Conserved anterior/posterior tissue asymmetry encodes proximal/distal positional information during axolotl limb regeneration.

While mammalian limb regeneration is limited to the distal fingertips, axolotls can regenerate entire limbs, providing a model system to uncover conserved patterning mechanisms. Although anterior-posterior (A/P) signaling is required for limb regeneration, how A/P patterning is coordinated across limb segments remains unclear. Using spatial transcriptomics and our SpatialFlux analysis pipeline, we found that A/P genes are asymmetrically distributed along the proximal-distal (Pr/Di) axis of both uninjured and regenerating axolotl limbs. This asymmetric boundary coincides with distally enriched

Transradial Amputation With Nerve Management: Case Presentation and Surgical Technique.

Transradial amputation of appropriate length preserves forearm rotation and facilitates prosthetic use; however, high rates of phantom limb pain and neuropathic medication use persist. This case describes a multidisciplinary approach to transradial amputation that integrates targeted muscle reinnervation, regenerative peripheral nerve interfaces, and bone and soft tissue management to optimize pain control and prosthetic success. A 29-year-old man with a severe ballistic injury and complex regional pain syndrome underwent transradial amputation with targeted muscle reinnervation and regenerati

Barthel Index-Based Activity Evaluation at Admission Predicts Wound Prognosis in Chronic Limb-Threatening Ischemia.

BACKGROUND: The Barthel Index (BI) is a reliable tool for assessing the level of independence in activities of daily living (ADLs). Current risk stratification for chronic limb-threatening ischemia (CLTI) primarily focuses on anatomical severity and comorbidities, with less emphasis on functional status. Although several studies have examined its prognostic utility in patients with CLTI, the association between BI at admission and wound healing outcomes remains unclear, particularly in the context of multidisciplinary treatment strategies. METHODS: This retrospective single-center study inclu

Who Controls the Prosthetic Leg? Voluntary Control Beyond the Microprocessor Knee.

Microprocessor-controlled knees (MPKs) have meaningfully advanced the safety and confidence of many transfemoral prosthesis users, yet a persistent clinical paradox remains: improved device intelligence does not automatically translate into greater everyday activity or full functional recovery. This editorial argues that the value of an advanced prosthetic leg is realized not when the device assumes control on the user's behalf, but when it is coupled with the user's own voluntary control and human-device coordination. Drawing on evidence that objective activity does not consistently increase

Associations of activities of daily living and their trajectories with the risk of diabetes-related lower-limb amputation: evidence from the HRS and ELSA longitudinal cohorts.

BACKGROUND: Diabetes-related lower-limb amputation (DLLA) is a severe complication of diabetes associated with considerable disability and mortality. However, the associations of activities of daily living (ADL) and their longitudinal trajectories with DLLA risk remain unclear. METHODS: Data from the Health and Retirement Study (HRS; n = 3007) and the English Longitudinal Study of Ageing (ELSA; n = 833) were analyzed. ADL trajectories were identified using Group-Based Trajectory Modeling. Associations of ADL levels and trajectories with diabetes-related lower-limb amputation (DLLA) were evalu

Individuals with a trans-tibial amputation present trunk and lower limbs biomechanical asymmetry during sit-to-stand transitions.

INTRODUCTION: Individuals with a transtibial amputation (iTTA) are usually fitted with a prosthetic leg. However, it does not prevent functional limitations or compensatory strategies during daily activities, such as sit-to-stand transition (STS). Compensatory strategies may be linked to higher risk of secondary joint conditions. OBJECTIVE: This study aimed to assess compensatory strategies using kinetic asymmetry and trunk kinematics among iTTA compared with healthy counterparts during STS. METHODS: Nine iTTA and 9 healthy subjects performed 5 STS. Kinematic and kinetic data were recorded u

Phase-Based Haptic Feedback for Improved Neural Control of a Robotic Prosthetic Ankle: A Preliminary Study.

While neural control of robotic prosthetic legs through direct electromyographic (dEMG) control has shown promise for restoring human-prosthesis coordination, many persons with amputations struggle to generate the appropriate electromyographic (EMG) control signals for locomotion. The objective of this study was to develop a novel, phase-based haptic feedback system to train persons with transtibial amputations (PWTA) to produce appropriate muscle activation patterns for dEMG control of a robotic ankle while walking. The system provided (1) instructional feedback to guide the timing of residua

Development and application of a prosthetist-specific rectification template based on artificial intelligence for the fabrication of transfemoral prosthetic sockets.

