A novel allograft combined with modified purse-string suture fixation for annulus fibrosus repair: biomechanical evaluation in a porcine lumbar disc model.

BACKGROUND: Annulus fibrosus (AF) defects may permit recurrent nucleus pulposus extrusion and are difficult to repair because of limited vascularity and persistent mechanical loading. Commercial bone-anchored annular closure devices are intended to occlude large post-discectomy defects, whereas tissue-derived grafts may provide an alternative defect-filling strategy. However, the immediate sealing performance of allograft-based AF repair remains incompletely characterized. PURPOSE: This study developed an allograft designed to stably repair AF defects and evaluated its sealing performance in

Video-assisted thoracoscopic diaphragmatic plication in a ventilator-dependent patient with an elevated and dysfunctional hemidiaphragm.

Unilateral hemidiaphragm dysfunction can reduce ventilatory reserve, but its contribution to weaning failure is difficult to isolate in a critically ill patient. A 61-year-old woman with asthma and a chronically elevated left hemidiaphragm developed severe pneumonia and acute hypercapnic respiratory failure, with an arterial partial pressure of carbon dioxide (PaCO2) of 164.9 millimeters of mercury (mmHg), followed by recurrent extubation failure and tracheostomy. Pulmonary embolism was excluded. Bronchoscopy, microbiological treatment, nutritional support, and treatment for suspected autoimmu

Does meniscal tear morphology independently predict repair failure? A context-adjusted analysis of 500 consecutive arthroscopic meniscal repairs.

PURPOSE: To determine whether meniscal tear morphology independently predicts repair failure following arthroscopic meniscal repair after adjustment for patient-, meniscal- and repair-related characteristics and to quantify the incremental prognostic value of tear morphology beyond established clinical predictors. METHODS: A retrospective multicenter cohort study included 500 consecutive patients undergoing primary arthroscopic meniscal repair with a minimum follow-up of 24 months. Repairable tear morphologies included longitudinal vertical, bucket-handle, radial, horizontal cleavage, complex

Intraparenchymal Occipital Cyst Formation after Contralateral Dural Arteriovenous Fistula Obliteration: A Case Report.

Postoperative intracranial cyst formation is a rare complication. We report a case of an air-containing cyst in the contralateral left occipital lobe parenchyma after ligation of a dural arteriovenous fistula of the right transverse sinus. We reviewed related literature and discussed the mechanisms of this rare complication. A 59-year-old man presented with a right transverse sinus dural arteriovenous fistula with cortical reflux discovered incidentally during a brain checkup. The patient underwent surgical ligation of the draining vein combined with spinal drainage for intraoperative brain re

Arthroscopic Posterior Cruciate Ligament Tibial Spine Avulsion Fixation Using a Suture Pull-Through Single-Tunnel Technique.

INTRODUCTION: Posterior cruciate ligament (PCL) tibial avulsion fractures are uncommon intra-articular injuries that may cause persistent posterior instability, loss of motion, and functional impairment if displaced fractures are not anatomically reduced. Arthroscopic suture pull-through fixation offers stable transosseous fixation while avoiding complications associated with screw fixation. AIM: The aim of this study was to evaluate the functional and radiological outcomes of arthroscopic PCL tibial avulsion fixation using a suture pull-through single-tunnel technique. MATERIALS AND METHODS

Holt-Oram syndrome presenting with failure to thrive due to a hemodynamically significant patent ductus arteriosus: the importance of early cardiac evaluation in infants with radial ray abnormalities.

BACKGROUND: Holt-Oram syndrome (HOS) is a rare autosomal dominant disorder caused by pathogenic variants in the TBX5 gene and characterized by radial ray malformations associated with congenital heart defects and/or cardiac conduction abnormalities. Although atrial and ventricular septal defects are the most common cardiac manifestations, other congenital cardiac lesions may occur. Early recognition of cardiac involvement is essential to prevent complications and improve outcomes. CASE PRESENTATION: We report the case of an 8-month-old female infant presenting with failure to thrive, feeding

Retroperitoneoscopic lymphatic disconnection for non-filarial chyluria in Vietnamese adults: a case series.

