Slow Transit Constipation: Pathophysiological Perspectives and Management Updates.

Slow transit constipation (STC) is a chronic colonic motility disorder characterized by markedly delayed transit, leading to reduced bowel movements, abdominal discomfort, and significant quality-of-life impairment. It predominantly affects women and is associated with abnormalities in enteric neuronal signaling, smooth muscle contractility, interstitial cells of Cajal, gut peptides, bile acid homeostasis, and autonomic regulation. Secondary causes of constipation and structural lesions must be excluded before the diagnosis of STC, with colonic transit studies serving as the gold standard. Com

Histopathological and Molecular Features of Adenomatous and Serrated Colon Adenomas, Characteristics and Overlapping Features, Challenges in the Nomenclature.

OBJECTIVE: Colorectal cancer develops from precursor epithelial polyps, including tubular adenomas, villous or tubulovillous adenomas, sessile serrated lesions, and traditional serrated adenomas. Although diagnostic criteria for these lesions have been established, overlapping features often complicate classification. This study aimed to define the characteristics and shared features of colorectal adenomas. MATERIAL AND METHODS: A total of 140 adenomas were evaluated, comprising 71 conventional adenomas, 34 sessile serrated lesions, and 35 traditional serrated adenomas. Hematoxylin and eosin-

SegFormer-based boundary-aware polyp segmentation with adaptive multi-branch fusion.

Accurate definition of colorectal polyps for the prevention of cancer is challenging due to high variability in appearance and indefinite boundaries. In this work, we present a SegFormer-aided framework comprising a transformer encoder (SegFormer-B4) and a multi-branch fusion head in low-, high-, and all-level scales. Both tasks (edges and segmentation) are predicted by each branch and task-specific, learnable softmax weights then fuse branch logits. The framework is optimized under a composite objective comprising (i) deeply supervised soft dice similarity coefficient (DSC) loss for region ov

Integrated Single-Cell and Spatial Profiling of MMP Gene Expression in Colorectal Cancer.

Increased matrix metalloproteinase (MMP) expression has long been recognized as a common feature of colorectal cancers (CRCs), yet less is known about how these enzymes interact to impact cancer progression. Taking advantage of single-cell and spatial transcriptomic data, we analyzed the cell-type-specific and spatial expression of MMPs in CRCs. Distinct colon cancer-associated fibroblast (CAF) subtypes were found to express different MMP combinations, including MMP1/3-expressing and MMP11-expressing CAFs. Conversely, myeloid cells (monocytes, macrophages, and dendritic cells) expressed varyin

Juvenile polyposis in a SMAD4-mutated child: A call for early surveillance.

We report the case of a 10-year-old boy with hereditary hemorrhagic telangiectasia (HHT) and a family history of SMAD4-related juvenile polyposis syndrome (JPS), presenting with hypoferritinaemia unresponsive to oral supplementation. Endoscopic evaluation revealed multiple gastrointestinal polyps, including duodenal, gastric, and colonic lesions. Histology confirmed a tubular adenoma with low-grade dysplasia. Genetic analysis identified a heterozygous pathogenic SMAD4 variant (c.1549_1550del), confirming the diagnosis of JPS. This case illustrates the early onset and clinical complexity of SMA

Polymorphic EBV-positive post-transplant lymphoproliferative disorder of the colon mimicking EBV-positive mucocutaneous ulcer: a case report.

BACKGROUND: Epstein-Barr virus (EBV)-driven lymphoproliferative disorders (LPDs) are important complications of post-transplant immunosuppression. EBV-positive mucocutaneous ulcer (EBV-MCU) is typically a localized, solitary, sharply circumscribed ulcer with an indolent clinical course, whereas post-transplant lymphoproliferative disorder (PTLD) may be multifocal, disseminated, and life-threatening; however, these entities can overlap histologically on limited gastrointestinal biopsies. CASE PRESENTATION: We report a 60-year-old woman with autoimmune hepatitis/primary sclerosing cholangitis (

Integrative Transcriptomic and Proteomic Profiling Identifies S100P as a Potential Functional Biomarker for Sessile Serrated Lesions.

BACKGROUND: Sessile serrated lesions (SSLs) account for 15% of colorectal cancers (CRCs) but detection remains difficult due to flat morphology, mucinous features, and subtle histology. AIMS: This study aimed to identify novel and functionally relevant biomarkers of SSLs using transcriptomic screening and multi-omics validation. METHODS: Paired SSL and normal mucosa specimens (n = 6) underwent RNA sequencing. Differentially expressed genes (DEGs) were filtered for membrane or secretory proteins and validated across TCGA and adenoma transcriptomes. Functional significance was assessed using C

Optimizing bowel preparation regimens for colon capsule endoscopy: an umbrella review (overview of systematic reviews).

BACKGROUND: Colon capsule endoscopy (CCE) is a minimally invasive alternative to colonoscopy, but its diagnostic performance depends entirely on adequate mucosal cleansing (adequate cleansing rate, ACR) and complete colonic transit (completion rate, CR). These requirements impose stringent preparation demands. Existing systematic reviews (SRs) show substantial heterogeneity in recommendations. OBJECTIVES: To identify optimal CCE preparation strategies. DESIGN: Umbrella review (overview of SRs). METHODS: A comprehensive literature search was conducted through November 2025 for SRs and meta-a

Non-keratinizing immature squamous metaplasia in the pancreas mimicking malignancy: a diagnostic pitfall in cytology.

