INTRODUCTION: Periprosthetic and peri-implant femoral fractures (PPF) include periprosthetic hip fractures (periPC), periprosthetic knee fractures (periPR), as well as peri-implant fractures associated with an osteosynthesis nail (periCL). Although risk factors for PPF are widely studied, there is scarce evidence on differential risk factors for each subtype. The objective of this study is to compare 30-day and 1-year mortality rates among the three PPF subtypes and identify differential risk factors between them.
METHODOLOGY: A retrospective cohort study conducted in a tertiary hospital, inc

AIMS: Despite the benefits of carbon fiber (CF) implants, concerns over their higher cost relative to titanium implants have limited their broader adoption in orthopaedic oncology. Our study compared the costs and postoperative complications associated with titanium and CF intramedullary nails (IMNs) in patients with femoral bone metastases.
METHODS: We conducted a retrospective analysis of patients treated with titanium or CF IMNs for impending or completed pathologic femoral fractures due to metastatic bone disease between 2016 and 2023. Inpatient costs, 30-day costs, and 30-day postoperati

Closed diaphyseal femoral fractures in children with cerebral palsy classified as Gross Motor Function Classification System (GMFCS) levels IV/V are frequently managed with flexible intramedullary nails (FIN). This study aimed to evaluate the survival of FIN with distal ends buried within the distal femoral metaphyseal cortex. Secondary outcomes included the occurrence of complications, especially those requiring reoperation. Children were divided into two groups based on the location of the distal nail. In group A, the nail ends were buried within the distal femoral metaphyseal cortex. In gro

PURPOSE: Short proximal femoral nail antirotation (PFNA) fixation is commonly performed for intertrochanteric fractures and is typically inserted without reaming. However, limited reaming may be required in cases of a narrow femoral canal, severe femoral bowing, or inadvertent selection of an oversized nail. Unlike long nails, reaming for short PFNA is confined to the isthmus, and its effects have not been well described. This study aimed to evaluate the association between limited reaming during short PFNA fixation and clinical outcomes.
METHODS: We conducted a retrospective matched cohort s

Treatment-emergent central sleep apnea (TECSA) can occasionally be observed when obstructive sleep apnea (OSA) is managed with most therapeutic approaches, including the mandibular advancement device (MAD). Although TECSA appears to be a low-incidence condition, evidence is lacking on its mechanism and possible health risks. The primary objective of this critical review is to provide guidance to clinicians managing OSA with MAD. Prompt return to a sleep medicine specialist is recommended, however, when a patient continues to present complaints or reports potentially associated with central sle

BACKGROUND: Percutaneous pedicle screw fixation (PPSF) is widely used for thoracolumbar fractures because it is minimally invasive and facilitates early recovery. However, some patients experience marked pain in the ultra-early postoperative period, which may hinder early mobilization and compromise enhanced recovery after surgery (ERAS). This study aimed to identify factors associated with immediate severe pain within 0-8 h after PPSF and to evaluate the discriminatory performance of a multivariable model.
METHODS: We retrospectively included 598 consecutive patients with thoracolumbar fract

BACKGROUND: Periprosthetic patellar fractures (PPPFs) following total knee arthroplasty (TKA) remain a challenging complication with high rates of fixation failure and functional impairment. Optimal surgical management for type II PPPF remains unclear. This study compared outcomes between transtibial tension band wiring (TTTBW) and heavy suture fixation (HSF).
METHODS: A retrospective review identified 18 patients with type II PPPF treated between 2020 and 2023 (HSF, n = 12; TTTBW, n = 6). Primary outcomes included implant survival and extensor mechanism function, assessed by extension lag, f

INTRODUCTION: Pelvic fractures in elderly patients are associated with significant pain, functional decline, and prolonged rehabilitation, with the majority managed non-operatively. In the conservative management paradigm, intact upper extremity function is critical for mobilization with assistive devices; concurrent upper extremity (UE) fractures may therefore complicate recovery. The aim of this study was to evaluate the clinical outcomes associated with this injury combination in patients with conservatively managed pelvic fractures.
METHODS: A retrospective cohort study was conducted at a

BACKGROUND: Disruption of the distal tibiofibular syndesmosis is a common complication of ankle fractures that can result in chronic instability and early osteoarthritis. This study aimed to compare syndesmotic widening and complications after plate fixation versus intramedullary fibular nailing. We hypothesized that fibular nailing would be associated with more syndesmotic widening but fewer complications.
MATERIALS AND METHODS: Patients treated with plate or nail fixation for ankle fractures at a level 1 trauma center were identified through query of the electronic medical record. Data incl
