Comply or don't fly: Airport security screening experiences of people who have limb loss and limb difference.

BACKGROUND: People who have limb loss (amputation etiology) or limb difference (congenital etiology) can experience additional challenges with airport security screening due to their assistive device or prosthesis. However, perspectives on airport security screening from people with limb loss and limb difference remain unreported. OBJECTIVE: This survey elicited experiences of airport security screening in the United States from people who have limb loss and limb difference to inform recommendations for the Transportation Security Administration. METHODS: The Amputee Coalition conducted an a

Step-specific distribution of whole-body angular momentum in unilateral transfemoral sprinters at maximal speed.

Unilateral transfemoral (TF) sprinters run with pronounced mechanical asymmetries between the prosthetic and intact limbs. Whole-body angular momentum (WBAM) provides a mechanical descriptor of whole-body rotation, yet how WBAM differs between prosthetic and intact steps across planes at maximal sprinting speed remains unclear. We investigated three-dimensional WBAM in nine unilateral TF sprinters running at maximal speed with a running-specific prosthesis. For each step, we quantified peak-to-peak WBAM-range, net integrated WBAM (iWBAM), and positive and negative angular-momentum contribution

Gait and Plantar Pressure Analysis in Charcot Neuroarthropathy: A Clinic-Based Biomechanical Study.

INTRODUCTION: Charcot Neuroarthropathy (CN) is a progressive joint disorder resulting from peripheral neuropathy and repetitive microtrauma. Management prioritizes offloading to prevent deformity, ulceration, infection, and amputation. While three-dimensional gait analysis can be useful for disease staging and guiding management, its cost and limited accessibility limit routine clinical use. This study evaluated plantar pressure and two-dimensional gait kinematics in CN patients using a relatively low-cost clinic-based approach. METHODS: Following institutional review board approval, this cro

Outcomes of simultaneous microsurgical replantation of complete amputations at the distal third of the right forearm and left wrist: a case report and 22-month follow-up.

Microsurgical replantation of simultaneous bilateral upper extremity amputations is a complex procedure requiring a highly coordinated multidisciplinary team. Functional outcomes depend on anatomical level of injury, duration of ischemia, extent of crushing, degree of contamination, and quality of postoperative rehabilitation. We report a 45-year-old male industrial worker who sustained complete simultaneous amputations through the distal third of the right forearm and at the left wrist following an industrial paper-cutting machine accident. Total ischemia time was 8 h. Two microsurgical teams

Quantifying the unique needs of women with lower limb loss: A national, cross-sectional, survey-based needs assessment.

INTRODUCTION: Despite the increasing population of women with limb loss, data to sufficiently characterize potential gender-based differences are lacking, as scientific literature on this topic is often not specifically designed to assess the unique needs of women. Fragmentary information has indicated that women experience a variety of physical and psychological challenges that differ from those experienced by men, but this information has yet to be fully explored. This is the first exploratory needs assessment to combine population-specific physical and psychosocial assessments, as well as g

Meta-analysis of adverse outcomes following soft tissue reconstruction of complex open lower limb fractures.

AIMS: Complex open lower limb fractures are associated with considerable morbidity. Soft tissue reconstruction is fundamental in the management of severe injuries. This systematic review and meta-analysis was conducted to pool data on key adverse outcomes following treatment of these injuries including total flap failure, infection and amputation. METHODS: A PRISMA-compliant systematic review and meta-analysis was conducted (PROSPERO CRD420251130703) by searching the MEDLINE, Embase and Cochrane CENTRAL databases from inception to August 2025. Data were extracted in duplicate by 5 reviewers.

Predictive Value of Preoperative Inflammatory and Nutritional Indices for Revision Surgery After Major Lower Extremity Amputation in Patients With Wagner Stage 5 Diabetic Foot.