This report describes a challenging case in which atypical immature squamous metaplasia was misinterpreted as malignancy. A 69-year-old man presented with abdominal pain and loss of appetite. Imaging revealed mild pancreatic duct dilation, parenchymal enlargement, and increased fat attenuation in the transverse mesocolon. Endoscopic ultrasound revealed a hypoechoic lesion in the pancreatic body. The serum amylase level was markedly elevated (1,785 U/L), consistent with acute pancreatitis. Repeated pancreatic juice cytology examinations demonstrated atypical epithelial clusters, which raised co

Imaging manifestations of malignancy in inflammatory bowel disease: patterns, pitfalls and pearls.

Patients with inflammatory bowel disease (IBD) are at increased risk of gastrointestinal and hepatobiliary malignancies arising in the setting of chronic inflammation, dysplasia, stricturing disease, and fistulizing complications. Although endoscopic surveillance remains the cornerstone for dysplasia detection, imaging plays an increasingly important complementary role in identifying invasive malignancy, evaluating transmural and extra-intestinal disease, and detecting lesions in segments inaccessible or poorly amenable to endoscopic assessment. Distinguishing malignant transformation from chr

Inflammatory bowel disease-associated colorectal dysplasia: the expanding morphologic landscape and diagnostic challenges.

Although most dysplastic lesions in inflammatory bowel disease (IBD) resemble sporadic adenomas, several less well-recognized morphologic variants have recently been described. These variants can be diagnostically challenging because of their subtle cytologic atypia and/or overlap with regenerative changes, often resulting in misclassification as benign, reactive, or indefinite for dysplasia. Furthermore, the use of different classification systems has led to inconsistent terminology and definitions across the literature and in clinical practice. Nevertheless, accurate recognition of these les

Concurrent Intestinal Tuberculosis and Colon Cancer in a Young Patient: A Diagnostic Challenge.

Concurrent intestinal tuberculosis and colonic adenocarcinoma are rare and pose significant diagnostic challenges due to overlapping clinical and endoscopic features. We report a 25-year-old woman presenting with right iliac fossa pain, anorexia, and 10-kg weight loss. Colonoscopy showed a proliferative ascending colon growth with adjacent cecal ulcers. Histopathology confirmed adenocarcinoma from the growth, while catridge-based nucleic acid amplification test from the ulcers detected Mycobacterium tuberculosis. The patient received antitubercular therapy, followed by right hemicolectomy and

GastroMalign: Vision Transformer-Based Framework for Early Detection and Malignancy-Risk Stratification for High-Risk Gastrointestinal Lesions.

Background: Current artificial intelligence (AI) systems in gastrointestinal (GI) endoscopy primarily emphasize binary detection or static classification, providing limited support for the graded assessment of malignant potential that underpins clinical decision-making. We developed GastroMalign, a transformer-based framework designed to stratify GI lesions according to ordinal disease severity while maintaining clinical interpretability, addressing this unmet need in endoscopic risk assessment. Methods: This retrospective development and validation study used the publicly available GastroVisi

Case Report: Malignant transformation of deep infiltrating endometriosis: a report of two cases and literature review.

Deep infiltrating endometriosis (DIE) is a severe form of endometriosis that rarely undergoes malignant transformation, and when it does, diagnosis is often difficult. Here, we report two cases of long-standing DIE that progressed to endometrioid adenocarcinoma. The first case involved a 44-year-old woman with pelvic DIE complicated by hydronephrosis, who underwent radical surgery with ureteroneocystostomy. Postoperative pathology confirmed well-differentiated endometrioid adenocarcinoma, and she remained disease-free after receiving chemotherapy and targeted therapy. The second case was a 47-

Molecular profiling of visible polypoid and invisible conventional intestinal-type low-grade dysplasia in patients with idiopathic inflammatory bowel disease.

AIMS: Little is known about the molecular features of visible polyps with low-grade intestinal-type dysplasia in patients with inflammatory bowel disease (IBD). To better understand their origins and biological potential, we sought to genomically profile these lesions and compare them with invisible low-grade dysplasia and sporadic adenomas from non-IBD patients. METHODS: 22 polyps within areas of colitis, 13 polyps outside areas of colitis, 10 foci of invisible dysplasia from patients with IBD and 6 sporadic tubular adenomas from non-IBD patients were analysed using the OncoPanel assay. Colonic Diaphragmatic Disease: The Concerning Dark Depths of NSAIDs.

Non-steroidal anti-inflammatory drugs (NSAIDs), widely used for their therapeutic benefits, are associated with significant gastrointestinal complications, including the rare occurrence of colonic diaphragm disease (CDD). Characterized by diaphragm-like fibrotic strictures that narrow the intestinal lumen, CDD often leads to symptoms such as chronic anemia, bowel obstruction, and gastrointestinal bleeding. This report discusses a case of NSAID-induced CDD in a 58-year-old male with a history of prolonged NSAID use for osteoarthritis. The patient's clinical course was marked by progressive fati

Diagnostic workout of glandular malignant lesions of the bladder according to the 5th WHO classification.

Glandular lesions involving the bladder are less common than conventional urothelial carcinoma, and they are often diagnostically challenging diseases, carrying different clinical outcomes. As a group, they encompass both primary and secondary neoplasms, with sometimes overlapping morphological features. In this scenario, proper clinical information is important, in that secondary involvement of the bladder may occur by direct extension or lymphatic/hematogenous spread from carcinomas at other sites, comprising prostate, colon, cervix, breast, and lung. According to the 5th edition of the WHO
