Asperginase-based chemotherapy regimen for acute lymphoblastic leukemia: A cohort study with promising outcomes on adults.

BACKGROUND: Acute lymphoblastic leukemia (ALL) is a malignancy of lymphoid progenitor cells. Asparaginase-based regimens are highly effective in pediatric ALL, but concerns over toxicity have limited their use in adults. This study evaluates the incidence of adverse events following asparaginase treatment in the adult ALL population. MATERIALS AND METHODS: In this retrospective cohort study, we assessed drug-related adverse events among 67 adults with ALL treated with either a hyper-central vascular access device (CVAD) (n = 34) or an asparaginase-containing regimen (n = 33). Those under aspa

Femoral Nerve Palsy as a Complication of Pavlik Harness Treatment for Developmental Dysplasia of the Hip: Ten-Year Prospective Study.

BACKGROUND: Femoral nerve palsy (FNP) is a recognized complication of Pavlik harness (PH) treatment for developmental dysplasia of the hip (DDH), but its true incidence and clinical significance may be underestimated. This prospective study aims to determine the incidence, risk factors, and clinical course of FNP in infants undergoing PH treatment, and to evaluate whether FNP predicts treatment failure. METHODS: A 10-year prospective study (2015 to 2025) was conducted on all infants treated with a PH for Graf II to IV DDH at a specialist center. Patients were reviewed weekly with clinical and

Higher risk of recurrence in partially thrombosed cerebral aneurysms post-WEB (Woven EndoBridge) device treatment: insights from the WorldWideWEB Consortium registry.

BACKGROUND: The Woven EndoBridge (WEB) device is a prevalent treatment for intracranial aneurysms. While many studies have assessed the obliteration rate post-WEB embolization, few have focused on long-term outcomes in partially thrombosed aneurysms. OBJECTIVE: To assess whether partially thrombosed aneurysms are at higher risk of recurrence or retreatment following WEB embolization compared with non-thrombosed aneurysms. METHODS: We evaluated data from 22 academic institutions, focusing on previously untreated cerebral aneurysms treated with the WEB device. Logistic regression was utilized

Treatment of Disturbed Sleep in Progressive Supranuclear Palsy: a randomized, remote, double-blinded, 6-week cross-over design study protocol comparing zolpidem, suvorexant, and placebo.

BACKGROUND: Prior research identified profound sleep disruption in progressive supranuclear palsy (PSP). The hypothalamus and brainstem, areas that help regulate sleep/wake patterns, are among the earliest affected brain regions in PSP disease progression. Comparing polysomnography and quantitative-neuropathology metrics, we identified relative sparing of wake-promoting nuclei in PSP compared to Alzheimer's disease, though PSP had more disrupted sleep. It led to the hypothesis that PSP patients have hyperinsomnia (or hyposomnia, little sleep) due to degeneration of sleep nuclei with a preserva

Prenatal and postnatal exposure to endocrine-disrupting chemicals: Implications for extremely preterm infants, narrative review.

Preterm birth affects approximately 13.4-15 million infants annually and remains a leading cause of neonatal morbidity and mortality. Increasing evidence suggests that endocrine-disrupting chemicals (EDCs) may represent an important and underrecognized contributor to adverse outcomes in extremely preterm infants (<28 weeks' gestation). These infants experience a "double-hit" exposure-prenatally via placental transfer and postnatally through intensive care interventions-during critical windows of development. EDCs, including phthalates, bisphenols, parabens, persistent organic pollutants, per-

Nuclear CK1δ as a critical determinant of PER:CRY complex dynamics and circadian period.

BACKGROUND: Up to 84% of women who undergo operative vaginal delivery receive neuraxial analgesia. However, little is known about the association between neuraxial analgesia and neonatal morbidity in women who undergo operative vaginal delivery. The authors hypothesized that neuraxial analgesia is associated with a reduced risk of neonatal morbidity among women undergoing operative vaginal delivery. METHODS: Using United States birth certificate data, the study identified women with singleton pregnancies who underwent operative vaginal (forceps- or vacuum-assisted delivery) in 2017. The autho

Hemodialysis elevates postoperative seizure risk without worsening overall outcomes after surgical clipping of unruptured intracranial aneurysms.

Patients with end-stage renal disease on long-term hemodialysis (HD) are at increased surgical risks due to chronic uremia, vascular fragility, and associated comorbidities. Limited data exist regarding outcomes of surgical clipping of unruptured intracranial aneurysms in patients on HD. We retrospectively reviewed the data of 425 patients who underwent surgical clipping of unruptured intracranial aneurysms. Among the 425 patients, 15 were on maintenance HD. We performed 1:2 propensity-score matching, adjusting for patient demographics, comorbidities, and aneurysmal characteristics, to yield 3

A comparative study of targeted end-tidal carbon dioxide versus standard management to guide resuscitation in emergency department cardiac arrest in Thailand: study protocol for a three-arm randomised controlled trial.

BACKGROUND: Cardiac arrest managed in the emergency department (ED) is associated with suboptimal survival outcomes. End-tidal carbon dioxide (EtCO2) monitoring during cardiopulmonary resuscitation (CPR) reflects cardiac output, tissue perfusion, and pulmonary blood flow. Observational evidence suggests that higher EtCO2 levels during CPR are associated with return of spontaneous circulation (ROSC), but the effect of actively targeting higher EtCO2 thresholds remains uncertain. The primary objective of this trial is to determine whether CPR guided by higher EtCO2 targets improves sustained ROS

Preprocedural vascular sheath insertion reduces hospital mortality in high risk PCI patients.

High-risk patients undergoing percutaneous coronary intervention (PCI) are prone to serious complications, such as ischemia-reperfusion injury, hemodynamic collapse, and cardiogenic shock. While mechanical circulatory support (MCS) may mitigate these risks, its prophylactic use is limited by increased healthcare costs and device-related complications. Preemptive insertion of femoral arteriovenous sheaths, a novel strategy designed to facilitate rapid MCS deployment in emergency situations, may address these concerns, but its clinical benefits remain unclear. In this multicenter retrospective s

Acute effects of caffeine withdrawal on headache among regular caffeinated coffee drinkers.

Headaches are among the most common neurological disorders, often leading to significant disability and reduced quality of life1,2. However, much remains unknown about the triggers that precipitate headaches3. Coffee is one of the most widely consumed beverages worldwide, and coffee withdrawal is thought to be associated with headache4,5. However, research linking caffeine withdrawal to headache in healthy adult populations is largely limited to observational studies prone to confounding or single snapshot assessments prone to recall bias6-8. While small, near-term randomized trials have been

Operative predictors of mortality and neurologic outcomes after modified Bentall for acute aortic dissection: A systematic review and meta-analysis.

IntroductionThe modified Bentall procedure is frequently required in acute type A aortic dissection (ATAAD) with root involvement, a high-risk subgroup with substantial early mortality and neurologic complications. Prior studies have examined operative predictors such as cardiopulmonary bypass (CPB) time, cross-clamp duration, circulatory arrest, and conduit type, but findings remain inconsistent. This systematic review and meta-analysis aimed to determine whether specific operative parameters influence early mortality or neurologic outcomes following the Bentall procedure in ATAAD.MethodsA sy
