Rare Coexisting Hepatocellular and Gallbladder Carcinomas: A Case Report.

BACKGROUND Multiple primary malignancies in a single patient are considered to be quite rare. However, due to the wider availability of advanced imaging methods and, more regular check-ups, they are becoming more common in clinical practice. To determine the diagnosis of multiple primary malignancies, each tumor must be completely separate and cannot be from metastasis of one or the other. Coexisting hepatocellular carcinoma and gallbladder carcinoma are extremely rare, with only a few case reports published. We believe that to determine the most effective therapeutic management, the results o

Therapeutic hyperthermia promotes lipid export and HSP70/90 during machine perfusion of human livers.

Liver transplantation is the only curative option for end-stage liver disease. Donor shortages necessitate the use of higher risk donor livers, including fatty livers, which are more susceptible to ischemia-reperfusion injury. Machine perfusion has improved graft utilization and is typically performed at hypothermic (8-12°C) or normothermic (35-37°C) temperatures. Here we studied the impact of mild hyperthermia (40°C) as a therapeutic intervention for fatty livers using in-depth proteomic and lipoprotein profiling of whole organ perfusion and precision-cut liver slices. We observed proteomic c

Preoperative plasma cell-free DNA chromosomal instability predicts microvascular invasion in hepatocellular carcinoma: a prospective study.

BACKGROUND: Microvascular invasion (MVI) has been recognized as a risk factor for early recurrence after hepatectomy in patients with hepatocellular carcinoma (HCC). This study aimed to estimate the performance of an ultrasensitive chromosomal aneuploidy detector (UCAD) model for preoperative MVI prediction in operable HCC patients based on plasma cell-free DNA (cfDNA). METHODS: A prospective study included HCC patients who underwent surgery in 2021. Preoperative peripheral plasma samples of eligible patients were collected to extract cfDNA, which was then subject to next generation sequencin

A novel off-liver sinusoidal progenitor cells in supporting transplantable bioliver engineering.

Liver transplantation is the only curative option for liver failure, which is currently limited by organ shortages. Although liver tissue engineering (LTE) with a strategy of decellularization/recellularization opens a new window to overcome this limitation, its challenges include the lack of specific seed cell sources and appropriate methods to support recellularization. In this preliminary study, both in vitro and in vivo experiments were performed. In vitro, we used novel off-liver progenitors of liver sinusoidal endothelial cells (LSECs) isolated from bone marrow mesenchymal stem cells (NG

Management of high-grade ovarian adenocarcinoma in an intraperitoneal pelvic renal transplant recipient.

BACKGROUND: The increasing number of organ transplant recipients has led to a rise in cancer diagnoses in this population. Kidney transplant recipients, in particular, have a two-to-four-fold higher risk of developing cancer due to immunosuppressive therapy. The management of gynecologic cancers in this subset of patients presents unique challenges due to anatomical considerations, immunosuppression, and the nephrotoxicity of oncologic treatments.Case Presentation.A 44-year-old woman with a history of polycystic kidney disease underwent a bilateral nephrectomy followed by a living-related intr

Pediatric partial heart xenotransplantation as an early use case of xenograft tissue.

Xenotransplantation is a promising advancement in the field of transplantation that could eliminate deaths on the waitlist and provide an unlimited supply of on-demand organs for those in need of this life-saving therapy. The results of preclinical studies in orthotopic heart xenotransplantation have shown that non-human primate models can consistently survive 9 months post-transplant. However, early clinical results in orthotopic heart xenotransplantation have been subpar compared to traditional orthotopic heart transplantation as the longest surviving patient survived for 60 days with a comp

Quantitative assessment of liver graft function using functional automated whole-liver score derived from gadoxetic acid-enhanced MRI: a prospective cohort study.

OBJECTIVES: Accurate and timely assessment of liver graft function is of great significance for the survival of liver grafts and liver transplant recipients. This study aimed to investigate the correlation between the functional automated whole-liver score (FAWLS) system derived from gadoxetic acid-enhanced magnetic resonance imaging (MRI) and quantitative measurements of liver graft function. Additionally, it sought to determine the utility of FAWLS in evaluating the severity of liver graft injury and monitoring treatment response following therapy, through an association analysis with liver

Early-stage transplant responses in miniature-liver implants based on decellularization-recellularization technology.

