BACKGROUND: Chronic kidney disease (CKD) is characterized by low levels of the anti-aging protein α-Klotho and accelerated cardiovascular (CV) morbidity. Short-term treatment with growth hormone (GH) was shown to enhance soluble Klotho (sKlotho), the circulating form of α-Klotho, and endothelial function in patients with CKD. We hypothesized that long-term GH treatment in pediatric patients with CKD improves sKlotho levels and CV morbidity.
METHODS: We performed a case-cohort study within the Cardiovascular Comorbidity in Children with Chronic Kidney Disease (4C) study including 101 children

BACKGROUND: While obesity confers a survival advantage, weight loss adversely affects the survival of patients undergoing haemodialysis. However, given the limited information regarding its long-term effects on mortality and cardiovascular events, the health benefits of weight gain remain uncertain, particularly in Asian patients undergoing haemodialysis.
METHODS: In a prospective multicentre cohort of patients undergoing haemodialysis in South Korea, patients whose dry weight was recorded at baseline and after 1 year were analysed. Patients were stratified into five groups according to annua

Hepatic osteodystrophy (HOD) is a metabolic bone disorder associated with chronic liver disease (CLD), marked by disrupted bone remodelling, reduced mineralization, and altered osteoblast-osteoclast dynamics. Despite its clinical relevance, mechanistic understanding of the liver-bone axis remains limited due to the shortcomings of conventional in-vitro and in-vivo models in capturing inter-organ crosstalk. To address this, we developed dual-organ perfusion-based micro-physiological devices (MPDs) that integrates human-derived liver and bone tissues-like scaffolds enabling unidirectional perfus

Hepato-renal crosstalk is a complex biological communication between liver and kidneys mediated by various factors, including cellular, endocrine, and paracrine molecules. This interaction highlights the functional consequences that damage in one organ can have on the other. In particular, the liver and kidney play a pivotal role in maintaining body homeostasis, as they are both involved in the excretion of toxic bioproducts and drugs. The overlap of liver and kidney disease has both therapeutic and prognostic implications. Therefore, a better understanding of the mechanisms involved in the pa

Despite the efficacy of hemoperfusion in bilirubin elimination, conventional adsorbents face inherent limitations in simultaneously optimizing the adsorption capacity and biocompatibility. Similarly, while hemodialysis demonstrates effective clearance of small-molecule toxins, including unconjugated bilirubin, its therapeutic efficiency for BSA-conjugated bilirubin remains suboptimal due to molecular binding characteristics. In this study, collagen was utilized as the core material, enabling effective binding between its amino and carboxyl groups and bilirubin through electrostatic interaction

BACKGROUND: The prognostic utility of quantitative positron emission tomography (PET) with myocardial blood flow (MBF) measurements in patients with prior coronary artery bypass grafting remains unestablished. We evaluated PET-derived myocardial flow reserve (MFR) and coronary vascular resistance (CVR) for risk stratification in patients who have undergone coronary artery pass grafting.
METHODS: This retrospective study included consecutive patients undergoing Rubidium-82 PET myocardial perfusion imaging between May 2017 and November 2023. MFR and CVR were defined as the ratio of stress to re

Aspergillus mastoiditis with skull base osteomyelitis is an exceptionally rare but life-threatening infection in patients undergoing peritoneal dialysis (PD). We report a 60-year-old man with diabetic kidney failure on PD who developed invasive Aspergillus flavus mastoiditis progressing to skull base osteomyelitis. Despite early mastoidectomy, PD catheter removal, and voriconazole therapy achieving therapeutic levels, the infection advanced to involve the carotid sheath and parapharyngeal space, resulting in severe neurologic and systemic complications. Molecular sequencing confirmed A. flavus

Essential hypertension represents the main risk factor for coronary events, including acute myocardial infarction, coronary syndromes, and unstable angina, as well as for chronic coronary artery disease (CAD). It also increases the risk of peripheral artery disease and chronic kidney disease, thus contributing to worsen prognosis, deteriorating quality of life, favouring unplanned hospitalizations due to cardiovascular (CV) causes, and affecting elevated costs for health care systems. Moreover, hypertension may contribute to development and progression of left ventricular (LV) remodelling and

BACKGROUND: Coronary artery disease (CAD) is a major contributor to cardiovascular events in individuals with diabetes. Quantification of coronary atherosclerotic burden is now feasible from coronary computed tomography angiography (CTA) whereas positron emission tomography (PET) enables quantitative assessment of myocardial perfusion. We studied the prognostic implications of quantitatively measured coronary plaque burden and myocardial perfusion in diabetic vs. non-diabetic patients with suspected CAD.
METHODS: In this observational cohort study, 1311 symptomatic patients with suspected CAD

BACKGROUND: Chronic kidney disease (CKD) is associated with a persistent inflammatory state that contributes to adverse outcomes. Biomarkers such as interleukin-6 (IL-6), C-reactive protein (CRP), and high-sensitivity CRP (hs-CRP) have been linked to mortality and cardiovascular events, but the consistency and clinical relevance of these associations remain unclear.
MATERIALS AND METHODS: We performed a systematic review and meta-analysis of 81 studies, with a median follow-up duration of 3.0 years (IQR 2.0-4.2), following the PRISMA checklist and a PROSPERO-registered protocol (CRD4202562159

INTRODUCTION: Osteomalacia is an often-overlooked manifestation of post-transplant bone disease that may persist or newly develop in kidney transplant recipients because of pre-existing chronic kidney disease-mineral and bone disorder, ongoing immunosuppression, and alterations in calcium-phosphate metabolism. Severe vitamin D deficiency, hypophosphatemia, and secondary hyperparathyroidism create a metabolic milieu that favors osteoid mineralization defect and leads to debilitating skeletal pain and fragility fractures.
OBJECTIVE: This case report documents the clinical course, diagnostic wor

BACKGROUND: Fluid monitoring is critical for patients on maintenance hemodialysis. Bioimpedance enables estimation of fluid volumes from measures of electrical tissue properties. However, empirical equations are needed to approximate key variables, especially in wrist-to-ankle bioimpedance measurements, introducing potential errors.
SUMMARY: Here, we provide a technical overview of electrical impedance, derivation of fluid volumes from different bioimpedance methods and electrode setups, as well as sources of error including the assumption of constant resistivity, constant body temperature, a

BACKGROUND: Chronic Kidney Disease (CKD) complications, like cardiovascular complications, are one of the leading causes of mortality. Managing the biochemical profile is essential in slowing the progression of CKD and its associated consequences. This study aimed to assess the effect of cinacalcet add-on therapy on clinical outcomes in patients with End-Stage Renal Disease (ESRD) on Hemodialysis (HD) who developed Secondary Hyperparathyroidism and are receiving hemodialysis. METHOD: A mixed retrospective-prospective cohort multicenter study was conducted in three hospitals in Saudi Arabia bet

Encapsulating peritoneal sclerosis (EPS) is a rare condition characterised by recurrent episodes of intestinal obstruction and perforation, usually due to prolonged peritoneal dialysis exposure, resulting in abdominal catastrophes often requiring open abdomen management (OAM). Dynamic negative pressure wound therapies (NPWTs) can facilitate definitive closure in the open abdomen, but consensus recommendations lack high-quality, cohort-specific data. We performed a retrospective analysis on the use of NPWTin this patient cohort to assess its effectiveness when compared to management with static
