The effect of pretransplant blood transfusion (PBT) and histocompatibility matching on kidney allograft survival was studied in immunosuppressed dogs. PBT was found to be beneficial for related and unrelated mismatched kidney grafts. In contrast, after withdrawal of the immunosuppressants, transfused recipients of related and unrelated matched grafts rejected the kidney significantly more often than non-transfused dogs. Crossimmunisation for minor histocompatibility antigens may be responsible for this adverse effect, but could not be demonstrated in vitro by serology or mixed lymphocyte react

Seventy-nine first living related donor transplants performed in Gothenburg, Sweden, from April 1977 to October 1980 were studied. The patient survival was 97% at 30 months. The graft survival in HLA-identical sibling combination was 100%. Thirty previously non-transfused recipients of one-haplotype mismatched grafts were given 2 units of third-party blood as preparation for transplantation. The graft survival was 92% at 30 months. No patients became sensitized against their donor. Thirty-one recipients of haplotype-mismatched grafts, previously transfused for medical reasons, had a graft surv

Matching for the HLA-A and -B antigens was found to have a significant influence on the outcome of 459 cadaveric first transplants, with a difference in graft survival of 21% at one year between best and worst matched grafts (p = 0.003). Matching for the HLA-DR antigens seemed to have an even greater influence. In a material of 226 prospectively typed transplants, a difference in graft survival of 35% at one year between best and worst matched transplants was found (p = 0.03). In the HLA-DR matched transplants there was no significant effect of matching for the HLA-A and -B antigens. An effect

The outcome of 461 prospectively HLA-A, -B and -C typed and 193 prospectively HLA-DR typed cadaveric kidney transplants in one center was followed. We found a significant beneficial effect on graft survival both of HLA-A and -B as well as of HLA-DR matching between donor and recipient, while no effects of HLA-C compatibility could be detected. The effect of HLA-DR matching was clearly more pronounced than that of HLA-A and -B matching, and a possible influence of matching for HLA-A and -B could only be seen in the HLA-DR mismatched combinations. Pretransplant blood transfusions were associated

Platelet concentrates from HLA-compatible donors frequently provide satisfactory posttransfusion platelet responses in patients who, for immunologic reasons, are refractory to pooled random-donor platelet transfusions. The use of donors who have HLA antigens "crossreactive" with those of the recipient has increased the number of donors available for individual patients. Occasionally, successful transfusions occur with donors mismatched for HLA. Immunologic explanations include a restricted pattern of HLA antibody production and a variable expression of HLA antigens on the platelet surface. Imm

The results of cadaveric renal transplantation in one centre over 12 years are reviewed. One hundred and sixty-eight grafts have been performed in 147 recipients. At one year after transplantation, patient survival rate was 86%, and graft survival rate was 63%. Analysis of factors influencing graft survival show that the age of recipients, the lack of blood transfusions, and the use of poorly matched grafts (three to four HLA mismatches against 0 to two mismatches), all had adverse effect on survival. Death was a significant cause of graft loss in patients over the age of 45 years. Patients wh

The effect of blood transfusions on kidney graft survival in D/DR-matched host-donor combinations was investigated in rhesus monkeys treated with azathioprine and prednisolone. If host-donor combinations were mismatched for D/DR and no transfusions were given (controls), graft survival ranged from 9 to 22 days. D/DR matching alone led to survival times of 13 to 66 days with 62% of the animals showing a prolonged survival time ( greater than 22 days). However, when three pretransplant blood transfusions were given in D/DR-matched combinations, the range of graft survival was even better: 19 to

An analysis of HLA (A and B) recipient-donor matching on the outcome of 105 cadaver kidney transplants performed at the Medical College of Virginia transplant center revealed that there is a significant, overall difference (P = 0.03) between recipients receiving kidneys mismatched at two or less HLA loci versus recipients mismatched at three or more loci, the fewer mismatch category showing a better graft survival. The effect of blood transfusion prior to transplantation was studied and found to improve the outlook, especially in the best matched groups. The number of transfusions does show a

Noninbred rabbits, matched with regard to the major histocompatibility complex (RLA-A and RLA-D loci) but mismatched for Ig allotypes, served as donors (adult) and recipients (newborn) of lymphoid cells. Lasting chimerism regularly followed the transfer of 1 x 10(8)-3 x 10(8) spleen, lymph node, or bone marrow cells, as indicated by the continued production of Ig with allotypic determinants of both donor and recipient. Typically, Ig of donor allotype accounted for 25-50% of total allotypic Ig at 4 wk of age and the amount of donor Ig produced remained stable for up to 20 mo. Total allotypic Ig

There is no evidence of ultrastructural damage as assessed by scanning and transmission electron microscopy on the exposure of group A and AB human platelets to the appropriate anti-A and anti-AB alloantibody and complement. There was also no evidence of platelet aggregation under the above circumstances. These in vitro findings support the view that an ABO mismatch is not an absolute contraindication to platelet transfusion.

