The influence of blood transfusions, HLA, A and B, DR matching was studied in adult patients transplanted in one centre. The conclusions were: 1. Pre-transplant blood transfusion(s) are a prerequisite for graft acceptance. 2. In transfused patients with no HLA A and B mismatch, there is only 8% graft loss due to irreversible rejection. 3. In transfused patients with a functioning graft after six months, a decrease in the number of reversible rejection crises is observed in patients with no HLA A and B mismatch or with identity for HLA B7 or HLA B8. 4. 80 cases of anti-Rho(D) treatment after mismatched transfusions are reviewed to compare the intramuscular (i.m.) and intravenous (i.v.) route of treatment. No severe reactions such as renal failure occurred with either method of anti-D treatment. If the Rh-immune globulin is injected i.m., 20 micrograms anti-D per ml red cells are used. For i.v. anti-D administration, 10-12 micrograms anti-D are suggested. The i.v. method of anti-D treatment is recommended for the future. Furthermore, a scheme of treatment for after mismatched transfusions is described.

In order to investigate the influence of pretransplant blood transfusions on renal graft survival, the results in 43 recipients with 10 or more transfusion (group A) were compared with those in 48 recipients with less than 10 or no transfusions (group B). In both groups cadaveric kidneys with mainly 3 or more mismatched histocompatibility antigens were transplanted. The relationships between immune reactivity after blood transfusions, subsequent kidney allograft survival, and donor selection were studied in dogs. Animals with a high as well as low serological immune reactivity toward antigens contained in blood transfusion were observed.
