Transanal excision of rectal lesions using a single port robot: outcomes of initial 81 resections.

Transanal excision of rectal lesions remains challenging due to limitations in visualization and access to the cephalad margin, often resulting in high rates of piecemeal excision and positive margins. Transanal Minimally Invasive Surgery (TAMIS), developed using laparoscopic instruments, aimed to improve transanal exposure and reach. We present our experience with single-port robotic TAMIS (SP rTAMIS) to assess its safety and efficacy. A prospectively maintained database of SP robotic colorectal resections performed between October 2018 to September 2024 was retrospectively reviewed to identi

Clinical Application of the London Classification in 3D High-Definition Anorectal Manometry: Prevalence, Limitations, and Complementary Metrics.

BACKGROUND: The London classification (LC) standardizes the interpretation of anorectal manometry, but real-world data using high-definition three-dimensional anorectal manometry (3D HD-ARM) remain limited. OBJECTIVE: To characterize anorectal manometric patterns in a large 3D HD-ARM cohort applying the LC and to assess additional functional parameters not included in the classification. METHODS: We retrospectively analyzed 300 consecutive 3D HD-ARM studies with balloon expulsion testing (BET). Clinical subgroups were defined using validated questionnaires: St. Mark's score for anal incontin

[Sphincter-preserving surgery for rectal cancer--From limits to perfection].

Sphincter-preserving therapy for low rectal cancer has always been a difficult and hot topic in colorectal cancer. Advanced understanding of the biological behavior of rectal cancer and the pelvic fascial anatomy have laid the theoretical foundation for sphincter preservation, shifting the surgical paradigm from radical resection alone toward both oncological safety and function preservation. Guided by the principle of TME and supported by minimally invasive technologies, a variety of surgical methods including low anterior resection(LAR), Bacon, intersphincteric resection(ISR), conformal sphi

Long-term outcomes and anorectal functional status of patients with anal squamous cell carcinoma treated with the modern technique of intensity-modulated radiotherapy.

BACKGROUND: Intensity-modulated radiation therapy (IMRT) may reduce mortality in patients with squamous cell carcinoma of the anus (SCCA), but there are limited data in long-term outcomes. This study reported 5-year outcomes in SCCA patients treated with IMRT-based definitive chemoradiation. METHODS: Sixty-five consecutive patients with T1-4N0-3M0 SCCA treated with IMRT-based concurrent chemoradiation (CCRT) from Jan 1, 2010, were retrospectively reviewed. Treatment outcomes included overall survival (OS), disease-free survival (DFS), anal cancer-specific mortality (ACSM), local-regional failu

A ten-year evaluation of the incidence of obstetric anal sphincter injury with a reduced episiotomy rate.

INTRODUCTION: The role of episiotomy in preventing obstetric anal sphincter injury (OASIS) remains controversial. Liberal use of episiotomy has been reduced locally. This study aimed to review the incidence of OASIS in our unit over the past decade given the reduced episiotomy rate. METHODS: A retrospective study was conducted in a single tertiary obstetrics and gynaecology unit. All singleton vaginal deliveries, including normal and instrumental deliveries, between 2012 and 2021 were included. Data were retrieved from the hospital electronic delivery database between July 2022 and June 2023.

Risk Factors for Colostomy after Chemoradiotherapy in Patients with Squamous Cell Carcinoma of the Anus: A Nationwide, Multicenter Analysis in Japan.

OBJECTIVES: This study investigated the incidence of colostomy, aiming to identify its risk factors following chemoradiotherapy (CRT) for squamous cell carcinoma of the anus (SCCA) in a large Asian cohort, and to clarify the prognostic significance of a complete response (CR). While CRT is the standard of care for SCCA, the risk of colostomy remains a major concern due to its potential impact on a patient's quality of life. Prior studies focused mainly on Western populations, and data from Asian cohorts are limited. METHODS: We conducted a retrospective multicenter study of 246 patients with

Spatiotemporal remodelling of the cloacal region determines the position of the anal opening in mouse embryos.

Conventional embryological models propose that the urorectal septum (URS) fuses with the cloacal membrane (CM) to form separate exits for the hindgut and urogenital sinus (UGS). However, previous studies have been re-evaluating this assumption, and the precise morphogenetic mechanisms determining the position of the anal opening remain unclear. To elucidate the spatial and temporal dynamics of cloacal development, we analysed mouse embryos from embryonic day (E)11.5 to E13.5 using serial midsagittal histological sections combined with three-dimensional reconstruction. We focused on the positio

From the anal canal to the pancreas: diagnostic challenges of radiologic surveillance in detecting rare but potentially curable metastases - a case report.

INTRODUCTION: Pancreatic metastases from anal canal squamous cell carcinoma (SCC) are exceptionally rare. To date, only one previous case has been reported. The present case represents the second documented instance, highlighting the unusual metastatic pattern and the challenges in diagnosis and treatment. Due to the rarity of such metastases, their clinical presentation and optimal management remain poorly understood. CASE PRESENTATION: We describe a 64-year-old woman diagnosed with locally advanced anal canal SCC, with diagnosis delayed as she had deferred medical consultation, who then rec

Establishing the safety of waterbirth for mothers and their babies: the POOL cohort study with nested qualitative component.

BACKGROUND: Intrapartum water immersion analgesia has been recommended by the National Institute for Health and Care Excellence since 2007, but high-quality evidence relating to the safety of waterbirth for mothers and their babies was lacking. PRIMARY STUDY OBJECTIVE: To establish whether, in the case of 'low-risk' women who use water immersion during labour, waterbirth, compared to birth out of water, is as safe for mothers and their babies. METHODS: A cohort study with non-inferiority design. SETTING: Twenty-six National Health Service organisations in England and Wales. PARTICIPANTS: T

Normalization of Dyssynergic Bear Down Patterns in the Seated Position Predicts Normal Balloon Expulsion Testing on High-Resolution Anorectal Manometry.

INTRODUCTION: Bear down patterns during anorectal manometry are classically evaluated in the left lateral decubitus position. Testing in the seated position may more accurately reflect functional outcomes. METHODS: Overall, 384 adults undergoing anorectal manometry were assessed for dyssynergia in the left lateral decubitus and seated positions. Logistic regression evaluated associations with balloon expulsion testing (BET) outcomes. RESULTS: Normalization of dyssynergic patterns in the seated position occurred in 3.9% (n = 19) resulting in higher odds of successful BET (<1 minute) (OR 30.31
