Comparative multi-omic analysis reveals conserved and derived mechanisms of fin and limb regeneration.

Comparative studies of vertebrate appendages offer a powerful framework for uncovering shared components of an ancestral regeneration toolkit. Here, we employed a multi-omics comparative approach leveraging the regenerative capacity of the axolotl, zebrafish, and Polypterus senegalus, a fish capable of full fin regeneration. We identified conserved markers of proximal and distal blastema territories, shared activation of DNA damage repair, hif1a-mediated hypoxia response, and sequential activation of pro- and anti-inflammatory program. Apical epithelial ridge markers were expressed in both the

Neuromuscular ultrasound and nerve conduction studies as complementary tools for screening of diabetic peripheral neuropathy.

Diabetic Polyneuropathy (DPN) is a common debilitating complication of Diabetes Mellitus (DM), causing pain which may progress to numbness and motor involvement. DPN may lead to diabetic foot ulcers and possible amputation. Thus, DPN affects the patients' quality of life and poses a financial burden if not managed early. Nerve conduction studies (NCS) have been used as an objective method for diagnosis of DPN. Neuromuscular Ultrasound (NMUS) is a more comfortable tool used to complement NCS in diagnosis of many diseases. The study aimed to investigate NMUS as a possible screening tool for DPN.

Period Prevalence Surveys: A New Approach to Measuring Phantom Limb Pain in Amputees.

BACKGROUND: The quantification of phantom limb pain (PLP) is challenging due to the heterogeneity of patient experiences. This study evaluated the use period prevalence questionnaires in assessing PLP in patients with lower limb osseointegrated implant (OI) prostheses. METHODS: The study included patients who received single-stage lower limb OI prostheses at our institution. Period prevalence questionnaires included questions regarding frequency and duration of PLP prior to OI implantation and in the last year postoperatively. The frequency of PLP was assessed by asking participants how often

Lower limb osseointegrated prosthetics: are we standing on the edge of a new era?

BACKGROUND: The advent of osseointegrated prosthetic systems marks a significant milestone in the evolution of limb reconstruction and rehabilitation. Osseointegration implants are a direct skeletal interface for artificial limbs, introduced as bone-anchored limb prostheses that may improve many limitations of traditional socket-based prosthetics, such as discomfort, poor fit, skin irritation, and limited range of motion. MAIN BODY: Since the discovery of the osseointegration phenomenon in 1940, several osseointegrated implant systems, such as OPRA, ILP, OPL, and ITAP, with varying biomechani

Risk-Stratified Predictive Analysis of Docking Site Outcomes in Lower Extremity Bone Transport: Identifying High-Risk and Low-Risk Zones for Large Segmental Defect Management.

Background: Reconstruction of limbs with extensive bone loss often requires complex surgical procedures, which can be technically demanding, time-consuming, and physically and psychologically burdensome for patients. Historically, the lack of alternatives for large bone defects often led to primary amputation. Modern musculoskeletal practice allows for reconstruction using autologous or allogeneic bone grafts, or through more complex procedures such as the Masquelet technique or distraction osteogenesis. However, these methods share a common challenge: the need for a docking site procedure in

Osseointegration Group of Australia Accelerated Protocol-3 (OGAAP-3): protocol for a stratified approach to surgical technique and rehabilitation in lower limb amputees.

INTRODUCTION: The Osseointegration Group of Australia Accelerated Protocol-3 (OGAAP-3) presents the latest iteration of a structured, prospective, clinical-based protocol for single-stage osseointegration surgery in lower limb amputees. Building on the foundational work of OGAAP-1 and OGAAP-2, this protocol incorporates a decade of institutional experience and over 1000 cases using the Osseointegrated Prosthetic Limb system. OGAAP-3 introduces stratified rehabilitation categories-immediate, standard, slow and delayed loading-based on intraoperative findings, residual limb characteristics and p

Microsurgical replantation of severely crushed and contaminated amputated limbs in children: Case reports and surgical insights.

Severely crushed and contaminated pediatric amputations present unique challenges for limb salvage, especially when critical structures are damaged. We report two pediatric cases-one hand and one foot amputation-successfully replanted using meticulous microsurgical techniques. A key component of our approach was microsurgical debridement under high magnification, which enabled removal of deeply embedded contaminants without sacrificing additional tissue. Both patients achieved limb survival and functional recovery after long-term follow-up. Detailed sensory outcomes, range of motion, skin cove

Correlation between metabolic cost and perceived comfort during human-in-the-loop optimization of a prosthetic foot.

