A Contemporary Look at the Landscape of Treatment of Tricuspid Regurgitation: A Review.

IMPORTANCE: Untreated severe tricuspid regurgitation carries a poor prognosis. We aim to provide a contemporary review of the anatomy, clinical manifestations, and diagnostic and management strategies, including medical, surgical and transcatheter options. By synthesizing current knowledge, this review seeks to equip clinicians with the insights necessary to navigate the complexities of TR treatment. OBSERVATIONS: Tricuspid regurgitation is predominantly secondary to annular dilation and leaflet tethering but can also be associated with cardiac implantable electronic device leads and primary

Disproportionate impairment in diffusing capacity predicts pulmonary hypertension with an elevated pulmonary vascular resistance in COPD.

BACKGROUND: Current guidelines for the evaluation of chronic obstructive pulmonary disease (COPD) do not recommend screening for pulmonary hypertension (PH), despite the high prevalence and impact on outcomes. A simple screening tool to identify patients with an elevated pulmonary vascular resistance (PVR) is urgently needed, as they may benefit from PH-specific therapy and more urgent referral for lung transplantation. RESEARCH QUESTION: We sought to examine whether a ratio of forced expiratory volume in 1 s (FEV1) to diffusing capacity (DLCO) predicts haemodynamic patterns in COPD. STUDY D

Reduction of Erroneous ST-segment Elevation Myocardial Infarction Diagnoses: A Successful Nurse-led Quality Improvement Project.

An ST-segment elevation myocardial infarction is a life-threatening condition within the spectrum of acute coronary syndrome, with well-recognized implications for short-term and long-term health outcomes. Retrospective review of our institutional electronic health records identified patterns of inaccurate acute coronary syndrome diagnostic assignment, including the presence of erroneous ST-segment elevation myocardial infarction diagnoses in discharge records. Such erroneous diagnoses in the durable health record may increase risk to patients through inadequate risk assessment and inappropria

GLIDE Score is associated with procedural success in patients undergoing direct transcatheter tricuspid valve annuloplasty.

AIMS: The GLIDE Score (septolateral gap, predominant jet location, image quality, chordal structure density, and en-face jet morphology) may predict successful tricuspid transcatheter edge-to-edge repair. This study aimed to evaluate its predictive value in transcatheter tricuspid valve annuloplasty (TTVA). METHODS AND RESULTS: This study was performed on 204 consecutive patients who underwent TTVA between 2018 and 2023 at two tertiary German centres. The GLIDE Score was assessed using preprocedural transoesophageal echocardiograms.Residual tricuspid regurgitation (TR) grade ≤ I was achieved

A p53-responsive microRNA-100 released from cardiomyoblasts serves as an early biomarker for acute coronary syndrome.

Coronary artery disease, a leading cause of mortality, underscores the need for biomarkers that can predict acute coronary syndrome (ACS) occurrence and outcomes. MicroRNAs (miRNAs) are increasingly recognized as potential markers. This study aimed to identify a plasma miRNA signature in ACS patients undergoing elective cardiac catheterization and to investigate miR-100's potential as an ACS prognostic biomarker. Plasma samples from 100 patients with suspected ACS were analyzed. qPCR revealed significantly elevated plasma miR-29a-3p, miR-100, miR-192, and miR-194-5p in ACS patients, and multip

Translational Strategies for Developing and Evaluating Transcatheter Devices in Swine Heart Failure Models.

Developing a therapeutic transcatheter device for cardiovascular diseases requires precise preclinical large animal models and robust evaluation methods. Three key aspects are essential for evaluating the safety and efficacy of a device: (1) types of swine heart failure models, (2) appropriate echocardiographic methods, and (3) the specific implantation approaches in swine. Common heart failure models include rapid pacing heart failure models, volume overload models, and ischemic models. Among the ischemic models, several approaches are performed to replicate the pathological conditions, such

Novel deep learning CCTA-FFR for detecting functionally significant coronary stenosis: Comparison with iFR.

BACKGROUND: Deep learning-based fractional flow reserve derived from coronary CT angiography (CT-FFR) enables noninvasive assessment of lesion-specific ischemia. Onsite CT-FFR systems provide near-real-time physiologic evaluation at the workstation, potentially reducing unnecessary invasive testing. This study evaluated the diagnostic performance of a novel onsite deep learning CT-FFR algorithm compared with invasive instantaneous wave-free ratio (iFR). METHODS: We retrospectively analyzed 44 patients (44 lesions) who underwent clinically indicated coronary CT angiography (CCTA) and invasive

Implications of Potential Undiagnosed HFpEF on Symptomatic Response With Coronary Revascularization.

