Rh(D)-negative individuals are relatively rare in the Chinese population, and clinical experience with Rh(D)-incompatible allogeneic haematopoietic stem cell transplantation (allo-HSCT) remains limited. Compared with ABO incompatibility, the immunohaematological consequences of Rh(D) mismatch in allo-HSCT are less well defined. Herein, we report a single-centre retrospective case series of four patients receiving Rh(D)-incompatible allo-HSCT at our institution, including two Rh(D)-negative recipients with Rh(D)-positive donors and two Rh(D)-positive recipients with Rh(D)-negative donors. BACKGROUND: Severe shortages of blood for transfusion are a persistent global health crisis, with the burden disproportionately concentrated in low-income countries. Diverse stakeholder perspectives on blood availability might inform improvements. We aimed to use a comprehensive approach to identify the challenges, facilitators, and innovations within the blood transfusion continuum (BTC) in three different Kenyan settings.
METHODS: In this qualitative study, we interrogated the BTC in Siaya, Nakuru, and Turkana counties as part of the Pathways for Innovation in Blood Transfusion Systems in K

BACKGROUND: This study assessed de novo human leukocyte antigen (HLA) antibodies associated with blood transfusions in a paediatric kidney transplantation cohort, associated risk factors and outcomes.
METHODS: Patients with kidney disease at Royal Children's Hospital (Melbourne, Australia) who were transfused and/or received a kidney transplant between 2000 and 2018 were included. HLA antibody results from pre- and post-transfusion serum specimens, blood donor typing (where available) and clinical and laboratory data were collected. A control group comprised non-transfused patients from the s

OBJECTIVES: Matched sibling donors (MSDs) were preferred for haematopoietic stem cell transplantation (HSCT) in severe aplastic anaemia (AA) when lacking MSDs, unrelated donors (URDs) and haploidentical (Haplo) donor would be considered, but limited by increased graft-versus-host disease (GVHD) and recommended only in experienced centres. This study aimed to compare the efficacy and safety of umbilical cord blood transfusion added URD-HSCT (URD-cord-HSCT) to MSD-HSCT and Haplo-HSCT in AA patients.
METHODS: Patients who underwent URD-cord-HSCT (n = 26), MSD-HSCT (n = 26) or Haplo-HSCT (n = 52)

INTRODUCTION: Skin allografts are used to cover wounds in patients with severe burns. Owing to the allogeneic nature of the tissue, rejection often limits its effectiveness to a few weeks. The aim of this study was to analyse the factors influencing sensitisation in skin allograft recipients and to evaluate the role of donor-recipient HLA mismatches in the humoral response of sensitised patients.
PATIENTS AND METHODS: Eighteen patients were enrolled in this single-centre, retrospective cohort study. Adult burned skin allograft recipients were selected through a retrospective chart review (201

BACKGROUND: Human leukocyte antigen (HLA) matching is critical for donor selection in allogeneic hematopoietic stem cell transplantation (allo-HSCT). Each HLA locus is normally biallelic; however, exceptionally rare cases with more than two alleles at a single locus have been reported.
CASE PRESENTATION: We describe a 17-year-old male with mixed phenotype acute leukemia (T/myeloid) who was found to carry three alleles at both the HLA-B and HLA-C loci during pre-transplant evaluation. Family-based HLA typing revealed that the additional alleles were inherited as part of a single maternal haplo

BACKGROUND: Red blood cell (RBC) transfusion practices during veno-arterial extracorporeal membrane oxygenation (VA-ECMO) are heterogeneous, and the benefit - harm balance remains uncertain. Current recommendations rely on predefined hemoglobin thresholds and do not provide criteria for individualized transfusion. As the primary goal of RBC transfusion is to optimize oxygen delivery (DO₂) to meet oxygen consumption (VO₂), a strategy guided by a surrogate of DO₂/VO₂ mismatch - central venous oxygen saturation (ScvO₂)- may be more appropriate than fixed liberal transfusion thresholds.

Donor-specific HLA antibodies (DSA) are associated with graft failure after HLA-mismatched allogeneic hematopoietic stem cell transplantation (HSCT). The most widely used method for identifying anti-HLA antibodies is the single-antigen bead (SAB) assay. Although this method has high sensitivity, it also detects natural HLA antibodies, which are considered to have low clinical significance. Instead, detection of reactivity between recipient serum and donor cells, such as lymphocyte crossmatching, may be useful for HSCT donor selection. We evaluated the efficacy of immunocomplex capture fluoresc

INTRODUCTION: The 2015 transition from International Classification of Diseases, Ninth Revision to International Classification of Diseases, Tenth Revision (ICD-9 to ICD-10) in the U.S. aimed to improve diagnostic and procedural precision. However, no validated methodology exists for translating complication codes, posing a barrier to longitudinal surgical outcomes research. This study evaluates a structured approach to map ICD-9 to ICD-10 complication codes for high-risk operations, including esophagectomy, pancreaticoduodenectomy, and hepatectomy.
METHODS: We developed an 8-step process usi

Unrelated cord blood transplantation (CBT) is an important alternative donor option for adult patients with myelodysplastic syndrome (MDS); however, the prognostic significance of cord blood unit characteristics following CBT remains unclear and may vary according to disease risk. We conducted a nationwide retrospective study of 1767 adult patients with MDS who underwent first single-unit CBT between 1999 and 2023, including 529 patients with low-risk and 1238 with high-risk disease at transplantation. Cord blood unit characteristics were not significantly associated with disease-free survival

