Real-world safety and efficacy of lecanemab in the treatment of Alzheimer's disease: a systematic review and meta-analysis.

BACKGROUND: Despite the approval of lecanemab for Alzheimer's disease (AD), evidence regarding its performance outside controlled trial settings remains scarce, leaving its real-world safety and clinical utility largely unknown. METHODS: We searched PubMed, Embase, and Web of Science (through October 1, 2026) for real-world studies of lecanemab in AD. Outcomes included ARIA, discontinuation, infusion-related reactions, serious adverse events, MMSE, MoCA, CDR-SB, and amyloid PET. Random-effects models with REML and Hartung-Knapp adjustment were used. Subgroup analyses were stratified by region

Initiating levodopa pump therapy: a qualitative study of patient and caregiver perspectives.

BACKGROUND: Parkinson's disease (PD) is a progressive neurodegenerative disorder affecting motor and non-motor functions. When oral medication no longer provides adequate control, device-aided therapies (DAT), including levodopa infusion therapy, may be introduced. Although DAT can improve symptom management, little is known about how people with PD (PwPD) and family caregivers (FC) experience the initiation process. This study explored PwPD and their FC perceptions during decision-making, therapy initiation, and subsequent adaptation to levodopa pump therapy. METHODS: This qualitative study

Sevuparin in children with severe malaria: a phase I safety and dose-finding trial.

Mortality rates for African children with severe malaria remain high. Sevuparin, a heparin-like compound, as an adjunctive treatment, may improve outcomes due to its effects on Plasmodium falciparum-infected erythrocytes. A Phase I dose-finding trial in Kenyan and Zambian children with severe malaria aimed to identify the maximum tolerated dose (MTD) of sevuparin, given as three infusions (at 0, 8 and 16 h post-enrolment) using the continual reassessment method (ISRCTN32271864). The primary endpoint was dose limiting toxicity (DLT) defined as Activated Partial Thromboplastin Time (APTT) > 95 s

Dual benefit of esketamine-based opioid-free anesthesia on postoperative pain and mental health outcomes in patients undergoing bariatric surgery: a randomized controlled trial.

Patients with morbid obesity are at a high risk of comorbid depressive and anxiety symptoms. While bariatric surgery is an effective treatment, conventional opioid-based anesthesia carries significant perioperative risks. Opioid-free anesthesia (OFA) presents a safer alternative. Esketamine, known for its dual analgesic and rapid antidepressant effects, is a promising component of OFA. However, clinical evidence supporting its simultaneous benefits for both physical and psychological recovery in bariatric patients remains limited. In this double-blind, randomized clinical trial, patients sched

ABCD-Trial: study protocol for a randomized, double-blind, placebo-controlled study in patients with Amyotrophic lateral sclerosis for the evaluation of efficacy and safety of B-Cell Depletion with rituximab (ABCD).

BACKGROUND: The exact disease-causing mechanisms in ALS remain incompletely understood. A possible cause is inflammation of neurons. Compared to other neurodegenerative diseases, neuroinflammation is pronounced in ALS, presumably leading to disease progression and neuronal death. Patients with ALS can have neuronal autoantibodies and most notably antibody-producing cells in the cerebrospinal fluid. The CD20-binding therapeutic antibody rituximab targets these antibody-producing B-cells. Rituximab is a medication commonly used in neurology. Single patients with ALS have already been treated wit

Nonoperative Management of an Acute Complete Achilles Tendon Rupture With Mesenchymal Stem Cells and Extracellular Vesicles: A Case Report.

An acute complete rupture of the Achilles tendon with a large intertendinous gap is conventionally regarded as a surgical indication, yet operative repair carries a significant burden on wound-healing and potential for neurovascular complications, whereas nonoperative care may be limited by re-rupture and incomplete functional recovery. Human umbilical cord-derived mesenchymal stem/stromal cells (hUC-MSCs) and their secreted extracellular vesicles (EVs) exert immunomodulatory, pro-angiogenic and pro-tenogenic paracrine effects that could support tendon healing. A healthy 45-year-old recreation

Pars plana vitrectomy under air infusion versus conventional fluid infusion for dense vitreous hemorrhage: a retrospective comparative study.

BACKGROUND: Vitreous hemorrhage (VH) remains a significant clinical challenge in vitreoretinal surgery. Conventional pars plana vitrectomy (PPV) under balanced salt solution (BSS) infusion is limited by poor intraoperative visualization when hemorrhagic vitreous mixes with the irrigating solution. Vitrectomy under air infusion has been proposed as an alternative approach that may overcome these limitations. This study aimed to compare the surgical efficiency and short-term outcomes of vitrectomy under air infusion versus conventional BSS infusion in the management of dense vitreous hemorrhage.

Intravitreal methotrexate as a prophylactic strategy for proliferative vitreoretinopathy in high-risk retinal detachment.

BACKGROUND: Proliferative vitreoretinopathy (PVR) remains the major cause of surgical failure in rhegmatogenous retinal detachment (RRD). This study evaluates the anatomical and functional outcomes, as well as the safety, of a standardized intraoperative intravitreal methotrexate infusion protocol during pars plana vitrectomy (PPV) in eyes with early stage PVR (grade B) or with advanced PVR (grade C). METHODS: A retrospective case series was conducted on 60 consecutive eyes of 59 patients with RRD and PVR grade B (n = 29) or grade C (n = 31) undergoing PPV with adjunct intraoperative methotre

Dexmedetomidine-based opioid-sparing anesthesia in video-assisted thoracoscopic surgery: a randomized controlled trial.