BACKGROUND: Although patients with heart failure with preserved ejection fraction (HFpEF) have poor quality of life (QoL) and a high coronary artery disease (CAD) burden, there remains limited evidence guiding revascularization in these patients, in part related to complexity in diagnosis. OBJECTIVES: This study aims to determine the prevalence of likely undiagnosed HFpEF in patients with CAD and a positive stress test result, as well as its therapeutic interaction with an invasive strategy on QoL. METHODS: Patients without known heart failure (HF) from the ISCHEMIA (International Study of C

Combining MELD-XI score and hemodynamic parameters enhances short-term prognostic value in patients undergoing transcatheter tricuspid valve interventions.

BACKGROUND: Hepatorenal dysfunction is a key prognostic factor in patients with symptomatic tricuspid valve regurgitation (TR). This study evaluated the prognostic value of hepatorenal dysfunction using the Model for End-stage Liver Disease (MELD), the modified MELD-XI score, and right heart hemodynamic parameters in patients undergoing transcatheter tricuspid valve interventions (TTVI). METHODS: This prospective, single-center cohort study included patients undergoing edge-to-edge or heterotopic tricuspid valve repair. The MELD and MELD-XI scores were calculated using creatinine, bilirubin a

Accuracy of site versus core laboratory interpretations of right heart catheterization hemodynamics.

An accurate interpretation of hemodynamics measured by right heart catheterization (RHC) is crucial for guiding the management of heart failure and pulmonary hypertension. We evaluated the concordance and variability between site-reported and core laboratory-adjudicated interpretations of invasive hemodynamic measurements.

Real-World Suitability of Patients for Transcatheter Tricuspid Valve Replacement.

Transcatheter tricuspid valve replacement (TTVR) has shown therapeutic promise for patients with severe tricuspid regurgitation (TR). However, some patients may not be eligible due to anatomic limitations. We sought to describe the outcomes of patients who were referred for transcatheter tricuspid valve intervention (TTVI) and were ineligible for TTVR. This was a single-center, retrospective study of 251 patients referred for TTVI from February 2024 to August 2025. All patients were considered by a multidisciplinary heart team and assessed for feasibility of commercial tricuspid valve repair o

Improved diagnostic performance of CT-derived FFR using 70-keV monoenergetic spectral CT in coronary artery disease.

To compare the diagnostic performance of coronary CT-derived fractional flow reserve (CT-FFR) calculated from conventional polychromatic CT images with 70-keV monoenergetic reconstructions derived from spectral CT data. This retrospective study enrolled patients who underwent spectral coronary CT angiography followed by invasive FFR measurements. The CT-FFR values were computed from conventional polychromatic and 70-keV monoenergetic images. Image quality was assessed using the contrast-to-noise ratio (CNR). Comparison of the CT-FFR with invasive FFR provided mean absolute error (MAE), Pearson

Zipping Versus Clover Repair in Transcatheter Edge-to-Edge Tricuspid Repair: Insights From the TRI-SPA Registry.

BACKGROUND: Transcatheter edge-to-edge repair is an established treatment for tricuspid regurgitation (TR) in nonoptimal surgical candidates. Two techniques have been described: zipping (or bicuspidization) and clover (or triple-orifice). This study aimed to compare the echocardiographic and clinical outcomes of these 2 techniques. METHODS: This multicenter registry included 288 patients undergoing tricuspid transcatheter edge-to-edge repair from June 2020 to December 2024. Patients were categorized into 2 groups (zipping and clover repairs). The primary study end point was the TR reduction a

Stress testing in the diagnosis and early screening of chronic thromboembolic pulmonary disease with and without pulmonary hypertension.

BACKGROUND: Chronic thromboembolic pulmonary disease (CTEPD) and its hemodynamic counterpart chronic thromboembolic pulmonary hypertension (CTEPH) are long-term sequelae of acute pulmonary embolism (PE), often underdiagnosed. Often, patients with CTEPD without PH may, however, develop exercise-induced PH (ExPH), a clinical condition detectable only under stress conditions. OBJECTIVE: This narrative review aims at evaluating current evidence on the role of stress testing in the early diagnosis, screening and clinical management of CTEPD and CTEPH, with a particular focus on exercise-induced he

Evaluation of diastolic function: machine learning improves classification of left ventricular filling pressure.

AIMS: Current recommendations for echocardiography-based classification of left ventricular filling pressure (LVFP) as normal or elevated, are based on an algorithm and parameter selection determined by human experts. We tested whether machine learning (ML) can improve classification of LVFP and investigated which parameters were deemed most important by different ML models. METHODS AND RESULTS: In a multicentre study, echocardiography was performed simultaneously with, or within 24 h of, heart catheterization in 250 patients. Eight different ML models were trained and tested using a nested c
