarrhythmias and transient electrocardiographic abnormalities involving the right precordial leads, potentially leading to diagnostic confusion with primary arrhythmic disorders. CASE REPORT A 55-year-old man with no known cardiovascular disease presented with palpitations, dyspnea, and diaphoresis. Electrocard

Complex Patent Foramen Ovale: Anatomic Variants, Technical Considerations, and Outcomes.

Patent foramen ovale (PFO) closure is a common intervention aimed at reducing recurrent stroke in patients with cryptogenic embolism. Complex PFO anatomies, characterized by features such as long tunnels, large defect sizes, atrial septal aneurysms, multifenestrated septa, prominent Eustachian valves or Chiari networks, and rigid or thickened septal tissue, pose significant challenges for successful device deployment and closure. Advances in imaging technologies, including transesophageal echocardiography, intracardiac echocardiography, and 3-dimensional imaging, have significantly enhanced th

Evaluating Standard Shielding Versus Novel Enhanced Radiation Protection Devices in the Cardiac Catheterization Laboratory.

BACKGROUND: Enhanced radiation protection devices (ERPDs) have the potential to reduce occupational risks in the cardiac catheterization laboratory. We aimed to investigate scatter radiation exposure to various cardiac catheterization laboratory members using 2 ERPDs. METHODS: We prospectively allocated cases to 3 different shielding conditions: standard shielding (group 1), EggNest Protect ERPD (group 2), and EggNest Complete ERPD (group 3). Real-time dosimetry was used to measure radiation exposure at 3 positions: primary operator (position 1), assistant (position 2), and circulating nurse

Association between the inferior vena cava collapsibility index and Fontan pressure in patients with Fontan physiology: a two-centre prospective pilot study.

INTRODUCTION: Patients with functional single ventricle anatomy undergo staged surgical palliation in early childhood culminating in the Fontan procedure whereby the inferior vena cava is connected to the pulmonary artery. Fontan circulation inherently requires elevated central venous pressure to maintain adequate pulmonary blood flow; however, excessive elevation is associated with morbidity and mortality. In adults, a key component of echocardiographic evaluation is the use of a "sniff test" as a surrogate of downstream right atrial pressure. This technique has not yet been validated as a me

Hemodynamic Characterization of a Circular Shunt After Arterial Switch in a Neonate to Guide Management Strategy.

A circular shunt is a complex and often critical condition in neonates after cardiac surgery. We report a case of a newborn with refractory hypoxemia and a circular shunt following an arterial switch operation. Cardiac catheterization provided a precise hemodynamic characterization and guided intervention. A trial balloon occlusion confirmed that closure would resolve the shunt, leading to successful device deployment and resolution of the patient's condition. Hemodynamic changes due to inadvertent partial pressure of arterial carbon dioxide (PaCO2) accumulation suggest that, in addition to fo

Quantitative Inferior T-Wave Inversion as a Marker of High Pulmonary Vascular Resistance and All-Cause Mortality in Patients Evaluated for Pulmonary Hypertension.

Right heart catheterization defines pulmonary hypertension (PH) hemodynamics, but simple markers of pulmonary vascular load remain useful. Inferior-lead T-wave inversion (TWI) is seen in right ventricular pressure overload; its association with pulmonary vascular resistance (PVR) and prognosis is uncertain. We retrospectively studied a cohort of 98 patients who underwent electrocardiography (ECG), echocardiography, and right heart catheterization within 48 h from 2021 to 2023. Inferior TWI amplitudes in leads II, III, and aVF were measured as absolute values and summed. Associations with PVR,

Prognostic Value of Pulmonary Elastance Methods in HFrEF: A Comparative Study.

BACKGROUND: Pulmonary vascular resistance (PVR) is commonly used to quantify right ventricular (RV) afterload in heart failure with reduced ejection fraction (HFrEF), yet it reflects primarily the steady component of pulmonary vascular load. Simplified pulmonary arterial elastance indices may better characterise the integrated arterial load experienced by the RV. The comparative prognostic value of different elastance formulations in HFrEF remains incompletely defined. METHOD: This retrospective study included 352 patients with HFrEF who underwent right heart catheterisation between 2018 and

Early-Onset Gastric Varices After Fontan Completion Associated with a Pre-Existing Gastro-Renal Shunt.

Gastroesophageal varices are recognized complications of Fontan-associated liver disease, but clinically significant varices have generally been reported more than 10 years after Fontan completion. The mechanisms of early variceal formation remain unclear. We report a child with tricuspid atresia who developed isolated gastric varices approximately 7 years after extracardiac Fontan completion in the presence of a pre-existing gastro-renal shunt (GRS). The patient underwent bidirectional Glenn palliation at 9 months of age. Contrast-enhanced computed tomography (CT) performed before Fontan comp

The Prognostic Value of Entropy from T1 Mapping in Pulmonary Arterial Hypertension.

BACKGROUND: The prognostic value of left ventricular (LV) entropy from T1 mapping in pulmonary arterial hypertension (PAH) remains unexplored. STUDY DESIGN AND METHODS: This prospective study enrolled patients with PAH who underwent cardiovascular magnetic resonance (CMR) within 72h of right heart catheterization (RHC). Myocardial native T1, extracellular volume fraction (ECV) and entropy were calculated in mid-ventricular short-axis slice. The primary endpoint was defined as all-cause mortality or lung transplantation. Outcomes were further analyzed across groups stratified by survival statu
