Diagnostic workout of glandular malignant lesions of the bladder according to the 5th WHO classification.

Glandular lesions involving the bladder are less common than conventional urothelial carcinoma, and they are often diagnostically challenging diseases, carrying different clinical outcomes. As a group, they encompass both primary and secondary neoplasms, with sometimes overlapping morphological features. In this scenario, proper clinical information is important, in that secondary involvement of the bladder may occur by direct extension or lymphatic/hematogenous spread from carcinomas at other sites, comprising prostate, colon, cervix, breast, and lung. According to the 5th edition of the WHO

Improved effect of antibiotic treatments on the hippocampal spatial memory dysfunction of mice induced by high fluoride exposure: Insight from assembly processes and co-occurrence networks of gut microbial community.

High fluoride exposure was widely demonstrated to be related with brain memory impairment. Since the absorption of F- enters the body mainly through the gastrointestinal tract, studying the effects of excessive intake of fluoride on brain memory function in various gut microbiome states might have profound implications for the prevention of fluorosis because growing evidence revealed the significance of the "microbiota-gut-brain" axis (MGBA). In the present study, we aimed to illustrate the potential mechanism of gut microbiota on high fluoride exposure-induced hippocampal lesions and spatial

Clinicopathologic Characteristics of Immune Checkpoint Inhibitor-related Pancreatitis.

The advent of immune checkpoint inhibitors (ICIs), although associated with adverse events, has heralded a new era in cancer therapy. ICI-related pancreatitis is a rare adverse effect of ICIs. The nonspecific clinical manifestations have posed diagnostic challenges, and the detailed histologic features remain largely unknown. This study aims to characterize the clinical and histopathologic features of ICI-related pancreatitis to increase awareness and improve diagnostic accuracy. We retrospectively identified 5 specimens from 4 patients from our database and consultation practice. We reviewed

Narrative review: this or that?-uncommon challenges in mediastinal pathology.

BACKGROUND AND OBJECTIVE: Accurate diagnosis of mediastinal tumors is of critical importance to establish appropriate therapy. However, these lesions are relatively uncommon and may be challenging to evaluate, particularly in small biopsy specimens. Thymomas and thymic carcinomas are the most common primary malignant tumors of the mediastinum, but the site can be affected by many other neoplasms that can pose significant difficulty in diagnosis. The objective of this article is to bring awareness to these rarer tumors and offer a diagnostic approach using ancillary techniques guided by clinica

Large colonic lipomas presenting as a rare cause of adult bowel obstruction and intussusception: A two-case surgical series.

INTRODUCTION AND IMPORTANCE: Colonic lipomas are uncommon benign tumors of mesenchymal origin that are typically asymptomatic but can lead to significant complications, including bowel obstruction or intussusception, when they grow large. PRESENTATION OF CASE: We report two cases of symptomatic colonic lipomas presenting with features of intussusception and bowel obstruction. The first case involved a 70-year-old male with chronic abdominal pain, hematochezia, and obstructive symptoms, in whom colonoscopy and CT imaging revealed a large lipoma of the ascending colon causing ileocolic intussus

Chilaiditi Syndrome With Colonic Gastrointestinal Stromal Tumor (GIST): An Unusual Overlap.

Chilaiditi syndrome is a rare condition characterized by the interposition of the colon between the liver and diaphragm, often mimicking other abdominal pathologies. While gastrointestinal stromal tumors (GISTs) are the most common mesenchymal tumors of the GI tract, they rarely originate from the colon. We report a 69-year-old man presenting with progressive right upper quadrant pain, low-grade fever, weight loss, altered bowel habits, sluggish bowel sounds, and a positive Murphy's sign. Initial imaging with an erect X-ray showed air under the right hemidiaphragm, prompting suspicion of intra

Colonic Lipoma Mimicking Gastrointestinal Stromal Tumor: A Case Report of a Diagnosis Pitfall.

Colonic mesenchymal tumors represent a diagnostic challenge due to their rarity and the overlap of clinical and imaging features with other subepithelial lesions, particularly gastrointestinal stromal tumors (GISTs). Histopathological evaluation remains essential to guide appropriate management and establish prognosis. We report the case of a 64-year-old woman with a three-year history of abdominal distension, altered bowel habits, and occult gastrointestinal bleeding. Repeated colonoscopies revealed a large subepithelial lesion in the transverse colon, highly suggestive of GIST, though repeat

Laparoscopic resection of cecal duplication cyst in a 28 years old male: a rare case report.

BACKGROUND: Cecal duplication cysts are a rare congenital anomaly, particularly uncommon in adults. With no clear etiology behind it, duplication cysts typically appear before 2 years of age. Their presentation is diverse, often mimicking other right lower quadrant pathologies. We report a case of an adult cecal duplication cyst treated successfully with laparoscopic resection, highlighting diagnostic challenges and management considerations. Mixed lesions in premalignant colorectal polyps.

Colonoscopists occasionally encounter mixed lesions with overlapping histological features. The frequency and combinations of such mixed colorectal polyps remain poorly understood. We comprehensively analyzed the frequencies and combinations of histologically mixed premalignant colorectal polyps. This study had a retrospective, single-center, cross-sectional design. Patients who underwent colonoscopy at the Toyoshima Endoscopy Clinic between March 2022 and July 2023 were eligible. We excised lesions diagnosed as conventional adenomas or clinically significant serrated polyps. Among these polyp

Gel immersion in endoscopy: Exploring potential applications.

