Left bundle branch area pacing (LBBAP) is now a common implantation strategy for both pacing and resynchronization based device therapies. While LBBAP has been (somewhat paradoxically) demonstrated to be effective in cases of preexisting left bundle branch (LBB) block, and criteria for LBB capture have been established; minimal focus has been towards the intrinsic refractory rate of the LBB. Herein, we demonstrate a case of LBBAP rate exceeding the refractory period of the LBB, resulting in alternating LBBAP capture and non-selective septal pacing. Implicit understanding of this physiology has

AIMS: Left bundle branch area pacing (LBBAP) has emerged as an alternative to cardiac stimulation via right ventricular pacing and cardiac resynchronization therapy using coronary sinus leads. The approach utilizes dedicated three-dimensional guiding catheters for lead placement. Our objective was to evaluate the feasibility and safety of a simplified approach of implantation of an LBBAP electrode without a dedicated guiding catheter.
METHODS: This was a prospective single-center proof-of-concept evaluation. Patients with an indication for dual-chamber pacemaker implantation were consecutivel

Endovascular aneurysm repair, a treatment for abdominal aortic aneurysms, carries the risk of recurrence due to endoleaks following stent graft implantation. Since these leaks do not present with specific symptoms, regular follow-up is necessary; however, now, only imaging-based monitoring is available. This study proposes an ultrathin flexible sensor inserted endovascularly with a stent to detect type I endoleaks, which pose the highest rupture risk. A coplanar capacitive sensor was attached to the stent graft using a specially developed flexible thermal adhesive to monitor blood leakage, wit

BACKGROUND: Although leadless pacemakers are traditionally implanted utilizing femoral venous access, the right internal jugular vein is an alternative site that may be preferable for patients with complex vasculature or contraindications to femoral access. The purpose of this study is to evaluate the safety, feasibility, and procedural efficiency of the AVEIR VR leadless pacemaker implantation via the right internal jugular venous access compared to traditional right femoral venous access in adult patients.
METHODS: This is a retrospective, single-center study of all adult patients who recei

BACKGROUND: The transjugular approach has recently gained attention as a viable alternative for leadless pacemaker implantation. However, clinical data in Asian populations, who often present with smaller body size and vascular anatomy, remain limited. This study aimed to evaluate the safety and efficacy of the transjugular approach compared with the conventional transfemoral route in an Asian cohort.
METHODS: We retrospectively analyzed 112 patients who underwent leadless pacemaker implantation between March 2021 and June 2025 at a single center. Patients were categorized by access route: tr

Cardiac implantable electronic devices (CIEDs) are widely used to manage rhythm disorders. However, implanters' training still follows an apprenticeship model, conducted in vivo from the outset, with wide institutional variability and lacking objective performance criteria to assess competence. Using objective validated metrics, this study evaluated procedural skills of early-career operators licensed to independently perform implantations and explored potential benefits of additional simulation-based training. Fifteen CIED implanters from six European countries underwent a three-task simulati

Cardiac implantable electronic device upgrade and downgrade procedures are increasingly being performed. Whilst the most appropriate guideline-recommended device may have been followed during a patient's initial procedure, the requirements of patients can change over time. This could be due to worsening of cardiac function due to detrimental effects of pacing itself or the diagnosis, development, or progression of another cardiac comorbidity. Device downgrades are also performed when a patient's clinical state changes and are often considered in patients with increased frailty and comorbidity.

Objective: This analysis compares new (G2) versus old (G1) generations of transcatheter heart valves (THVs) in transcatheter aortic valve replacement (TAVR) procedures, focusing on key outcomes: post-procedural conduction disturbance (CD) and permanent pacemaker implantation (PPMI). We aim to determine whether G2 valves reduce these specific complications and thereby improve patient outcomes compared with G1. Methods: From February 2015 to September 2022, 1468 patients underwent TAVR at the university clinic in Kiel. After applying exclusion criteria, a final cohort of 1182 patients were analy

