Study of the integrated process of cutting and longitudinal upsetting of high-precision workpieces using the energy of elastic unloading of the press.

This paper investigates an integrated blank separation process combining cutting and plastic deformation (longitudinal upsetting) to ensure high volumetric and geometric accuracy. The relevance of the study stems from the need to produce high-quality blanks for cold forging and extrusion, where simple blanks made from long rolled products often exhibit unacceptable dimensional deviations. Based on 3D finite element modeling in DEFORM software and experimental studies, an optimal two-support cutting scheme was determined. Unlike cantilever (single-support) cutting, this scheme creates oppositel

Human Factors Analysis of Vendor-Induced Flow Disruptions in Orthopedic Surgery: Implications for Workflow Integration and System Safety.

OCCUPATIONAL APPLICATIONThis study found that vendors were involved in nearly one-third of all observed flow disruptions during orthopedic surgery, with a disproportionate share linked to coordination issues and protocol failures, including breaches of the sterile field. While vendors provide critical technical expertise on equipment and implants, their involvement can unintentionally blur role boundaries and disrupt team coordination in high-stakes environments. For ergonomics and human factors practitioners, these findings underscore the importance of designing systems that support clearer r

Responsible use of large language models in gastroenterology and hepatology.

Large language models (LLMs) and related generative artificial intelligence (AI) systems are rapidly entering clinical workflows, including in gastroenterology and hepatology, where text-heavy documentation, guideline-driven care, and high-volume patient messaging create a strong demand for decision support and automation. Deployment raises distinctive risks: hallucinations and unsafe recommendations, automation bias, privacy and confidentiality threats, inequitable care, intellectual property and licensing uncertainty, and unclear allocation of responsibility across clinicians, institutions,

Risk Stratification for ESBL-Producing Enterobacterales in Elderly Diabetic Patients with Urinary Tract Infections: A Multicenter Model to Support Empirical Antibiotic Decision-Making.

OBJECTIVE: Antimicrobial resistance among elderly diabetic patients with urinary tract infections (UTIs) poses a significant challenge for empirical antibiotic therapy. Delayed availability of microbiological susceptibility results often leads to treatment mismatch and inappropriate broad-spectrum antibiotic use. This study aimed to develop and validate a clinically interpretable risk stratification model to estimate the probability of extended-spectrum β-lactamase (ESBL)-producing Enterobacterales isolation prior to microbiological confirmation, thereby supporting early empirical antibiotic d

Stage-specific remodeling of wingless-related integration sites (WNT) signaling during oocyte-to-embryo transition in pigs.

The WNT signaling pathway is a central regulator of cell polarity, adhesion, cytoskeletal dynamics, and lineage specification during early embryonic development. Although its roles have been extensively studied in murine and human models, the temporal regulation and pathway architecture of WNT signaling during early porcine development remain poorly defined. Here, we performed a comprehensive transcriptomic analysis to characterize WNT pathway dynamics across key stages of pig in vitro development, including immature oocytes (IMO), mature oocytes (MO), zygotes (ZY), cleaved embryos (2-4 cells;

Cornering: A Geometric Framework for Surgical Dissection in Minimally Invasive Surgery.

BackgroundPrecise tissue dissection is a fundamental component of minimally invasive surgery (MIS), yet it is often taught implicitly. While triangulation and traction-countertraction are widely recognized as geometric principles for exposure and transection, the geometric conditions that enable the dynamic advancement of dissection along tissue interfaces have not been clearly articulated.MethodsWe performed a schematic geometric analysis of surgical dissection focusing on the relationships among traction direction, opposing tissue surfaces, and the dissecting edge. Based on this analysis, we

Imaging appropriateness in pediatric emergency department using the American College of Radiology criteria: an observational study.

BACKGROUND: Even though children are typically more susceptible to radiation-induced illnesses than adults, pediatric radiology is an essential part of contemporary practice. Consequently, one of the most important performance metrics for patient safety is the appropriateness of radiologic procedures. The appropriateness criteria of the American College of Radiology (ACR) are evidence-based guidelines designed to assist referring physicians and other healthcare providers in decision-making regarding diagnostic imaging. METHODS: Using ACR criteria as a reference, this study attempts to assess

Case Report: Revision surgery for a missed posterior dislocation of the humeral head in a shoulder fracture-dislocation and literature review.