The challenge of effectively eliminating air during gastrointestinal endoscopy using ultrasound techniques is apparent. This difficulty arises from the intricacies of removing concealed air within the folds of the gastrointestinal tract, resulting in artifacts and compromised visualization. In addition, the overlap of folds with lesions can obscure their depth and size, presenting challenges for an accurate assessment. Conversely, in intricately folded regions of the gastrointestinal tract, such as the stomach, intestine, and colon, insufficient delivery of air or CO2 into the cavity impedes l

Characterization of ITIH6, a novel inter-α-inhibitor family member with prognostic significance in cholangiocarcinoma.

The family of inter-α-trypsin inhibitors (ITI) includes proteins with a conserved light chain and a variable number of heavy chains. Here, we present the first characterization of ITIH6, a previously uncharacterized member of the ITI heavy chain (ITIH) family, using different bioinformatics analysis tools. ITIH6 contains the two canonical domains characteristic of ITIH proteins (VIT, vWA), but also harbors a large central region between the vWA domain and the C-terminus that is unique to ITIH6. Phylogenetic analysis further revealed that ITIH6 is most closely related to ITIH5, forming a separa

Diagnostic Challenges in Sessile Serrated Lesions: Progression to Adenocarcinoma in a High-Risk Patient.

BACKGROUND Sessile serrated lesions (SSLs) are precursors in approximately 20-30% of colorectal cancer (CRC) cases, often characterized by B-Raf proto-oncogene (BRAF) mutations and CpG island methylator phenotype (CIMP), and follow the serrated neoplasia pathway. Histopathological differentiation between SSLs with dysplasia and conventional adenomas is diagnostically challenging because their visual and pathologic features often overlap. SSLs with dysplasia carry a high malignant potential and can progress rapidly to invasive carcinoma, underscoring the need for accurate classification and tim

Plasmablastic lymphoma in Crohn's disease complicated by enterocutaneous fistula: Radiologic clues and interventional strategy.

Plasmablastic lymphoma (PBL) is a rare and aggressive subtype of diffuse large B-cell lymphoma. The potential association between Crohn's disease and lymphoproliferative disorders remains controversial. We present an unusual case of plasmablastic lymphoma arising in a patient with long-standing rectocolonic Crohn's disease, complicated by an enterocutaneous fistula. The patient underwent surgery including sigmoidectomy, fistula disconnection, colostomy, and wide skin excision. Histopathology confirmed plasmablastic lymphoma involving the colon, rectum, and skin. Our case is notable for the cha

Diagnosis and Treatment of Metastatic Colon Cancer in Pregnancy First Presenting as Multiple Liver Masses: A Case Report.

Colorectal cancer (CRC) is the second most common cancer in women in Japan. However, it is uncommon during pregnancy. CRC diagnosis during pregnancy is often complicated and delayed due to the overlapping of symptoms, such as abdominal pain and nausea, with those of pregnancy and the limitations placed on potential diagnostic imaging and testing because of concerns for the fetus. A 39-year-old woman was referred from a local hospital at 32 weeks gestation after persistent right abdominal pain, which prompted an ultrasound that showed multiple liver lesions suggestive of malignancy. A combinati

Stereotactic ablative body radiotherapy for primary kidney cancer (TROG 15.03 FASTRACK II): a non-randomised phase 2 trial.

BACKGROUND: Stereotactic ablative body radiotherapy (SABR) is a novel non-invasive alternative for patients with primary renal cell cancer who do not undergo surgical resection. The FASTRACK II clinical trial investigated the efficacy of SABR for primary renal cell cancer in a phase 2 trial. METHODS: This international, non-randomised, phase 2 study was conducted in seven centres in Australia and one centre in the Netherlands. Eligible patients aged 18 years or older had biopsy-confirmed diagnosis of primary renal cell cancer, with only a single lesion; were medically inoperable, were at high

Leiomyomatous Lesions of the Colon: Two Case Reports with Radiological Features, Pathological Correlations, and Literature Review.

Colonic leiomyomatous lesions are smooth muscle tumors including both benign leiomyoma and malignant leiomyosarcoma. They usually occur in elderly and middle-aged groups. They are rare, representing 3% of all gastrointestinal leiomyomas. Descending and sigmoid colon are the most commonly affected segments of colon. Patients are usually asymptomatic but occasionally they may present with abdominal pain, hemorrhage, and intestinal obstruction. Radiological findings for these lesions are variable and overlapping with other diagnoses but usually manifest as large lesions with lobulated margins, ex

SPECT/CT in the Diagnosis of Ectopic Gastric Mucosa-Meckel's Diverticulum.

Aim  The imaging of Meckel's diverticulum (MD) is based of accumulation of Tc-99m pertechnetate in the ectopic gastric mucosa (EGM) content. Although the diagnostic accuracy of this imaging modality is high, there are some overlap patients with coexisting gastrointestinal bleeding and false positive causes hampering diagnostic power. The aim of this study was to evaluate the possible contribution of single-photon emission computed tomography/computed tomography (SPECT/CT) in EGM-MD diagnosis and to determine the indication of this additional imaging modality.
