Challenges in Diagnosing Necrotizing Sarcoid Granulomatosis: The First Case Reported From Indonesia.

Necrotizing sarcoid granulomatosis (NSG) is a rare disease characterized by granulomatous and necrotic features as well as vasculitis, and it primarily affects the lungs, with occasional extrapulmonary manifestations. The first documented case was in Jakarta, Indonesia. A 71-year-old male presented with prolonged fever, a neck mass, and multiple mediastinal lymphadenopathy. The disease was initially suspected as lung tuberculosis, but a surgical biopsy of the left thyroid lobe confirmed the NSG pattern. Treatment with oral prednisolone led to positive outcomes, as evidenced by radiological imp

Establishment and evaluation of a naked-eye diagnostic assay for tuberculosis utilizing reverse isothermal amplification-assisted CRISPR-Cas in resource-limited settings.

INTRODUCTION: The current scenario of tuberculosis (TB) caused by Mycobacterium tuberculosis (MTB) has presented an almost insurmountable challenge to hospitals with high patient numbers. Delayed diagnosis of TB is a major hurdle in preventing the employment of efficient therapeutics, leading to the development of drug resistance. Hence, an easily accessible diagnostic method, particularly for resource for resource-limited settings, is pertinent for the rapid identification of MTB-infected patients. In pursuit of developing such an assay, the present study offers a CLAP-TB (CRISPR-Cas coupled

Antigen-specific polyfunctional cytotoxic T cells differentiate intraocular from peripheral blood immune responses in posterior uveitis.

OBJECTIVES: Peripheral blood is frequently used to study the immune response in human uveitis because of the inaccessibility of ocular tissue samples. To determine whether peripheral blood immune cells accurately reflect the intraocular immune response, we compared the T-cell profiles and antigen-specific cytokine responses between paired vitreous and peripheral blood samples from patients with sight-threatening posterior uveitis. METHODS: We collected paired vitreous and peripheral blood mononuclear cells (PBMCs) from 24 patients with posterior uveitis. Multi-parametric flow cytometry was em

Rapid detection of brucellosis by a two-signal vertical flow immunoassay based on sea urchin-like gold nanoparticles.

Brucellosis, a chronic zoonotic disease caused by Brucella, poses a significant threat to global public health and safety. While vertical flow immunoassay (VFI) is an effective tool for detecting brucellosis, its low sensitivity and poor quantitative ability significantly limit its application in brucellosis detection. Therefore, in this study, a dual-mode signal output technique was designed for rapid detection of brucellosis using the photothermal properties of sea urchin-like gold particles (ULGNs). ULGNs were prepared using the hydroquinone reduction method, and their localized surface pla

Validation of the COTS Calculator for Tubercular Uveitis: Predictive Performance and Diagnostic Utility in an Indian Cohort.

PURPOSE: Diagnosing ocular tuberculosis (OTB) is clinically challenging due to its paucibacillary nature and lack of definitive diagnostic tests. The Collaborative Ocular Tuberculosis Study (COTS) Calculator was developed as a clinical decision-support tool to guide anti-tubercular therapy (ATT) initiation. This study externally validates the COTS Calculator in a high-burden Indian cohort and assesses the additive value of radiological and immunological testing. METHODS: This retrospective cohort study included 196 OTB patients treated between 2015 and 2022 at a tertiary eye care center in So

Exploiting induced pluripotent stem cell-derived retinal pigment epithelium to unravel host-pathogen interaction in ocular tuberculosis: a reverse translational in vitro model.

Mycobacterium tuberculosis (Mtb) can infect the retinal pigment epithelium (RPE) cells. Current in vitro research models for ocular tuberculosis (OTB) only rely on RPE cell culture approaches. Until now it remains unclear why only a minority of patients with active systemic tuberculosis (TB) develops concurrent OTB. There is significant variation in the clinical manifestations of OTB, which is potentially influenced by ethnic differences and diversity in mycobacterial strains. To better understand the immunopathobiology of OTB, particularly an individual's susceptibility to Mtb-infection and t

Current concepts in the diagnosis of ocular tuberculosis: A narrative review.

