OBJECTIVE: Women from ethnic minority groups have higher rates of obstetric anal sphincter injuries (OASI) and often face barriers to follow-up care. This study aimed to design and pilot a questionnaire to explore ethnic minority women's postnatal experiences after OASI, identify barriers and facilitators to attending specialist perineal clinics, and assess the feasibility of this approach to inform a future mixed-methods study.
METHODS: The questionnaire was co-designed with input from community stakeholders and piloted among three groups at a UK hospital serving a large ethnic minority popu

Anal canal squamous cell carcinoma (ACSCC) is a rare malignancy, accounting for approximately 2% of all colorectal cancers. Brain metastases originating from this primary site are exceedingly uncommon and scarcely documented. The aim of this study is to review the literature-reported cases of brain metastases from ACSCC and to supplement this analysis with the description of a clinical case. A systematic review of the literature was conducted in accordance with PRISMA guidelines to identify published cases of brain metastases from ACSCC. The clinical and therapeutic features of these cases wer

INTRODUCTION: FI adversely affects nearly every aspect of daily life. Patients often avoid telling symptoms, and physicians may neglect to inquire about FI symptoms. This study aims to audit the point prevalence of FI, and risk factors among participants older than 50 years old. MATERIALS AND METHODS: Participants older than 50 years whose complaints other than FI were included into the study. They were strictly questioned about FI symptoms according to Rome 4 criteria. Demographics, comorbidities, history of anal surgery, and obstetric history were recorded. An anorectal examination was perfo

IMPORTANCE: Perineal wound complications following an obstetric anal sphincter injury (OASI) repair may affect pelvic floor recovery, continence, and sexual functioning, yet are understudied. Understanding risk factors for recurrent complications after OASI may aid in future delivery planning.
OBJECTIVE: This study aimed to identify characteristics associated with wound complications and compare rates of recurrent OASI and perineal wound complications after subsequent vaginal delivery in patients with and without prior wound complications.
STUDY DESIGN: This retrospective cohort study of pat

OBJECTIVE: Anastomotic stricture (AS) is a common complication following low-level anastomosis of the colon and rectum in rectal cancer (RC) surgery. The aim of this study is to evaluate whether intraoperative stay sutures combined with postoperative anal dilatation reduce the incidence of AS after low anterior resection with defunctioning stoma. METHODS: A total of 240 patients with low RC who underwent treatment at the Ninth Medical Center of the PLA General Hospital between February 2019 and May 2024 were randomly assigned to four groups (n = 60 each): one experimental group and three contr

OBJECTIVE: The optimal mode of subsequent delivery following obstetric anal sphincter injury (OASI) is arguable. This pilot study assessed the impact of using our suggested transperineal ultrasound (TPUS) based algorithm on recurrent OASI and cesarean deliveries (CD) rates, as well as anal incontinence, in a subsequent delivery.
METHODS: This retrospective observational study included women following OASI who sought consultation regarding the safety of a subsequent vaginal delivery (VD). The study group included women who underwent a comprehensive evaluation in a dedicated postpartum perineal

Surgical excision of perianal tumors presents significant technical challenges due to limited exposure, proximity to the anal sphincter complex, and an increased risk of contamination from the anal canal. Adequate visualization while preserving sphincter function and maintaining sterility is essential to ensure safe oncologic resection. Conventional retraction methods may provide suboptimal exposure in this anatomically constrained region. We describe a technical adaptation using the Alexis® wound retractor (Applied Medical Resources Corporation, Rancho Santa Margarita, California, United Stat

To aid the criminal investigation of sexual assault with anal penetration, we developed the enzyme-linked immunosorbent assay (ELISA) to identify Chloride Channel Accessory 1 (CLCA1), a rectal mucosa-secreted protein, in samples collected from foreign objects at the crime scene or from the suspect’s body. The developed procedure was highly specific to samples swabbed from the rectal mucosa and might be sensitive enough to test a trace sample collected from the crime scene. In addition, most of the sexual assault–related contaminants tested in this study showed less influence on the ELISA detec

CONTEXT: Stratified mucin-producing intraepithelial lesions (SMILEs) and invasive stratified mucin-producing carcinoma (ISMC) are well studied in the uterine cervix. These lesions have recently been described in the anorectal canal; however, understanding about their clinicopathologic features and clinical implications is limited.
OBJECTIVE: -To investigate the clinicopathologic features and behavior of SMILE/ISMC in the anorectal canal.
DESIGN: We identified 4 cases containing SMILE/ISMC in the anorectal canal between 2013 and 2024. Clinical data and histopathologic slides were reviewed.

Prolapse of the gravid uterus, which cannot be spontaneously reduced through the anal orifice, is extremely rare. It is considered to be incarcerated and constitutes a clinical and therapeutic emergency, as it endangers fetal and maternal life. However, few cases have been reported in the literature. Because of the rarity of these prolapses of the gravid uterus and their clinical presentation, especially in young women in rural areas with armed conflicts, in the form of a mass protruding from the anal canal with a reduction in the volume of the abdomen, the clinical diagnosis provides guidance

BACKGROUND: High-definition anorectal manometry (HD-ARM), balloon expulsion test (BET), and magnetic resonance defecography (MR defecography) are important tools in studying constipation. Our aim was to evaluate the prevalence of functional defecation disorders and structural anorectal alterations in patients with chronic constipation and compare the findings of HD-ARM and BET with MR defecography.
METHODS: Retrospective cohort study including patients with functional constipation or irritable bowel syndrome with constipation (IBS-C), refractory to medical therapy, submitted to HD-ARM, BET, a

Rectovaginal fistulae are uncommon but highly morbid, with presentations ranging from vaginal defaecation to subtle symptoms such as flatus, discharge, or recurrent infection. Imaging and contrast studies may be inconclusive for short, low-lying tracts, and ultrasound techniques can be limited by false-negative results. A structured examination under anaesthesia therefore remains central to definitive diagnosis and operative planning. The St Mark's protocol standardises this assessment using a reproducible five-stage sequence: (1) direct proctovaginal inspection with evaluation of the rectovag

INTRODUCTION: The aim of this study was to evaluate self-reported pelvic floor symptoms 12 months postpartum, in women after a first-time vaginal delivery complicated by an obstetric anal sphincter injury, according to body mass index. The hypothesis was that dyspareunia, urinary incontinence, and anal incontinence would be more common in overweight and obese women compared with normal weight women 12 months postpartum.
MATERIAL AND METHODS: This is a population-based cohort study. Altogether, 6595 primiparous women with a first-time vaginal delivery complicated by an obstetric anal sphincter

INTRODUCTION AND HYPOTHESIS: Obstetric rectal buttonhole tears (ORBT) are isolated rectal mucosal lacerations with intact anal sphincters. These injuries are frequently overlooked during delivery, resulting in delayed diagnosis, rectovaginal fistula formation, and the need for surgical referral.
METHODS: A descriptive case series was conducted on seven women referred for ORBT repair between X across Y under Z (60,000 deliveries). All patients had intact anal sphincters with confirmed rectal mucosal tears identified at the time of delivery through systematic digital rectal examination and subs
