HLA-A29-Positive Birdshot Chorioretinopathy in Egyptian Patients.

PURPOSE: To report the first HLA-A29‑positive Birdshot Chorioretinopathy (BSCR) patients from Egypt. METHODS: Two patients, from 2 separate cities along the Egyptian Nile Valley, were found to have chorioretinopathy with characteristic "birdshot" pale ill-defined choroidal lesions arranged in a radial fashion emerging from the optic disc, with a relatively quiet anterior segment. On OCT, both patients showed varying degrees of photoreceptor disruption and choroidal structural changes related to long-standing inflammation. RESULTS: HLA-A29 testing was positive in both patients. Both patients

Presumed ocular tuberculosis presenting as a subfoveal choroidal granuloma in a healthcare professional: a case report.

BACKGROUND: Ocular tuberculosis (OTB) is a great masquerader and may occur without pulmonary symptoms, making its diagnosis challenging. In endemic regions like Malaysia, clinicians should consider OTB in patients with unexplained posterior segment ocular lesions. This report describes presumptive OTB with underlying tuberculous lymphadenitis in a healthcare professional; however, the source of tuberculosis (TB) exposure could not be established. CASE DESCRIPTION: A 36-year-old primary care physician with 12 years of clinical experience presented with a 1-day history of sudden wavy line visio

[Tuberculous granuloma of the iris: a case report].

A 29-year-old man presented to Department of Ophthalmology, Affiliated Hospital of Nantong University with a 14-day history of redness and decreased vision in the right eye. Uncorrected visual acuity in the right eye was 0.02 (not improvable with correction). Slit-lamp examination revealed mutton-fat keratic precipitates on the corneal endothelium, anterior chamber inflammation, and two iris masses. Systemic evaluation suggested cervical lymph node tuberculosis, and empirical anti-tuberculosis therapy was initiated. After one month of treatment, the original iris lesion regressed, but new nodu

Masquerade Syndrome Presenting as Optic Neuritis and Retinitis Due to Pituitary Germinoma.

Masquerade syndrome is a collective term for various ocular diseases presenting with clinical features resembling intraocular inflammation. Delays in definitive diagnosis and treatment can not only lead to irreversible vision loss but also impact the patient's prognosis regarding survival. We report the case of a 26-year-old male patient who presented with impaired vision in the right eye, which had persisted for six months; neuroretinitis was diagnosed. Although infectious neuroretinitis was suspected, tests for pathogens such as Mycobacterium tuberculosis, Toxoplasma, Toxocara, and Bartonell

Tubercular Uveitis: Multimodal Imaging, Immunologic Stratification, and a Practical Framework for Diagnosis and Management.

Tubercular uveitis (TBU) remains difficult to diagnose and monitor because microbiologic confirmation is uncommon and clinical phenotypes overlap with other inflammatory chorioretinal disorders. This narrative review integrates evidence from multimodal imaging, immune testing, systemic tuberculosis evaluation, molecular diagnostics, biomarkers, treatment response, and monitoring to support a practical probability-based approach to suspected TBU. A targeted search of PubMed, Embase, Web of Science, the Cochrane Library, and Google Scholar was conducted for English-language literature through Ma

Gastric cytomegalovirus disease mimicking locally advanced gastric adenocarcinoma in a patient with newly diagnosed AIDS: A case report and literature review.

INTRODUCTION: Gastric cytomegalovirus (CMV) disease is an uncommon manifestation of tissue-invasive CMV infection and, in rare instances, may mimic gastric malignancy. Recognition of this pseudomalignant presentation is important to prevent misdiagnosis and inappropriate oncologic management. CASE PRESENTATION: A 65-year-old woman presented with abdominal pain, diarrhea, recurrent post-prandial vomiting, poor oral tolerance, and a 20-kg weight loss over one year. Upper gastrointestinal endoscopy showed a large ulcerated antral mass with raised borders, highly suspicious for gastric adenocarci

Nocturnal behaviours, stridor and recurrent respiratory failure in anti-IgLON5 disease.

Anti-IgLON5 disease is a rare autoimmune encephalopathy with concurrent tauopathy, characterised by sleep dysfunction and brainstem involvement. A 77-year-old woman developed obstructive sleep apnoea and recurrent hypercapnic respiratory failure requiring repeated intubation, initially misdiagnosed as epilepsy. Video recordings captured elaborate nocturnal behaviours, including praying and offering alms during sleep, alongside inspiratory stridor and sleep attacks during eating. Serial polysomnography documented progressive loss of sleep spindles and undifferentiated non-rapid eye movement sle

The Spectrum of Subretinal Abscess: Insights from a 15-Year Single‑Center Experience.