BACKGROUND: Posterior shoulder dislocation is uncommon and therefore prone to being missed. When accompanied by a proximal humeral fracture, complex fracture lines may obscure the radiographic signs of dislocation. Inadequate standardization of the imaging workup may consequently result in inappropriate management and treatment failure. CASE PRESENTATION: We report a young patient in whom posterior shoulder dislocation was missed at the initial assessment because axillary and scapular Y-view radiographs were not obtained and the available imaging was insufficiently interpreted. The dislocatio

A comparative study of the risk of delayed cerebral ischemia after clipping and coiling of ruptured intracranial aneurysms: a systematic review and meta-analysis.

INTRODUCTION: Delayed cerebral ischemia (DCI) is one of the main causes of disability and death following surgical treatment of ruptured intracranial aneurysms. Many studies combine data from patients undergoing clipping and coiling; however, this approach may be inappropriate. The purpose of this study was to compare the incidence of DCI between the two surgical methods and to explore, separately for each method, the predictive factors of DCI. METHODS: This study systematically searched relevant articles on DCI after intracranial aneurysm surgery published in the PubMed, EMBASE, and Web of S

Insulinoma after Roux-en-Y gastric bypass successfully treated by endoscopic ultrasound-guided radiofrequency ablation.

Post-bariatric hypoglycemia (PBH) can significantly impact patient quality of life and is generally referred to as postprandial hyperinsulinemic hypoglycemia or late dumping. In this report, we present the case of a 55-year-old man with a recent history of Roux-en-Y gastric bypass (RYGB) surgery and multiple admissions owing to hypoglycemic coma postoperatively. Initially, the hypoglycemic episodes were interpreted as being caused by PBH. Due to symptom severity, a fasting test was performed, revealing hypoglycemia and inappropriate hyperinsulinemia, suggestive of an insulinoma. Further comput

Non-invasive in vivo monitoring of transplanted human intestinal organoids using bioluminescence.

BACKGROUND: Xenotransplantation of pluripotent stem cell-derived human intestinal organoids (HIOs) into animal hosts models early human small intestinal development. However, traditional methods to assess HIO engraftment and expansion involve invasive, non-survival procedures. We developed a novel HIO platform featuring stable intestinal epithelial expression of a red-shifted luciferase, enabling sensitive, longitudinal in vivo tracking of HIO xenografts. METHODS: Human embryonic stem cell (hESC) line H9 was lentivirus-transduced with a plasmid encoding red-shifted luciferase under a ubiquiti

Dual-functional 3-aminobenzenesulfonic acid additive enhances cellulose pretreatment in DMSO-rich ionic liquid systems.

Ionic liquids (ILs) are effective agents for cellulose pretreatment but are hindered by high cost and high viscosity. While dilution with dimethyl sulfoxide (DMSO) reduces IL viscosity and consumption, high DMSO content (IL/DMSO ≤4:6, w/w) reduces pretreatment efficacy. Herein, 3-aminobenzenesulfonic acid (ABS) was introduced as an additive to enhance pretreatment efficiency within a 1-allyl-3-methylimidazolium chloride ([Amim]Cl)-DMSO system (4:6, w/w). ABS operates through two mechanisms: (1) binding to [Amim]+ to release Cl- (Proven by 1H and 35Cl NMR spectroscopy, ESI-MS, and molecular dyn

Missing-data-aware machine learning prediction of in-hospital major adverse cardiovascular events after primary percutaneous coronary intervention for ST-segment elevation myocardial infarction.

Machine learning (ML) offers opportunities to improve prognostication after ST-segment elevation myocardial infarction (STEMI), but real-world registries frequently contain incomplete data, and inappropriate handling of missingness may degrade performance. We retrospectively evaluated 659 consecutive STEMI patients undergoing PCI during the index admission. The primary outcome was in-hospital major adverse cardiovascular events (MACE). Eighty clinical, electrocardiographic, laboratory, echocardiographic, and angiographic variables were analyzed, with up to 40% missingness in some predictors. A