In a 2-year trial at a single center, prospective HLA-DR matching for cadaver renal transplantation exerted a stronger effect than either HLA-A and B matching or blood transfusion. One-year cumulative graft survivals for two-DR-matched organs was 92%. Grafts matched for one DR antigen had a cumulative 1-year graft survival of 65% whereas grafts matched for zero DR antigens had a cumulative 1-year survival of 41%. When all cadavers with less than two identifiable DR antigens were excluded from this analysis, however, the graft survivals of the groups known to be mismatched for one or two DR ant

The influence of pre-transplant blood transfusions and haemodialysis has been analyzed in 191 consecutive living related first transplants. A significantly increased graft survival was seen among patients having received blood transfusions and transplanted with a 1 haplotype disparate kidney, while no effect could be observed in HLA-identical transplantations. No significant effect could be detected on recipient survival. Four patients to receive 1 haplotype mis-matched grafts were sensitized against their potential donors by transfusions, and therefore not transplanted with living related don

Our experience from the Gothenburg material indicates that the graft prognosis is highly dependent on pretransplant blood transfusions, to a lesser degree on avoidance of two mismatches for the HLA-B-locus and, not the least, on the use of methylprednisolone antirejection therapy. It seems that a rather moderate dosage of methylprednisolone allows us to prolong and repeat the courses according to individual needs, and this may be the main reason for the significantly improved graft function at one year. The experiences gained and the lessons learned from the transplant programme in Gothenburg

Although HLA-matched platelet transfusions are of value in the support of some alloimmunized thrombocytopenic patients, poor posttransfusion increments can be observed following HLA-matched platelet transfusions and conversely good posttransfusion increments may result after HLA-mismatched platelet transfusions. We have explored the possibility that in vitro assays in addition to HLA typing might better select compatible donors for refractory recipients. Our studies suggested that a platelet migration inhibition assay is predictive of platelet transfusion responses in HLA-compatible and incomp

The mixed lymphocyte culture (MLC) is known to react to major histocompatibility complex disparities but is insensitive to minor histocompatibility antigens. To determine which modifications could amplify reactivity to these antigens, we have studied the effects of presensitization on normally nonresponsive MLC in dogs. Lymphocytes, primed in vitro to and restimulated with cells of a DLA-identical sib, did not react to these specific cells, in contrast to normal proliferation to other stimulators. However, lymphocytes obtained after in vivo sensitization against a DLA-identical donor showed in

One hundred ninety-one consecutive living related transplants performed from 1969 to the end of 1978 have been analyzed for the effect of pretransplant blood transfusions. Superior graft survival was observed in transfused patients transplanted with a one HLA haplotype-disparate kidney, whereas no effect of blood transfusions could be observed on the survival of HLA-identical transplants. The frequency of first rejection episodes was significantly reduced in transfused compared to nontransfused one haplotype-mismatched transplants, while no influence of blood transfusions was seen in patients

To investigate the influence of matching for HLA-DR antigens in renal transplantation, we assessed the outcome of 170 prospectively HLA-typed cadaveric kidney transplantations performed since 1977 in one center. We found a beneficial effect on graft survival of HLA-DR compatibility between donor and recipient (P < 0.05). A possible effect of matching for the HLA-A and B antigens could be seen only in the HLA-DR-mismatched combinations. Pretransplantation blood transfusions were associated with increased graft survival only in patients receiving HLA-DR mismatched transplants (P < 0.02). We conc

In order to select MLC incompatible one-haplotype related donor-recipient pairs that would achieve better graft survival and in an effort to alter the recipient immune response, 45 patients received three fresh blood transfuions from their prospective kidney donors. Recipient sensitization was evaluated by cross-match testing weekly sera obtained during and after the blood transfusions against donor T- and B-lymphocytes at 5 C (cold) and 37 C (warm). Thirteen (29%) of the 45 potential related recipients developed a positive warm T-cell cross-match or a persistent warm B-cell cross-match to the

Transfusion reactions range from mismatching of blood causing reaction, to the transmission of diseases such as hepatitis through the improper screening of blood. This paper discusses guidelines for transfusions of various blood fractions or components, reasons for transfusions of red blood cells rather than whole blood, the problem of blood platelets, the prohibition against fibrinogen, and the various volume expanders.
