Transjugular Leadless Pacemaker Implantation in an Adolescent With Central Hypoventilation Syndrome: A Case Report.

BACKGROUND: Permanent pacing in pediatric patients is complicated by small body size, vascular access limitations, and the need for durable long-term management. Leadless pacemakers offer an emerging alternative that minimizes infection and lead-related complications, yet vascular access remains a key challenge in young patients. CASE PRESENTATION: We report a 14-year-old male with congenital central hypoventilation syndrome (CCHS; PHOX2B polyalanine repeat mutation 20/26) who presented with recurrent nocturnal oxygen desaturation secondary to prolonged sinus pauses despite tracheostomy-assis

Left Bundle Branch Area Pacing as a Rescue Strategy After Failed Cardiac Resynchronization Therapy in a Patient With Persistent Left Superior Vena Cava, Absent Coronary Sinus Branches, After Bilateral Maze Procedure: A Case Report.

BACKGROUND Cardiac resynchronization therapy can fail in the presence of anatomical variations, such as persistent left superior vena cava, which can preclude coronary sinus cannulation and left ventricular lead placement. Conduction system pacing, particularly left bundle branch area pacing, has emerged as a physiological alternative for achieving ventricular resynchronization in such anatomically challenging situations and may provide an effective solution when standard cardiac resynchronization therapy (CRT) is not feasible. CASE REPORT We report the case of a 78-year-old man with chronic h

Subclavian chronic total occlusion angioplasty facilitates cardiac implantable electronic device upgrades: A single-center case series.

BACKGROUND: Subclavian venous occlusion is a frequent complication of transvenous cardiac implantable electronic device (CIED) implantation, with reported incidences exceeding 30%. Although often asymptomatic, chronic total occlusion (CTO) creates a major technical barrier during device revision, particularly for cardiac resynchronization therapy (CRT) upgrades. Conventional strategies such as lead extraction or contralateral tunneling may increase procedural risk or compromise future vascular access. Subclavian CTO venoplasty is a minimally invasive alternative to re-establish ipsilateral acc

Percutaneous Pacemaker Lead Implantation in Patients With Univentricular Circulation and Selected Patients With Biventricular Circulation: Intracavitary and Intramyocardial Approaches.

BACKGROUND: Epicardial pacemaker lead implantation in patients with univentricular circulation following total cavopulmonary connection with an extracardiac conduit and without fenestration necessitates median sternotomy. Transvenous access, although possible and previously described, may be technically challenging in this population and carries an increased risk of lead-related thrombosis within the subsystemic atrium or ventricle. Surgical implantation usually requires mechanical ventilation and is associated with procedural morbidity. A percutaneous approach represents a minimally invasive

Impact of end-screw configuration on peri-implant fracture risk in distal femoral plating.

BACKGROUND: Locked plating of distal femoral fractures is associated with a risk of secondary peri-implant fractures predominantly resulting from stress concentrations around the proximal plate end. This study aimed to investigate whether the strength of healed femoral bone-locking compression plate constructs can be increased by modifying the end screw type to minimize the risks of peri-implant femoral fractures. METHODS: Twenty-four artificial femurs were fixed with Distal Femur Locking Compression Plates (DF-LCP) in four different end screw configurations (n = 6): bicortical nominal lockin

Rebuilding the pelvis: advances in robotic-assisted management of complex pelvic fractures.

Complex pelvic fractures are infamously challenging to fix surgically because of their fine anatomy and proximity to vital neurovascular structures. Traditional open reduction and internal fixation (ORIF) improves stability but is complicated by excessive blood loss, longer operative time, and morbidity. Robotic-assisted surgical methods, i.e. Robot-Assisted Fracture Reduction (RAFR) and the TiRobot platform, provide a paradigm shift toward precise, minimally invasive fracture reduction and fixation. The RAFR system blends preoperative high-definition 3D CT imaging with intraoperative cone-bea

Preoperative imaging predictors of subsidence of 3D-printed artificial cervical vertebral bodies: a cohort study of 102 patients with nomogram development and validation.