BACKGROUND: Dexmedetomidine has increasingly been incorporated into opioid-sparing anesthetic strategies during video-assisted thoracoscopic surgery (VATS) because of its analgesic properties and minimal respiratory depression. However, dexmedetomidine remains associated with dose-related hemodynamic adverse effects, particularly bradycardia and hypotension. We evaluated whether a reduced-dose dexmedetomidine regimen decreases intraoperative opioid requirements while maintaining acceptable hemodynamic safety during VATS. METHODS: In this prospective randomized controlled trial, 68 patients un

Precision fluid therapy in sepsis: fluid responsiveness vs. fluid tolerance.

Intravenous fluid is the most frequently administered drug in septic shock and one of the least precisely dosed. Two decades of trials have progressively dismantled the assumption that more fluid is better: protocolized early goal-directed therapy failed to improve on contemporary usual care, high-volume bolus protocols caused frank harm where organ support was limited, and restrictive strategies applied after an initial resuscitation have proven neutral rather than superior. What has replaced a universal volume prescription is a pair of questions that must be asked separately for each patient

Safety of lecanemab in real-world patients with Alzheimer's disease: a prospective, observational case series supported by FAERS signal analysis.

OBJECTIVE: Lecanemab, a humanized immunoglobulin monoclonal antibody, has been approved for the treatment of Alzheimer's disease (AD). Although clinical trials have demonstrated its favorable safety profile, real-world studies are crucial for identifying rare adverse events (AEs) and evaluating post-marketing safety. This study aimed to evaluate the safety of lecanemab by combining a clinical prospective, observational case cohort with data from the FAERS database, thereby providing a robust basis for the identification of potential safety signals. METHODS: A prospective and observational stu

Switching from Intravenous to Subcutaneous Infliximab in Children with Inflammatory Bowel Disease: A Systematic Review and Meta-Analysis.

PURPOSE OF REVIEW: Subcutaneous infliximab (IFX-SC; CT-P13 SC/infliximab-dyyb/Remsima-SC/Zymfentra) is an approved maintenance option after intravenous (IV) induction in adults with inflammatory bowel disease (IBD), but it is not approved for patients < 18 years, and the pediatric evidence remains dispersed across small, largely observational cohorts. This review synthesizes the published pediatric experience of transitioning from IV to SC infliximab, examining maintenance of remission, drug concentrations, safety, and acceptability, and situates it within the adult evidence base and the disti

Management of acute infusion-related reactions in AAV gene therapy guided by mechanistic insight.

Infusion-related reactions (IRRs), serious immune events that occur either during or within six hours of an infusion, can occur with adeno-associated virus (AAV) gene therapy. IRRs may be attributed to IgE-mediated hypersensitivity, leading to treatment discontinuation due to concern about progressive, life-threatening allergic reactions. IRRs were observed in two participants who received an AAV8 gene therapy (DTX401) for glycogen storage disease type Ia and in one participant who received an AAV9 gene therapy (UX701) for Wilson's disease. All of these IRRs were associated with a rapid infusi

NMDAR subunit GluN1 dynamics in urinary astrocyte-derived extracellular vesicles in anti-NMDAR encephalitis.

BACKGROUND AND OBJECTIVES: Longitudinal measurement in vivo of molecular changes in the central nervous system (CNS) is important to investigate and guide treatment of rapidly changing neurological diseases. Here, we show how a novel protocol for urinary astrocyte-derived extracellular vesicles (uADEVs) can be used non-invasively to track the dynamic trajectory of the N-methyl-D-aspartate receptor (NMDAR) subunit GluN1 in a patient with anti-NMDAR encephalitis. METHODS: We collected 18 morning urine samples from the patient over 34 days of hospitalisation, as well as 15 samples from a healthy

Autologous gene therapy versus allogeneic transplantation for sickle cell disease and transfusion-dependent β-thalassemia: Choosing between 2 definitive therapies.

Autologous hematopoietic stem and progenitor cell (HSPC) gene therapy and allogeneic hematopoietic cell transplantation (allo-HCT) can both provide durable hematologic and clinical correction for sickle cell disease (SCD) and transfusion-dependent β-thalassemia (TDT). Allo-HCT has decades of follow-up and established rescue pathways, but outcomes depend on donor type, HLA matching, graft source, conditioning, and graft-versus-host disease (GVHD) prophylaxis. Gene therapy removes donor dependence, alloreactive graft rejection, and GVHD, but requires adequate HSPC collection, successful manufact

Renin-Angiotensin System Activation Drives Early Cardiorenal Dysfunction in Normotensive Prediabetes.

INTRODUCTION: Prediabetes is increasingly recognised as a stage at which cardiovascular and renal dysfunction begin to develop despite the absence of overt metabolic disease. The mechanisms underlying these early alterations remain poorly understood. Because adipose tissues surrounding blood vessels and kidneys can locally generate components of the renin-angiotensin system (RAS), we hypothesised that subclinical RAS activation contributes to early cardiorenal dysfunction in prediabetes. METHODS: Using a pre-established normotensive, non-obese rat model of prediabetes induced by 12 weeks of m
