Intraprocedural Cardiac Arrest in Patients Undergoing Percutaneous Coronary Interventions in the United States: An Analysis of the NCDR CathPCI Registry.

BACKGROUND: Cardiac arrest (CA) during percutaneous coronary interventions (PCI) is a catastrophic procedural complication. Contemporary rates of CA during PCI are unknown. OBJECTIVES: This study sought to understand contemporary rates of CA, associated mortality, and identify predictors of CA during PCI. METHODS: Patients with CA during PCI from 2018 to 2023 were identified from the National Cardiovascular Data Registry CathPCI Registry. Trends in intraprocedural CA (IPCA) incidence and in-hospital death were evaluated. Predictors of IPCA and risk-adjusted mortality were identified. RESULT

Native Spleen T1 Mapping for the Assessment of Hepatic Congestion in Children With Congenital Heart Disease.

BACKGROUND: Congestive hepatopathy in congenital heart disease (CHD) can lead to portal hypertension. Liver native T1 relaxation time (LT1) was reported to be associated with elevated central venous pressure and liver injury in CHD. This study investigated the utility of spleen native T1 relaxation time (ST1) in children with CHD, particularly those with Fontan circulation, and its association with hemodynamic parameters and biomarkers of liver injury. METHODS AND RESULTS: This study included 120 patients with CHD (biventricular repair, 39; bidirectional Glenn shunt, 31; and Fontan circulatio

"Interstitial lung disease with associated severe pulmonary hypertension is rarely idiopathic by clinical or pathologic measures".

BACKGROUND: Interstitial lung disease (ILD) patients who develop severe pulmonary hypertension (PH) have rapidly progressive disease and increased mortality, but their clinical and pathologic phenotypes are poorly defined. Improved understanding and identification of these patients will expedite beneficial PH-targeted therapeutics and referral for lung transplantation. METHODS: We conducted a comprehensive retrospective study of a large cohort of ILD patients who underwent lung transplant at UCLA. Multidisciplinary Discussion (MDD) was used to establish ILD etiologies incorporating all availa

Single-view echocardiography-based deep learning for pulmonary hypertension detection: a multi-level supervised and explainable approach.

BACKGROUND: Early identification of pulmonary hypertension (PH), particularly mild disease, remains challenging. We aimed to develop and validate a deep learning (DL) model for noninvasive PH screening using single-view transthoracic echocardiography (TTE), without reliance on Doppler measurements. METHODS: In this retrospective study, apical four-chamber (A4C) TTE cine loops were collected between January 2023 and June 2025. A total of 144 patients with PH confirmed by right heart catheterization (RHC), 17 RHC-confirmed non-PH patients, and 95 non-PH controls defined by TTE under strict incl

[Pulmonary hypertension associated with interstitial lung diseases: current state of the art].

Pulmonary hypertension associated with interstitial lung diseases (PH-ILD) is a common and severe complication of interstitial lung diseases. Diagnosis is challenging and relies on a combination of clinical, functional, biological, radiological and echocardiographic findings. Definitive diagnosis is established by right heart catheterisation, which confirms the presence of pre-capillary pulmonary hypertension associated with increased pulmonary vascular resistance. Until 2021, and the INCREASE study, all randomised controlled trials evaluating therapies approved for pulmonary arterial hyperten

Temporary Left Ventricular Unloading With Impella as a Bridge to Emergency Surgery for Acute Mitral Bioprosthetic Failure.

Structural valve deterioration usually declares itself gradually, but a bioprosthetic leaflet that tears away from a stent post can produce catastrophic regurgitation without warning. In the mitral position, the left atrium has not had time to become compliant, and the resulting rise in pulmonary capillary pressure can flood the lungs within hours. A 68-year-old woman who had undergone mitral valve replacement with a bovine pericardial bioprosthesis 13 years earlier developed sudden dyspnea. Her oxygen saturation was 58%, the chest radiograph showed almost complete bilateral opacification, and

Association Between Micra Transcatheter Pacing System Curvature and Capture Threshold During Implantation.

BACKGROUND: No previous studies have examined the relationship between Micra transcatheter pacing system (TPS) curvature and acceptable thresholds during implantation. This study aimed to evaluate the association between Micra TPS curvature and pre-deployment capture thresholds. METHODS: The Micra TPS was successfully implanted with the assistance of right ventriculography in 180 patients. The curvature of the Micra TPS prior to deployment was analyzed using archived right anterior oblique (RAO) images from the first deployment, with the aid of Origin software (OriginLab Corporation, USA), wh

An integrated fluid-dynamic and structural pipeline to assess atrial hemodynamics before and after left atrial appendage occluder deployment in a patient-specific anatomy.

Atrial fibrillation is associated with a high risk of thromboembolic events, often mitigated by left atrial appendage occlusion (LAAO) when anticoagulation is contraindicated. This study presents a patient-informed computational workflow integrating pre-operative computational fluid dynamics (CFD), finite element (FE) modeling of device deployment, and post-operative CFD informed by the FE-deformed configuration. A proof-of-concept case was analyzed considering three atrial wall stiffness values to explore biomechanical variability. Pre-implant CFD simulations identified regions of blood stasi

Persistent Supraventricular Tachycardia Following Central Venous Catheter Insertion: When Withdrawal of the Mechanical Stimulus Is Not Enough.