BACKGROUND: Subsidence of an interbody implant, defined as its sinking into adjacent bone, is a common complication that can compromise spinal fusion outcomes and may require revision surgery. Custom 3D-printed titanium vertebral bodies are increasingly used for anterior spinal reconstruction. However, their reported subsidence incidence (15–30%) remains comparable to conventional cages. Early identification of high-risk patients is therefore crucial. This study aimed to identify preoperative imaging predictors of 3D-printed vertebral body subsidence and develop a nomogram for individualized r

Design of 3D-printed hydroxyapatite intervertebral fusion cages in a patient specific framework.

Fusion surgery involves replacing degenerated intervertebral discs with artificial implants, usually composed of titanium alloys or PEEK, requiring bone grafting to assist tissue growth. This study aims to evaluate numerically 3D-printed porous ceramic implants as a bioactive alternative. Clinical computed tomographies of an adult patient were used to reconstruct the geometry and to assign mechanical properties to L1 and L2 vertebrae, utilizing a patient-specific anisotropic micromechanics-based model. The influence of different microstructural choices on a porous hydroxyapatite-based scaffold

Short-same segment fixation utilizing differential screw application with sequential reduction for thoracolumbar burst fractures without neurological deficit via Wiltse approach: a technical strategy to enhance reduction and radiological outcomes.

BACKGROUND AND OBJECTIVE: Posterior distraction reduction (DR) technique with polyaxial screw fixation may fail to achieve sufficient central endplate reduction, potentially resulting in residual cavities, postoperative vertebral collapse and hardware failure. The biomechanical benefits of mixed screw types remain insufficiently explored. This study aims to evaluate clinical and radiographic outcomes of a novel transpedicular prying reduction (TPR) technique after axial distraction performed via the Wiltse approach using a sequential monoaxial-monoaxial-polyaxial screw configuration in patient

Towards optimal valve prescription for transcatheter aortic valve replacement (TAVR) surgery: a machine learning approach.

Transcatheter Aortic Valve Replacement (TAVR) has emerged as a prominent, minimally invasive treatment for patients with severe aortic stenosis, a life-threatening cardiovascular condition. Multiple transcatheter heart valves (THV) have been approved for use in TAVR, but current guidelines regarding valve type prescription remain a topic of ongoing debate within the medical community. We propose a data-driven clinical support tool to identify the optimal valve type with the objective of minimizing the risk of permanent pacemaker implantation (PPI), a predominant postoperative complication. We

[Perioperative complications during transvenous pacemaker and defibrillator implantation].

Despite major advances in cardiovascular implantable electronic devices (CIEDs) and implantation techniques, perioperative complications remain clinically relevant. Reported rates of relevant complications after transvenous CIED implantation range from 2 to 11% and may adversely affect patient outcomes. Major complications include access-related vascular injuries, venous thrombosis, pneumothorax and hemothorax, and cardiac complications such as lead perforation or tricuspid valve dysfunction, lead dislodgement, unrecognized malposition of the right ventricular lead in the left ventricle, arrhy

Pulmonary Vein Isolation in Elderly Patients With Atrial Fibrillation and Symptomatic Sick Sinus Syndrome: A Case Series.

In patients with atrial fibrillation (AF), sinus node disease (SND) represents a potentially reversible concomitant condition. Initial treatment with catheter ablation (CA) aimed at restoration of sinus rhythm may provide an alternative to pacemaker implantation. In this case series, 15 elderly patients (>65 years) with AF and SND underwent CA, with follow-up monitoring provided by implantable loop recorders (ILR). Despite CA of AF, pacemaker implantation was necessary in 40% of patients during a mean follow-up period of 37 months.

Transcatheter Aortic Valve Replacement for Severe Bicuspid Aortic Valve Stenosis.

Surgical aortic valve (AV) replacement has been the standard treatment for patients with severe symptomatic degenerative AV stenosis (AS). In recent years, transcatheter AV replacement has emerged as an established alternative in selected patient populations, supported by robust evidence in tricuspid AS. However, there has been limited evaluation of transcatheter AV replacement in bicuspid AV AS. Recently, several observational studies have demonstrated the feasibility and safety of transcatheter AV replacement in bicuspid AV AS, and the use of newer-generation devices has shown encouraging ou