INTRODUCTION: Retroperitoneal laparoscopic surgery is a widely applied technique in the management of renal, adrenal, ureteral, and retroperitoneal lymphatic disorders. A representative condition is chyluria, a rare disease caused by a lymphatic-urinary fistula, most commonly involving the ureter and renal pelvis. The retroperitoneal approach provides direct access to the affected structures while minimizing intra-abdominal manipulation, thereby reducing the risks of bowel adhesions and visceral injury. This technique enables ligation, excision, or suturing of abnormal lymphatic vessels, poten

Median Arcuate Ligament Syndrome in an Elderly Patient: A Symptomatic Post-stenotic Aneurysm and Systemic Atherothrombotic Disease.

Median arcuate ligament syndrome (MALS) is atypical in elderly patients, in whom it frequently coexists with severe atherosclerotic disease. We report the case of a 77-year-old male with marked mitral regurgitation who presented with intestinal claudication and malnutrition. CT angiography demonstrated MALS associated with a fusiform aneurysm of the celiac trunk with dissection, along with 60% atherosclerotic stenosis of the superior mesenteric artery. Given the limiting ischemic presentation, the patient underwent laparoscopic release of the ligament. The procedure was complicated by avulsion

Surgical Ligation for Partial Anomalous Pulmonary Venous Connection With Dual Drainage in an Elderly Patient.

BACKGROUND: Partial anomalous pulmonary venous connection with dual drainage into both the superior vena cava (SVC) and left atrium (LA) is a rare congenital anomaly, with few reported cases. CASE SUMMARY: A 78-year-old woman presented with exertional dyspnea. Cardiac computed tomography revealed that the right pulmonary vein drained into both the SVC and LA, resulting in a significant left-to-right shunt. We ligated the anomalous connection with a median sternotomy. This approach allowed avoidance of complex reconstructions and cardioplegic arrest. DISCUSSION: Simple ligation may represent

Delayed post-traumatic left gastric artery pseudoaneurysm presenting with massive upper gastrointestinal hemorrhage: a case report.

INTRODUCTION AND IMPORTANCE: Post-traumatic visceral artery pseudoaneurysms are rare vascular entities but carry a high risk of catastrophic hemorrhage. They most commonly involve the hepatic artery, while pseudoaneurysms of the left gastric artery are exceptionally uncommon. Delayed presentation several years after trauma is particularly rare and poses significant diagnostic challenges. PRESENTATION OF CASE: We report the case of a 37-year-old woman who presented with massive hematemesis and hemodynamic instability. She had a history of childhood abdominal trauma following a road traffic acc

Reconstruction Versus Primary Closure in Salvage Total Laryngectomy: Impact on Complications and Functional Outcomes - a Clinical Case Series.

BACKGROUND: Salvage total laryngectomy (STL) remains a difficult procedure in patients with recurrent or persistent laryngeal neoplasms after radiotherapy and chemoradiotherapy. The choice of the most appropriate method for pharyngeal closure continues to be debated, especially in relation to the possible associated postoperative complications and functional recovery. OBJECTIVE: To compare postoperative complications and functional outcomes between primary pharyngeal closure and pectoralis major flap reconstruction in patients undergoing salvage total laryngectomy. MATERIALS AND METHODS: A r

Sleeve fractures of the patella: current concepts in diagnosis and surgical management.

PURPOSE OF REVIEW: Patella sleeve fractures are the most common patellar fracture in children yet remain frequently missed. Additionally, the options for their repair have grown. This review revisits how the injury is recognized and how the expanding range of fixation choices should be navigated. RECENT FINDINGS: Recent cohorts confirm that recognition rests on the examination rather than the radiograph, as the injury is frequently confused with Sinding-Larsen-Johansson syndrome. In sleeve fractures that are displaced or minimally displaced and in which the extensor mechanism is disrupted, op

Implementation of Spring Assisted Cranioplasty at a Single Institution.

Spring-assisted cranioplasty (SAC) is a minimally invasive surgical technique for the treatment of craniosynostosis that may reduce operative morbidity compared with traditional open cranial vault remodeling procedures. A retrospective review was conducted of all patients who underwent SAC between May 2019 and August 2024 at a single academic institution serving a socioeconomically vulnerable population. Demographic, clinical, and operative variables were collected, including suture involvement, spring configuration, perioperative outcomes, complications, and need for secondary procedures. A t

Nonabsorbable Versus Delayed-Absorbable Sutures in Vaginal Uterosacral Ligament Suspension: A Retrospective Cohort Study.

