Metacontrol-related aperiodic and periodic neural activity in cognitive aging: enhancing the neural signal-to-noise ratio through anodal transcranial direct current stimulation.

Metacontrol, the ability to adapt cognitive control to task demands, declines with age and is thought to be reflected in aperiodic and periodic neural dynamics. Given that anodal transcranial direct current stimulation (atDCS) can modulate cortical excitability via membrane potential shifts, we tested whether atDCS alters the neurophysiological signatures of metacontrol in younger and older adults. In a mixed design, younger and older participants performed a Go/Nogo task under both atDCS and sham stimulation conditions; resting-state EEG data were also acquired. Aperiodic activity was analyze

Limited Accuracy of Prognostic Models for Predicting in-Hospital Mortality Among Patients with Refractory Status Epilepticus Admitted to Resource-Limited Intensive Care Units Compared with Those Treated Outside the Intensive Care Unit.

BACKGROUND: The prognostic value of scoring systems for status epilepticus (SE), including the Status Epilepticus Severity Score (STESS), modified STESS (mSTESS), and Epidemiology-Based Mortality Score in SE (EMSE), has been demonstrated primarily in studies from high-income countries (HICs). However, data on their performance in subgroups such as older adults, patients with refractory SE (RSE) in intensive care unit (ICU), and populations from low- and middle-income countries (LMICs) remain limited. METHODS: Retrospectively analysis was performed on data from the Eugenio Espejo Hospital SE d

No effect of anodal high-definition transcranial direct current stimulation during motor sequence learning in older people.

Transcranial direct current stimulation (tDCS) targeting the primary motor cortex (M1) has previously been shown to enhance motor learning in older adults. However, findings across studies are inconsistent, which may be partly due to variability in stimulation parameters and montages across experimental protocols. We aimed to replicate the effect of 1 mA conventional anodal tDCS over the M1 hotspot during one session of motor sequence learning on retention (24 h) compared to sham stimulation in older adults. In addition, we aimed to compare the effect of high definition (HD tDCS) versus conven

Manual asymmetries and cortical modulation: relationships between motor control and interhemispheric connectivity.

Manual asymmetries in goal-directed aiming have been linked to interhemispheric interactions, yet it remains unclear whether experimentally modulating hemispheric activity alters both motor behavior and interhemispheric functional coupling. We investigated how bilateral transcranial direct current stimulation (tDCS) targeting primary motor cortices modulates manual asymmetries and interhemispheric connectivity during aiming. Twelve healthy right-handed male adults completed a protocol comprising three sessions: dominant hemisphere inhibition (DHI; cathode over C3/anode over C4), non-dominant h

Tumor-Intrinsic HEXB Drives Endothelial-To-Mesenchymal Transition and Confers Resistance to Anti-Angiogenic Therapy in Glioblastoma.

Although anti-angiogenic (anti-VEGF) therapy is widely used for glioblastoma (GBM), its efficacy is severely limited by acquired resistance. Here, we identify endothelial-mesenchymal transition (Endo-MT) in brain endothelial cells as the primary driver of this resistance, fundamentally modulated by hexosaminidase B (HEXB). Mechanistically, HEXB directly binds and stabilizes heat shock protein 60 (HSP60) by inhibiting its ubiquitination and degradation. This stabilization upregulates insulin-like growth factor binding protein 1 (IGFBP1), thereby promoting the Endo-MT process. Addressing its cli

Cognitive load and prefrontal neuromodulation shape exploration, risk-taking, and information seeking in experience-based decision making.

Experience-based decision making requires individuals to balance exploration and exploitation under uncertainty, a balance that may be influenced by cognitive demands and prefrontal neuromodulation. We examined the joint effects of trivia-based cognitive load and anodal transcranial direct current stimulation (tDCS) over the left dorsolateral prefrontal cortex (DLPFC) in 60 healthy adults. The primary 2 × 2 between-subjects design crossed Stimulation (anodal tDCS vs sham) with Cognitive-Load Condition (Trivia-Based Load vs No-Trivia Control). Within the Trivia-Based Load subgroup, Lower-Load a

Enhanced neuroprotection effects of transcranial direct current stimulation combined with 6-Gingerol in copper-overloaded traumatic brain injury via mGluR1.

OBJECTIVE: To investigate the neuroprotective effects and underlying mechanisms of transcranial direct current stimulation (tDCS) combined with 6-Gingerol (6G) in a rat model of traumatic brain injury (TBI) complicated by copper overload, with particular emphasis on microglial polarization and mGluR1 signaling. METHODS: A controlled cortical impact (CCI) was used to establish TBI in rats, followed by induction of copper overload via oral administration of copper sulfate. Animals were randomly assigned to sham, TBI, copper-overloaded TBI, and copper-overloaded TBI treated with 6G, tDCS, or the

The Effect of Cerebellar tDCS On Inhibitory Control in Healthy Adults: Exploring Trait Moderation.

