The incidence of clinically significant cystoid macular edema (CME) following cataract surgery has risen sharply with the widespread utilization of intraocular lenses. To evaluate the factors contributing to the incidence of CME, the senior author has divided 1808 of his own cataract cases into three groups according to procedures used: 1) intracapsular cataract extraction (ICCE) with no lens implantation; 2) ICCE with implantation of an iris-supported lens; and 3) extracapsular cataract extraction (ECCE) with implantation of a posterior chamber lens. The limited experience with LRK precludes a valid comparison with IOLs and extended-wear contact lenses. Only observations, unsupported by valid statistical analysis, are possible. A randomized prospective clinical trial of feeder vessel photocoagulation for proliferative sickle cell retinopathy was performed. In Chicago there were 34 argon laser-treated eyes and 30 control eyes. In Kingston there were 53 xenon arc treated eyes and 50 control eyes. Twenty-nine eyes, representing an infection rate of 4.9%, developed corneal ulcers following corneal transplant surgery; 26 with available data are presented. All occurred in the graft or at the wound margin. The median postoperative time to ulcer development was 5.5 months. All eyes were on topical steroids when the ulcer developed. Energy-dispersive x-ray analysis (EDXA) was used to study nine intraocular lenses from one manufacturer. Residual polishing compound was found on two of three dry-sterilized lenses from a "hot lot" associated with a high incidence of sterile hypopyon. Similar material was not present on normal lenses. Liquid fluorescein (2%) was instilled into the inferior cul-de-sac and the precorneal tear film and the cornea were observed utilizing a specialized examination routine. Following the initial fluorescein instillation and observations, sequential fluorescein instillations were performed at five-minute intervals six additional times. The major complications with extended wear lenses are due to hypoxia and lens deposits. Certain patients tend to deposit proteins and lipids on the lens surface with daily wear and at a greater frequency during prolonged wear. A build-up of these deposits can be associated with decreased vision, conjunctival irritation, and giant papillary conjunctivitis. In a survey of 162 keratoconus patients, we set out to investigate individual conditions, such as asthma, hay fever, connective tissue syndromes, and eye rubbing, and to determine their relationship of keratoconus. It was found that the prevalence of asthma rises from 0.4% to 1% in the control group to 17.9% in the keratoconus group, or an eighteenfold increase. In addition, the incidence of hay fever in keratoconus patients was found to be 35.7%. This presentation recounts the original description of CME and the more recent developments associated with intravenous fluorescein angiography in its study and diagnosis. The work of previous authors to show the incidence of CME and its possible prognosis are reviewed. Our findings in a retrospective study agree with the other authors and show that there is no particular increase in CME following intraocular lens implantation regardless of the type of surgical procedure used.