Decellularization and recellularization can help address the gap between organ demand and availability. Although several whole organ engineered liver constructs have been reported, they are unable to maintain long-term function. Additionally, the effect of the early stage on cells after implantation is not described in detail. To better understand which factors affect initial cell viability, we inoculated primary hepatocytes into a miniature recellularized liver (RCL). In this study, RCL grafts based on the decellularized right lobe of a mouse were populated with 1 × 107 primary hepatocytes of

Preimplantation Histopathologic Assessment of Deceased Donor Kidneys: Recommendations from the Banff Time-Zero Biopsy Working Group.

Transplantation using kidneys from deceased donors with marginal clinical characteristics is increasing to maximize access to transplant. Preimplantation biopsies of such kidneys are often obtained to determine the degree of chronic changes as an objective supplement to donor clinical-based assessment of organ quality, particularly in the United States. There is conflicting evidence and heterogenous practice about how preimplantation biopsies should be performed, interpreted, and incorporated into organ disposition decisions. In 2022, the Banff Foundation for Allograft Pathology tasked its Tim

Evolutionary dynamics of recurrent hepatocellular carcinoma under divergent immune selection pressures.

Hepatocellular carcinoma (HCC) is a highly lethal, aggressive malignancy. Little is known about the evolutionary trajectories of HCC and how clinical decision-making could be informed based on biopsies of the initial tumour. Here, we report the whole-exome sequencing of a unique series of resected HCC tumours and matched recurrences. This cohort included patients who received a liver transplant and who were immunosuppressed at time of recurrence, in comparison to patients who underwent liver resection for HCC and immunocompetent at time of recurrence, therefore facilitating analyses of immune

Diabetes and transplantation.

Transplantation is both a cause of, and a therapy for, diabetes. Solid organ transplantation requires immunosuppressive regimens that frequently cause temporary or permanent hyperglycaemia, which can influence the outcome of allograft function and also increase cardiovascular mortality and morbidity. Post-transplant diabetes should be actively screened for and managed in the early post-transplant period, and should also be screened for long term in all solid organ transplant recipients. Transplantation of whole pancreas or pancreatic islets is a long-established therapy for people with type 1

Microphotopheresis for the Treatment of Graft-Versus-Host Disease in Pediatric Patients.

INTRODUCTION: Extracorporeal photopheresis (ECP) is an attractive treatment for graft-versus-host disease (GVHD) following allogeneic hematopoietic stem cell transplantation due to the lack of systemic immunosuppression. Modifications of ECP without leukapheresis are being actively studied in clinical practice around the world. A low-volume method of extracorporeal photochemotherapy-micro-ECP-has been developed at the Rogachev Federal Scientific and Clinical Centre of Pediatric Hematology, Oncology and Immunology. We present the results of using micro-ECP to treat acute and chronic GVHD. PATI

The Role of Machine Perfusion for Liver Transplantation in Children.

Pediatric liver transplantation (PLT) is the only lifesaving procedure for children with advanced liver disease and tumors. Despite significant progress and overall success in PLT, the field continues to face challenges due to procedural complexity across a heterogeneous age group, especially in infants and small children. Despite the use of living donor grafts, waitlist mortality remains disproportionately high in these younger patients. The unique challenges in PLT-including limited donor availability and higher risk of complications-underscore the need for improved strategies to evaluate an

Fatigue, Sleep Problems, and Self-Efficacy Before and After Heart Transplantation: A 5 Year Follow-Up.

INTRODUCTION: Fatigue is a troublesome symptom after heart transplantation and is poorly studied in a long-term perspective. One hypothesis is that there is a strong relationship between fatigue, sleep problems, and self-efficacy, and that fatigue decreases self-efficacy. The aim of this study was to explore self-reported fatigue from the time on the waiting list to 5 years after heart transplantation and its association with self-efficacy and sleep problems. METHODS: In this multicenter, longitudinal cohort study 48 heart recipients, 12 women and 36 men with a median age of 57 years, were fo

Kidney Transplantation in the Era of Climate Change: Environmental, Structural, and Ethical Implications for Future-Ready Practice.

<p>Background: Kidney transplantation and dialysis are the two main modalities of kidney replacement therapy, and both are increasingly challenged by the current climate emergency landscape. Dialysis has long been scrutinised for its high energy and water demands, but transplantation, while generally more sustainable over the long term, also warrants critical evaluation concerning environmental accountability, equity, and resilience. Summary: In this review, we compare the environmental and structural dimensions of dialysis and transplantation, while examining how climate change uniquely affec