The prosthetic socket is the most critical component of a lower limb prosthesis, requiring precise customization to the individual's residual limb. This study presents proof-of-concept for a novel artificial intelligence (AI)-driven rectification template for transfemoral sockets tailored to a single prosthetist. Using a dataset of nine persons with transfemoral amputation, the study workflow required the manual casting and 3D scanning of unrectified and rectified plaster positives, anatomical landmark identification, and unsupervised training of an algorithm using Principal Component Analysis

Spotted lanternfly (Lycorma delicatula) nymphs demonstrate robust jumping and gripping performance following limb loss.

The spotted lanternfly (Lycorma delicatula) is an invasive hemipteran spreading across the United States. Eradication campaigns, such as "See It, Squish It," rely on mechanical elimination. However, lanternflies often escape by executing hindlimb-driven leaps. Although escape rates remain unquantified, the physical nature of these eradication attempts likely results in individuals surviving with damaged/missing limbs. Lanternflies do not exhibit limb autotomy, and it remains unclear how well they tolerate injury. To evaluate the effects of limb loss on locomotor performance, we amputated a sin

The Cost of Assistance: High Prosthetic Knee Torque Reduces Biological Joint Work But Induces Symmetry Tradeoffs in Knee Power and Center-of-Mass Velocity During Stair Ascent.

Stair ascent is a biomechanically demanding activity for individuals with transfemoral amputation, often requiring significant overreliance on the residual hip and sound biological joints to compensate for the lack of positive power in conventional passive prostheses. Although powered prostheses can generate positive power imitating biological legs, the optimal torque levels required to balance biological joint effort reduction with interlimb symmetry remain unknown. This study evaluated the effects of systematically increasing prosthetic knee torque on the biomechanics of stair ascent in eigh

A Synthetic Digital Anatomy Approach to Determine the Effects of Tissue Composition and Alignment on Transfemoral Residual Limb Mechanics.

The soft tissue envelope within the residual limb influences prosthetic fit, function, and user comfort. The purpose of this study was to examine the influence of residual limb tissue composition and skeletal alignment on its mechanical properties during axial loading. We hypothesized that deviations in the adipose and muscle volume of the limb, as well as frontal plane alignment, will alter the mechanical stiffness and energy loss. We constructed multi-material synthetic residual limb models with varying tissue compositions and mechanically tested them under cyclical axial compression. Stiffn

Amputees and non-amputees adapt similarly to split-belt treadmill assistance.

Individuals with lower-limb amputation experience higher walking energy cost and discomfort than able-bodied controls when using traditional passive prostheses. Powered prosthetics aim to reduce energy cost and improve comfort, but have not shown empirical metabolic benefits approaching those of comparable exoskeletons or simulation studies. One theorized explanation is that amputees adapt less quickly, and less effectively, to walking assistance than able-bodied participants. Previous work has shown that split-belt walking is an effective test of adaptation to an assistance condition, and tha

Barefoot ambulation following partial foot amputation: A systematic review of biomechanical outcomes.

BACKGROUND: Partial foot amputation (PFA) is increasingly performed due to rising prevalence of diabetes and peripheral vascular disease. While PFA may preserve gait and reduce energy expenditure compared with transtibial amputation, biomechanical deficits are common. This review aimed to evaluate biomechanical outcomes during baref following PFA. METHODS: A systematic review was conducted. MEDLINE, Embase, CINAHL, SCOPUS and Web of Science databases were searched for studies reporting biomechanical outcomes in adults with PFA without prosthesis. Eligible outcomes included spatiotemporal metr

Do Arterial Allografts Outperform Venous in Cryopreserved Bypass for Limb Salvage? A Systematic Review and Meta-Analysis of Infrainguinal Revascularization Outcomes.

BACKGROUND: Cryopreserved allografts remain a viable option for infrainguinal revascularization in limb salvage when autologous veins are unavailable or prosthetic material is undesirable. Uncertainty persists regarding comparative outcomes by allograft type (arterial vs venous) and clinical indication (infectious vs noninfectious settings). This systematic review and meta-analysis summarizes outcomes and key evidence gaps. METHODS: A systematic search of studies reporting infrainguinal reconstructions with cryopreserved arterial or venous allografts was conducted according to Preferred Repor

Historical Perspective of the Surgical Innovations to Improve Prosthetic Control.

Over the course of history, humanity has continuously pursued ways to restore the upper extremity after amputation. Both technology and new techniques have helped bring surgeons closer to recreating the biological upper extremity than ever before. Surgical innovations such as vessel ligation, proximal osteotomies, muscle flap closures, and infection control have their basis in historical accounts of amputation care. The 21st century has seen advances in surgical pain control after amputation, improved myoelectric prosthetic control, and the potential for sensory feedback. The combination and i