PURPOSE: To report the clinical outcomes and microbiological spectrum of subretinal abscess (SRA) over a 15-year period at a tertiary eye care center in South India. METHODS: Retrospective review of electronic medical records at a tertiary eye care institute for patients clinically diagnosed with SRA from 2009 to 2024. SRA was classified as tubercular (TB-SRA) and non-tubercular (non-TB SRA) based on clinical and microbiological evidence. RESULTS: Forty-three patients (50 eyes) were included (83.7% male; 16.3% bilateral); 15 patients (34.9%) had TB-SRA and 28 (65.1%) had non-TB SRA. There wa

Myelin oligodendrocyte glycoprotein-antibody disease (MOGAD) with leukodystrophy-like presentation.

Myelin oligodendrocyte glycoprotein-antibody disease (MOGAD) is an immune-mediated demyelinating disorder, distinct from MS, which typically affects the optic nerve, spinal cord, brain and/or brainstem. Diffuse white matter involvement that resembles a leukodystrophy has been reported only in children. We present a woman aged 29 years with new right optic neuritis on a background of previously unexplained, longstanding, bilateral, reduced visual acuity. MR scan of the brain and orbits showed confluent, bilateral, posterior predominant white matter changes reminiscent of a leukodystrophy, along

Epithelial Keratitis Secondary to Presumed Ocular Tuberculosis: A Report of Two Cases.

This report presents two rare cases of presumed epithelial keratitis secondary to ocular tuberculosis (TB), highlighting an unusual clinical presentation that may be underrecognized. Two male patients from a TB-endemic region presented with unilateral, chronic epithelial keratitis in a semicircular pattern, with mild anterior stromal infiltration and reduced corneal sensitivity. Both were unresponsive to standard topical and systemic treatments including antivirals and corticosteroids. Diagnostic evaluation included Mantoux testing and systemic screening. Mantoux tests were positive in both pa

Associations Between Prior Lung Diseases and Risk of Lung Cancer in Populations With No Smoking History: A Systematic Review and Meta-Analysis.

BACKGROUND: Individuals who do not smoke account for a growing proportion of lung cancer (LC) incidence, but the impact of prior lung diseases on lung cancer risk in this population remains unclear. RESEARCH QUESTION: What is the association of TB, asthma, and chronic bronchitis (CB) with lung cancer risk among individuals who do not smoke? STUDY DESIGN AND METHODS: PubMed and Embase were searched from inception through July 15, 2025, for studies examining the association between TB, asthma, or CB with lung cancer risk in populations older than 18 years who never smoked. Tuberculosis Masquerading as Behcet's Disease-Pseudo Bechet's Syndrome: A Case-Based Review of Literature.

Behcet's disease is a chronic, multisystem variable vessel vasculitis characterised by recurrent oral and genital ulcers, ocular inflammation and a wide range of systemic manifestations. Pseudo-Behcet's syndrome refers to a condition that mimics these clinical features but arises from distinct etiologies. We present a case of a 33-year-old male with year-long recurrent oral ulcers and intermittent abdominal pain, followed by scrotal ulcers, severe fatigue, weight loss and appetite loss. Chest imaging demonstrated bilateral consolidations with cavitation and right-sided pleural effusion, prompt

Management of adverse events in TB care and active TB drug safety monitoring.

Tuberculosis (TB) remains the leading cause of death from a single infectious agent, with 10.8 million cases reported in 2023. While treatment is generally effective, both drug-susceptible (DS-TB) and drug-resistant TB (DR-TB) regimens are associated with adverse events (AEs) that compromise adherence and outcomes. In this viewpoint, we highlight the most clinically relevant AEs: hepatotoxicity, cutaneous reactions and ocular toxicity in DS-TB; and linezolid-associated neuropathy, myelosuppression, QT prolongation and hepatotoxicity in DR-TB. We argue that structured safety monitoring and inte

Multidisciplinary management of thyroid eye disease (TED) in patients with latent tuberculosis or chronic hepatitis B.

BACKGROUND: Intravenous methylprednisolone (IVMP) is a first-line treatment for active moderate-severe or sight-threatening thyroid eye disease (TED), but the presence of systemic infections such as latent tuberculosis (TB) or chronic hepatitis B virus (HBV) act as relative contraindications due to the risk of reactivation. There is limited data to guide clinicians in this high-risk group. METHODS: A retrospective case series was conducted involving five TED patients with relative contraindications to immunosuppression. Each patient included had a EUGOGO severity of moderate-to-severe. Multid

Postmarketing adverse events of biologics and Janus kinase inhibitors in patients with atopic dermatitis.

BACKGROUND: New targeted drugs, including biologics and Janus kinase inhibitors (JAKi), have been approved for the treatment of atopic dermatitis (AD). Given their widespread clinical use, a comprehensive real-world study of their adverse events (AEs) is warranted. OBJECTIVES: This study aims to characterize of new targeted drugs related AEs, and to compare the AE profiles of biologics and JAKi. METHODS: Disproportionality metrics, including the reporting odds ratio, proportional reporting ratio, information component, and empirical Bayesian geometric mean, were employed to determine AE sign
