Early real-world experience with thoracoabdominal branch endoprosthesis from a physician-initiated multicenter postmarket registry.

OBJECTIVE: Recent commercial approval of the thoracoabdominal multibranch endoprosthesis (TAMBE) has enabled wide dissemination of four-vessel multibranched endovascular aortic repairs in the United States. However, the real-world performance benchmarks remain incompletely characterized. This study evaluated 30-day clinical outcomes of patients receiving TAMBE across diverse practice settings. METHODS: We performed a retrospective study of commercial TAMBE implants across multiple US centers between May 2024 and November 2025. The primary outcomes were 30-day mortality and technical success.

Preoperative alignment and risk of proximal junctional failure : a framework for upper instrumented vertebra selection in adult spinal deformity.

AIMS: Proximal junctional kyphosis (PJK) remains a major complication after surgery for adult spinal deformity (ASD). While postoperative alignment is a recognized modifiable risk factor, objective methods for selecting the upper instrumented vertebra (UIV), a key modifiable factor, are lacking. We aimed to determine whether preoperative sagittal alignment, specifically cervicothoracic alignment, predicts the risk of PJK, and whether this risk can be mitigated by UIV selection, focusing on factors available at the time of surgical planning. METHODS: From a multicentre, prospective ASD registr

The role of visceral debranching in hybrid thoracoabdominal aortic aneurysm repair: A narrative review of indications, techniques, and outcomes.

Thoracoabdominal aortic aneurysm (TAAA) repair remains associated with significant morbidity and mortality, especially in elderly and comorbid patients. Although total endovascular repair using fenestrated or branched endografts has broadened therapeutic options, anatomical limitations, patient frailty, and logistical constraints still prevent a purely endovascular approach in many cases. For this subset, hybrid repair with visceral debranching has emerged as an alternative strategy, though its role remains controversial. This narrative review explores the rationale, indications, technical app

[CNGOF statement on instrumental vaginal deliveries using vacuum extraction].

The Obstetrics Committee of the CNGOF, consulted by the CNP GOGM at the request of the ANSM, has reviewed the safety of obstetric vacuum extraction. Instrumental delivery may be performed to shorten the expulsive phase, particularly in cases of fetal heart rate deceleration. The available instruments are the vacuum, forceps, and spatulas, with the choice depending on the clinical situation and the operator. In France, approximately 12% of births involve instrumental assistance, a rate that has remained stable since 2016. The vacuum is used in about two-thirds of these cases - a proportion simi

Cerebrovascular Events in Aortic Stenosis: From Native Valve Disease to TAVR-Specific Risk and Prevention.

Aortic stenosis (AS) is associated with a heightened burden of cardiovascular comorbidities, atrial fibrillation (AF), and progressive valvular calcification, all of which may contribute to cerebrovascular events across the disease continuum. However, true epidemiologic evidence establishing AS as an independent stroke risk factor remains limited, and much of the contemporary concern regarding stroke has shifted toward interventional management. Transcatheter aortic valve replacement (TAVR) has become the dominant therapy for severe AS, but periprocedural and early postprocedural stroke remain

Association of wearable-device-measured fragmented sedentary behavior with cardiovascular disease, mortality, and life expectancy.

OBJECTIVES: It remains unclear whether the degree of fragmentation modifies the adverse health outcomes associated with prolonged sedentary behavior. This study aims to investigate the associations of objectively measured sedentary patterns with incident cardiovascular disease (CVD), mortality, and life expectancy. METHODS: This prospective cohort study included 89 010 UK Biobank participants (median age 63.5 years, 56.5% women) who provided 3 to 7 valid days of wrist accelerometer data. Daily sedentary time and number of sedentary breaks were the metrics of interest. Four overall sedentary b

Pacemaker Syndrome: A Narrative Review.

Since the first pacemaker implantation in 1958, cardiac pacing has transformed the management of bradyarrhythmias and conduction system disease. Despite major technological advances and the widespread use of dual-chamber and biventricular pacing systems, pacemaker therapy may be complicated by pacemaker syndrome, a clinical entity resulting primarily from atrioventricular (AV) dyssynchrony. Pacemaker syndrome is characterized by a constellation of cardiovascular and neurologic symptoms including fatigue, dyspnea, dizziness, hypotension, palpitations, and neck pulsations. Although initially ass