BACKGROUND Major lower extremity amputation is a severe complication of diabetic foot disease, leading to high morbidity and repeat surgeries. Although systemic inflammatory and nutritional markers have been studied as prognostic factors in diabetic foot ulcers, their role in predicting revision surgery after major amputation is unclear. This study aims to evaluate the link between preoperative inflammatory and nutritional indices and revision surgery after major lower extremity amputation. MATERIAL AND METHODS This multicenter retrospective study reviewed medical records of patients with Wagn

Staged bilateral reverse sural artery flap reconstruction for severe frostbite of both feet in a patient with schizophrenia: A case report.

RATIONALE: Bilateral, weight-bearing heel defects after frostbite are difficult to reconstruct, and outcomes depend heavily on postoperative off-loading and patient cooperation. We report a limb-salvage strategy for severe bilateral foot frostbite that occurred during acute psychosis in a patient diagnosed with schizophrenia. PATIENT CONCERNS: A 25-year-old man developed acute psychotic symptoms and walked barefoot in subzero conditions for about 12 hours, resulting in deep frostbite of both heels and multiple toes. During admission he had poor insight and repeatedly attempted weight bearing

Surgical management and limb salvage outcomes in orthopedic trauma patients with concomitant popliteal artery injury: a single-center retrospective analysis.

BACKGROUND: Traumatic popliteal artery injury often occurs with complex orthopedic and soft-tissue damage and requires coordinated vascular, skeletal, decompressive, and wound management. We evaluated real-world treatment patterns and limb-salvage outcomes in a single-center cohort of patients with combined orthopedic trauma and popliteal artery injury. METHODS: We retrospectively reviewed 48 hospitalization records from 46 patients treated between 2002 and 2025. Repeated hospitalizations related to the same injury were reconciled into 46 independent patient/injury episodes. The primary outco

Innovative Patient Positioning for Proximal Femoral Nailing in a Bilateral Above-knee Amputee: A Case Report and Technical Note.

INTRODUCTION: Proximal femur fractures in amputees present unique surgical challenges, primarily due to the inability to use standard traction table attachments. Bilateral above-knee amputations further add to this difficulty. The literature mainly describes unilateral amputees and delayed fractures in previously amputated limbs, while reports involving bilateral above-knee amputations are rare. The challenge becomes greater when the fracture and bilateral traumatic amputations occur in the same index trauma, because stump length, soft-tissue condition, fluoroscopic access and infection risk a

Clinical course and management of extremity frostbite in populations under forced movement in border regions.

BACKGROUND: Frostbite is a serious cold injury that can progress to deep tissue damage and even amputation. The increasing number of cases among refugee and migrant populations who are forcibly mobile in border regions necessitates a detailed evaluation of clinical course and risk factors. This study aimed to assess the demographic characteristics, clinical course, laboratory findings, and treatment outcomes of patients presenting with frostbite-related extremity injuries. METHODS: This retrospective study included a total of 154 patients who presented with a clinically confirmed diagnosis of

Outcomes of Open and Endovascular Interventions in Octogenarians with Infrainguinal Chronic Limb-Threatening Ischemia.

BACKGROUND: Despite prior studies showing that lower extremity revascularization in the elderly population could be performed safely, there continues to be significant debate regarding the outcomes in octogenarians with chronic limb-threatening ischemia (CLTI). The choice between bypass and endovascular therapy (ET) in this population is controversial. This study aimed to examine outcomes of bypass and ET in octogenarians presenting with CLTI using the Vascular Quality Initiative (VQI) database. METHODS: The VQI database was queried for all patients with infrainguinal CLTI that received bypas

Analgesic consumption after major lower extremity amputation - a Danish nationwide register-based study from 2010-2021.

OBJECTIVES: Persistent pain after major lower extremity amputation (MLEA) is common. This study aimed to investigate opioid and non-opioid analgesic consumption, defined by drug group, one year before and after MLEA and to investigate risk factors for postoperative opioid use. METHOD: Through the Danish National Patient Registry, we identified 7,012 patients, ≥50 years, alive one year postoperatively, with first-time MLEA performed in 2010-2021, divided in below-knee (BKA) and above-knee amputations (AKA). Prescription redemptions were used as a proxy for analgesic use, which was analysed by

Modified tibiofibular synostosis vs. Burgess stump in a bilateral amputee: a case report.

