Low radial and axial force stent retriever reduces symptomatic subarachnoid hemorrhage after mechanical thrombectomy for acute middle cerebral artery and medium vessel occlusion: a prospective pilot study.

BACKGROUND: Subarachnoid hemorrhage (SAH) is a well-recognized complication after mechanical thrombectomy (MT) and may adversely affect clinical outcomes. SAH commonly results from vessel injury due to overextension or displacement during device retrieval. This risk is particularly concerning in smaller, tortuous vessels, such as in medium vessel occlusions (MeVO). This study evaluated whether a novel low radial and axial force stent retriever (Tron FX II) could reduce post-procedural SAH without compromising recanalization outcomes. METHODS: The study comprised two components: (1) bench test

Role of antiseizure medication in postoperative late seizure and survival among patients with newly diagnosed glioblastoma: A multicenter retrospective study.

BACKGROUND: We conducted a multicenter study to investigate the utility of postoperative antiseizure medication (ASM) in the occurrence of late seizure in glioblastoma. In addition, we analyzed whether postoperative ASM affects survival. METHODS: 1132 consecutive patients with newly diagnosed glioblastoma at 39 centers were enrolled. In patients treated with postoperative ASM (levetiracetam [LEV], lacosamide [LCM], or perampanel [PER]), as well as in those who received no medication, the cumulative incidence of late seizure, adverse events, and survival outcomes were analyzed. RESULTS: In to

Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Structural basis for Mg²⁺-glycine synergy in the tandem glycine riboswitch revealed by cryo-EM.

Part I of this article discussed the potential functional importance of genetic mutations and alleles of the human cytochrome P450 2D6 (CYP2D6) gene. The impact of CYP2D6 polymorphisms on the clearance of and response to a series of cardiovascular drugs was addressed. Since CYP2D6 plays a major role in the metabolism of a large number of other drugs, Part II of the article highlights the impact of CYP2D6 polymorphisms on the response to other groups of clinically used drugs. Although clinical studies have observed a gene-dose effect for some tricyclic antidepressants, it is difficult to establ

Flow diverter braid deformation following treatment of cerebral aneurysms: incidence, clinical relevance, and potential risk factors.

BACKGROUND: Although flow diverters (FDs) have benefited from several technical improvements, recently concerns have arisen regarding the braid stability after implantation. Thus, we investigated frequency, predictive factors, and clinical impact of the phenomenon of FD braid deformation (FDBD). METHODS: Consecutive intracranial aneurysms (IAs) treated with various FDs, between January 2018 and July 2023, were reviewed to identify FDBD (defined as the deformation of a FD without any external force applied to it). Patient, aneurysm, procedural, and FD characteristics were retrieved and analyze

Association Between Cannula Size of Extracorporeal Membrane Oxygenation and Prognosis in Patients With Out-of-Hospital Cardiac Arrest - A Secondary Analysis of the SAVE-J II Study.

BACKGROUND: Selecting an appropriate cannula size is crucial for achieving an adequate extracorporeal membrane oxygenation (ECMO) flow rate. However, the association between ECMO cannula size and the prognosis of patients with out-of-hospital cardiac arrest (OHCA) has not been fully elucidated. We examined the associations between ECMO cannula size and neurological outcomes and survival at discharge in patients with OHCA who received ECMO. METHODS AND RESULTS: This is a secondary analysis of the Study of Advanced life support for Ventricular fibrillation with Extracorporeal circulation in Jap

Impact of porcelain aorta on outcomes in transfemoral aortic valve implantation (TAVI) with balloon-expandable valves.

BACKGROUND: Current guidelines recommend transcatheter aortic valve implantation (TAVI) for patients with aortic stenosis and porcelain aorta (PA). Neurological outcomes of patients with PA undergoing TAVI with modern valves require clarification as most trials examined balloon-expandable valves (BEV) and self-expandable valves in intermediate or high-risk patients, but not specifically in patients with PA. Our aim was to compare outcomes, including stroke and mortality, in well-matched patients with and without PA who received BEV during transfemoral TAVI procedures. METHODS: Consecutive pat

The effect of delayed-release antibiotics on the rate of postoperative wound infection for implanted neuromodulatory devices.

OBJECTIVE: Implanted neuromodulatory devices are important for the surgical management of pain, epilepsy, movement disorders, and spasticity. These devices are surgically implanted, but many must be periodically explanted and replaced as batteries deplete or devices fail. Replacement surgery risks further infections that require device explantation, incur large financial costs, and reduce patient quality of life. To reduce infection rates, a single surgeon began using vancomycin-impregnated absorbable calcium sulfate delayed-response beads with neuromodulatory device and intrathecal pump prima

Textbook outcomes as a novel patient-centric metric to inform carotid revascularization.

OBJECTIVE: The recent National Coverage Determination surrounding carotid stenting and shared decision-making has ushered in an era of patient-centric carotid care. However, historical carotid intervention endpoints have lacked patient-centered nuances to inform clinical decisions. Accordingly, we aimed to create a comprehensive novel, patient-centric textbook outcome (TO) to inform treatment paradigms. METHODS: We developed a novel composite TO for patients who underwent carotid revascularization reflecting a combination of patient-centric outcomes derived from previous patient interviews an

Disability and associated outcomes among patients suffering periprocedural strokes after carotid artery stenting.

OBJECTIVE: Perioperative stroke after carotid artery stenting (CAS) is rare. However, the degree of disability and long-term effects from a postoperative stroke remain unclear. Our goal was to assess the degree of disability from a stroke after transcarotid artery revascularization (TCAR) and transfemoral CAS (TFCAS) for asymptomatic carotid artery disease, and the associated 1-year impact on subsequent neurological events and mortality. METHODS: The Vascular Quality Initiative CAS registry (2016-2023) was queried for CAS performed for asymptomatic disease. Patients with a postoperative strok

Effectiveness of human albumin for clinical outcome in aneurysmal subarachnoid hemorrhages: a protocol for randomized controlled (HASH) trial.

BACKGROUND: Aneurysmal subarachnoid hemorrhage (aSAH) is a dreadful acute neurological condition with an overwhelmingly high rate of associated morbidities and mortality. Despite leaping advancement in neurosurgical techniques and imaging modalities, there is no substantiative improvement in the overall prognosis for aSAH. Cerebral vasospasm remains the predominant cause of associated morbidities. Human albumin has been used in different neurological conditions, including head trauma, intracerebral hemorrhages, and ischemic strokes, with favorable outcomes. However, its beneficial use in aSAH

American Heart Association Automated External Defibrillator Symposium: Summary and Recommendations.

The American Heart Association (AHA) introduced public access defibrillation more than 30 years ago. Since then, we have seen the growth of public access defibrillation programs across many settings within communities. However, despite high expectations that the availability of automated external defibrillators (AEDs) and more integrated public access defibrillation programs would dramatically increase cardiac arrest survival, AEDs are used in the United States in only 4% of out-of-hospital cardiac arrests and survival rates have remained disappointingly low. In follow-up to a recent Internati

Rate and fate of incidental durotomies in spine surgery.

BACKGROUND: Incidental Durotomy (ID) is a well-recognized complication from spine surgery and its occurrence is often linked to negative outcomes following spine surgery. However, there is conflicting evidence in the literature regarding the risk factors, incidence and outcome following ID. This study aims to assess the rate and fate of ID in spine surgery. METHODS: A retrospective review of a prospectively maintained database all adult patients who underwent spinal surgery by a single board certified neurospine surgeon between January 2016 and October 2024 was performed to identify all patie