BACKGROUND: Human-in-the-loop optimization provides a standardized approach to enhance the effectiveness of assistive technology for gait by tuning devices to the needs of the individual user. Metabolic cost is frequently used as optimization objective. However, the assessment of metabolic cost requires relatively long-lasting physiological steady-state conditions. Subjective user preference could be an interesting alternative optimization objective that could shorten trial duration. We aim to explore whether the measured metabolic cost of walking correlates with the perceived comfort of the u

Guided ultrasonic wave monitoring for osseointegration assessment of an intraosseous transcutaneous amputation prosthesis.

This study investigated the feasibility of guided ultrasonic wave monitoring of bone attachment to uncemented orthopaedic implants during the rehabilitation process (osseointegration), which is crucial for implant stability and long-term survival. Experiments were conducted using a simplified three-layer synthetic bone model of an intraosseous transcutaneous amputation prosthesis (ITAP) implant, used for femoral amputee patients, where epoxy curing simulated the bone ingrowth process associated with increasing bone-implant interface layer stiffness, representing the early stages of osseointegr

Differences in phantom limb, residual limb, and bodily pain during pain recall and increasing activity intensity in persons with unilateral lower limb amputation.

BACKGROUND: Pain is a common experience in the phantom limb, residual limb, and other parts of the body following amputation. However, formal pain surveys typically do not differentiate between these different pain subtypes, leading to a lack of relevant data to clinically diagnose patients' pain and provide targeted treatment. OBJECTIVE: To characterize the phantom limb, residual limb, and bodily pain experiences and quality of life of patients with major lower limb amputation and compare these to self-evaluations of overall pain at time the survey, during the past month during rest, light d

Movement speed of an autonomous prosthetic limb shapes embodiment, usability and robotic social attributes in virtual reality.

Autonomous robotic prostheses can aid individuals with limb loss regain functionality and near-normal appearance. However, to psychologically integrate such limbs into one's body image, it is necessary to understand how movement characteristics, such as movement speed, affect the sense of embodiment. Using a VR simulation, we investigated how the speed of autonomous lower-arm movements affects embodiment and user-perception. Nineteen healthy participants embodied an amputated virtual avatar with a prosthetic lower left-arm moving autonomously at six different speeds following minimum-jerk traj

Can We Develop a Web-based Nomogram That Could Predict the Risk of Local Recurrence After Treatment for Giant Cell Tumor of Bone of the Extremities by Curettage?

BACKGROUND: For giant cell tumor of bone (GCTB) in the extremities, joint-preserving surgery (curettage) is often favored over en bloc resection to maintain postoperative function, although it carries a notable risk of local recurrence. We decided to develop a nomogram to predict the risk of local recurrence after curettage of GCTB of the extremities so physicians can shorten the interval between radiographs, add CT or MRI, and even monitor tartrate-resistant acid phosphatase 5b levels during surveillance for patients at particularly high risk of local recurrence. Simulated gait training improves joint loading symmetry in unilateral transfemoral bone-anchored limb users.

Bone-anchored limbs (BALs) are promising alternatives for addressing socket-related pathologies, but evidence suggests movement and joint loading asymmetries persist 12-months following transfemoral BAL implantation. Gait training targeting biomechanical symmetry is common in rehabilitation for people with transfemoral amputation. However, it is unknown if people with transfemoral BALs can achieve symmetrical gait due to absent anatomical structures. Our objective was to determine if gait symmetry is feasible in transfemoral BAL users and investigate the biomechanical effects of enforcing symm

Estimation of Recommended Damping Coefficient Range to Achieve Target Peak Knee Flexion in a Passive Prosthetic Knee.

PURPOSE: There is a lack of predictive biomechanical models and publicly accessible empirical data that quantitatively explore the effect of damping magnitude on the peak knee flexion angle during the swing phase. METHODS: A three-step framework estimates a recommended damping coefficient range required to achieve able-bodied peak knee flexion during swing. A recommended damping range (0.29- 0.56 × 10 - 2 [ - ] ) was estimated using experimental able-bodied gait data and adjusted based on expected walking speed and shorter duration of swing flexion common in transfemoral prosthetic gait.