Despite trans-tibial amputation being the most common major amputation, consensus on optimal stump management remains limited. Cohort studies comparing Ertl-tibiofibular synostosis (TFS) and Burgess-type stumps (BTS) have yielded conflicting results. We present the case of a bilateral trans-tibial amputee with a modified TFS on the right and a conventional BTS on the left. Instead of traditional myodesis, a myoplastic sling was created attaching residual tendons distal to the bone bridge to improve tendon sliding and perceptive feedback. Outcomes were assessed via load distribution, temporospa

Cumulative operative burden is associated with poorer function after segmental bone transport for lower-extremity long-bone defects.

OBJECTIVE: Segmental bone transport offers biological reconstruction and limb preservation for extensive lower-extremity long-bone defects, but treatment often involves prolonged external fixation, planned stages, and unplanned reoperations. We evaluated reconstructive and functional outcomes and examined the association between cumulative operative burden and final patient-reported function. METHODS: This retrospective single-centre study screened 44 patients treated between January 2012 and March 2023. Thirty-six patients, representing 37 reconstructed long bones, had complete final outcome

Long-Term Mortality After Major Lower Extremity Amputation for Dysvascular or Infected Limbs.

OBJECTIVE: Previous studies have reported poor survival rates after major amputation for dysvascular or infected limbs. In this study, we examined contemporary long-term survival rates and causes of death among patients undergoing major lower extremity amputation. METHODS: Institutional medical records were queried for patients undergoing either above-knee (AKA) or below-knee amputation (BKA) for chronic limb-threatening ischemia (CLTI) or severe infection, from 2020-2023. Mortality rates were evaluated using Kaplan-Meier survival curves, and Cox proportional hazards models were constructed t

Plantar Perfusion Imaging for Peripheral Arterial Disease Screening: A Proof-of-Concept Study.

The diagnosis of peripheral artery disease (PAD) typically relies on specialized equipment such as ultrasound. The delayed PAD detection of these approaches may lead to amputation and even death. To achieve rapid and ubiquitous PAD screening, we propose a novel concept of camera-based plantar perfusion imaging (CPPI) for PAD diagnosis and severity classification. Specifically, we performed a simulation trial that used an RGB camera to record the plantar video of 20 subjects and a cuff with different pressures applied to the left leg to simulate different degrees of lower limb blockage. We gene

FedEMG: Achieving Generalization, Personalization, and Resource Efficiency in EMG-Based Upper-Limb Rehabilitation Through Federated Prototype Learning.

Upper extremity amputation, often necessitated by traumatic injuries, significantly impacts an individual's well-being. This paper addresses the critical challenges of deploying deep learning for real-time electromyography-based gesture recognition in prosthetic control: generalization across users and time, the personalization-generalization trade-off, and computational constraints. We propose Federated Electromyography (FedEMG), a novel Federated Prototype Learning (FPL) framework that leverages a prototype-based approach for efficient knowledge transfer and a unique adaptive personalization

Medicinal Plants Facilitate the Recovery of Diabetic Foot Ulcer by Regulating Macrophages: Latest Evidence and Insights.

Diabetic Foot Ulcer (DFU) is a major complication of diabetes that mostly affects the lower extremities, with a high incidence and recurrence rate in approximately 15% of patients with diabetes. The complexity of diabetic wounds poses a substantial challenge for clinical recovery, underscoring the need to investigate novel therapeutic approaches. Medicinal plants have been used to treat ulcers for centuries. Recently, there has been a growing focus on the development of topical preparations derived from medicinal plants that target macrophages as an adjuvant therapy for DFU. Macrophages have b