STATEMENT OF PROBLEM: The manual segmentation of anatomic structures in cone beam computed tomography (CBCT) scans such as the mandibular canal is time-consuming, operator-dependent, and prone to variability, limiting efficiency and consistency in implant planning. Whether automated CBCT segmentation is more accurate remains unclear.
PURPOSE: The purpose of this systematic review and meta-analysis was to synthesize evidence on deep learning, primarily U-Net architectures, for automatic segmentation of the mandibular canal in CBCT imaging. The segmentation accuracy and clinical relevance for d

BACKGROUND: Tricuspid valve transcatheter edge-to-edge repair (T-TEER) is increasingly used in patients with severe tricuspid regurgitation (TR) at high surgical risk. Long-term outcomes in those with transvalvular cardiac implantable electronic devices (CIEDs) remain insufficiently characterized.
OBJECTIVES: The aim of this study was to evaluate procedural and clinical outcomes of T-TEER in patients with CIED leads in a real-world cohort.
METHODS: Among 3,025 patients undergoing T-TEER at 26 centers (2016-2024), 851 (28.1%) had transvalvular CIED leads. Residual TR at discharge and follow-u

BACKGROUND: Transtricuspid cardiac implantable electronic devices (CIEDs) complicate the management of tricuspid regurgitation (TR). Transcatheter tricuspid valve annuloplasty (TTVA) offers a promising approach due to minimal interaction with leaflets and transvalvular CIEDs, though real-world evidence is limited.
METHODS: This bi-center, retrospective study includes 204 consecutive patients who underwent TTVA with the Cardioband (Edwards Lifesciences) for severe symptomatic TR. Patients were divided into CIED carriers and non-CIED carriers. CIED carriers were further classified into those wi

Background and Objectives: Implantation of cardiac implantable electronic devices (CIEDs) is increasingly performed in elderly and comorbid patients, for whom minimizing perioperative complications-including pain and systemic drug use-is critical. Traditional local infiltration often provides insufficient analgesia. The ultrasound-guided PECS II block, an interfascial regional technique, offers promising analgesic benefits in thoracic wall procedures but remains underutilized in cardiac electrophysiology. Materials and Methods: We conducted a prospective, controlled, non-randomized clinical st

BACKGROUND: In patients with a cardiac implantable electronic device (CIED) using a transvenous right ventricular (RV) lead, transcatheter tricuspid valve replacement (TTVR) results in RV lead entrapment.
OBJECTIVES: The authors sought to evaluate outcomes of patients with pre-existing transvenous RV leads undergoing EVOQUE TTVR.
METHODS: Patients with a CIED referred for TTVR at 3 centers were evaluated preoperatively by the heart team. Lead and clinical assessments were performed at baseline and in follow-up per local clinical practice. The primary outcome was the nature and incidence of m

Cardiac implantable electronic devices (CIEDs) have increased the quality and duration of life for millions of patients. As their utilization continues to grow exponentially, clinicians need to diagnose and manage several device-associated complications that may arise during the CIED life cycle. This primer is tailored for the general internal medicine physician or hospitalist, who will inevitably take care of patients with CIEDs, to provide a contemporary update on the incidence and clinical manifestations of the 4 most common CIED complications, with the latest evidence to guide clinical man

BACKGROUND: Cardiac implantable electronic devices (CIEDs) play a vital role in modern electrophysiology but are prone to complications, necessitating transvenous lead extraction (TLE) in certain cases. Recent studies suggest sex-based disparities in cardiovascular outcomes, yet little is known about sex differences in TLE complications.
OBJECTIVE: This study aimed to evaluate sex-related differences in peri-procedural outcomes following TLE.
METHODS: We conducted a retrospective, observational cohort study using the National Inpatient Sample (NIS) from 2016 to 2019. Patients undergoing TLE

BACKGROUND: Left ventricular unloading is needed in patients on extracorporeal life support (ECLS) with severely impaired left ventricular contractility to avoid stasis and pulmonary congestion, and to promote LV recovery. The presence of thrombi in the LV precludes the use of conventional active unloading methods such as transaortic microaxial pumps or apical LV vents. We describe placement of a vent cannula via the left atrial appendage (LAA) as a useful bailout option.
