DisNet : Learning interpretable depression representations in speech.

Speech-based depression detection (SDD) offers an objective and convenient method for depression screening and intervention. However, existing deep learning methods lack interpretability, which prevents them from revealing their findings for the quantitative detection of depression. To address these issues, we propose an interpretable Depression screening Network (DisNet), which contains a Learnable frequency-domain FilterBank (LFB) module and a Hierarchical speech Representation Extraction (HRE) module. LFB utilizes a learnable filterbank to select effective frequency bands in speech for gene

Code Blue Awareness in Dental Students: A Pilot Project to Develop Awareness in the Clinic.

INTRODUCTION: The "Code Blue" system is a standardised emergency response protocol designed to ensure rapid intervention in critical medical situations, particularly cardiopulmonary arrest. Although mandatory in Türkiye since 2011, inappropriate activations still occur, often due to limited awareness among healthcare staff, including dental students. This pilot project aimed to assess dental students' knowledge and awareness regarding Code Blue and to examine whether these outcomes differ according to the level of clinical training. MATERIALS AND METHODS: A cross-sectional survey was conducte

Implantable cardioverter-defibrillator use in patients with left ventricular assist device: prediction of ventricular arrhythmia using the VT-LVAD score.

BACKGROUND: The survival benefit of implantable cardioverter-defibrillators (ICD) in patients with left ventricular assist device (LVAD) remains unproven. The VT-LVAD score was developed to stratify arrhythmic risk and may help identify patients most likely to benefit from ICD therapy. We aimed to retrospectively assess its ability to identify patients at higher risk of ventricular arrythmias and to describe ICD-related complications in a population of patients with ICD and LVAD. METHODS: A total of 63 primary continuous-flow LVAD implantation were performed at our institution between January

Management of severe dysplastic nevus (high-grade dysplasia): Italian recommendations for good clinical practice.

"Dysplastic nevus" (DN) is a histopathological term used to describe nevi with cytological atypia and architectural disorder. With the 2018 World Health Organization (WHO) classification, the grading system for atypia/dysplasia was simplified into two categories: low-grade dysplasia and high-grade dysplasia (severely atypical dysplastic nevus [SDN]); this classification is currently in use, and its criteria are applied by pathologists for the diagnosis of DN. Actually, there are currently no definitive guidelines for managing patients with DN. This lack of clear direction has historically led

Assessment of Antibiotic Prescribing Patterns Among Dentists and their Impact on Antimicrobial Resistance in Oral Infections.

BACKGROUND: Antibiotics are frequently prescribed in dental practice for the management of oral infections and as prophylaxis for invasive procedures. Inappropriate prescribing practices contribute significantly to the emergence of antimicrobial resistance (AMR), posing a growing public health concern. METHODS: A cross-sectional observational study was conducted among 150 dental practitioners and 120 patients presenting with odontogenic infections. Data regarding antibiotic choice, dosage, duration, and indication were collected using structured questionnaires and prescription audits. Microbi

Understanding Sustainability in Operating Theaters: An Ethnographic Study to Determine Drivers of Unsustainable Behaviors.

BACKGROUND: Climate change is the biggest threat to human health. Paradoxically, the healthcare sector is a major contributor to climate change, and operating theaters are among the highest sources of emissions. Unsustainable practices are actions that compromise environmental, social, and financial sustainability, leading to unnecessary resource use, avoidable harm to the wider population, and reduced ability to provide effective healthcare in the future. Drivers of unsustainable practices and barriers to sustainability in practice (a top priority identified by the James Lind Alliance Priorit

A Risk Model Incorporating the Novel Inflammatory Biomarker CD64 for Predicting Bloodstream Infection in Suspected Cases.

Background/Objectives: Bloodstream infection (BSI) is a significant cause of mortality. The availability of a convenient tool for predicting the risk of BSI at the early stage would be beneficial for clinicians, allowing them to improve the outcomes of BSI and avoid antibiotic overuse. Methods: A multivariate prediction model was constructed based on conventional laboratory test results and novel serum inflammatory markers in a cohort of patients with suspected BSI over a one-year period using least absolute shrinkage and selection operator (LASSO) and logistic regression. Results: BSI was con

Frequency of inappropriate endovenous truncal ablation therapy within the vascular quality initiative varicose vein registry.

