Request for aortography on 82 patients in one hospital in 1978 were assessed to determine the effect of the investigation on the patient's subsequent management. In 49% of cases the request was fully justified. In the remaining 51% of patients, none of whom had reconstructive surgery, the request was considered either inappropriate (38%) or of marginal value (13%). Thirty-three patients with acute purulent otitis media and mastoiditis caused by Gram-negative bacilli are presented. The main features of the disease include: predilection for young male infants, a high rate of complications that include sepsis, mastoiditis and osteomyelitis of the base of the skull. A study was performed to analyse antibiotic usage in a surgical department, with the object of providing a basis for improving antibiotic policies. Preventable deaths from small bowel obstruction result from misdiagnosis, inappropriate delay in operation, inadequate preoperative preparation, and a poorly performed operative procedure. A systematic approach to each of the factors includes repeat physical examination, x-rays and CBC four hours after initial study in questionable cases, a pre-planned therapeutic interventions timetable, adequate intravascular volume and electrolyte levels, and a carefully designed operation which includes as little bowel manipulation and entrance as possible.

Pulmonary infection due to Pneumocystis carinii is now recognized as the leading cause of death from infection in patients with a hematologic malignancy who are in remission. Submucous Resektion (SMR) operation ascribed to Killian-quite commonly taught-has serious deficiencies in some clinical situations. The more rational and conversative technique of septoplasty is explained and its principles and indications discussed. Postsurgical deformities after inappropriate SMR (i.e. septal perforation) can be avoided by performing a well executed conversative septoplasty. Complications of SMR as well as septoplasty are discussed. Some basic principles and indications for closure of nasal septal perforations are described.

Exocrine pancreatic function was studied after meal stimulation and intravenous hormone stimulation with secretin and cholecystokinin in 10 patients with duodenal ulcers before, and 3 to 4 months after, highly selective vagotomy.