Transcranial direct current stimulation (tDCS) research on inhibitory control (IC) has largely focused on prefrontal targets, with inconsistent results, while the cerebellum's contribution to executive control networks remains underexplored. This study examines whether cerebellar anodal tDCS modulates IC in healthy adults, using both proactive (Go/No-Go) and reactive (Stop Signal) inhibition measures, and explores whether dispositional traits moderate stimulation effects. In a double-blind, sham-controlled, within-subjects design, 40 participants received active and sham cerebellar tDCS (2 mA,

Early life stress caused glucose homeostasis impairment and anxiety-like behavior along with oxidative stress in adolescent male rat offspring: Protective effect of saffron extract.

Early life stress (ELS) significantly contributes to metabolic and psychological disorders by triggering oxidative stress and disrupting the hypothalamic-pituitary-adrenal (HPA) axis. This research examined ELS effects on redox status, along with anxiety-like behavior, in adolescent male Wistar rats, also assessing saffron (Crocus sativus L.) extract's potential benefits. Rats were exposed to foot shock stress from postnatal days 15-19, with treatment groups receiving saline or saffron extract (10 mg/kg) via intraperitoneal injection. Control groups were maintained without stress exposure. Ele

Psychiatric, anesthetic, and procedural characteristics of modified electroconvulsive therapy courses in adolescents treated in a tertiary child and adolescent psychiatry service: a 15-year retrospective cohort study.

BACKGROUND: Evidence on electroconvulsive therapy (ECT) in adolescents is largely retrospective and rarely reports psychiatric outcomes together with anesthetic and procedural characteristics. We characterized modified ECT courses in a tertiary child and adolescent psychiatry service. METHODS: This retrospective cohort included adolescents aged 13-18 years treated with modified ECT from 2010 to 2025; the ECT course was the unit of analysis. Illness severity was rated with the Clinical Global Impression-Severity (CGI-S) scale at admission and discharge. The primary outcome was a composite of a

Non-invasive neuromodulation for drug-resistant epilepsy: a contemporary structured review of mechanisms, clinical evidence and future directions.

Drug-resistant epilepsy (DRE) affects nearly one-third of people with epilepsy and remains a major cause of disability despite continued advances in antiseizure medications and surgical therapies. For patients who are not candidates for resective surgery or invasive neuromodulation, non-invasive neuromodulation (NIN) has emerged as a promising strategy to modulate epileptic networks while avoiding the risks associated with implanted devices. This structured narrative review examines current evidence across the principal NIN modalities, including repetitive transcranial magnetic stimulation (rT

Molecular mechanisms of electroconvulsive therapy modulates excitation-inhibition balance in major depression disorder.

BACKGROUND: The excitation-inhibition (E/I) imbalance is a major pathological characteristic of major depressive disorder (MDD). However, whether/how electroconvulsive therapy (ECT) modulates neuronal homeostasis or E/I balance in MDD and its molecular basis is still largely unknown. METHODS: In this study, longitudinal rs-fMRI data from 46 patients with MDD receiving ECT and 46 healthy controls were included. The Hurst index was used to characterize the E/I balance, and mixed-effects model was constructed to assess the interactive effects of ECT on the Hurst index and functional connectivity

Neuro-oncological emergencies requiring critical care intervention.

Patients with primary or metastatic brain tumours can deteriorate abruptly, and the acute physician is often the first to respond. This review covers the recognition and initial management of the neurological emergencies most likely to need critical care: raised intracranial pressure from peritumoural oedema and herniation; seizures and status epilepticus, including non-convulsive status epilepticus (NCSE); intracranial haemorrhage; acute hydrocephalus; and venous thromboembolism. It also covers two treatment-related emergencies that the general physician must now recognise: immune effector ce

Session-dependent effects of 6-Hz frontoparietal tACS on visuospatial working-memory accuracy.

Visuospatial working memory (VSWM) is supported by distributed fronto-parietal networks and is critically dependent on coordinated neural oscillations, particularly in the theta band (4-8 Hz). Non-invasive neuromodulation via transcranial alternating current stimulation (tACS) has been proposed to enhance cognitive function by entraining these oscillations. The present study investigated the effects of theta-frequency (6 Hz) tACS applied over the left fronto-parietal network (F3/P3) on VSWM performance using the Corsi Block Tapping Task. In a within-subjects design, participants completed four