Cardiac arrhythmias are a recognised complication of central venous catheterisation and are usually attributed to mechanical irritation of the endocardium by the guidewire or catheter tip. These arrhythmias typically resolve following withdrawal of the mechanical stimulus. We report a 42-year-old man with heart failure with reduced ejection fraction and diabetic ketoacidosis who developed an abrupt regular narrow-complex tachycardia at approximately 217 beats/min immediately following advancement of a right internal jugular central venous catheter. The arrhythmia was associated with transient

Robust Approach to Measure Time-Resolved Septal Curvature in Pediatric Pulmonary Hypertension Patients Using Standard Short-Axis Ventricular Volume Contours: A Retrospective, Cross-Sectional Study.

BACKGROUND: Right heart catheterization is the gold standard for diagnosis and monitoring of pediatric pulmonary hypertension (PH). Noninvasive cardiovascular magnetic resonance (CMR) and echocardiography can additionally provide volumetric and functional assessment of the right ventricle and possibly serve as a non-invasive method for surveillance of PH severity using ventricular septal curvature (SC), interventricular septal (IVS) angle, or eccentricity index (EI). However, these septal wall measurements require manual interaction and can be observer dependent. This study presents the result

Estimation of Endothelin Levels in Chronic Obstructive Pulmonary Disease Patients with Pulmonary Hypertension: A Clinical Insight.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is a major global health issue, with pulmonary hypertension (PH) as a severe complication linked to increased morbidity and mortality. Endothelin-1 (ET-1) is a vasoconstrictor peptide and a potential biomarker for PH in COPD due to its role in vascular remodeling and hemodynamic stress. OBJECTIVES: This study aimed to assess serum ET-1 levels in COPD patients with and without PH to evaluate its correlation with hemodynamic and metabolic parameters and evaluate its diagnostic potential. METHODOLOGY: A cross-sectional study enrolled 90 s

Left ventricular transmural pressure and ventilatory inefficiency during exercise in advanced heart failure.

BACKGROUND: Baseline intracavitary haemodynamic measurements frequently demonstrate an inconsistent relationship with exertional performance. This may reflect an incomplete interpretation of left ventricular (LV) filling pressures. LV transmural pressure difference (ΔPTM = LV end-diastolic pressure [LVEDP]-right atrial pressure [RAP]) integrates LV filling pressure with right ventricular/pericardial constraint and may provide a composite representation of resting haemodynamic conditions. Ventilatory efficiency, assessed by the minute ventilation-carbon dioxide production (VE/VCO2) slope during

Elevated plasma eosinophil-derived neurotoxin is associated with echocardiographic pulmonary vascular involvement in stable COPD.

BACKGROUND: Pulmonary vascular involvement is an important dimension of chronic obstructive pulmonary disease (COPD) heterogeneity, but its biological features may not be fully represented by airflow limitation, gas-transfer impairment, or resting hypoxemia. eosinophil-derived neurotoxin (EDN) is a circulating eosinophil-derived protein whose relationship with pulmonary vascular involvement in stable COPD is unknown. METHODS: This single-center observational cohort study included 53 clinically stable participants with COPD and 10 healthy controls. COPD participants were classified by echocard

Functional Recovery Patterns and Baseline Cardiac Power Output During Impella 5.5 Support in Heart Transplant Candidates: A Descriptive Study.

BACKGROUND: The Impella 5.5 microaxial pump facilitates ambulatory mechanical circulatory support via axillary implantation, providing a model for active prehabilitation in critically ill heart transplant candidates. Whether patients with severely reduced baseline cardiac power output (CPO) can achieve meaningful ambulation during Impella 5.5 support remains uncertain. METHODS: This is a retrospective descriptive study of consecutive adults with cardiogenic shock (SCAI stages C-E) who received an Impella 5.5 as a bridge to heart transplantation between April 2022 and September 2024. Pulmonary Artery Denervation for Persistent Pulmonary Hypertension After Transcatheter Mitral Edge-to-Edge Repair: A Pilot Study and First Application in the Post M-TEER Setting.

Pulmonary hypertension (PH) is common in patients undergoing transcatheter mitral edge-to-edge repair (M-TEER) and is an established determinant of adverse outcomes after intervention. Importantly, persistent PH after technically successful M-TEER remains associated with ongoing symptoms, right ventricular dysfunction and poor prognosis, yet no dedicated treatment strategy has been established. The aim of this study is to evaluate the safety and efficacy of pulmonary artery denervation (PADN) in patients with persistent PH following successful M-TEER. Six patients (median age 69.0 years; 1 fem

Diagnostic performance of artificial intelligence-enabled electrocardiography for pulmonary hypertension: a systematic review and meta-analysis.

Pulmonary hypertension (PH) is a progressive, under-recognized condition with substantial morbidity and mortality. Its non-specific presentation and multimodal diagnostic pathway contribute to delayed diagnosis. Artificial intelligence (AI)-enabled electrocardiography (ECG) may offer a scalable approach to case finding. This PRISMA-DTA systematic review and meta-analysis was registered in PROSPERO (CRD420251241142). MEDLINE, Embase, Web of Science, Scopus, and IEEE Xplore were searched through July 2026, with Google Scholar and citation searching used as supplementary sources. Eligible studies