INTRODUCTION AND HYPOTHESIS: The optimal suture material for uterosacral ligament suspension (USLS) remains uncertain. We compared surgical outcomes between nonabsorbable and delayed-absorbable sutures in vaginal USLS. METHODS: This retrospective cohort study included patients who underwent vaginal hysterectomy with USLS for uterine prolapse and completed 1-year follow-up. The primary outcome was composite surgical failure at 1 year, defined as vaginal bulge symptoms, apical descent greater than half of the total vaginal length, anterior or posterior descent beyond the hymen, or retreatment f

Sequential thoracoscopic thoracic duct occlusion and intra-abdominal lymphatic intervention for feline refractory idiopathic chylothorax.

Surgical management of feline idiopathic chylothorax remains unpredictable, with reported success rates varying widely despite combined procedures. We report a case of refractory idiopathic chylothorax in a cat that was successfully managed through a strategic, sequential surgical approach guided by indocyanine green (ICG) fluorescence. Initially, the cat underwent thoracoscopic thoracic duct occlusion (TDO) via clipping, but chylous effusion persisted. A 2nd surgery was conducted to achieve definitive thoracic duct ligation using sutures combined with additional clipping, yet the effusion rem

Arcuate fracture of the fibular head: two-tunnel surgical technique.

Avulsion fractures of the fibular head, commonly manifesting as the "arcuate sign" on radiographs, are indicative of posterolateral corner (PLC) injuries of the knee and, if inadequately managed, may lead to chronic instability and degenerative joint changes. This technical note describes a novel surgical technique for the fixation of arcuate fractures following failed anchor repair. The procedure involves dual tunnel formation in the fibula and strategic FiberTape passage to achieve anatomical reduction and stable fixation of the avulsed fragment. Key advantages of this approach include a low

Editorial Commentary: Suture Tape Augmentation for Revision Anterior Cruciate Ligament Reconstruction: Finding Ways to Improve on Previous Failures.

When a primary anterior cruciate ligament reconstruction fails, we look for something to do differently the second time around, and suture tape augmentation has increasingly become part of that conversation in revision surgery. There are several known risk factors for graft failure-younger age, higher activity level, smaller graft diameter, and increased tibial slope-but it remains unclear which patients, if any, would benefit from suture augmentation in the primary setting. Suture augmentation may well have a role in revision anterior cruciate ligament reconstruction, particularly for soft-ti

A single-aortotomy end-to-side technique simplifies murine aortic transplantation.

BACKGROUND: Transplant arteriosclerosis remains a major cause of late graft failure after solid organ transplantation. Murine aortic transplantation models have provided important insights into the mechanisms underlying chronic allograft rejection, but the surgical procedure remains technically demanding. We developed a modified end-to-side aortic transplantation technique utilizing a single recipient aortotomy to simplify vascular anastomosis in mice. METHODS: Recipient infrarenal aortas were occluded proximally and distally using microvascular clamps. A single longitudinal aortotomy was cre

Round versus tape sutures with interference fit anchors perform similarly in cadaveric glenoid and greater tuberosity biomechanical testing.

PURPOSE: To determine whether suture shape (flat-tape vs. round-wire) influences biomechanical performance at the suture-anchor-bone interface in interference-fit anchors used for rotator cuff and glenoid labrum repair. METHODS: Four matched pairs of human cadaveric shoulders were used for the glenoid model, with six anchor sites per glenoid. Five matched pairs of cadaveric humeri were used for the rotator cuff model, with three anchor sites per humerus. Each model tested multiple suture-anchor constructs under cyclic loading followed by load-to-failure. Biomechanical outcomes included cyclic

A Case of a Cardiac Re-Rupture Caused by the Removal of Pericardial Patches Late after a Sutureless Repair for a Cardiac Rupture.

INTRODUCTION: Surgical treatment is the only reliable therapeutic option for a left ventricular free wall rupture (LVFWR). There are 2 major surgical techniques, namely, sutureless repair and sutured repair; a ventricular pseudoaneurysm is a late postoperative complication. We herein report a case of a re-ruptured left ventricular pseudoaneurysm after a sutureless bovine pericardial patch repair. CASE PRESENTATION: The patient was a 79-year-old woman who underwent a sutureless pericardial patch repair for a blow-out-type left ventricular rupture following an anterior myocardial infarction 10
