Elevated combined pulmonary vascular resistance obtained by cardiovascular magnetic resonance and cardiac catheterisation on pre-Fontan assessment is associated with adverse postoperative Fontan outcomes.

OBJECTIVE: We analysed cardiac MRI and catheterisation variables during pre-Fontan evaluations for associations with major adverse cardiac events including Fontan takedown, mechanical circulatory support, heart transplantation, or death. METHODS: In this single-centre retrospective study, we gathered pre-operative MRI and catheterisation data for all patients who underwent Fontan operation at Children's Medical Center, Dallas, from April 2017 to November 2022. Patients were grouped according to the presence or absence of adverse events, and MRI and catheterisation parameters were compared bet

Pediatric restrictive cardiomyopathy with rapid complete atrioventricular block progression.

UNLABELLED: Restrictive cardiomyopathy (RCM) is a rare pediatric cardiomyopathy characterized by ventricular diastolic dysfunction, and poor prognosis that often requires heart transplantation. We report the case of a 7-year-old boy with RCM complicated by complete atrioventricular block (CAVB) in whom pathological examination of the explanted heart demonstrated extensive fibrosis involving the cardiac conduction system. At 1 year and 9 months of age, the patient presented with arrhythmia. Echocardiography revealed marked biatrial enlargement and small ventricles. Cardiac catheterization demon

Sonographic Diagnosis and Anatomic Mapping of a Radial Artery Pseudoaneurysm Following Transradial Cardiac Catheterization.

Radial artery pseudoaneurysm is a rare complication of transradial catheterization. We report a 74-year-old woman on rivaroxaban who developed a pulsatile distal forearm mass 1 day after transradial cardiac catheterization. High-frequency ultrasound showed a rounded extra-arterial lesion (7.6 × 8.9 × 7.0 mm) not involving all wall layers, with thrombus, a discrete communicating neck, turbulent bidirectional "yin-yang" color Doppler, and preserved distal flow. Anatomic mapping showed the lesion abutting adjacent tendons and the palmar cutaneous branch of the median nerve. Surgical excision and

Automated phase recognition in the cardiac catheterization laboratory with deep learning.

BACKGROUND: An efficient workflow in the cardiac catheterization laboratory is important to guarantee quality of care and keep healthcare affordable. Automated surgical process monitoring of diagnostic coronary angiograms (CAGs) has the potential to increase workflow efficiency, reduce non-clinical workload, and optimize scheduling. The aim of this study is to automatically classify workflow phases of CAGs using deep learning models trained on a real-world dataset. METHODS: With an observational study we collected a dataset, composed of an integration of video recorded CAGs, clinical data, an

Percutaneous closure of an iatrogenic left atrial-aortic perforation complicated by cardiac tamponade and coronary vasospasm during transseptal puncture: a case report.

BACKGROUND: Iatrogenic atrial-aortic perforation during transseptal puncture is a rare but potentially life-threatening complication. Percutaneous closure using dedicated occluder devices has emerged as a less invasive alternative to surgical repair in selected patients. CASE PRESENTATION: A 68-year-old woman with hypertension and long-standing persistent atrial fibrillation (≥ 12 years) underwent catheter ablation. Pre-procedural imaging showed a markedly elevated left atrial (LA) volume (171 mL) with predominantly lateral enlargement. During a second transseptal puncture attempt, the sheath

Transcatheter Intervention Is Associated With Visceral Decongestion in Patients With Severe Tricuspid Regurgitation.

INTRODUCTION: Tricuspid regurgitation (TR) is frequently associated with severe venous congestion, increasingly recognized as a key factor in the development of cardiorenal syndrome. While transcatheter tricuspid valve intervention (TTVI) has shown to be effective in TR reduction, their impact on visceral venous congestion remains unknown. The aim of our study was to investigate visceral venous congestion in patients with severe TR undergoing TTVI. METHODS: We prospectively enrolled 30 patients undergoing TTVI for severe TR. Renal venous blood flow was assessed by Doppler ultrasound examinati

Early outcomes of transcatheter vacuum-assisted mass extraction in the left heart and the aortic arch.

BACKGROUND: Transcatheter vacuum-assisted mass extraction (TVME) in the systemic circulation is increasingly being reported. However, its feasibility, effectiveness, and safety remain poorly defined. AIMS: We sought to investigate the feasibility, effectiveness and safety of TVME in the left heart and aortic arch. METHODS: Consecutive patients from 6 centres in the United States and Canada were included. Feasibility was defined as technically successful mass extraction without unplanned cardiac surgery or intraprocedural mortality. The effectiveness endpoint was feasibility with successful d

Outcomes of rescue procedures in patients undergoing unsuccessful deployment of transcatheter mitral valve replacement.

BACKGROUND: Perivalvular leak (PVL), valve embolization, and migration of valve in mitral position are critical complications of transcatheter mitral valve replacement (TMVR). To date, rescue strategies, and outcomes following unsuccessful initial TMVR deployment remain poorly documented. METHODS: Between March 2016 and December 2024, 86 patients with high risk of open-heart surgery underwent TMVR with a balloon expandable valve at a single institution. 19.8% (17/86) of patients experienced unsuccessful first TMVR (unsuccessful group) due to valve embolization, migration, significant PVL, or

A rare cause of cardiac syncope: aorto-right atrial connection.

Syncope in children is commonly benign, yet rare cardiovascular anomalies may be causative. We present a child with recurrent syncope, ultimately diagnosed with an aorto-right atrial connection. Transcatheter closure was successfully performed with an Amplatzer Duct Occluder II. This case emphasizes considering aorto-atrial connections in pediatric syncope with continuous murmurs and highlights the efficacy of percutaneous closure.

Cardiac tumour in paediatrics: the role of multimodal imaging and therapeutic catheterisation.

We present the case of a 14-year-old male patient referred to paediatric cardiology for the incidental finding of a cardiac murmur and initially diagnosed with asymmetric septal hypertrophic cardiomyopathy. Due to deterioration in his functional class (NYHA II/IV), he was admitted to our institution for further evaluation. A hyperechoic mass measuring 35.7 mm × 39.4 mm was identified in the interventricular septum, with central vascularisation and a significant obstructive gradient in the right ventricular outflow tract (112 mmHg). Cardiac magnetic resonance imaging confirmed a highly vascular

Multi-center study of CMR versus cardiac catheterization with endomyocardial biopsy for pediatric transplant surveillance: study protocol.

Notwithstanding advances in post-transplant care, acute allograft rejection (AR) remains the leading cause of mortality in pediatric heart transplant recipients (PHTX). Endomyocardial biopsy (EMB) continues to be the standard of care for AR detection, despite being invasive, costly, and associated with patient discomfort and potential complications. Non-invasive alternatives, including cardiac magnetic resonance imaging (CMR) with T1 and T2 parametric mapping and blood biomarkers (BB) such as cell-free DNA (cfDNA) and microRNA (miRNA), have shown promise in the adult transplant population, but

Additive Value of Acetylcholine Testing in ANOCA: A US-Based Multicenter FlowLab Study.

BACKGROUND: Angina with non-obstructive coronary arteries (ANOCA) is an underrecognized cause of angina with non-universal diagnostic techniques and patient management strategies. Among ANOCA testing, vasospasm provocation is not always performed mainly due to differences in institutional availability. AIMS: Using the FlowLab Study, we quantified vasospasm prevalence among patients with ANOCA at US centers and compared physician decision-making, endotype distribution, and management between sites that did and did not include vasospasm testing in their workup. METHODS: The FlowLab Study was a
