Pain Outcomes Following Modern External Ring Fixation Compared with Internal Fixation for Severe Open Tibial Fractures: A Secondary Analysis of a Prospective Randomized Trial (FIXIT).

BACKGROUND: It is unclear whether postoperative pain differs by treatment type for patients with severe open tibial fractures. METHODS: We performed a secondary analysis of data from the FIXIT study. Adults with severe open tibial fractures were randomized to undergo definitive modern external ring fixation (n = 122) or internal fixation (n = 132). Primary outcomes were pain intensity and interference at 6 and 12 months, measured by the Brief Pain Inventory. Secondary outcomes were Numeric Pain Rating Scale (NPRS) scores and the incidence of moderate to severe pain. Post hoc subanalysis compa

Radial entry for ESIN fixation of pediatric radial fractures: a 10-year cohort study with focus on neurological complications.

BACKGROUND: Elastic stable intramedullary nailing (ESIN) is an established treatment for pediatric radius fractures. However, the optimal surgical entry point remains debated due to differing complication profiles. The radial approach carries a risk of superficial branch of the radial nerve (SBRN) irritation, whereas dorsal approaches have been associated with extensor tendon injuries. This study evaluates the neurological complication profile of the radial approach and provides clinically relevant risk factors. METHODS: A retrospective single-center study was performed including 285 pediatri

Does the operative technique (MIPO vs. ORIF) influence surgical outcomes and complications in proximal humerus fractures? A matched-pair analysis.

INTRODUCTION: Proximal humerus fractures account for approximately 4-6% of all fractures and are among the most common fractures in the elderly. Surgical treatment is increasingly applied for displaced and unstable fracture patterns. The two most commonly used techniques are open reduction and internal fixation (ORIF) via a deltopectoral approach and minimally invasive plate osteosynthesis (MIPO) using a deltoid-split or anterolateral approach. ORIF allows direct fracture visualization and anatomical reduction but is associated with more extensive surgical exposure. MIPO aims to preserve soft

Reinforcement K-wires reduce the risk of tibial plateau and lateral cortex fractures during open wedge high tibial osteotomy: a prospective comparative cohort study.

BACKGROUND: Open wedge high tibial osteotomy (OWHTO) may be complicated by iatrogenic fractures of the tibial plateau or lateral cortex, potentially compromising osteotomy stability. Reinforcement with intra-operative Kirschner wire (K-wire) has been proposed to mitigate this risk, but robust clinical evidence is limited. This study aimed to evaluate whether intra-operative reinforcement wires decrease the incidence of iatrogenic fractures during OWHTO and to assess their impact on clinical and radiological outcomes. METHODS: A prospective multi-centre comparative cohort study was conducted b

Hybrid versus single internal fixation strategies in minimally invasive surgery for multiple rib fractures: a retrospective comparative analysis.

BACKGROUND: Surgical stabilization of rib fractures typically employs a single type of internal fixation material. While this "Single Strategy" offers the advantage of procedural consistency and facilitates a rapid learning curve for surgeons, it often fails to address the heterogeneity of multiple rib fractures (MRF). Factors such as fracture location, segment length, degree of comminution, and associated intrathoracic visceral injuries frequently render a single material insufficient for all fracture types within a single patient. Consequently, we adopted a clinical approach utilizing multip

What is the optimal implant removal timing following hook plate fixation in distal clavicle fractures: a retrospective analysis from subacromial osteolysis cases.

BACKGROUND: Clavicular hook plates are widely used for the fixation of distal clavicle fractures, yet prolonged retention of the hook plate is associated with subacromial osteolysis (SAO), a common complication that may lead to poor functional outcomes. The optimal timing for implant removal remains unclear. PURPOSE: This retrospective study aims to identify the predictive factors for SAO and determine the optimal timing for implant removal that balances the prevention of complications with sufficient fracture healing. METHODS: This retrospective review was conducted on 92 patients who underwe

Percutaneous versus subcutaneous titanium elastic nail tips: complication profiles by age group in pediatric forearm shaft fractures.

BACKGROUND: Titanium elastic nailing (TEN) is widely used for the treatment of unstable pediatric forearm fractures. Although subcutaneous nail tips remain the standard technique, superficial (exposed) nail tip placement has been described as an alternative strategy to facilitate implant removal. However, evidence regarding complication profiles associated with different TEN tip management techniques remains limited, particularly with respect to age-related differences. This study addresses three critical gaps: (1) comparative complication rates between superficial and subcutaneous TEN placeme

Management of Distal Femoral Periprosthetic Fractures after Total Knee Arthroplasty: Fixation versus Revision Reconstruction.

The incidence of periprosthetic distal femur fractures after total knee arthroplasty (TKA) is increasing in parallel with the increasing number of primary TKA procedures being performed. This number will continue to rise going forward. Treatment decisions depend on patient factors, fracture characteristics, and implant stability. It is important to have a thorough understanding of the available modalities for treatment, including their indications, advantages, and disadvantages, to effectively manage these injuries and optimize outcomes. Nonoperative treatment is indicated in select cases but

Intramedullary nailing versus sliding hip screw for AO/OTA 31-A2 and 31-A3 trochanteric fractures: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Trochanteric femoral fractures (TFFs) are frequent injuries in older adults, with unstable patterns (AO/OTA 31-A2 and A3) carrying a higher risk of complications. Intramedullary nailing (IMN) and sliding hip screw (SHS) are the two main surgical options. However, guideline recommendations differ, and prior meta-analyses are limited. This study aimed to systematically compare the efficacy and safety of IMN versus SHS in treating adult patients with unstable TFFs based on randomized controlled trials (RCTs). METHODS: MEDLINE, Embase, and CENTRAL (January 2008-March 2025) were search
