Küntscher Y-nailing is indicated for unstable proximal fractures of the femur in the elderly (5-7,10). The Williams modified Y-nail is an interlocking Y-nail that was designed to extend the indications beyond those of the Küntscher Y-nail. Sixty-eight patients with idiopathic scoliosis treated with Zielke instrumentation are reviewed. Comparison of Groups A and B demonstrates a statistically significant reduction in the incidence of rod fracture by modifying preoperative selection criteria and operative technique as described. Fifty-eight patients who had scoliosis or kyphosis of varying etiologies were followed for a minimum of two years (average, forty-four months) after segmental spinal instrumentation using sublaminar wires. In eight (19 per cent) of the patients who had been operated on for scoliosis one or both rods broke. The average time that had elapsed before the breakage was discovered was twenty-three months. In 1980 a review of lower limb amputation over a 3.5 year period between 1974 and 1978 was reported from our centre. More recently 193 amputations were performed for peripheral vascular disease over a similar 3.5 year period, representing an increase of 33 per cent in the amputation rate during the last 6 years. Sixty-four adult patients (87 diaphyseal forearm fractures) were treated by plating. The authors have observed forty-two cases of fractures and dislocations of the spine in children. In twenty-eight cases there was a complete spinal cord lesion and in four cases a partial cord lesion with some recovery. Ten children had no neurological lesion. Manual traction reduction without anesthesia and immediate immobilization in a spica cast were used to treat eighty-five children ranging in age from birth to ten years old who had closed femoral-shaft fractures and no associated injuries. The status of the fibula is an index of the severity of injury. Open tibial fracture with associated fractured fibula is more likely to develop complications. Antibiotics should be started promptly. Judicious debridement is essential but bone fragments should be left whenever possible. When there is any question about contamination, the wound is best left open. Rigid occlusive dressings promote epithelial ingrowth. Fracture distraction and plate fixation are associated with a high incidence of complications.