The selection of the best available donor is crucial for patients' outcome after allogeneic stem cell transplantation (allo-SCT). In the absence of fully Human leucocyte antigen (HLA) -matched donors, mismatched unrelated donor (9/10-MUD) or haploidentical donor (haplo) can be considered. No consensus has been reached on the best alternative and large real-world data are warranted to support decisional processes. We compared the outcome of 1413 patients with myeloid malignancies undergoing allo-SCT from 9/10-MUD with anti-thymocyte-globulin (ATG) (n = 1134) or haplo with post-transplant cyclop

BACKGROUND: Passenger lymphocyte syndrome (PLS) is an alloimmune complication of ABO-mismatched solid organ or hematopoietic stem cell transplantation characterized by the production of antibodies by donor-derived B lymphocytes against the recipient's red blood cell antigens. Its incidence, risk factors, and management remain poorly defined because many cases are subclinical, and most reports are case based. This study aimed to summarize the available literature on PLS, focusing on its incidence, immunohematologic mechanisms, clinical features, and management.
METHODS: We systematically searc

Although HLA-matched related donor transplantation (MRDT) is considered the preferred graft source when available, cord blood transplantation (CBT) is an alternative source, with several studies suggesting a potent graft-versus-leukemia effect. We compared outcomes between CBT and MRDT in acute myeloid leukemia (AML) not in remission and examined how pre-engraftment immune reaction (PIR), graft-versus-host disease (GVHD), and HLA mismatch affect CBT outcomes. We retrospectively analyzed 334 patients with AML not in remission who underwent first allo-HSCT using either single-unit CBT (n = 309)

Allogeneic hematopoietic cell transplants (HCTs) offer a curative option for patients with life-threatening hematologic diseases. While stem cell infusion reactions are common, they are generally not severe and typically do not affect engraftment. We present a rare case of isolated secondary cold agglutinin syndrome (CAS) in a donor, complicating the graft infusion and causing poor graft function (PGF) in a recipient. The 27-year-old recipient, diagnosed with refractory T-cell acute lymphoblastic leukemia (ALL), underwent a 9/10 HLA-mismatched unrelated peripheral blood stem cell transplant. T

BACKGROUND: ABO-mismatched kidney transplantation (KTx) expands donor availability but increases risks of antibody-mediated rejection and passenger lymphocyte syndrome (PLS). While rituximab (Rit) potentially mitigates these complications, conventional high-dose regimens (375 mg/m2) elevate infectious and hematologic toxicity. We implemented low-dose Rit induction (200 mg/body) for desensitization in minor/major ABO-mismatched and DSA-positive KTx, evaluating its efficacy and safety over 15-years.
METHODS: This single-center retrospective cohort (May 2009-April 2024) analyzed 161 adult KTx re

BACKGROUND: Passenger lymphocyte syndrome (PLS) is a hematologic complication of ABO blood-type minor incompatibility that can occur following both solid-organ and hematopoietic stem cell transplant (HSCT). HSCT patients usually require red blood cell (RBC) transfusions until they achieve full erythroid engraftment. It is unclear whether the blood bank should honor the recipient's blood type or the donor's blood type at the time of issuing RBC products for minor ABO-incompatible HSCT patients prior to full engraftment. To avoid this confusion, blood group O RBCs are often transfused. However,

Donor-specific antibodies (DSA) are preformed recipient antibodies directed against donor HLA antigens, most frequently observed in haploidentical or mismatched allogeneic hematopoietic cell transplantation (HCT). DSA assessed by multiplexed bead-based immunoassays at mean fluorescence intensity (MFI) values >1 000 are associated with increased risk of graft failure, graft-versus-host disease (GvHD), and mortality, supporting combined pharmacological and plasma-based depletion in absence of matching donors. We retrospectively analyzed all adult HCTs performed between 2009 and 2025 at the Unive

Immune-mediated alloimmunization and hemolytic anemia can complicate ABO-incompatible hematopoietic stem cell transplantation (HSCT). After major ABO-incompatible HSCT, clinical manifestations may include pure red cell aplasia and delayed hemolysis. Passenger lymphocyte syndrome and delayed hemolysis are typical complications of minor ABO-incompatible HSCT. Because delayed hemolysis after major or minor ABO mismatch may be indistinguishable, the terms host-associated alloimmune hemolytic anemia and graft-associated alloimmune hemolytic anemia have been tentatively proposed to distinguish these

BACKGROUND: The size mismatch between a liver graft and the recipient can result in complications and poor survival after pediatric whole liver transplantation (WLT).
METHODS: A retrospective study was designed, which included 114 recipients. Multiple variable predictors of graft loss suggested the graft-to-native-liver weight ratio (GNLWR), and then the GNLWR was calculated and categorized into 2 groups. The demographic, operation, complications, and survival analysis data were collected and compared.
RESULTS: Group 1 with a GNLWR < 0.41 had a higher incidence of hepatic artery thrombosis (

Organ transplantation is a life-saving treatment for patients with end-stage organ failure but requires lifelong immunosuppression that can result in significant complications. Achieving stable and durable donor-specific tolerance, whereby immunosuppression can be stopped without precipitating graft rejection, holds the promise to circumvent these problems. While transient inhibition of the CD40/CD154 costimulatory pathway delays transplant rejection in animal models, and antibodies blocking this interaction are currently in clinical trials, the efficacy of anti-CD154 (αCD154) in mouse models