OBJECTIVE: Chronic venous disease is a pervasive problem, with a spectrum of severity and an equally diverse range of potential treatment settings and proceduralists. The adoption of endovenous ablation (EVA) techniques has facilitated the growth in office-based and ambulatory procedures. Concurrently, the impact of external factors including market compensation and insurance policies is known to influence procedural patterns away from evidence-based therapy. Using the Vascular Quality Initiative Varicose Vein Registry (VQI-VVR), we aimed to investigate trends in EVA therapies and ascertain tr

The biorhythm concept revisited: A narrative critical review and epistemological analysis of a persisting but scientifically invalid term in chronobiology.

The term biorhythm, introduced in the early 20th century by Wilhelm Fliess, a German otolaryngologist and associate of Sigmund Freud, refers to the supposed existence of universal and deterministic biological cycles governing human physical, emotional, and intellectual capacities. Fliess proposed that human behavior and physical performance follow fixed 23-day (physical), 28-day (emotional), and 33-day (intellectual) cycles - sinusoidal and predetermined from birth. Despite the lack of experimental validation and the absence of any identified physiological mechanisms, this term continues to ap

Mechanistic insights into sulfate-driven performance adaptation and membrane fouling in a UASB-SBR-AXMBR system: metabolic network reconstruction and microbial community succession.

Sulfate-rich wastewater poses considerable challenges to the operational stability of biological treatment systems. This study investigated the long-term (294 days) response of a combined UASB-SBR-AXMBR process to stepwise increases in sulfate concentration from 100 to 2000 mg/L. The system maintained stable carbon and nitrogen removal performance under sulfate stress, with COD removal exceeding 90.3% and total nitrogen removal stabilizing at 85.5% via a partial nitritation-anammox (PN/A) pathway. However, high sulfate loading significantly intensified membrane fouling, with the primary drivin

Beyond the obvious: pneumothorax masquerading as a cardiac emergency.

A non-smoking housewife in her 70s with underlying hypertension, dyslipidaemia and kyphoscoliosis presented with chest pain and acute respiratory distress. Reduced breath sounds were noted over the right lung. Initial ECG showed ST-segment elevation in leads V3-V5, and focused echocardiography showed septal wall hypokinesia, suggestive of myocardial infarction. Fibrinolysis therapy was started due to percutaneous coronary intervention service limitation. Despite treatment, there were no signs of improvement. A right-sided pneumothorax was detected on CXR done near the end of fibrinolysis. Cons

Gonadal recovery in men with prolactinomas on cabergoline: 6-month testosterone levels as a predictor of testosterone replacement timing.

PURPOSE: Hypogonadotropic hypogonadism is a frequent manifestation of prolactinomas in men. Dopamine agonists can restore gonadal function; however, the timeline and predictors of recovery are not well defined. This study focuses on hypogonadotropic hypogonadism exclusively among men with prolactinomas. METHODS: We retrospectively analyzed 84 men with prolactinomas treated with cabergoline (CAB) for at least 12 months, without prior pituitary surgery or radiotherapy. Hypogonadotropic hypogonadism was defined as morning testosterone < 300 ng/dL with low or inappropriately normal luteinizing ho

Virtual Reality illusions systematically perturb balance in young and older adults.

BACKGROUND: Virtual reality (VR) illusions-manipulated visual cues that alter perceived motion-have shown potential to perturb balance and elicit postural adaptations. While younger adults demonstrate clear responses to illusions, less is known about how older adults respond to similar perturbations during dynamic tasks. Understanding age-related differences in these responses is critical for developing inclusive, VR-based balance rehabilitation interventions. OBJECTIVES: This study aimed to (1) determine whether older adults respond to VR illusions differently than younger adults, (2) evalua