Ocular tuberculosis (OTB) is one of the extrapulmonary manifestations caused by Mycobacterium tuberculosis (Mtb). If untreated, it can result in poor visual prognosis. Prompt diagnosis of OTB is met with challenges. The gold standard for the diagnosis of OTB is the direct demonstration of the Mtb in ocular tissues or ocular fluids either by Ziehl-Neelsen Stain, culture or molecular diagnostic techniques such as polymerase chain reaction. This is onerous owing to the paucibacillary nature of the disease, small quantity of samples, and low sensitivity and specificity of molecular diagnostic test

Specific retinal vasculitis in HLA-A29 birdshot retinochoroiditis, distinguishing it from other causes of vasculitis: A pilot study.

PURPOSE: Human leukocyte antigen-A29 (HLA-A29) birdshot retinochoroiditis (BRC) is a noninfectious uveitis affecting independently the retina and the choroid. While the choroidal involvement is characterized by a stromal choroiditis, the retinal features of BRC were less well defined until a recent study determined a specific pattern of retinal involvement with seven features including profuse posterior retinal leakage, thick fluorescein angiography sheathing/staining, profuse disc hyperfluorescence, macular edema with foveal sparing, and circulatory arteriovenous pseudo delay. The aim of this

Tuberculous endogenous endophthalmitis presenting acute anterior uveitis with hypopyon.

A 27-year-old male with pain, redness, and vision loss in the right eye was diagnosed with acute anterior uveitis with hypopyon and treated with topical corticosteroid and cycloplegic eyedrops. Two weeks later, he developed severe vitritis. Then, after 1 month of oral corticosteroid treatment, his condition did not improve; therefore, he underwent pars plana vitrectomy with intravitreal antimicrobial injection in the right eye with suspicion of endogenous endophthalmitis. Polymerase chain reaction (PCR) from vitreous aspirate were negative for herpes viruses, eubacterial and panfungal genomes.

Visual Recovery Following Linezolid Cessation in an MDR-TB Patient: Detailed Case Analysis.

Multidrug-resistant tuberculosis (MDR-TB) is characterized by resistance to at least isoniazid and rifampicin. Linezolid is an antibiotic used for drug-resistant Gram-positive bacteria and is a treatment option for MDR-TB. However, its use is associated with optic neuropathy, presenting as acute worsening and bilateral vision loss, typically within 4 months of therapy. A 47-year-old male with MDR-TB relapsed during the sixth month of an individualized treatment regimen at Dr. Soetomo General Academic Hospital, Surabaya. The patient presented with weakness and anemia, receiving a regimen includ

Tuberculosis as a significant cause of uveitis-related blindness: current referral trends at a tertiary uveitis center in Indonesia.

OBJECTIVES: Uveitis may lead to blindness if improperly treated, yet the data on blindness due to uveitis, particularly, in low-resource countries, remain limited. In high-tuberculosis (TB) burden countries such as Indonesia, active systemic TB is among the leading causes of infectious uveitis. The combined impact of blindness, uveitis, and TB presents a substantial health and socioeconomic burden. This study aimed to assess the current situation of uveitis referrals, particularly, on the proportion of uveitis with active systemic TB and the proportion of blindness in TB-related uveitis. METH

A naked-eye biosensing system based on one-pot RPA-CRISPR/Cas12a driver G4-hemin self-assembly for Mycobacterium tuberculosis.

INTRODUCTION: The rapid and accurate identification of Mycobacterium tuberculosis (MTB) is essential for effective tuberculosis (TB) control. However, conventional diagnostic methods for MTB suffer from limitations such as low sensitivity, poor specificity, high cost, reliance on specialized instruments, and complex, time-consuming procedures. To address these challenges, there is an urgent need for a simple, rapid, and highly sensitive detection method that can be deployed in point-of-care settings. METHODS: We developed a one-pot biosensing system combining recombinase polymerase amplificat

Manipulating Zika virus RNA tertiary structure for developing tissue-specific attenuated vaccines.

Traditional live attenuated vaccines (LAVs) are typically developed through serial passaging or genetic engineering to introduce specific mutations or deletions. While viral RNA secondary or tertiary structures have been well-documented for their multiple functions, including binding with specific host proteins, their potential for LAV design remains largely unexplored. Herein, using Zika virus (ZIKV) as a model, we demonstrate that targeted disruption of the primary sequence or tertiary structure of a specific viral RNA element responsible for Musashi-1 (MSI1) binding leads to a tissue-specif
